THE MANAGEMENT OF TONGUE BITE IN A PATIENT OF CEREBRAL PALSY AFTER DENTAL TREATMENT UNDER GENERAL ANESTHESIA -CASE REPORTT- (뇌성마비환자의 전신마취 하 치과치료 후 혀 깨물기 손상 관리)

  • Trauma to the lips and tongue can occur by accidental self-biting after dental treatment. After local anesthesia, it is likely that the patient may feel painless even in biting the tongue. In case of young children and disabled patients, the dentists should be careful not to bite the tongue. In this report, we present a case of deep lingual laceration due to biting the tongue in the course of dental treatment under general anesthesia. A 33 year-old male was transferred to our hospital to treat tongue laceration. Before 2 hour on arrival, he had received dental care under general anesthesia at a dental hospital for the disabled because of cooperation difficulty and cerebral palsy. During recovery from general anesthesia, he tried to bite his own tongue involuntary. The doctors and nurses tried to prevent the patient from being injured. Despite these efforts, massive bleeding occurred from the injured sites of the tongue. Because we could not communicate with him, we decided to evaluate the extent of the injury and treat the injured sites under general anesthesia. The laceration wound was sutured for nearly 1 hr general anesthesia. During recovery we inserted mouth prop into the oral cavity to prevent further injuries from tongue biting. After full recovery from general anesthesia he didn't try to bite his tongue. After 4 hour admission, he was discharged without other complications.

  • Cerebral Palsy is a genetic term referring to abnormalities of motor control caused by damage to a child's brain early in the course of development. Due to the impairment of balanced perioral muscle development, the prevalence of malocclusions in patients with cerebral palsy such as maxillary protrusion is high. But most clinicians may feel uncomfortable to treatment of these problems. Here a case report about mitigation of maxillary anterior teeth protruded in patient with cerebral palsy. 8y 4m old boy who have cerebral palsy visited our dental hospital. He showed severely protrusive maxillary anterior teeth with mouth breathing and could not close his mouth. He and his mother wanted to improve dental and facial esthetic problem. Specially designed or modified intraoral fixed appliance and rubber elastic chain was used in the therapy. Treatment carried out for 8 months and we could observe maxillary incisor angle was improved and mouth breathing habit was stopped. In conclusion, modified fixed appliance therapy for the patients with cerebral palsy might be useful. Continuous rehabilitation training of lips should be followed after treatment to correct imbalance of muscle tone.

  • Pleomorphic adenoma is the most common of all salivary gland tumors, constituting over 50% of all cases of tumors of both major and minor salivary gland origin and approximately 90% of all benign salivary gland tumors. Of the major salivary glands, the parotid gland is the most common site of the pleomorphic adenoma. It may occur, however, in any of the major gland or in the widely distributed intraoral accessory salivary glands. The palatal glands are frequently the site of origin of tumors, and other parts of origins are as follows: upper and lower lip, buccal mucosa, tongue and occasionally other sites. The majority of the lesions are found in patients in the fourth to sixth decades, but they are also relatively common in young adults and have been known to occur in children. It is somewhat more frequent in women than men. The term "mixed tumor" has masquaeraded under a great variety of names throughout the years (e.g., enclavoma, branchioma, endothelioma, enchondroma), but the term "pleomorphic adenoma" suggested by Willis characterizes closely the unusual histologic pattern of the lesion. The accepted treatment for this tumor is surgical excision. The intraoral lesions can be treated somewhat more conservatively by extracapsular excision. In general, Lesions of the hard palate should be excised with the overlying mucosa, while those in lining mucosa, such as the lips, soft palate and buccal mucosa often can be treated successfully by enucleation or extracapsular excision. In our hospital, we experienced two patients who were identified pleomorphic adenoma which occurred at buccal mucosa, submandibular gland. The lesions were successfully treated by surgical excision.

