A Case of Pneumonia with Septic Shock Due to Nocardia farcinia in Liver Transplant Patient (간이식 환자에서 발생한 Nocardia farcinia 폐렴 및 패혈성 쇼크 1예)

  • Nocardia farcinia, an aerobic, gram-positive bacilli actinomycetes of the genus Nocardia, is an uncommon pathogen found in humans. The most common Nocardia infection sites are the lung, central nervous system, and skin. Even though hematogenous dissemination can occur, isolation of the organism from blood cultures is very rare. We report a case of Nocardia infection that was isolated on blood cultures. A 59-year-old male with a medical history that includes a liver transplantation 6-years prior due to hepatocellular carcinoma secondary to chronic hepatitis B, developed pneumonia and was transferred to Severance Hospital. At the time of admission, the patient's initial exam showed hypothermia, tachypnea, and hypotension. His chest radiograph showed severe pneumonia and a large abscess on left upper lobe. Under the presumptive diagnosis of bacterial pneumonia or other opportunistic infection, we started broad spectrum antibiotics. However, he developed Nocardia sepsis, rapidly deteriorated, and subsequently died.

Dosimetric Characteristics of Edge $Detector^{TM}$ in Small Beam Dosimetry (소조사면 선량 계측을 위한 엣지검출기의 특성 분석)

  • In this study, we evaluated an edge detector for small-beam dosimetry. We measured the dose linearity, dose rate dependence, output factor, beam profiles, and percentage depth dose using an edge detector (Model 1118 Edge) for 6-MV photon beams at different field sizes and depths. The obtained values were compared with those obtained using a standard volume ionization chamber (CC13) and photon diode detector (PFD). The dose linearity results for the three detectors showed good agreement within 1%. The edge detector had the best linearity of ${\pm}0.08%$. The edge detector and PFD showed little dose rate dependency throughout the range of 100~600 MU/min, while CC13 showed a significant discrepancy of approximately -5% at 100 MU/min. The output factors of the three detectors showed good agreement within 1% for the tested field sizes. However, the output factor of CC13 compared to the other two detectors had a maximum difference of 21% for small field sizes (${\sim}4{\times}4\;cm^2$). When analyzing the 20~80% penumbra, the penumbra measured using CC13 was approximately two times wider than that using the edge detector for all field sizes. The width measured using PFD was approximately 30% wider for all field sizes. Compared to the edge detector, the 10~90% penumbras measured using the CC13 and PFD were approximately 55% and 19% wider, respectively. The full width at half maximum (FWHM) of the edge detector was close to the real field size, while the other two detectors measured values that were 8~10% greater for all field sizes. Percentage depth doses measured by the three detectors corresponded to each other for small beams. Based on the results, we consider the edge detector as an appropriate small-beam detector, while CC13 and PFD can lead to some errors when used for small beam fields under $4{\times}4\;cm^2$.

An Analytical Solution of Flow and Progressive Wave-Induced Residual Pore Water Pressure in Seabed (흐름과 진행파에 의한 해저지반 내 잔류간극수압의 해석해)

  • This study extended the Lee et al.'s (2015a) solution which improved the existing analytical solution for prediction of the residual pore water pressure into progressive wave and flow coexisting field. At this time, the variation of incident wave period and wave length should be incorporated to Lee et al.'s (2015a) analytical solution, which does not consider flow. For the case of infinite thickness, the new analytical solution using Fourier series was compared to the analytical solution using Laplace transformation proposed by Jeng and Seymour (2007). It was verified that the new solution was identical to the Jeng and Seymour's solution. After verification of the new analytical solution, the residual pore water pressure head was examined closely under various given values of flow velocity's magnitude, direction, incident wave's period and seabed thickness. In each proposed analytical solution, asymptotic approach to shallow depth with the changes in the soil thickness within finite soil thickness was found possible, but not to infinite depth. It is also identified that there exists a discrepancy case between the results obtained from the finite and the infinite seabed thicknesses even on the same soil depth.

