유한요소법을 이용한 설측 치아교정시 전치부 후방견인에 관한 연구 (Study on the Retraction of Anterior Teeth for the Lingual Orthodontics with the Three-Dimensional Finite Element Method)

  • The orthodontic surgery including lingual orthodontics has recently attracted a person's attention due to its functional and esthetic appreciation. The skeletal anchorage with the miniscrew is newly adopted in the lingual orthodontics to assist the upholding ability. The appliciation needs to understand the mechanism of the orthodontic appliance and its related orthodontic correction for optimal orthodontic treatment. There is, however, few information about the qualitative and quantitative effect of the orthodontic appliance with the miniscrew has not been well identified. In this paper, three dimensional finite element analysis is introduced to the lingual orthodontics in order to investigate the effect of the anterior retraction force on the miniscrew and transpalatal arch wire. The analysis determines the adequate location of the miniscrew and the point of force application of the anchorage system in the lingual orthodontics. The analysis results demonstrate the effect of the position of the miniscrew and the transpalatal arch wire on the lingual orthodontics.

Alveolar bone thickness around maxillary central incisors of different inclination assessed with cone-beam computed tomography

  • Objective: To assess the labial and lingual alveolar bone thickness in adults with maxillary central incisors of different inclination by cone-beam computed tomography (CBCT). Methods: Ninety maxillary central incisors from 45 patients were divided into three groups based on the maxillary central incisors to palatal plane angle; lingual-inclined, normal, and labial-inclined. Reformatted CBCT images were used to measure the labial and lingual alveolar bone thickness (ABT) at intervals corresponding to every 1/10 of the root length. The sum of labial ABT and lingual ABT at the level of the root apex was used to calculate the total ABT (TABT). The number of teeth exhibiting alveolar fenestration and dehiscence in each group was also tallied. One-way analysis of variance and Tukey's honestly significant difference test were applied for statistical analysis. Results: The labial ABT and TABT values at the root apex in the lingual-inclined group were significantly lower than in the other groups (p < 0.05). Lingual and labial ABT values were very low at the cervical level in the lingual-inclined and normal groups. There was a higher prevalence of alveolar fenestration in the lingual-inclined group. Conclusions: Lingual-inclined maxillary central incisors have less bone support at the level of the root apex and a greater frequency of alveolar bone defects than normal maxillary central incisors. The bone plate at the marginal level is also very thin.

  • In these days, the orthodontic surgery including lingual orthodontics has attracted a person' attention due to its functional and esthetic appreciation. The delivery of the optimal orthodontic treatment is greatly influenced by clinician' ability to predict and control the tooth movement by applying force system to dentition. The skeletal anchorage system with the miniscrew has been used recently in the lingual orthodontics to assist the anchorage control. Precise understanding of the force system produced from the various orthodontic appliances is necessary. However, the qualitative and quantitative effect of the miniscrew has not been identified well. In this paper, three dimensional finite element analysis is introduced on the lingual orthodontics to investigate the effect of anterior retraction force on the miniscrew and transpalatal arch wire. The purpose of this study is to determine the location of the miniscrew and the point of force application of the anchorage system in the lingual orthodontics. The analysis results indicate the efficient position of the miniscrew and the transpalatal arch wire in the lingual orthodontics.

Displacement pattern of the anterior segment using antero-posterior lingual retractor combined with a palatal plate