Oral and Pharyngeal Cancer Among the Arab Population in israel from 1970 to 2006

  • Background: Israeli Arabs are considered as a developing society characterized by poverty and high levels of smoking among men. The purpose of this study was to describe their incidence, mortality and survival rates for oral and pharyngeal cancer between the years 1970-2006. Studies such as this in the Arab world, where the population is almost the same as the Arab population in Israel, are rare. Methods: The incidence and survival data were derived from all relevant registered data at the National Cancer Registry. The group of lesions included cancer of the lips, tongue, buccal mucosa, floor of the mouth, salivary glands, gums, palate and pharynx. Morphological description was according to WHO classification. Results: Most diagnosed patients were male. The mean age was 54.4 years, and mean years of survival were 3.83. The oropharynx was the most common site (28.3%) while the palate was the least frequent (3.12%). Squamous cell carcinoma (SCC) was the most common histological feature (66.3%), while basal cell carcinoma (BCC) was the least (3.9%). The overall 5 years survival rate was 59.4%, this being highest for BCC (82.1%), while SCC was significantly lower (56.2%) (p<0.001). Lip cancers survived better than other sites. Conclusions: Data from this society are similar to other developing societies in the majority of the results. The incidence of oral and pharyngeal cancer is lower among the Arab population, in comparison to the Jewish population in Israel.

  • This study carefully explored today's preferable beauty images, analyzed Korean adult purchasing trends of functional underwear and market analysis of the most popular items and their designs. Information and methods are based largely on reference book material as well as analysis of real domestic data. The paper's research timeline simply spans the last three years, as in accordance with project parameters. Helping improve body shape, foundation literally means 'substructur' and 'base'. The formativeness in these adult foundations typically has three main category classifications. First, natural modern image. This is where we find natural beauty including that which embodies a cool modern sense. Here, you can often see such patterns as either stripes and dots, or simply no patterns at all. Colors are green, red-brown, incarnadine, or khaki from a natural or basic human body color. Its products are marketed as both solid and durable eco-friendly materials, and lace-seethrough. Fashion's natural modern image is undoubtedly the most popular style purchase choice among both genders. Second, sweet romantic image. This mainly consists of two concepts : a sweet and romantic theme, and visual appeal. Patterns include flowers, lips, heart, stars, candies and various other appealing characters. Its main colors are pink, yellow, red, purple, and violet. Products are decorated with laces, frills and ribbons. Since only men's semi-girdles carry such designs, this fashion foundation is largely a women's world. While men show an obvious interest in foundation, they still mostly prefer the choice of natural modern images. Third, mystic sexy image. This is defined as all feminine images of mysterious and classic sexual beauty. Patterns are variously expressed as paisley, flowers, and geometric. Colors are largely purple, blue-green, royal-blue, cobalt, and black. Products are commonly decorated with beading, spangles, hot-fixes and so on. While this image can be found in all kinds of women's foundations, it typically appears only in men's corsets and girdles.

  • Harlequin ichthyosis (HI, OMIM #242500) is one of the most severe skin diseases among the autosomal recessive congenital ichthyoses, with high morbidity and mortality, particularly in newborns. Clinically, it is characterized by a typical appearance of generalized, thick, yellowish, hyperkeratotic plates with deep erythematous fissures on the skin. Herein, we present the case of a newborn girl with HI that was genetically confirmed by targeted gene panel analysis. The premature baby was encased in an opaque white membrane with erosion covering the skin of the entire body except the lips, with her hands and feet restricted by the membrane. Humidification, emollient, and retinoic acid treatment were started; the thick ichthyosis gradually peeled off and the underlying skin was only covered with thin scales. Targeted gene panel analysis using next-generation sequencing and validation with Sanger sequencing and quantitative polymerase chain reaction analyses confirmed compound heterozygous mutations of the ABCA12 gene (p.N1380S and a partial gene deletion encompassing exon 9). The parents were carriers for each of the identified mutations. Early recognition of the genetic etiology of congenital ichthyosis can, thus, facilitate genetic counseling for patients and their families.

