Efficacy and Tolerability of Moclobemide Compared with Amitriptyline in Dysthymic Disorder (기분부전장애에서 Moclobemide와 Amitriptyline의 치료 효과와 내약성 비교 연구)

  • Background : Since dysthymia begins in late childhood or adolescence and has a chronic course, long-term pharmacotherapy may be required. New generation antidepressant, moclobemide, with more acceptable side effect profiles, is effective in the treatment of dysthymia. The main objective of this study was to determine whether they exhibit comparable efficacy and tolerability in dysthymia to amitriptyline. Method and Materials : The efficacy and tolerability of the moclobemide and amitriptyline, were compared in a eight-week single-centre double-blind study in patients(n=37) with dysthymia using he HAMD-17, the Clinical Global Impression Scale(CGI), the Montgomery-Asberg Depression Rating Scale (MADRS), Efficacy Index-Therapeutic Index(EITE), 4-point Index Side Effect Scale(4-PISES), and Efficacy Index- Side Effect Scale(EISE). Results : A total of 37 patients entered the study, 19 were randomly assigned to the moclobemide group and 18 to be amitriptyline group. Demo-graphic and illness characteristics were similar in both groups. There were no significant difference between two groups at the total 17-HDRS score, the HAMD-17% improvement, the total MADRS score, CGI response, and the EITE. In the comparison of EISE between two groups, the scores of the moclobemide group were relatively lower than the amitriptylinen group in full treatment. And the differences were significant(moclobemide group $1.39{\pm}0.61$ ; amitriptyline group $2.00{\pm}0.85$, p<.001). At the 4-PISE, There was no serious or treatment threatening side effects. And there was no specific difference in side effects between two groups. The moclobemide group reported higher EIR scores than the amitriptyline group at every follow up day, but the differences were not significant. And, there was no significant differences in the scores of five HRQOL subcategories which is compared between two groups at every follow up days. Conclusions : In terms of 17-HDRS and MADRS, moclobemide and amitriptyline are equally effective at least in allevating dysthymic symptoms. But moclobemide tended to be less troubling and better tolerated than amitriptyline. Therefore, moclobemide treatment can be used as a safe, and higher satisfactory treatment strategy for the dysthymia.

A Study on the Circadian Blood Pressure Rhythm of Diabetic Patients (당뇨병 환자의 혈압 일주기 리듬에 관한 조사연구)

  • This study was performed to investigate the relationship between reversed circadian blood pressure and risk factors of peripheral vascular disease in non-insulin-dependent diabetes mellitus (NIDDM) subjects. The subjects in this study were 18 NIDDM patients who were hospitalized in a medical unit of an university medical center located in Incheon, Korea, between November, 1998 and March, 1999. Blood pressure was measured with a mercury sphygmomanometer by 2 trained examiners every 2 hours during 24 hours. NIDDM subjects were divided into a dipper group and non-dipper group. Dippers are defined as those who show a mean nighttime blood pressure(BP) drop of more than 10% compared with daytime BP. Non-dippers are defined as those who show a mean nighttime BP drop of less than 10%, or an elevation in BP compared with daytime BP. Daytime BP included values obtained between 6 a.m. and 10 p.m. Night time BP included values obtained between 10 p.m. and 6 a.m. Data was analyzed by SPSS/PC package. Chi-square( $^2$) test was used for the comparison of sex between The dipper group and non-dipper group. Mann-Whitney test was used for comparisons of values of the risk factors of peripheral vascular disease and the frequency of complications of diabetes between the dipper group and non-dipper group. The results are as follows. There were no significant differences in daytime systolic, diastolic, and mean blood pressures between the dipper group and non-dipper group. However, night time systolic, diastolic, and mean blood pressures in the non-dipper group were significantly nigher than those in the dipper group (p=.021). There were no differences in sex, age, body, weight, duration of diabetes, serum lipid levels, BUN and HbA1c between the two groups. On the contrary, 87.5% of non-dipper group subjects showed having hypertension, 30% of dipper group subjects showed having hypertension and this difference was statistically significant (p=.018). All of the non-dipper group subjects (N=8) showed having at least one diabetic complication. However, 40% of the dipper group subjects (N=10) showed having no diabetic complication at all and this difference was also statistically significant (p=.049). There were no significant differences in frequency of nephropathy, neuropathy and retinopathy between the dipper group and non-dipper group.