  • Objective: To evaluate and compare the effects of two appliances on the en masse retraction of the anterior teeth anchored by temporary skeletal anchorage devices (TSADs). Methods: The sample comprised 46 nongrowing hyperdivergent adult patients who planned to undergo upper first premolar extraction using lingual retractors. They were divided into three groups, based on the lingual appliance used: the C-lingual retractor (CLR) group (group 1, n = 16) and two antero-posterior lingual retractor (APLR) groups (n = 30, groups 2 and 3). The APLR group was divided by the posterior tube angulation; posterior tube parallel to the occlusal plane (group 2, n = 15) and distally tipped tube (group 3, n = 15). A retrospective clinical investigation of the skeletal, dental, and soft tissue relationships was performed using lateral cephalometric radiographs obtained pretreatment and post en masse retraction of the anterior teeth. Results: All groups achieved significant incisor and canine retraction. The upper posterior teeth did not drift significantly during the retraction period. The APLR group had less angulation change in the anterior dentition, compared to the CLR group. By changing the tube angulation in the APLR, the intrusive force significantly increased in the distally tipped tube of group 3 patients and remarkably reduced the occlusal plane angle. Conclusions: Compared to the CLR, the APLR provides better anterior torque control and canine tipping while achieving bodily translation. Furthermore, changing the tube angulation will affect the amount of incisor intrusion, even in patients with similar palatal vault depth, without the need for additional TSADs.

Comparison of inclination and vertical changes between single-wire and double-wire retraction techniques in lingual orthodontics

  • Objective: The Heat Induction Typodont System (HITS), used in some recent studies, has a distinct advantage over previous tooth movement simulation methods. This study aimed to compare inclination and vertical changes between the single-wire and double-wire techniques during en masse retraction with different lengths of lever arms in lingual orthodontics using an upgraded version of the HITS. Methods: Duet lingual brackets, which have two main slots, were used in this study. Forty samples were divided into four groups according to the length of the lever arm (3-mm or 6-mm hook) and the retraction wire (single-wire or double-wire). Four millimeters of en masse retraction was performed using lingual appliances. Thereafter, 3-dimensional-scanned images of the typodont were analyzed to measure inclination and vertical changes of the anterior teeth. Results: Incisor inclination presented more changes in the single-wire groups than in the double-wire groups. However, canine inclination did not differ between these groups. Regarding vertical changes, only the lateral incisors in the single-wire groups presented significantly larger values than did those in the double-wire groups. Combining the effect of hook lengths, among the four groups, the single-wire group with the 3-mm hook had the highest value, while the double-wire group with the 6-mm hook showed the least decrease in crown inclination and extrusion. Conclusions: The double-wire technique with an extended lever arm provided advantages over the single-wire technique with the same lever arm length in preventing torque loss and extrusion of the anterior teeth during en masse retraction in lingual orthodontics.

Evaluation of mandibular cortical bone thickness for placement of temporary anchorage devices (TADs)

  • Objective: In this study, we measured the cortical bone thickness in the mandibular buccal and lingual areas using computed tomography in order to evaluate the suitability of these areas for application of temporary anchorage devices (TADs) and to suggest a clinical guide for TADs. Methods: The buccal and lingual cortical bone thickness was measured in 15 men and 15 women. Bone thickness was measured 4 mm apical to the interdental cementoenamel junction between the mandibular canine and the 2nd molar using the transaxial slices in computed tomography images. Results: The cortical bone in the mandibular buccal and lingual areas was thicker in men than in women. In men, the mandibular lingual cortical bone was thicker than the buccal cortical bone, except between the 1st and 2nd molars on both sides. In women, the mandibular lingual cortical bone was thicker in all regions when compared to the buccal cortical bone. The mandibular buccal cortical bone thickness increased from the canine to the molars. The mandibular lingual cortical bone was thickest between the 1st and 2nd premolars, followed by the areas between the canine and 1st premolar, between the 2nd premolar and 1st molar, and between the 1st molar and 2nd molar. Conclusions: There is sufficient cortical bone for TAD applications in the mandibular buccal and lingual areas. This provides the basis and guidelines for the clinical use of TADs in the mandibular buccal and lingual areas.