Comparison of Chlorine, Chlorine Dioxide and Ozone as Disinfectants in Drinking Water (정수소독공정에 이용되는 염소, 이산화염소, 오존 소독제의 비교, 고찰에 관한 연구)

  • The experiments for the characterization of inactivation were performed in a series of batch processes with the total coliform as a general indicator organism based on chlorine, chlorine dioxide and ozone as disinfectants. The water sam-ples were taken from the outlet of settling basin in a conventional surface water treatment system that is provided with the raw water drawn from the mid-stream of the Han River. The inactivation of total coliform was experimentally ana-lyzed for the dose of disinfectant contact time, pH, Temperature and DOC. The nearly 2.4,3.0,3.9 log inactivation of total coliform killed by injecting 1 mか1 at 5 minutes for chlorine, chlorine dioxide and ozone. For the inactivation of 99.9%(3 log), Disinfectants required were 1.70, 1.00 and 0.60 mか1 for chlorine, chlorine dioxide and ozone, respec-tively. The higher the pH is, the poorer the disinfections effects are in the range of pH 6-9 by using chlorine and ozone. But the irfluence of pH value on killing effects of chlorine dioxide is weak. The parameters estimated by the models of Chick-Watson, Hom, and Selleck from our experimental data obtained for chlorine are: log(N/ $N_{0}$ )=-0.16 CT with n= 1, log(N/ $N_{0}$ )=-0.71 $C^{0.87}$T with n$\neq$1, for Chicks-Watson model, log (N/ $N_{0}$ )= -1.87 $C^{0.47}$ $T^{0.36}$ for Hom model. For chlorine dioxide are: log(N/ $N_{0}$ )= -1.53 CT with n = 1, log(N/ $N_{0}$ )= -2.29 $C^{0.94}$T with n$\neq$1,, for Chicks-Watson model, log(N/ $N_{0}$ )= -3.64 $C^{0.43}$ $T^{0.24}$ for Hom model and for ozone are: log(N/ $N_{0}$ )= -2.59 CT with n = 1, log(N/ $N_{0}$ )= -2.28 $C^{0.36}$T with n$\neq$1, for Chicks-Watson model, log(N/ $N_{0}$ )= -4.53 $C^{0.26}$ $T^{0.19}$ for Hom model.19/ for Hom model.

  • We reviewed 30 cases of superior vena cava syndrome in adult patients who were seen at the Yeungnam University Hospital from January 1985 to June 1990. The results were as follows : 1. The male-to-female ratio was 6.5:1, and the most patients were in the age group between the sixth and seventh decades. 2. The most common symptoms were dyspnea (87%) and followed by cough (63%), facial swelling (63%) and chest pain (44%) and the physical signs were dilated neck vein (97%), facial edema (93%) and facial flushing (45%) in order of frequency. 3. The simple chest x-ray findings were superior mediastinal widening (90%), right hilar mass (77%) and pleural effusion (31%). 4. Diagnosis was made by history and physical examination (100%), chest C-T scan (100%), simple chest x-ray (97%), bronchoscopy with biopsy (40%) and so on. 5. 21 cases of patients were confirmed by histology : 14 cases (46%) of bronchogenic ca. 4 cases (14%) of lymphoma, 3 cases (10%) of metastic lung ca. Of bronchogenic ca. small cell ca was 7 cases (23%), squamous cell ca, 5 cases (17%), and unclassified ca was 2 cases (6%). 6. In response of treatment, the clinical improvement was achieved in 18 cases with radiotherapy alone. 1 case with chemotherapy only, and 6 cases with radio-chemotherapy.

A Research on the Calligraphic Critique of Seongjeok Jeong-Jik Lee - Based on 'Wongyo-Jinjeok' of Wongyo Gwang-Sa Lee (석정 이정직의 서예비평 연구 - 원교 이광사의 『원교진적』을 중심으로 -)

  • This thesis is an introduction and critique of the recently released 'Wongyo-Jinjeok(원교진적)'. 'Wongyo-Jinjeok' is the critique of Seokjeong Jeong-Jik Lee (석정 이정직, 1841-1910), a practical scientist and writer during the last years of the Korean Empire, on the calligraphy of Wongyo(원교) Gwang-Sa Lee (이광사, 1705-1777). Even though whether or not Seokjeong follows the flow of Donggukjinche(동국진체) is to be determined by the specialists in this field, this thesis is based on the view that Seokjeong was influenced by Donggukjinche. The academic value of 'Wongyo-Jinjeok' is Seokjeong's preface and epilogue which critiques Wongyo's writing. 'Wongyo-Jinjeok'is a collection of calligraphic specimens from the 18 pieces of Chinese poetry Wongyo had written before and after June 1756 which was the year after he was banished to Booryung. Seokjeong critiqued the writing of Wongyo from the perspective of calligraphic history in the preface and epilogue of 'Wongyo-Jinjeok'. Seokjeong had been positive about Wongyo's taking after the pre-Wangheejee calligraphic style. But at the same time, Seokjeong thought that Wongyo's ability to create was limited by the public morals of that time. Such thought of Seokjeong can be interpreted as an evaluation of Wongyo's calligraphy as having been externally stern but failing to transcend the realm of mastery to the realm of creation.

