Multiple Liver Abscesses Associated with Ventriculoperitoneal Shunt Infection : Case Report and Review of the Literature

  • Liver abscess following ventriculoperitoneal (VP) shunting occurs very rarely. We report an unusual case of multiple liver abscesses caused by Staphylococcus capitis in a 50-year-old compromised woman due to a complicating VP shunt infection. We reviewed the nine cases of VP shunt complications reported in the English literature, and speculated that the most likely pathogenetic mechanism in our case is an infected peritoneal tip that migrated to and penetrated the liver, which subsequently caused the formation of multiple liver abscesses. The patient was successfully treated with percutaneous aspiration, drainage of the abscesses, intravenous antibiotics, and shunt revision. Awareness and vigilance of the possibility of liver abscess formation caused by VP shunt infection will help establish an early accurate diagnosis and therapeutic strategy.

  • There are many of anaerobic culture methods and equipments for isolation and cultivation of anaerabic bacteria, but most of these methods are used without pre-reduced media. Gas-jet method is a recommend. able method for the culture of anaerobes, resently developed. Bacteriological studies were experimented of liver abscess of cattle by the use of gas. jet method. The results were summarised as follows; 1. Gas-jet method for anaerobic culture are expedient for the making of pre-reduced media, maintaining of oxygen free condition in the culture tube, picking of bacteria from colony and colony counting etc. 2. A 121 strains of facultative anaerobic and anaerobic bacteria were isolated from liver abscess of 27 head of cattle, and the isolated anaerobic bacteria were as follows. Peptostreptococcus spp. 7 strains Acid aminococcus fermentans 1 Veillonella spp. 1 Bacterioides spp. 6 Bifidobacterium spp. 4 Arachinia propionica 2 Lactobacillus spp. 4 Propionibacterium acnes 1 3. Liver abscess were infected with many of bacteria, about $10^3-10^9$ numbers per gram of abcessed tissue. Almost of abscess were mixed infection of various bacterial species rather than simple species.

  • Congenital solitary liver abscess in newborns is an extremely rare and serious disease. Only one case was reported in the literature. Most a liver abscesses have a postnatal origin. We encountered a premature infant who had a low. Apgar score and an elevated right diaphragm at birth. Pyogenic solitary liver abscess was diagnosed by ultrasound and computed tomography. Staphylococcus aureus was cultured from ultrasound guided aspiration fluid.

  • Both primary hepatoma and amebic liver abscess are probably the most common space-occupying disease of the liver in Korea, and the importance of their accurate distinction can not be overemphasized. Radiogold hepatoscanning has been widely used at St. Mary's Hospital, and recently blood disappearance rate of colloidal $^{198}Au$ has been introduced in the study of hepatic ailments. A review of the literature revealed few previous publications concerning the differential features of the two conditions although some characteristic scan findings of each condition have been described. The present study is aimed at a critical review and analysis of various photoscan findings and blood disappearance rate determinations of colloidal $^{198}Au$ of 34 cases of primary hepatoma and 30 cases of amebic liver abscess. Among many scan manifestations, prominence of the uninvolved liver tissue, "hemispherical cold area," and splenic uptake are much more commonly encountered in primary hepatoma and the blood disappearance rate tends to be far more frequently delayed in primary hepatoma than in abscess providing at least a statistically useful basis of distinction of the two diseases.

  • Pyogenic liver abscess (PLA) is rare in healthy children. We report a case of PLA in an immunocompetent 12-year-old boy. Percutaneous catheter drainage was performed for the abscess. In addition, parenteral antibiotics were administered for 3 weeks. Klebsiella pneumoniae was detected in the culture of blood and drained fluid. Here, we present this case and a brief review of the literature on this subject.

  • Klebsiella pneumoniae is known to cause an invasive syndrome characterized by primary liver abscess associated with metastatic infection. The characteristics of the metastatic infection involving the musculoskeletal system in this invasive syndrome are not well understood. The authors present a case report of a patient who developed abscesses of the lower extremities along with abscesses of multiple organs, such as the liver and eye, caused by K. pneumoniae. The patient was diagnosed early, and the infection was successfully controlled after several surgical treatments.

  • Staphylococcus epidermidis is a normal inhabitant of skin, throat, mouth, vagina, and urethra. It is not usually pathogenic, particularly in immunocompetent hosts. This report describes a case of a pyogenic liver abscess caused by Staphylococcus epidermidis in a healthy 12-year-old boy. He was admitted to Seoul St. Mary's Hospital with intermittent fever for 6 days. Findings on abdominal computed tomography (CT) showed a mass measuring $7.0{\times}6.5$ cm in the right hepatic lobe. Culture of the abscess resulted in growth of Staphylococcus epidermidis as a causative organism. The patient was successfully treated with intravenous administration of antibiotics and percutaneous drainage of the abscess.

F-18 FDG Uptake in an Eosinophilic Liver Abscess Mimicking Hepatic Metastasis on PET/CT Images (PET/CT에서 간전이로 오인되었던 호산구성 간농양의 F-18 FDG 섭취 증가)

  • A 61-year-old man had a F-18 FDG PET/CT scan for evaluation of a common bile duct cancer identified on CT. The PET/CT image showed a hypermetabolic mass in the common bile duct, and a focal area of increased F-18 FDG uptake in segment IV of the liver, which corresponded to a hypoattenuated lesion on non-enhanced CT, and was consistent with hepatic metastasis. The patient underwent choledochojejunostomy with hepatic resection, and pathologic findings were compatible with an eosinophilic abscess in the liver. This case demonstrates that F-18 FDG uptake by an eosinophilic abscess can mimic hepatic metastasis in a patient with a malignancy.

  • Tuberculosis is not a common cause of liver abscess and it is rarely considered in the differential diagnosis of a patient with hepatic mass. We report a case of tuberculous abscess of liver and spleen in a 15-year-old boy who presented with abdominal pain, fever and weight loss. The ultrasonographic and computed tomographic scan of the abdomen revealed multiple cystic lesions in the liver and spleen. Mycobacterium tuberculosis was cultured from the ascitic fluid and biopsy specimen of lymph node. Follow up CT scan of the abdomen after anti-tuberculosis medication for eighteen months showed complete resolution of the cystic lesions with calcified nodules.
