The left ventricle wall motion simulation during systolic and diastolic stages of the heart (심장의 수축 및 이완기에서의 좌심실 벽 움직임 시뮬레이션)

  • 심장의 수축 및 이완기에서의 좌심실 벽 움직임은 허혈 및 심근경색증과 같은 심장질환에서 영상적 진단의 주요한 특징이다. 심장은 동적 기관으로써 진단을 위해서는 속도와 같은 4차원 파라미터의 추정이 필요하다. 본 논문에서는 심장의 좌심실 형태 및 움직임을 모델링하여 동적으로 가시화하는 방법을 제시한다. 본 논문에서는 좌심실을 Dynamic Gaussian Blob 모델로 근사화하였다. 이 모델은 가우시안 함수 기반 FEM 요소와 superellipsoid를 통합한 것으로 좌심실의 형태 및 벽의 움직임을 물리기반 방법에 의해 묘사할 수 있다. 즉, 일련의 영상들로부터 좌심실 벽에 대응되는 3차원 점들을 추출한 후 이 점들에 작용되는 힘에 의해 박동하는 좌심실의 움직임을 추적한다. 이와 같은 좌심실 벽 움직임 시뮬레이션은 심장 움직임에 이상이 있는 질환의 진단을 위한 빠르고 간편한 보조 도구로써 사용되어질 수 있다.

Potts Shunt in Patients with Primary Pulmonary Hypertension

  • Idiopathic pulmonary arterial hypertension eventually leads to right-sided heart failure and sudden death. Its mortality rate in children is still high, despite improvements in pharmacological therapy, and therefore novel treatments are necessary. The Potts shunt, which creates an anastomosis between the left pulmonary artery and the descending aorta, has been proposed as a theoretically promising palliative surgical technique to decompress the right ventricle. We report the case of a 12-year-old girl with suprasystemic idiopathic pulmonary hypertension and right ventricular failure who underwent a Potts shunt for palliation with good short-term results.

Bilateral Atrioventricular Valve Dysplasia in a Middle Aged Turkish Angora Cat: A Case Report

  • A castrated, 6-year-old, male Turkish Angora cat with a history of respiratory distress was referred to the hospital. Physical examination revealed a cardiac murmur, and thoracic radiographic findings revealed pleural effusion and cardiomegaly. Echocardiography showed abnormality of the tricuspid and mitral valve, and color-flow Doppler imaging revealed regurgitation between both atrium and ventricle. Based on the echocardiographic examination, tricuspid valve dysplasia concurrent with mitral valve dysplasia was diagnosed. However, the patient died a week after treatment. In necropsy, bilateral atrioventricular valve dysplasia and left ventricular hypertrophy were confirmed. This is the first report to describe a middle age Turkish angora cat having bilateral atrioventricular valve dysplasia which has high mortality and only been reported rarely in cats. This case report also describes its clinical signs, diagnostic imaging findings, treatment and discussions how the patient could live long.

Repair of Postinfarction Ventricular Septal Defect (심근경색 후 발생한 심실중격결손의 수술)

  • Background: Ventricular septal defect(VSD) is rare but feared complication after acute myocardial infarction. The patient could survive by surgery only, but the surgical mortality is still high. We investigate the surgical result and predictors for early surgical mortality. Material and Method: Between August 1993 and February 2006, 8 patients (male, 2: female, 6) with postinfarct VSD underwent surgical repair. Seven patients had one-vessel disease of left anterior descending artery, and 6 of them had wide extension of infarction to posterior septal wall as well as anterior septal wall of both ventricles. One patient had concomitant coronary bypass grafting to a coronary lesion unrelated to the infarction. Two patients had concomitant tricuspid annuloplasty and 1 patient mitral valvuloplasty. Result: Surgical mortality was 37.5%(3 patients). They all had only one-vessel disease of left anterior descending artery, which made extended posterior septal wall infarction as well as anterior septal wall infarction of both ventricles. In preoperative M-mode echocardiographic study of left ventricle, they had lower ejection fraction than survivors($34.9{\pm}4.2\;vs.\;54.8{\pm}12.3$; p=0.036). Conclusion: Most of patients with postinfarction VSD had one-vessel disease of left anterior descending artery. Early surgical mortality occurred in the patients with poor ejection fraction of left ventricle and the wide anterior septal wall infarction extending to the posterior septum.