AN IN-VITRO WEAR STUDY OF INDIRECT COMPOSITE RESINS AGAINST HUMAN ENAMEL (법랑질에 의한 수종의 간접복합레진의 마모에 관한 연구)

  • Statement of problem: Second-generation indirect composite resins have been improved flexural strength, compressive strength, hydrolytic degradation resistance, wear resistance compared to first-generation indirect composite resins, but there are still some problems as hydrolysis and low wear resistance. Some manufacturers claim that wear resistance of their materials has been improved, but little independent study has been published on wear properties of these materials and the properties specified in the advertising materials are largely derived from in-house or contracted testing. Purpose: This study was to evaluate the wear of indirect composite resins (SR Adore, Sinfony, Tescera ATL) and gold alloy against the human enamel. Material and method: Extracted human incisors and premolars were sectioned to $2{\times}2{\times}2mm$ cube and embedded in the clear resin and formed conical shaped antagonist to fit the jig of pin-on-disk tribometer. Total 20 antagonists were stored in distilled water. Five disk samples, 24mm in diameter and 1.5mm thick, were made for each of three groups of indirect composite resins and gold alloy group, and polished to #2,000 SiC paper on auto-polishing machine. Disk specimens were tested for wear against enamel antagonists. Wear test were conducted in distilled water using a pin-on-disk tribometer under condition (sliding speed 200rpm contact load 24N, sliding distance 160m). The wear of the enamel was determined by weighing the enamel antagonist before and after test, and the weight was converted to volumes by average density. The wear tracks were analyzed by scanning electron microscopy and surface profilometer to elucidate the wear mechanisms. Statistical analysis of the enamel wear volume, wear track depth and wear tract width of disk specimens were accomplished with one-way ANOVA and the means were compared for significant differences with Scheffe's test. Results: 1. The enamel wear was most in gold alloy, but there were no statistically significant differences among all the groups (P>.05). 2. In indirect composite resin groups, the group to make the most shallow depth of wear tract was Sinfony, followed by Tescera ATL, SR Adoro (P<.05). Gold alloy was shallower than Sinfony, but there was no statistically significant difference between Sinfony and gold alloy (P>.05). 3. The width of wear tract of SR Adore was larger than the other groups (P<.05), and there were no statistically significant differences among the other groups (P>.05). 4. SEM analysis revealed that Sinfony and gold alloy showed less wear scars after test, Tescera ATL showed more wear scars and SR Adore showed the most. Conclusion: Within the limits of this study, Sinfony and gold alloy showed the least wear rates and showed similar wear patterns.

The Current State of Families using the In-Home Care Service and Satisfaction in Service According to Family Type (아이돌보미 서비스 이용가정 실태 및 가정유형에 따른 서비스 만족에 관한 연구 : 서울 거주 아이돌보미 서비스 이용 가정을 중심으로)

  • The purposes of this research were (1) to comprehend how families used and evaluated the in-home care service provided by the Healthy Family Support Center, and (2) to investigate the differences in the perceptions of service fee, satisfaction in and loyalty to service among the types of family categorized by their income. The data from 346 mothers or fathers whose children had received the in-home care service at least once were analyzed. One-way ANOVA and Scheffe or Tamhane post hoc test were used to test the hypotheses. The findings were as follows: 1. There were significant differences in perception of the service fee among the family types: the 'Da'-type families tended to perceive that the service fee was expensive and not cheaper than the similar services provided by other organizations. 2. The 'Ga'-type families scored significantly higher than the 'Na'-type families and the 'Da'-type families on satisfaction in service, and higher than the 'Da'-type families on loyalty to the service. 3. No significant difference was found on satisfaction in the performances of baby-sitters and staffs in charge of the service. The implications drawn from the study findings are discussed.

