A Clinical Study on 29 Cases of Vocal Cord Paralysis caused by Neoplasm (종양에 의한 성대마비 29예에 대한 임상적 고찰)

  • The authors observed the clinical status of 29 patients with vocal cord paralysis caused by tumor from April, 1983 to September, 1993 at Department of Otorhinolaryngology, Yongdong Severance hospital, Yonsei University College of Medicine. The results were as follows: 1) In the kinds of neoplasms, the most frequent were lung Ca. with 13 cases(44.8%), followed by 8 cases by thyroid Ca., 3 cases by neurogenic tumor, 2 cases by mediastinum tumor, cervical esophagus Ca., tracheal Ca., glomus jugulare were 1 case each. 2) In sex distribution, there were 18 cases of males and 11 cases of females with the male to female ratio being 1.8:1. In age distribution, most of the cases(10 cases ; 34.5%) were in the 7th decade. 3) In chief complaints, most of the cases(17 cases : 58.6%) had hoarseness only and aspiration, stridor, dyspnea, cough, dysphagia were present in some cases. 4) In site of the paralysed vocal cord, 21 cases were in the left cord. 5 cases in the right cord and 3 cases in the both cords. 5) In the position of paralysed vocal cord, most of the cases(23 cases : 79.3%) were in the parmedian position.

IgG4-Related Intracranial Hypertrophic Pachymeningitis : A Case Report and Review of the Literature

  • Hypertrophic pachymeningitis is an uncommon disorder that causes a localized or diffuse thickening of the dura mater. Recently, the possibility that IgG4-related sclerosing disease may underlie some cases of intracranial hypertrophic pachymeningitis has been suggested. We herein report the tenth case of IgG4-related intracranial hypertrophic pachymeningitis and review the previous literature. A 45-year-old male presented with left-sided focal seizures with generalization. Magnetic resonance imaging (MRI) revealed a diffuse thickening and enhancement of the right convexity dura matter and falx with focal nodularity. The surgically resected specimens exhibited the proliferation of fibroblast-like spindle cells and an infiltration of mononuclear cells, including predominantly plasma cells. The ratio of IgG4-positive plasma cells to the overall IgG-positive cells was 45% in the area containing the highest infiltration of plasma cells. On the basis of the above findings, IgG4-related sclerosing disease arising from the dura mater was suspected. IgG4-related sclerosing disease should be added to the pachymeningitis spectrum.

Clinical Evaluation of Spontaneous Pneumothorax - A review of 360 cases - (자연기흉의 임상적 고찰: 360례 보고)

  • We have observed 360 cases of spontaneous pneumothorax from January 1980 to May 1991 at the department of Thoracic and Cardiovascular Surgery, Chosun University Hospital. Males occupied 266 cases[73.9%] and females 94 cases[26.1%], and its ratio was 2.8: l. The age of patients ranged from neonate[5 days] to 84 years old. The site of pneumothorax was right in 50.3%, left in 43.3% and bilateral in 6.4%a. The clinical symptoms were frequently dyspnea, chest pain and coughing. The associated pulmonary lesions were shown pulmonary tuberculosis in 199 cases[55.3%], bullae in 54, pulmonary emphysema in 31, COPD in 17, pneumonia in 6, lung cancer in 5, paragonimiasis in 5, catamenial pneumothorax in 3 and unknown underlying pathology in 39 cases. The results of surgical management of spontaneous pneumothorax are followings: 288 out of 360 cases[80.0%] were cured by closed thoracotomy, 53 cases[14.8%] were cured by open thoracotomy. Open thoracotomy was the most effective procedure in persistent air leakage, recurrent pneumothorax, visible bleb or bullae on the chest X-ray, associated lesion, bilateral simultaneous pneumothorax, parenchymal incomplete lung expansion and bleeding after closed thoracotomy. The incidence of complication was developed in 10. ado and recurrent rate was seen in 10.6%. There was no operative death.

  • Objectives : The limited number of studies to date have mainly focused on the effect of obesity on the characteristics of plantar foot pressures. This study is designed to find the correlation between plantar pressure and obese index. Methods : This study assessed the body composition of 30 obese women using bioelectrical impidence analysis and Gaitview AFA-50. The static and dynamic plantar pressure was determined from electronic footprints captured using a capacitive pressure distribution platform during standing and walking. The data were analysed by independent t-test and Pearson Correlation. Results : Positive correlations were noted between body weight, body mass index(BMI), waist circumference(WC), waist-hip ratio(WHR) and difference of fore and rear plantar pressure. And negative correlations were noted between body weight, BMI, WC, WHR and difference of left and right plantar pressure. Conclusions : The findings of this pilot study suggest that body composition influences the waight distribution in overweight and obese subjects.