Molecular Weight and Structural Changes of Enzymatic Hydrolysate from Bombyx mori Fibroin (효소처리에 의한 Bombyx mori fibroin 가수분해물의 분자량 및 구조 변화)

  • This study examined the enzymatic digestion, average molecular weight distribution and structural changes of Bombyx mori silk gland fibroin using gel penneation chromatography and nuclear magnetic resonance methods. The pure-separation of calcium chloride-treated fibroin hydrolysates was carried out by gel filtration chromatography. Also, the effects of fibroin's enzymatic hydrolysis were investigated using an edible enzyme. The average molecular weight of three hydrolysate samples (silk gland fibroin (SF), SF-calcium chloride (SFC), and SFC-enzyme) were measured to compare their characteristics. The molecular weights of SF, SFC, and SFC-enzyme were approximately 108,000, 65,000, and 1,000 Da, respectively. Finally, we determined the structural characteristic changes of the different enzymatically digested samples by $^{13}C$ nuclear magnetic resonance methods. For the enzymatically digested fibroin, the glycine $^{13}C^{\alpha}$ resonance indicated that the amino acid was dramatically changed and/or separated out; however, this was not shown for the normal Bombyx mori silk gland fibroin.

Cholesterol Improvement Synergistic Effects of Fermented Soybean Grits Caused by Added with Mung Bean in vitro (녹두 첨가로 인한 탈지대두 Grits(Defatted Soybean Grits) 발효물의 in vitro 상에서의 콜레스테롤 개선능 상승효과)

  • This study was performed to investigate cholesterol improvement of fermented defatted soybean grits (FD) and FD added with 2.5, 5, 10% mung bean (FDM). The FD and FDM were prepared by the solid state fermentation using Bacillus subtilis NUC1 at $40^{\circ}C$ for 24 hr. More than 70% cholesterol adsorption of FD and FDM groups was shown. Particularly, FDM added with 2.5% mung bean (2.5% FDM) showed highest cholesterol adsorption by 90% among FD and FDM groups. 2.5% FDM showed 42% inhibition effect on HMG-CoA reductase, and significantly decreased the intracellular cholesterol contents in HepG2 cells. Apolipoprotein AI, CIII improvement effects of FD and FDM group in HepG2 cells showed most effects in the 2.5% FDM. The results suggest that FDM added with 2.5% mung bean may be beneficial to the prevention of hypercholesterol.

  • A 7 years old, female and neutered ferret (Mustela putorius furo) was presented to local animal hospital due to lethargy, anorexia and hypothermia. Radiography, ultrasonography and blood test were performed. On the basis of clinical signs and tests, this case was presumed to be lymphoma. In consideration of this ferret's condition, chemotherapy was carried out for the treatment. However, the ferret died a few days. After necropsy, metastatic tumors were observed over abdomen. All tumors were packed with homologous lymphocytes with pleomorphic changes and numerous mitotic figures. Consequently, this masses were diagnosed as B-cell gastrointestinal lymphoma, which were negative for CD3 on immunohistochemistry.

Caplan's Syndrome Presenting as Multiple Pulmonary Nodules (다발성 폐결절로 발견된 카플란증후군 1예)

  • We report a case of Caplan's Syndrome, which presented as multiple pulmonary nodules. A 58-year-old male was admitted to hospital due to multiple pulmonary nodules. In addition, the patient presented with multiple arthritis, and dyspnea on exertion. Rheumatoid arthritis had been diagnosed 35 years ago. The patient had worked as a stonemason for 20 years. Computed Tomography (CT) revealed numerous well-defined tiny nodules scattered in both lungs, which was suspicious of miliary tuberculosis or malignancy. The patient was started on antituberculous medications and referred to our hospital. First, a transbronchial lung biopsy was performed, which showed no evidence of granuloma. It was our opinion that the biopsy was insufficient, and a follow-up video-associated thoracoscopy was performed. The pathological report determined necrotizing granulomatous inflammation and silicosis on background. According to imaging studies, pathologic reports, and clinical symptoms, we concluded that the patient had Caplan's syndrome. We controlled his rheumatic medications, and instructed him to avoid exposure to hazardous dust.