Partial Sequence of the Bovine (Bos taurus coreanae) Myogenic Factor Encoding Gene MyoD

  • This experiment was carried out to isolate the partial bovine (Bos Taurus coreanae) myogenic factor encoding gene, MyoD, using the rat myogenic factor (MyoD) gene sequence and to compare the gene sequence between another myogenic factor (Myf 5) and MyoD gene of the bovine. To make the probe and isolate the MyoD gene, PCR was performed to amplify rat and bovine MyoD gene including exon I, II and intron I. The homology between mouse and bovine MyoD is high; bovine MyoD gene shows 17 different gene sequence region compared to rat MyoD. Among those, two regions have significant differences; one is the exon I part between 2834 and 2850 bp, the other is intron part between 3274 and 3303 bp of the mouse. At this region homology was 40% in the former and 50% in the latter. Homology between bovine MyoD and Myf5 was 83% in the exon 1. Especially exon I in the Myf5 602-617 bp and 651-683 bp have significant differences. These results suggest that MyoD gene have a similar gene structure in mouse and bovine and MyoD and Myf5 of the bovine, at least in part, have a similar expression and activity.

Comparison of marginal bone loss and patient satisfaction in single and double-implant assisted mandibular overdenture by immediate loading

  • PURPOSE. The purpose of this study was to compare the coronal bone level and patient satisfaction in 1-implant and 2-implant assisted mandibular overdentures. MATERIALS AND METHODS. Twenty patients who had maladaptive mandibular dentures were treated in this study. Patients were randomly divided into two groups. The first group received 1 implant (Simple line II, Implantium, South Korea) in their mandibular midline and the second group received 2 implants in their B and D regions (according to Misch's category). If the primary stability of each implant was at least 60 ISQ, ball attachment was placed and denture relined with soft liner. After 6 weeks, retentive cap incorporated with hard acrylic resin. In the 6 and 12 months recalls, periapical digital radiograph were made and visual analogue scale questionnaires were used to record patient satisfaction. The Friedman test was done for comparing the presurgical and postsurgical parameters in each group and the U-Mann Whitney test (P<.05) was done for comparison of post-treatment results between the two groups. RESULTS. All implants achieved sufficient primary stability to be immediately loaded. Patient satisfaction was high, and there were no significant differences between two groups (P>.05). In addition, mean marginal bone loss was $0.6{\pm}0.67$ mm in the first group and $0.6{\pm}0.51$ mm in the second group, after 12 month. Mean marginal bone loss showed no significant differences between two groups. CONCLUSION. This preliminary one-year result indicated that mandibular overdentures anchored to a single implant can be a safe and cost-effective method as a starting step for implant-overdenture treatment.

STUDY ON VERTICAL DISPLACEMENT OF SOFT TISSUE UNDER DISTAL EXTENSION PARTIAL DENTURE BASE BY FUNCTIONAL IMPRESSION (유리단 국소의치의 기능 인상에 의한 연조직의 수직적 변위량에 관한 연구)

  • Distal extension partial dentures are supported by both the relatively rigid teeth and the resilient mucosa. So impression techniques of residual alveolar ridge in case of distal extension partial denture have particular importance in order to broad distribution of the masticatory force. McLean recognized the need for recording the tissues supporting distal extension partial denture base in functional form to equalize the resilient and non-resilient support, and this was called functional impression. Many investigators proposed various techniques of the functional impression for a distal extension partial denture, but only a little studies were performed about displacement of soft tissue under distal extension partial denture base. The purpose of this study is to investigate the amount of vertical displacement of the soft tissue under distal extension partial denture base by different functional impression techniques. Impression techniques used were Z.O.P. Impression, Selective Tissue Placement Impression, Functional Relining Impression. Measurement of the vertical displacement of soft tissue were made with Depth Gauge and Measuring Platform. A Anatomic Impression was used as a control. The results were tested statistically using 3 way ANOVA and Scheffe test. The followings were the results obtained from this study. 1. The greatest amount of soft tissue displacement was observed in the center of the retromolar pad. 2. No significant differences were found between the crest of alveolar ridge and the buccal shelf area. 3. The greatest soft tissue displacement was observed in Functional Relining Impression using Iowa wax, and the least displacement was observed in Selective Tissue Placement Impression using murcaptan rubber base. 4. No significant differences were found between finger pressure and biting pressure in Z.O.P. Impression, but greater displacement was observed by biting pressure than finger pressure in Functional Reling Impression.