Word-boundary and rate effects on upper and lower lip movements in the articulation of the bilabial stop /p/ in Korean

  • In this study, we examined how the upper and lower lips articulate to produce labial /p/. Using electromagnetic midsagittal articulography, we collected flesh-point tracking movement data from eight native speakers of Seoul Korean (five females and three males). Individual articulatory movements in /p/ were examined in terms of minimum vertical upper lip position, maximum vertical lower lip position, and corresponding vertical upper lip position aligned with maximum vertical lower lip position. Using linear mixed-effect models, we tested two factors (word boundary [across-word vs. within-word] and speech rate [comfortable vs. fast]) and their interaction, considering subjects as random effects. The results are summarized as follows. First, maximum lower lip position varied with different word boundaries and speech rates, but no interaction was detected. In particular, maximum lower lip position was lower (e.g., less constricted or more reduced) in fast rate condition and across-word boundary condition. Second, minimum lower lip position, as well as lower lip position, measured at the time of maximum lower lip position only varied with different word boundaries, showing that they were consistently lower in across-word condition. We provide further empirical evidence of lower lip movement sensitive to both different word boundaries (e.g., linguistic factor) and speech rates (e.g., paralinguistic factor); this supports the traditional idea that the lower lip is an actively moving articulator. The sensitivity of upper lip movement is also observed with different word boundaries; this counters the traditional idea that the upper lip is the target area, which presupposes immobility. Taken together, the lip aperture gesture is a good indicator that takes into account upper and lower lip vertical movements, compared to the traditional approach that distinguishes a movable articulator from target place. Respective of different speech rates, the results of the present study patterned with cross-linguistic lenition-related allophonic variation, which is known to be more sensitive to fast rate.

Treatment effects of mandibular total arch distalization using a ramal plate

  • Objective: The purpose of this study was to evaluate treatment effects after distalization of the mandibular dentition using ramal plates through lateral cephalograms. Methods: Pre- and post-treatment lateral cephalograms and dental casts of 22 adult patients (11 males and 11 females; mean age, $23.9{\pm}5.52years$) who received ramal plates for mandibular molar distalization were analyzed. The treatment effects and amount of distalization of the mandibular molars were calculated and tested for statistical significance. The significance level was set at p < 0.001. Results: The mandibular first molar distalization at the crown and root were 2.10 mm (p < 0.001) and 0.81 mm (p = 0.011), respectively. In the evaluation of skeletal variables, there was a significant increase in the Wits appraisal (p < 0.001). In the evaluation of the soft tissue, there was no significant effect on upper lip position, but the lower lips showed a significant retraction of 2.2 mm (p < 0.001). Conclusions: The mandibular molars showed a significant amount of distalization accompanied by limited extrusion and mesiobuccal rotation of the crowns. A ramal plate may be a viable device for mandibular total arch distalization in Class III patients who are reluctant to undergo orthognathic surgery.

  • Deep burns of the face & lips often lead to scarring and contraction of the circum-oral tissues with a marked reduction in the size of the oral aperture. Such burns most commonly caused by electrical contact, for example, children sucking electric plugs, or as a result of chemical burns and the exposture to flame. Once having burns, burned tissue may contract and reduce considerably the size and mobility of the mouth ; therefore bring up the resultant functional disturbance, verbal difficulty, even digestive difficulty, and poor appearance, which fall into difficulty in social acceptability, caused by burns. In our department, 2 patients who were complain of functional limitations and esthetic problem owing to scar contracture, were visited, and we treated this microstomia with scar excision, graft and flap technique, and postoperative intensive physical therapy. We obtained relatively favorable results, thus report this cases with literature review.

A Method of Eye and Lip Region Detection using Faster R-CNN in Face Image (초고속 R-CNN을 이용한 얼굴영상에서 눈 및 입술영역 검출방법)

  • In the field of biometric security such as face and iris recognition, it is essential to extract facial features such as eyes and lips. In this paper, we have studied a method of detecting eye and lip region in face image using faster R-CNN. The faster R-CNN is an object detection method using deep running and is well known to have superior performance compared to the conventional feature-based method. In this paper, feature maps are extracted by applying convolution, linear rectification process, and max pooling process to facial images in order. The RPN(region proposal network) is learned using the feature map to detect the region proposal. Then, eye and lip detector are learned by using the region proposal and feature map. In order to examine the performance of the proposed method, we experimented with 800 face images of Korean men and women. We used 480 images for the learning phase and 320 images for the test one. Computer simulation showed that the average precision of eye and lip region detection for 50 epoch cases is 97.7% and 91.0%, respectively.
