Hospice and Palliative Care for the Terminal Patients with Colorectal Cancer (말기 대장직장암 환자의 호스피스 완화의료)

  • Purpose: Colorectal ranter is the 4th leading cause of cancer death in Korea and the prevalence is increasing continuously. This study was aimed to figure out the problems through the clinical consideration about terminal colorectal ranter patients who had died in hospice unit. Methods: We retrospectively reviewed the medical records in 78 patients with colorectal ranter who had admitted, received palliative care, and died in a hospice unit between April 2003 and November 2006. Results: The median age of patients was 59.6 years with 45 men (58%) and 24 women (42%). The median survival in hospice and palliative care was 36 days. The median hospitalization was 22 days. The most prevalent reason for admission was pain (38 patients, 49%), and the most common symptom was also pain (70 patients, 90%). Forty eight patients (62%) took analgesics before hospice referral. Twenty seven patients (65%) of 45 patients with intestinal obstruction have been performed palliative procedures. Median survival of patients with palliative procedure was higher than that of no palliative procedure group (47 days vs 19 days, P-value=0.005). Conclusion: The duration of hospice and palliative care was not enough to care the terminal colorectal cancer. Therefore, we suggest that proper education and information should be provided to physician, patients and their family members for effective hospice and palliative care.

A Study on Estimating Construction Equipment Annual Standard Operating Hours (건설기계 연간표준가동시간 산정에 관한 연구)

  • As use of construction equipment has been increasing continuingly, the proportion of equipment expense to the total construction cost has become higher. However, there is a difference between the equipment expenses section in 'Poom-Sam' and practical data, because 'Poom-sam' does not consider non-working days due to weather conditions, legal holidays and management conditions. Therefore, 'Poom-Sam' does not present a reasonable standard for estimating construction equipment expenses. In this study, to estimate realistic construction equipment operating hours, firstly, construction equipment was classified according to work, and weather conditions, in which each work could not be executed, were established. Then, weather data on Seoul and Busan($2004{\sim}2006$) and legal holidays were analyzed to suggest annual standard operating hours. The annual standard operating hours of earthmoving & excavating, compaction, and drilling equipment was estimated to be 1,430 hours, and lifting equipment, concrete paving equipment, asphalt paving equipment, concrete equipment, and crushing & conveying equipment were estimated to be 2,124 hours, 1,156hours, 1,188hours, 1,688hours, and 2,152hours respectively.

Tansport Rate of Chromium ion from Waste Water through the Liquid Surfactant Membrane Containing Carrier (운반체 함침 유화액막에 의한 폐수중 크롬(VI)의 이동속도)

  • The transport of chromium(VI) ion from waste water through the liquid surfactant membrane containing tri-n-octylamine as a carrier, was analyzed by a slab model and was investigated through experiments. For the experiment of membrane stability, concentrations of surfactant and liquid paraffin oil were analyzed. Extraction experiments were carried out to observe the effect of system variables, such as concentrations of carrier, and initial chromium(VI) ion in external aqueous phase at $25^{\circ}C$. It is concluded that the most stable formation of liquid membrane emulsion was obtained when surfactant concentration is above 3 wt.% and liquid paraffin oil concentration is 50 vol.%. The theoretical equation on the transport of chromium(VI) ion agreed well with the experimental results.

A Clinical Report on Ascites Due to Liver Cirrhosis and Metastatic Liver cancer (간경변과 전이성 간암으로 인한 복수 환자 1례)

  • Ascites is the retention of body fluids in abdominal cavity, which is induced by salt increase, and disorders in such things as water retention and albumin synthesis. Ascites are especially common in cases of progressed liver cirrhosis and metastatic cancer. One patient was diagnosed with liver cirrhosis and Metastatic cancer at Rt. liver from Lt. ovarian tumor by abdominal CT. To measure the response to treatment of ascites, these clinical symptoms were observed: weight, abdominal circumference, and lab findings. Clinical symptoms and ascites were improved after oriental medical treatment. Abdominal circumference decreased. Results suggest oriental medical treatment was effective for this patient for this disease, but more extensive research is needed.