Torque control during lingual anterior retraction without posterior appliances

  • Objective: To evaluate the factors that affect torque control during anterior retraction when utilizing the C-retractor with a palatal miniplate as an exclusive source of anchorage without posterior appliances. Methods: The C-retractor was modeled using a 3-dimensional beam element (0.9-mm-diameter stainless-steel wire) attached to mesh bonding pads. Various vertical heights and 2 attachment positions for the lingual anterior retraction hooks (LARHs) were evaluated. A force of 200 g was applied from each side hook of the miniplate to the splinted segment of 6 or 8 anterior teeth. Results: During anterior retraction, an increase in the LARH vertical height increased the amount of lingual root torque and intrusion of the incisors. In particular, with increasing vertical height, the tooth displacement pattern changed from controlled tipping to bodily displacement and then to lingual root displacement. The effects were enhanced when the LARH was located between the central and lateral incisors, as compared to when the LARH was located between the lateral incisors and canines. Conclusions: Three-dimensional lingual anterior retraction of the 6 or 8 anterior teeth can be accomplished using the palatal miniplate as the only anchorage source. Using LARHs at different heights or positions affects the quality of torque and intrusion.

  • 진단용 셋업 모형 제작시 전치의 치축을 조절하기 위하여 incisor inclination indicator란 장치가 소개되었다. 이것은 상하악 전치의 후방 견인시 토오크 조절을 위하여 사용된다. 본 논문에서는 incisor inclination indicator를 사용하여 전치부 설측 브라켓에 적절한 토오크를 처방하는 방법을 묘사하고 이렇게 적절한 토오크가 처방된 전치부 설측 브라켓으로 치료한 양악 전돌증 환자의 치료결과를 평가한다. 후방 견인시 상하악 전치는 토오크가 적절하게 조절되었다. 설측교정치료에 있어서 incisor inclination indicator는 기공 과정에서 부가적으로 사용되어 전치부의 후방 견인시 효과적으로 토오크 조절을 할 수 있는 것으로 생각된다.

정상교합자의 치열궁 설측형태에 관한 연구 (A STUDY ON THE LINGUAL MORPHOLOGY OF THE DENTAL ARCH IN NORMAL OCCLUSION)

  • This study was designed to get the knowledge of the structural characteristics of the lingual dental arch shapes of Koreans. The subjects consisted of 107 normal occlusions. The lingual structure which was divided into two groups, anterior and posterior part, was measured and analyzed. 33 pairs of the 107 samples were used to test the derived results. The findings of this study were as follows: 1. The mathmatical functions of the 12 curves which showed good fitting to the upper and lower anterior lingual arch were derived and flawed to the same scale using the computer graphics. 2. The results of the test showed a reasonable degree of accuracy to all of the 33 random samples. 3. The average degrees of posterior flexions were obtained and could be used as a initial guide at the wire-bending.

한국인 성인 정상교합자 치관의 설측면 특징에 관한 연구 (A MORPHOLOGIC CHARACTERISTICS STUDY OF CROWN OF LINGUAL SURFACE WITH NORMAL OCCLUSION IN KOREAN ADULTS)

  • The purpose of this study was to obtain the lingual morphology (size, angulation, contour, eminence) of adults with normal occlusion in order to provide the basic data for lingual bracket and lingual arch form in Korean. The subjects (Male: 50, Female: 50) were selected who have normal occlusion and dental casts were prepared. Tooth size, angulation, arch width, horizontal contour, lingual eminence were measured for all 28 teeth both upper and lower arch. The results were as follows: 1. Measuring items of all individual teeth for adults with normal occlusion were obtained. 2. In comparison tests, there was a statistically difference between intermolar widths $(\underline{6}\;to\;\underline{6})$ before occlusal reduction and intermolar width $(\underline{6}\;to\;\underline{6},\underline{7}\;to\;\underline{7})$ after occlusal reduction only all the other measuring items showed no statistically differences. 3. There was small variation in horizontal contour of lingual surface on lower incisors and upper and lower bicuspids. The other teeth showed somewhat greater variations. 4. There was offset between canine and bicuspid in upper arch while there was no prominent offset in lower arch.
