Influencing Factors on the Health Promotion Lifestyle Modification after Medical Examination (건강진단 후 수검자의 건강증진생활양식 변화의 영향요인분석)

  • Purpose: This study was to analyze changes of motivation and health-promoting lifestyle in 3 months after medical examination, and to identify the influencing factors on the change of health-promoting lifestyle. Methods: The subjects of this study were 81 adults who took medical examination at a general hospital health clinic in Cheonan city. The instruments used in this study were the motivation scale and modified HPLP. For data collection, the first survey was conducted from March 16 to April 19, 2001, and the second survey was carried out by mail three months after the medical examination from June 16 to July 19, 2001. Results: 1. The age of the subjects ranged from 26 to 66 years, 71.6% were male, and the major group was office workers(43.2%). 2. The average score of motivation scale was significantly improved in three months to 520.7(SD=82.7). All sub-scales of motivation scale, self-efficacy(t=-4.204, p=.000), perceived benefits(t=-4.263, p=.000), perceived barriers (t=4.305, p=.000), and emotional salience (t=-6.169, p=.000) showed significant improvements in 3 months. 3. The average score of health-promoting lifestyle was significantly increased to 62.5 (SD=9.8) (t=-5.111, p=.000) after 3 months. Health responsibility(t=-6.098, p=.000), eating habit(t=-3.625, p=.001), exercise(t=-2.557, p=.012), and smoking habit(t=-2.157, p=.034) showed significant improvement. But stress management was not changed at the significant level in 3 months(t=-1.832, p=.071). 4. As the result of multiple regression analysis, it was found that perceived barriers, self-efficacy and monthly mean income had a significant influence on health-promoting lifestyle in 3 months after the medical examination. These variables explained 42.4% of variance in health-promoting lifestyle in 3 months after the medical examination. Conclusion: Periodic medical examination and guidance for healthy lifestyle was effective to change the motivation and to improve health promoting lifestyle.

Development of Nutritional Counseling for Weight Reduction based on behavior modification through Internet (인터넷에서 행동 수정 이론을 적용한 체중 감량 상담 방법 개발)

  • The purpose of the study was to develop an internet nutritional counseling program using an expert system to assist obese people to lose weight through behavior modification. The internet counseling program for weight loss was developed by the accumulation of knowledge dealing with eating habits and exercising behaviors in expert system tool, Knowledge Engineering Agent (KEA) by a dietitian without any help of computer expert. To accumulate knowledge into KEA, survey was performed in 150 obese people, dietitians reviewed and consulted each survey case, and the consulted contents were learned and accumulated into KEA. Survey questionnaire was the same as that of the internet consulting program, and it included general characteristics, dietary habits, lifestyle, and exercise patterns related to obesity. Also, the dietitian selected proper factors inferred from the survey questionnaire of each case, and added the conclusions for them. Conclusions were made for helping clients to correct bad eating behaviors and accumulate good behaviors to lose weight. Counseling was divided into two parts; a two-week part and a daily part. Two-week counseling was performed based on 4 step questionnaires, and daily counseling was done for daily food consumption and physical activity. When clients answered survey questionnaires in a counseling internet program, the recommendations on how to eat, to exercise and to deal with stress in a real time for each case, was given. In conclusion, a counseling internet program for weight reduction can be used to give advices how to deal with obesity in a man-to-man way in a real time using KEA where nutritional knowledge based on behavior modification for weight loss was accumulated.

The Analysis of the Recent Research Trend of Traditional Korean Medicine Intervention with Woman Menopausal Symptoms (갱년기 여성 대상 한의약 중재요인의 국내 연구 동향분석)

  • Objectives : This study was conducted to survey on the research trends of Traditional Korean Medicine(TKM) intervention with woman menopausal symptoms in Korea. Method : We searched Korean research database by using keyword 'Menopause'. Korean research databases were Korean Studies Information Service System(KISS), Research Information Service System(RISS) and Korean Medicine Information System(OASIS). We classified articles identified by TKM treatments. Results : We analyzed 63 studies by classifying 4 interventions that consist of acupuncture treatment, herbal medicine, lifestyle modification, complex therapy. According to studies using acupuncture, Sameumgyo(三陰交, SP6) was the most treated. Soyosan(逍遙散) was most treated in herbal medicine studies, and massage was most used in lifestyle modification. Conclusion : A variety of treatment using TKM have been used for menopausal women. It is necessary to increase the level of evidence of TKM intervention through additional studies in the future.