A Study on Model Development for Urban Community Nursing Center (일개 도시 지역사회 간호센터 모형개발을 위한 요구조사)

  • Objectives: The purpose of this study was to identify basic health needs of adult clients to develop a community nursing center model in Seoul. Methods: Data were collected using a questionnaire survey from 894 adults registered at a public health center who were health management members, and visiting 4 community nursing centers, University of Wisconsin, Milwaukee during the period from July 1999 to January 2000. The data were analyzed using frequency, percentile, t-test, and ANOVA. Results: 1) The University of Wisconsin- Milwaukee School of Nursing has a long tradition of developing 4 community nursing centers with innovative health care programs. CNCs integrate the ability to implement and test effective intervention strategies with education, research, and practices of nursing students and faculty. They were designed to enhance the health status and quality of life for urban communities through the development of productive, outcome focused, collaborative partnerships among UWM-Nursing faculties and staff, other health and human service providers, consumers, and policy makers. It links the financial resources between UWM and 9 voluntary agencies and 12 public funding organizations including federal. state, and local governments. 2) Of the total health management members, 37.4% were reported to have at least one type of chronic disease such as hypertension, diabetes, and arthritis. Ten percent of them reported having obesity, and 44.2% reported lack of exercise. The health status of the subjects was within normal range in laboratory tests. However, female subjects showed more significant differences in obesity and cholesterol levels than male subjects. The subjects, who were in their 50s, showed more significant differences in obesity, SGOT, SGPT, and cholesterol levels than the subjects in other age groups. Conclusion: A community nursing center needs to be developed, that has a link between the nursing college and the public health center, with partnerships and a multidisciplinary approach. Based on the study results, exercise programs for middle aged adults are considered necessary. In particular, specific exercise programs for pre-menopausal women needs to be implemented in the future to prevent them from developing osteoporosis.

Factors Associated With Subjective Life Expectancy: Comparison With Actuarial Life Expectancy

  • Objectives: Subjective life expectancy (SLE) has been found to show a significant association with mortality. In this study, we aimed to investigate the major factors affecting SLE. We also examined whether any differences existed between SLE and actuarial life expectancy (LE) in Korea. Methods: A cross-sectional survey of 1000 individuals in Korea aged 20-59 was conducted. Participants were asked about SLE via a self-reported questionnaire. LE from the National Health Insurance database in Korea was used to evaluate differences between SLE and actuarial LE. Age-adjusted least-squares means, correlations, and regression analyses were used to test the relationship of SLE with four categories of predictors: demographic factors, socioeconomic factors, health behaviors, and psychosocial factors. Results: Among the 1000 participants, women (mean SLE, 83.43 years; 95% confidence interval, 82.41 to 84.46 years; 48% of the total sample) had an expected LE 1.59 years longer than that of men. The socioeconomic factors of household income and housing arrangements were related to SLE. Among the health behaviors, smoking status, alcohol status, and physical activity were associated with SLE. Among the psychosocial factors, stress, self-rated health, and social connectedness were related to SLE. SLE had a positive correlation with actuarial estimates (r=0.61, p<0.001). Gender, household income, history of smoking, and distress were related to the presence of a gap between SLE and actuarial LE. Conclusions: Demographic factors, socioeconomic factors, health behaviors, and psychosocial factors showed significant associations with SLE, in the expected directions. Further studies are needed to determine the reasons for these results.

Effects of Korean College Students' Use of English Reading Learning Strategies on Reading Comprehension (한국 대학생의 영어독해 전략이 독해에 미치는 영향)

  • The purpose of this study is to research the effects of English reading strategies on English reading comprehension by Korean college students. Reading strategy use was assessed through Oxford's self-report questionnaire in reading strategies. This study has three research questions. The first question was to investigate some reading strategies used by college students. The second question was to investigate the differences in reading strategies between two groups in gender. The third question was to investigate the differences in reading strategies of three college student groups according to their English proficiency estimated by reading scores. Some major findings of this study are as follows. First, college English learners use memory strategies most frequently of the six strategies, while using metacognitive strategies least frequently. Second, there exists a significant difference in reading strategies between the gender group. Third, there also exists a significant difference in reading strategies among the three groups divided according to English proficiency. This study shows that students' reading ability can be strengthened and motivated by some reading strategies in reading practice. It also means that it is necessary for English teachers to take into consideration the reading strategies suitable for the students in their reading classes.
