Treatment Patterns, Costs, and Survival among Medicare-Enrolled Elderly Patients Diagnosed with Advanced Stage Gastric Cancer: Analysis of a Linked Population-Based Cancer Registry and Administrative Claims Database

  • Purpose: To assess real-world treatment patterns, health care utilization, costs, and survival among Medicare enrollees with locally advanced/unresectable or metastatic gastric cancer receiving standard first-line chemotherapy. Materials and Methods: This was a retrospective analysis of the Surveillance, Epidemiology, and End Results-Medicare linked database (2000~2009). The inclusion criteria were as follows: (1) first diagnosed with locally advanced/unresectable or metastatic gastric cancer between July 1, 2000 and December 31, 2007 (first diagnosis defined the index date); (2) ${\geq}65$ years of age at index; (3) continuously enrolled in Medicare Part A and B from 6 months before index through the end of follow-up, defined by death or the database end date (December 31, 2009), whichever occurred first; and (4) received first-line treatment with fluoropyrimidine and/or a platinum chemotherapy agent. Results: In total, 2,583 patients met the inclusion criteria. The mean age at index was $74.8{\pm}6.0years$. Over 90% of patients died during follow-up, with a median survival of 361 days for the overall post-index period and 167 days for the period after the completion of first-line chemotherapy. The mean total gastric cancer-related cost per patient over the entire post-index follow-up period was United States dollar (USD) $70,808{\pm}56,620$. Following the completion of first-line chemotherapy, patients receiving further cancer-directed treatment had USD 25,216 additional disease-related costs versus patients receiving supportive care only (P<0.001). Conclusions: The economic burden of advanced gastric cancer is substantial. Extrapolating based on published incidence estimates and staging distributions, the estimated total disease-related lifetime cost to Medicare for the roughly 22,200 patients expected to be diagnosed with this disease in 2014 approaches USD 300 millions.

  • We describe the framework of a data-fuelled, interdisciplinary team-led learning system. The idea is to build models using patients from one's own institution whose features are similar to an index patient as regards an outcome of interest, in order to predict the utility of diagnostic tests and interventions, as well as inform prognosis. The Laboratory of Computational Physiology at the Massachusetts Institute of Technology developed and maintains MIMIC-II, a public deidentified high- resolution database of patients admitted to Beth Israel Deaconess Medical Center. It hosts teams of clinicians (nurses, doctors, pharmacists) and scientists (database engineers, modelers, epidemiologists) who translate the day-to-day questions during rounds that have no clear answers in the current medical literature into study designs, perform the modeling and the analysis and publish their findings. The studies fall into the following broad categories: identification and interrogation of practice variation, predictive modeling of clinical outcomes within patient subsets and comparative effectiveness research on diagnostic tests and therapeutic interventions. Clinical databases such as MIMIC-II, where recorded health care transactions - clinical decisions linked with patient outcomes - are constantly uploaded, become the centerpiece of a learning system.

대학 아카이브즈의 사진자료 관리에 관한 연구 - 부산대학교 도서관의 사례를 중심으로 - (A Study on the Photograph Management at the University Archives - The Case of the Pusan National University Library -)

  • 사진은 다른 기록물과 비교하여 물리적 특성과 정보를 담고 있는 형식의 차이 때문에 특별한 관리를 필요로 한다. 이에 대학 아카이브즈에서 빈번하게 이용되는 사진자료의 효율적인 이용과 관리를 위한 방안으로서, 먼저 대학 관련 사진이미지를 싣고 있는 대학 간행물에서 사진이미지를 추출하고, 서지정보를 파악하여 워크시트를 작성하고, 이를 토대로 사진자료의 서지데이터베이스를 구축하였다. 다음으로 대학 간행물로부터 추출해낸 사진이미지를 편집하고 이를 서지데이터베이스에 링크시킴으로서 사진의 서지정보 검색과 더불어 이미지를 즉시 확인할 수 있는 데이터베이스 구축과정을 사례를 중심으로 소개하고 몇 가지 문제점과 향후 개선방향에 대하여 살펴본다.