A Survey of Korean Medical Doctors on the Items and Importance of Lifestyle Management for Infertile Couple (난임 부부의 섭생 지도 항목과 중요도에 대한 한의사 대상 설문조사)

  • Objectives: This study was conducted to reflect Korean Medicine doctors (KMD)' perception of lifestyle management in the process of developing a Korean Medicine (KM) Clinical Practice Guidelines (CPG) of female infertility. Methods: We sent four e-mails containing links to online questionnaires to 25,286 KMD belonging to the Association of Korean Medicine. The questionnaire consisted of sexual intercourse, weight, diet, exercise, drinking, smoking, caffeine intake, stress etc. KMD responded to self-administered questionnaires, and we analyzed 786 responds. Results: KMD put stress first among the infertile women's lifestyle management items, weight and sexual intercourse were next. Also KMD put stress first among the spouses' lifestyle management items, drinking and sexual intercourse were next. In relation to sexual intercourse, 'Recognizing the Fertile Window' was the most important. In relation to stress in the emotional domain, 'Depression' and 'Anxiety' were the most important, and in relation to stress in the relationship domain, 'Deteriorating of marital relationship' was the most important. Conclusions: We surveyed the status of KMD' awareness of infertility lifestyle management for patient education, and reflected it in the modification of the CPG for female infertility.

The Effect of Follow-Up Management Service on Health Promotion: for High Risk Population Classified in Health Screening of National Health Insurance Corporation (건강검진 사후관리 서비스의 건강증진 효과 - 건강주의자를 대상으로-)

  • Objectives: This study aims to examine if the follow-up management service by National Health Insurance (NHI) for person at health risk leads to significant modification of the lifestyle and change of health status. Methods: Of persons who underwent health screening and were classified as having health risks after periodic health screening by NHI in 2003, persons who took the follow-up management service were selected as case group and persons who took no service were selected as control group. The DW database of NHI was used to explore the effect of the follow-up management service on the modification of health status and lifestyle. Chi-square tests were conducted with SAS 9.1 to examine the differences of health promotion effect between case vs. control group. Results: It was shown that of lifestyle behaviors, only exercise was significantly improved for case group compared with control group as the effect of the follow-up management service by NHI (2.98%p) (p<.0001). Further, morbidity rate for control group was 2% higher than that of case group (p <.0001), which indicates that persons who received the follow-up management service better maintained their health significantly than persons who did not. Conclusions: The present study shows that the appropriate follow-up management services need to be provided for maximizing potential effect of periodic health screening by NHI.

Chi-square tests and t-tests, pairs t-tests were conducted with SAS 9.1 to examine the differences of health promotion effect between case vs. control group. Results: It was shows that of lifestyle behaviors, only exercise is significantly improved for case group compared with control group as the effect of the follow-up management service by NHI (2.11%p) (p=0.0435). Further, morbidity rate for control group was 2.05% higher than that of case group (p=0.0002). These results indicate that persons who received the follow-up management service better maintained their health significantly than persons who did not. Conclusion: The present study shows that the appropriate follow-up management services need to be provided for maximaizing potential effect of periodic health screening by NHI.

A Behavior-Modification Approaches to Improved Exercise Performance for Athletes Through the Multipe Nutritional Counseling (영양상담을 통한 식사행동의 변화가 운동선수의 운동능력 향상에 미치는 효과)