Clinical Evaluation of Spontaneous Pneumothorax -A Review of 244 Cases- (자연 기흉의 임상적 고찰 -244례-)

  • From March, 1985, to June, 1993, 244 patients with 345 episodes of spontaneous pneumothorax treated at Koryo General Hospital were reviewed. Most of the patients were male, and the ratio of male to female was 8:1. The average age of the patients with spontaneous pneumothorax was 32.8 years old. The site of pneumothorax was revealed left side in 53.3%, right side in 42.6%, and bilateral in 4.1%. The cause of pneumothorax were shown primary spontaneous pneumothorax in 73.4%, and secondary spontaneous pneumothorax in 26.6%. The underlying pathologic lesion in secondary spontaneous pneumothorax showed pulmonary tuberculosis in 56patients[86.1%], COPD in 4patients[6.2%], bronchial asthma in 2patients[3.1%], lung cancer in 2patients[3.1%], and pneumoconiosis in a patient[1.5%]. The usual clinical symptomes were dyspnea, chest pain and chest discomfort. Recurrence rate was as follow; 2nd episode 33.6%, 3rd episode in 26.8%, and above in 4th episode in 18.2%. All the patient of pneumothorax was treated as following; Closed thoracostomy tube drainage in 127patients, bullectomy in 88patients, lobectomy in 5patients, wedge resection in 2patients, conservative treatment with oxygen therapy in 21patients, and video assisted thoracoscopic bullectomy in a patient. The course of treatment of all of the patients were smooth and uneventful.

A novel classification of anterior alveolar arch forms and alveolar bone thickness: A cone-beam computed tomography study

  • Purpose: This study classified alveolar arch forms and evaluated differences in alveolar bone thickness among arch forms in the anterior esthetic region using cone-beam computed tomography (CBCT) images. Materials and Methods: Axial views of 113 CBCT images were assessed at the level of 3 mm below the cementoenamel junction (CEJ) of the right and left canines. The root center points of teeth in the anterior esthetic region were used as reference points. Arch forms were classified according to their transverse dimensions and the intercanine width-to-depth ratio. The buccolingual alveolar bone thickness of each tooth was measured at 3 mm below the CEJ and at the mid-root level. Differences in the mean thicknesses among arch forms were analyzed. Results: Anterior maxillary arches could be classified as long narrow, short medium, long medium, and long wide arches. Significant differences in buccolingual alveolar bone thickness among the arch groups were found at both levels. The long wide arches presented the greatest bone thickness, followed by the long medium arches, while the long narrow and short medium arches were the thinnest. Conclusion: Arch forms were classified as long narrow, short medium, long medium, and long wide. The buccolingual alveolar bone thickness exhibited significant differences among the arch forms.

SEVEN-YEAR RETROSPECTIVE STUDY OF FACIAL FRACTURE (안면골 골절의 발생 양상에 관한 7년간의 추적조사연구)

  • This report constitutes a 7-year study of facial fractures based on a series of 616 patients who were treated for facial fractures as in-patients at Chosun University Dental Hospital between 1998 and April 2005. The following results were obtained : 1. The ratio of men to women was 5 : 1. 2. The major etiological factors were falls (36.2%), traffic accidents (23.4%), punches (18.1%), sports (8.3%), and accidents related to work (2.8%). 3. They were most frequent in the second (28.8%), first (26.6%), and third (12.3) decades, in that order. 4. The sites of frequent mandible fracture are the symphysis (32%), left mandibular angle (25.3%), and right mandibular angle (10.9%). 5. The most frequent maxillary fractures were zygomatic fractures (46.4%), Le Fort I fractures (28.8%), and Le Fort II (12.0%) fractures.