  • Information systems for application areas like office automation, customer service or computer aided manufacturing are usually highly interactive and deal with complex document structures composed of multiple media formats. For the realization of these systems, nonstandard database systems, which we call document databases, need to handle different types of coarse-and fine-grained document objects(like full-text documents, graphics and images), hierarchical and non-hierarchical relationships between objects(like composition-links and cross-references using hypertext structures) and document attributes of different types such as formatting/presentation information and access control. In this paper, we present the underlying data model for document databases based on descriptive markup languages that provide mechanisms for specifying the logical structure(or schema) of individual documents stored in the database. We then describe extensions to the data model for supporting notion of composite structures("join" operators for documents) --composition and hyperlinking mechanisms for representing compound documents and inter-linked documents as unique entites separate from their components. Furthermore, due to the interactive nature of the application domains, the database system in conjunction with clients(or browsers) has to support visual navigation and graphical query mechanisms. We describe the functionality of a new user interface paradigm called HyBrow for meeting the above mentioned requirements. The underlying implementation strategy is also discussed.discussed.

극대용량 서지 링크드 데이터 구축의 효율성을 위한 RDF 트리플 저장소 접근 최소화에 관한 연구 (Research on Minimizing Access to RDF Triple Store for Efficiency in Constructing Massive Bibliographic Linked Data)

  • 본 논문에서는 세계 최대 규모의 생의학 분야 서지 데이터베이스인 MEDLINE 전체를 링크드 데이터로 변환 구축하는 효율적인 방안을 제시한다. 이를 위해서 우선 MEDLINE 레코드 구조를 세부적으로 분석하여 적합한 RDF 스키마를 도출하고 각 레코드를 도출된 스키마에 유효한 RDF 파일로 변환하는 과정을 거친다. 본 논문에서는 변환된 레코드 단위의 모든 RDF 파일을 병합하여 이를 단일 RDF 트리플 저장소에 저장할 때 주어 URI 중복 확인 절차를 효율화하는 이중 일괄 등록 방법을 적용한다. 이 방법을 통해서 RDF 파일 단위로 링크드 데이터를 순차적으로 구축하는 방법과 비교했을 때 주어 URI 중복 제거를 위한 RDF 트리플 저장소 접근 횟수가 26,597,850회에서 2,400회로 감소하는 결과를 가져왔다. 따라서 본 연구의 결과는 대용량 서지 레코드 집합을 링크드 데이터로 변환하는 과정에서의 비효율성을 제거하고 신속성과 시의성을 확보할 수 있는 중대한 계기를 제공할 것으로 기대한다.

의학진단과 연계된 간호진단 및 중재 프로그램 개발 (Development of Computerized Program for Nursing diagnosis and Intervention linked to Medical Diagnosis)

  • Purpose: The actual nursing processes have been performed by individual nurses' judgment without any supporting programs in Korea. It is not easy for novice nurses to make accurate diagnoses and provide proper nursing interventions to patients. Therefore, we propose a computerized program for nursing diagnosis and intervention linked to medical diagnosis. Method: For the program, we have linked standardized nursing diagnosis and intervention classifications with medical diagnosis. It is premised that the program is connected to order communication system(OCS) in hospitals. Result: We provide a nursing information system with standardized database for nursing diagnosis and interventions so that nurses can make more accurate diagnosis and perform more adequate interventions. Conclusion: It is expected that the program will help the nurses perform their nursing processes more efficiently. And we expect the system can be used in many hospitals efficiently in the future after pilot operations are completed in some hospitals.

캐드 환경에서 플라스틱 사출 금형 설계 시스템의 개발 (Development of Plastic Injection Mold Design System on the CAD Environment)

  • In this work, we have been concerned with developing an intelligent mold design system for plastic injection molding on the AutoCAD. We have concentrated on building a viable environment, including a mold parts database and a menu-driven user interface. This provides a more interactive and interface for selection of optimal mold-base and mold parts in mold design system. This work presents a method which allows the designer to select the mold parts and mold-base directly within an AutoCAD environment. It can also automatically generate detailed 3D drawings of the mold parts and mold-base. The system shows its potential capability for future enhancement. Since the system is independent of the data, it could easily be extended to other mold-bases and mold parts. In addition, it can be linked to the molding analysis system by creating subtracted 3-D models.