  • Management of nutrient intakes through behavior-mordification can be important for improving exercise performance in athletes. The purpose of this study was to determine the effectiveness of nutritional counseling for improving exercise performance capability in athletes. The subjects were seven golfers and fourteen ju-do athletes from Hoseo university, and they have nor received professional hel[ from dieticians. Prior to nutritional counseling, their dietary intakes, lifestyles and dietary habits were evaluated. Nutritional counseling sessions were conducted by a trained dietician every 2-3 weeks for 3-4 month. After 5 counseling sessions, nutrient intakes, lifestyle and dietary habits were re-evaluated. Exercise performance capability was measured by maximal exercise stress test using treadmills. Individual data were assessed as quality index represents the better nutritional status. The common dietary and lifestyle problems were overeating binging, overeating, snacking and drinking alcohol prior to the nutritional counseling. After counseling sessions, the quality index of dietary habits seemed to decrease, especially in ju-do athletes, representing nutritional problems were resolved. Body fat and body mass index decreased in ju-do athletes, but not in golfers. Both athletes have shown to increase oxygen uptake at anaerobic threshold and maximal oxygen uptake status, which represents that the athletic performance capacity was improved after counseling sessions. Their respiratory quotients were decreased. In conclusion, nutritional counseling sessions over five times are an effective and efficient approach to change dietary habits to improve exercise performance capacity. Furthermore, clients can have good dietary habits and learn how to manago muscle strength by behavior modification through multiple nutritional counseling sessions. The Effects of a Comprehensive Life Style Modification Program on Glycemic Control and Stress Response in Type 2 Diabetes (스트레스 중재를 포함한 포괄적인 생활습관개선 프로그램이 제2형 당뇨병환자의 당조절과 스트레스 반응에 미치는 효과)

  • Purpose: This study was to evaluate the effects of a comprehensive life style modification program on glycemic control and stress response in type 2 diabetes. Method: The participants(n=34) with type 2 diabetes were divided into either a usual care(control) or treatment(experimental) group. The experimental group(n=21) received a program that was based on a comprehensive life style modification protocol at a weekly meeting for 16 weeks. They also participated in individually prescribed exercise and diet along with stress management and self monitoring. The participants were followed for 6 months, during which postprandial glucose, HbA$_{1C]$, and stress response inventory were measured. Result: The experimental group showed a significant lower postprandial glucose and stress response compared to those of the control group. However, there was no significant change in the HbA$_{1C]$ value in either group. Conclusions: These results suggest that a type 2 diabetes comprehensive lifestyle modification program may lead to clinical improvement in glycemic control and reduce the stress response.

Clinical Perspectives on Obesity in Children and Adolescents (소아청소년 비만의 임상적 이해)

  • The prevalence of obesity in children and adolescents is increasing worldwide. Obesity in children and adolescents not only increases the risk of transitioning to obesity in adulthood but also increases the risk of cardiometabolic diseases such as high blood glucose, high blood pressure, dyslipidemia, fatty liver, and hyperinsulinemia during childhood. The goal of treating obesity in children and adolescents is not to focus on weight loss but to help children reach a healthy weight while maintaining normal growth appropriate for their age and sex. To achieve this goal, regular physical activity and exercise, dietary modification, improvement of obesity-prone environmental factors, and behavioral changes are required for a healthy lifestyle. If appropriate weight control is not achieved through lifestyle modifications, pharmacotherapy may be considered for adolescents with severe obesity aged 12 and above. Recent clinical trials have reported the efficacy and safety of pharmacotherapy in severely obese adolescents. Currently, two medications can be prescribed in Korea for patients with obesity aged 12 and above: Orlistat and Liraglutide. However, despite effective weight control through drug treatment, body weight may increase again after treatment discontinuation. Therefore, it is crucial to evaluate adherence to health behaviors during visits and continue to educate on lifestyle modifications, even during pharmacotherapy, to minimize weight regain.

Adaptive Responses to Electrophilic Stress and Reactive Sulfur Species as their Regulator Molecules

  • We are exposed to numerous xenobiotic electrophiles on a daily basis through the environment, lifestyle, and dietary habits. Although such reactive species have been associated with detrimental effects, recent accumulated evidence indicates that xenobiotic electrophiles appear to act as signaling molecules. In this review, we introduce our findings on 1) activation of various redox signaling pathways involved in cell proliferation, detoxification/excretion of electrophiles, quality control of cellular proteins, and cell survival during exposure to xenobiotic electrophiles at low concentrations through covalent modification of thiol groups in sensor proteins, and 2) negative regulation of reactive sulfur species (RSS) in the modulation of redox signaling and toxicity caused by xenobiotic electrophiles.