Combined training improves body composition, balance, and muscle function in sarcopenia elderly

  • Purpose: Sarcopenia is defined as a decrease in muscle mass, strength, and function with age that affects overall body function. We aimed to investigate the effect of combined training on body composition, balance, and muscle function in sarcopenia elderly. Research design, data, and methodology: Twenty-eight sarcopenia elderly (age 74.9±4.5 years) were randomly assigned to an exercise, EG (n=14), or a control, CG (n=14), group. The EG performed an intervention consisting of combined exercise training (60-75 min) for a total of 12 weeks, three times a week. The CG maintained their usual daily lifestyle during the intervention period. We measured body weight, body mass index (BMI), % body fat, free fat mass, balance ability, peak torque in shoulder, knee, and lumbar joints normalized for bodyweight in one second. Results: The EG showed improved body composition (i.e., BMI, fat-free body mass, fat mass; all p < 0.031, η2 > 0.179), balance (i.e., right and left of static and dynamic balance and fast 10 m walk; all p < 0.049, η2 > 0.152), and muscular function (i.e., 90°/sec and 180°/sec peak power per kg bodyweight, 90°/sec average power per kg bodyweight, 180°/sec total work, and 180°/sec endurance ratio; all p < 0.045, η2 > 0.158). Conclusions: Combined exercise training improves muscle mass and strength, body composition, balance, and muscle function in sarcopenia elderly.

Z-DNA-Containing Long Terminal Repeats of Human Endogenous Retrovirus Families Provide Alternative Promoters for Human Functional Genes

  • Transposable elements (TEs) account for approximately 45% of the human genome. TEs have proliferated randomly and integrated into functional genes during hominoid radiation. They appear as right-handed B-DNA double helices and slightly elongated left-handed Z-DNAs. Human endogenous retrovirus (HERV) families are widely distributed in human chromosomes at a ratio of 8%. They contain a 5'-long terminal repeat (LTR)-gag-pol-env-3'-LTR structure. LTRs contain the U3 enhancer and promoter region, transcribed R region, and U5 region. LTRs can influence host gene expression by acting as regulatory elements. In this review, we describe the alternative promoters derived from LTR elements that overlap Z-DNA by comparing Z-hunt and DeepZ data for human functional genes. We also present evidence showing the regulatory activity of LTR elements containing Z-DNA in GSDML. Taken together, the regulatory activity of LTR elements with Z-DNA allows us to understand gene function in relation to various human diseases.

A New Method of Liver Size Estimation on Hepatic Scintigram -Hepato-abdominal Ratio (간신티그램상 간크기의 새로운 평가방법 -간.복부횡경비-)

  • Estimation of liver size is essential in the diagnosis of liver disease, Many approaches have been attempted in evaluation of liver size such as the measurement of length, area and volume. Among these, area and volume measurements are accurate but complicated, so we commonly use formerly introduced various linear measurements, but in scintigraphy one must calculate the actual liver size using rate of reduction, which is time consuming. Because of these reasons, we carried out present study to represent liver size by means of a simple liver measurement like we express the cardiac size by cardiothoracic ratio. Our cases consisted of 100 clinically normal subjects as the normal group and 50 patients suffering from liver disease and diagnosed to have hepatomegaly on abdominal palpation and scintigram at Dept, of Radiology of St. Mary's Hospital, Catholic Medical College during the period of 8 months from Jan. 1980. We measured the liver size using 4 linear diameters(Fig. 1). And as the reference measurement, the distance from the right margin of the liver to the left margin of the spleen was measured. We called this "abdominal transverse diameter(ATD)". The results were as follows; 1) The smallest value was recorded in the midline vertical diameter (MVD). It was $4.2{\pm}0.4cm$ in normal group and $5.0{\pm}0.6cm$ in the hepatomegaly group. 2) The diameter using other methods ranged from 5.6 to 7.2 cm in the normal group and from 6.3 to 7.5cm in the hepatomegaly group. 3) There was significant difference in the ratio of each diameter to ATD between the normal and hepatomegaly group (<0.01). We called this "hepato-abdominal ratio". 4) The "hepato-abdominal ratio" using MVD is $0.43{\pm}0.06$ in the normal group and $0.53{\pm}0.07$ in the hepatomegaly group. The "hepato-abdominal ratio" of MVD was most significantly different between normal and hepatomegaly group. 5) The tolerance limits(99%) of "hepato-abdominal ratio" using MVD is from 0.41 to 0.45 in the normal group and from 0.51 to 0.55 in the hepatomegaly group. Therefore, by reasons of error during measurement and convenience of memory, it was warranted to suggest hepatomegaly when "hepato-abdominal ratio" using MVD is more than 0.5 in the interpretation of hepatic scintigram.