Successful Left-Heart Decompression during Extracorporeal Membrane Oxygenation in an Adult Patient by Percutaneous Transaortic Catheter Venting

  • Venoarterial extracorporeal membrane oxygenation (VA ECMO) is widely used in patients with cardiogenic shock. Insufficient decompression of the left ventricle (LV) is considered a major factor preventing adequate LV recovery. A 40-year-old male was diagnosed with acute myocardial infarction, and revascularization was performed using percutaneous stenting. However, cardiogenic shock occurred, and VA ECMO was initiated. Severe LV failure developed, and percutaneous transaortic catheter venting (TACV) was incorporated into the venous circuit of VA ECMO under transthoracic echocardiography guidance. The patient was successfully weaned from VA ECMO. Percutaneous TACV is an effective, relatively noninvasive, and rapid method of LV decompression in patients undergoing VA ECMO.

Medical Imaging of Ventricular Septal Defect in Two Dogs (개에서 심실중격결손의 영상의학적 진단 2례)

  • Ventricular septal defects (VSDs) is an opening in the ventricular septum that allows blood to flow between the ventricles. Most ventricular septal defects are located in the upper ventricular septum and can be identified by auscultation. In this report, 2 dogs with heart murmur were diagnosed as VSD using radiography and echocardiography. In radiographs, bulging sign of the main pulmonary artery or the enlargement of the left ventricle was observed. The color Doppler examination showed the left-to-right shunting of blood via interventricular septal defect.

  • Cardiogenic syncope occurs due to arrhythmia (bradycardia and tachycardia) or decreased cardiac output, and if proper treatment is not provided, it can lead to acute sudden death. A detailed medical history and physical examinations are required to determine the cause of syncope, and clinical approaches, including 12-lead ECG, are important. The 12-lead ECG does not have a chest lead in the posterior wall of the left ventricle; therefore, ECG of the isolated posterior wall myocardial infarction caused by left circumflex artery occlusion is not observed with ST elevation. Therefore, the significantly higher appearance of ST depression and R waves than S waves from V1 to V3 of the chest lead must be interpreted meaningfully. Isolated posterior wall myocardial infarction is small in the area of myocardial necrosis, and tension is increased in the necrotic area due to the contraction of the normal myocardial muscle, which can cause ventricular wall rupture. Therefore, it is necessary to additionally check Beck's triad, such as jugular venous distension and decreased heart sound, in patients with low blood pressure with an isolated posterior wall myocardial infarction on 12-lead ECG in patients with syncope.

PDGFC, MARK3 and BCL2 Polymorphisms are Associated with Left Ventricular Hypertrophy in Korean Population

  • Left ventricular hypertrophy (LVH) refers to the expansion and the enlarged myocardium due to the increased resistance to ejection from the left ventricle to the aorta and/or the periphery, or the long-term burden imposed by the blood increase. Hypertension is a major risk factor that accounts for more than 50% of the causes of cardiovascular disease. If hypertension endure in the long term, the myocardium responds to abnormal heartbeat in the heart. Therefore, the prevalence of left ventricular hypertrophy also increases. As a result of genome-wide association study (GWAS) analysis for European people, PDGFC, MARK3, and BCL2 were related to blood pressures. In this study, the genetic polymorphisms of PDGFC, MARK3, and BCL2 were extracted and selected based on Korean genomic and epidemiologic data, and then logistic regression analysis was performed on LVH. As a result, one SNP (rs9307953) in PDGFC gene, four SNPs (rs6575983, rs17679475, rs2273703 and rs10141388) in MARK3 gene and two SNPs (rs17756073 and rs17070739) in BCL2 gene were statistically significant. The rs6575983 of the MARK3 gene showed the highest significance level ($P=7.2{\times}10^{-3}$) among the SNPs and the relative risk of 1.08 (95% confidence interval: 1.06 to 1.45). These results suggest that the polymorphisms of PDGFC, MARK3, and BCL2 not only affect European blood pressures but also correlate with LVH in Korean. These results suggest that increased understanding of the genetic correlations of the pathogenesis of LVH.