  • 실계통에서 운영되는 에너지관리시스템(EMS)의 계통해석 기능을 수행하기 위해서는 대규모 자료의 처리가 불가피하며 이에 따른 메모리의 사용에 대한 효율성, 즉 물리적인 자원의 제약성도 고려되어한 한다. 이러한 이유로 인하여 대규모 자료처리가 요구되는 실시간 시스템의 개발에서는 데이터에 대한 저장의 효율성과 접근의 용이성을 제공할 수 있는 자료설계 (Database Design)가 매우 중요하다 [1]. 이를 위하여 EMS에서는 고속의 연산속도에 대한 요구를 만족하며, 효율적이고 빠른 데이터 처리가 가능한 데이터저장기법이 적용된다. 본 논문에서는 상업용 EMS와 현재 개발중인 한국형EMS에서 적용되고 있는 자료저장기법인 연결형데이터저장기법 (Linked List Data Storage Scheme)에 대한 기능원리와 실제 적용에 대하여 소개하고자 한다.

SOP (Search of Omics Pathway): A Web-based Tool for Visualization of KEGG Pathway Diagrams of Omics Data

  • With the help of a development and popularization of microarray technology that enable to us to simultaneously investigate the expression pattern of thousands of genes, the toxicogenomics experimenters can interpret the genome-scale interaction between genes exposed in toxicant or toxicant-related environment. The ultimate and primary goal of toxicogenomics identifies functional context among the group of genes that are differentially or similarly coexpressed under the specific toxic substance. On the other side, public reference databases with transcriptom, proteom, and biological pathway information are needed for the analysis of these complex omics data. However, due to the heterogeneous and independent nature of these databases, it is hard to individually analyze a large omics annotations and their pathway information. Fortunately, several web sites of the public database provide information linked to other. Nevertheless it involves not only approriate information but also unnecessary information to users. Therefore, the systematically integrated database that is suitable to a demand of experimenters is needed. For these reasons, we propose SOP (Search of Omics Pathway) database system which is constructed as the integrated biological database converting heterogeneous feature of public databases into combined feature. In addition, SOP offers user-friendly web interfaces which enable users to submit gene queries for biological interpretation of gene lists derived from omics experiments. Outputs of SOP web interface are supported as the omics annotation table and the visualized pathway maps of KEGG PATHWAY database. We believe that SOP will appear as a helpful tool to perform biological interpretation of genes or proteins traced to omics experiments, lead to new discoveries from their pathway analysis, and design new hypothesis for a next toxicogenomics experiments.

한 번의 데이터베이스 탐색에 의한 빈발항목집합 탐색 (Frequent Patterns Mining using only one-time Database Scan)

  • 본 논문에서는 한 번의 데이터베이스 스캔으로 빈발항목집합들을 생성할 수 있는 효율적인 알고리즘을 제안한다. 제안하는 알고리즘은 빈발 항목과 그 빈발항목을 포함하고 있는 트랜잭션과의 관계를 나타내는 이분할 그래프(bipartite graph)를 생성한다. 그리고 생성된 이분할 그래프를 이용하여 후보 항목집합들을 생성하지 않고 빈발 항목집합들을 추출할 수 있다. 이분할 그래프는 빈발항목들을 추출하기위해 대용량의 트랜잭션 데이터베이스를 스캔할 때 생성된다. 이분할 그래프는 빈발항목들과 그들이 속한 트랜잭션들 간의 관계를 엣지(edge)로 연결한 그래프이다. 즉, 본 논문에서의 이분할 그래프는 대용량의 데이터베이스에서 쉽게 발견할 수 없는 빈발항목과 트랜잭션의 관계를 검색하기 쉽게 색인(index)화한 그래프이다. 본 논문에서 제안하는 방법은 한 번의 데이터베이스 스캔만을 수행하고 후보 항목집합들을 생성하지 않기 때문에 기존의 방법들보다 빠른 시간에 빈발 항목집합들을 찾을 수 있다.
