Methods to determine the volume of infrapatellar fat pad as an indicator of anterior cruciate ligament tear

  • Anterior knee pain is a major problem among adolescents and young adults especially those who participates in sports. The most common pathogenesis of anterior knee pain can arise from compression and shear forces in the patellofemoral joint. It is also caused by impingement of infrapatellar fat pad. Fat pad impingement can occur when the fat pad becomes swollen and inflamed due to a direct blow or chronic irritation. As a result, the bottom tip (or inferior pole) of the patella can pinch the fat pad. One of the many causes of swollen fat pad can be secondary to anterior cruciate ligament (ACL) injury. The aim of this study was to compare the infrapatellar fat pad volume in patients with acute ACL injury and a group of age-, gender-, and activity- matched controls with intact ligament. Axial magnetic resonance (MR) images have been performed on 32 patients with torn ACL and 40 control patients. The volume of the fat pad was measured digitally from MR image by using a 3d Reconstruction software, ellipsoidal approximation, and a MATLAB code. The results were compared between patients with torn ACL and control group. Patients with a torn ACL had a significantly larger fat pad than the controls (P=0.01). There was no significant difference between the methods used to measure the infrapatellar fat pad volume (P=0.83-0.87). Thus, lesions of the infrapatellar fat pad is often associated with ACL injury.

급성 외상성 비골근 탈구의 수술적 치료 (Surgical Treatment of Acute Traumatic Peroneal Tendon Dislocation)

  • Purpose: Acute traumatic peroneal tendon dislocation is relatively rare disease and their methods of treatment is controversial, that we want to assess the characters and outcomes of 8 patients with early surgical treatments. Materials and Methods: We evaluated the results of 8 patients who can follow up more than 28 months using sex, age, side, injury sports, concomitant injuries, Eckert and Davis classifications, anatomic variants, results and complications. Results: All of 8 patients was male, average age was 27, Right side was dominant (5/8), causal sports was variable. Concomitant injuries were distal tibiofibular ligament syndesmosis injury, Peroneus longus injury, lateral collateral ligament injury. On behalf of Eckert and Davis classifications 5 patients were Grade 1 and other 3 patients were Grade 2. 1 case of low lying peroneus brevis belly was found as an anatomic variants. 6 of patients shown excellent results, 2 patients were good. Post operative complications were discomfort of operation site and mild limited dorsiflexion on ankle joint. Conclusion: Careful history and physical exam is important for diagnosis. And surgical treatments can expect good results.

  • 저자들이 전방십자인대 파열과 함께 전방 불안정성을 보인 2례를 보존적으로 치료하여 좋은 임상적 결과를 얻은 바를 고려할 때, 급성기에 전방십자인대가 파열된 경우에도 전방십자인대를 싸고 있는 활액막이 파열되어 없어지지 않고 비교적 연속성이 잘 유지된 경우에는 보존적으로 치료함으로써 좋은 결과를 기대할 수 있을 것으로 사료되며 아울러 십자인대가 손상받았을 때 스스로 복원 능력이 있는 지에 대하여 새로운 연구가 필요하리라 사료된다.

  • Anterior cruciate ligament tear is a common disease of knee injury. We report 1 case of anterior cruciate ligament complete tear diagnosed by MRI(magnetic resonance imaging) imaging and 1 case after reconstruction. Outcomes were measured by visual analogue scale(VAS) and Lysholm knee scoring scale. We applied acupuncture treatment, bee venom injection, bongchuna and prescribed herbal medication. Patients who treated by oriental medical treatments had a significant effect on the pain decrease and range of motion and knee function. Oriental medical treatments are very useful on pain reduction and prevention of muscle contraction, leading to satisfied rehabilitation, as wee as diminishing recurrence after operation.

Palmar annular ligament desmitis in horses: retrospective study between desmotomy and desmectomy techniques

  • This study evaluated two different surgical techniques for the treatment of palmar annular ligament desmitis (PAL) in horses. The group 1 (G1) consisted of 11 Thoroughbred submitted to desmotomy, and G2 of 22 horses that underwent PAL desmectomy. There was no statistically significant difference between mean palmar annular ligament thickness in both groups evaluated (p = 0.800). In this study, the horses of G1 had 12 times more chance to return to physical activity, when compared to animals of G2 (p = 0.033; O.R. = 12.0; C.I.95% = 1,142 - 126,122). Desmectomy was more efficient in promoting the resolution of the injury and the return to sports activity.

  • Forestier's disease is a systemic rheumatological abnormality in which exuberant ossification occurs along ligaments throughout the body, but most notably the anterior longitudinal ligament of the spine. This disease is usually asymptomatic; however dysphagia, dyspnea, and peripheral nerve entrapment have all been documented in association with the disorder. We report a rare case of catastrophic neurologic damage caused by Forestier's disease accompanying ossification of the posterior longitudinal ligament.

  • In the present paper the current clinical knowledge about proprioception is given for the shoulder, knee, ankle, elbow and the radiocarpal joint. Proprioceptive capabilities are decreased after knee joint injury such as anterior cruciate ligament. Joint position sense is significantly improved by cruciate reconstruction. Thus, we review of the articles for the proprioception of the anterior cruciate ligament of the knee joint. The present information on proprioception will influence our clinical practice in the future. We should choose surgical procedures that not only reconstruct the anatomy, but also the neurophysiologic feed-back mechanism.

  • Lateral epicondylitis with rupture of the radial collateral ligament of the elbow has not been reported in the literature. We report on a case of a recreational golfer who had not received steroid injection and had no trauma history. The patient was treated with open surgical repair. At 2 years follow-up, satisfactory clinical and radiological outcomes were observed with return to pre-injury level. The authors report this case and review the literature.

This is a article about nonoperative management of foot and ankle injury. Therefore, this article includes various exercise technique, range of motion, stretching for muscle relaxation, proprioception training for rehabilitation. We recommend that orthopedic surgeon should discuss with patient and specialist for treatment plan after foot and ankle injury