Early Results of Extracardiac Fontan Operation (심장외폰탄수술의 조기성적)

  • Between August 1996 and August 1997, 22 patients underwent extracardiac Fontan operations. The basic diagnoses included univentricular heart of the right ventricular type (n=12); univentricular heart of the left ventricular type (n=4); tricuspid atresia (n=4); left isomerism, transposition of great arteries, ventricular septal defect and pulmonary stenosis (n=1); and criss-cross heart with uneven ventricle (n=1). The median age of the 14 men and 8 women was 29 months (range from 21 months to 26 years). Previous procedures included bidirectional cavopulmonary shunt (n=15, interval=15.6$\pm$3.4 months), Kawashima operation (n=4, interval=37.5$\pm$20 months), and classic Glenn shunt (n=1, interval=14 years). In 2 patients, extracardiac Fontan operations were done without any previous procedures. A 16- to 22-mm flexible Gore-Tex tube graft (n=18), Hemashield graft (n=3), or, alternatively, a nonvalved aortic allograft (n=1) was cut and anastomosed end-to-end between inferior vena cava and undersurface of pulmonary artery using Gore-Tex or Prolene suture in a running fashion. In risk Fontan patients (n=12), a communication between the extracardiac conduit and the right atrium was constructed. In the most 13 recent patients, the procedures were done without cross-clamping of the aorta and with a beating heart. Operative mortality was 9.1% (n=2). Complications included persistent chest tube drainage for more than 7 days (n=5), chorea (n=2), and low cardiac output (n=1). There were no late deaths. Follow-up echocardiogram (mean: 6 months) demonstrated satisfactory hemodynamic results in the surviving 20 patients. Potential advantages of this technique consist of minimization of surgical manipulation of atrial tissue, reduction or elimination of myocardial ischemia, creationof a uniform and stable inferior vena cava-to-pulmonary artery conduit, and increased flexibility and safety in certain high-risk patients such as those with increased pulmonary vascular resistance, pulmonary hypertension, and impaired ventricular function. Further investigations during a longer follow-up are needed to confirm the intermediate and long-term results, especially the reduction of late atrial arrhythmias.

The effect of Aconiti tuber butanol fraction on the rabbit heart microsomal $Na^+-K^+$-activated ATPase activity (부자 Butanol Fraction이 가토 심장근 Microsomal $Na^+-K^+$-activated ATPase 활성도에 미치는 영향)

  • Aconiti tuber butanol fraction shows positive inotropic effect on the isolated atrium of rabbit heart. To investigate the mechanism, the effect on microsomal ATPase activity of rabbit heart is observed. The microsomal fraction which contains the $Na^+$- and $K^+$-activated ATPase in the presence of $Mg^{++}$ is isolated from the left ventricle of rabbit heart. The microsomal ATPase activity is maximally stimulated at $Na^+$ and $K^+$ concentration of 100 mM and 10 mM respectively. Microsomal $Na^+-K^+$-activated ATPase is inhibited by ouabain and Aconiti tuber butanol fraction. Ouabain and Aconiti tuber butanol fraction depress $Na^+$-stimulation on microsomal ATPase activity, and the inhibitory effects are not completely reversed at $Na^+$ concentration of 300 mM. Also, $K^+$-stimulation on microsomal ATPase activity is inhibited by ouabin and Aconiti tuber butanol fraction and the inhibitions are not compeletely reversed at $K^+$ concentration of 30 mM. It is, therefore, suggested that the inhibitory effect of Aconiti tuber butanol fraction on the microsomal ATPase activity may contribute to leading to the positive inotropic effect.
