Lessons Learnt from an 11-year Experience with Lymphatic Surgery and a Systematic Review of Reported Complications: Technical Considerations to Reduce Morbidity

  • Complications experienced during lymphatic surgery have not been ubiquitously reported, and little has been described regarding how to prevent them. We present a review of complications reported during the surgical management of lymphedema and our experience with technical considerations to reduce morbidity from lymphatic surgery. A comprehensive search across different databases was conducted through November 2020. Based on the complications identified, we discussed the best approach for reducing the incidence of complications during lymphatic surgery based on our experience. The most common complications reported following lymphovenous anastomosis were re-exploration of the anastomosis, venous reflux, and surgical site infection. The most common complications using groin vascularized lymph node transfer (VLNT), submental VLNT, lateral thoracic VLNT, and supraclavicular VLNT included delayed wound healing, seroma and hematoma formation, lymphatic fluid leakage, iatrogenic lymphedema, soft-tissue infection, venous congestion, marginal nerve pseudoparalysis, and partial flap loss. Regarding intra-abdominal lymph node flaps, incisional hernia, hematoma, lymphatic fluid leakage, and postoperative ileus were commonly reported. Following suction-assisted lipectomy, significant blood loss and transient paresthesia were frequently reported. The reported complications of excisional procedures included soft-tissue infections, seroma and hematoma formation, skin-graft loss, significant blood loss, and minor skin flap necrosis. Evidently, lymphedema continues to represent a challenging condition; however, thorough patient selection, compliance with physiotherapy, and an experienced surgeon with adequate understanding of the lymphatic system can help maximize the safety of lymphatic surgery.

Percutaneous Transhepatic Removal of Migrated Biliary Stent from a Chronic Biloma Cavity (만성 담즙종 공동 내로 이동한 담도 스텐트의 경피경간적 제거)

  • Iatrogenic foreign bodies are a challenging complication to both the interventional radiologist and patient, resulting in impaired quality of life and substantial financial cost. The case report describes a successful percutaneous transhepatic removal of an intra-abdominal foreign body. A 72-year-old man underwent surgery for placement of a retrievable covered stent for refractory bile leakage after left hemihepatectomy. Three days after placement, stent folding and migration into a chronic biloma cavity occurred via the bile leakage site. By using a balloon catheter technique, the folded stent could be straightened and repositioned into the bile duct to minimize stent-strut injury during retrieval. The interventional approach could be a valid treatment option for intra-abdominal foreign bodies, as well as intravascular foreign bodies. A thorough understanding of devices and techniques can provide the interventional radiologist with valuable information regarding procedural planning and the management of iatrogenic foreign bodies.

An Efficient Broadcast Authentication Scheme with Batch Verification for ADS-B Messages

  • As a cornerstone of the next generation air traffic management (ATM), automatic dependent surveillance-broadcast (ADS-B) system can provide continual broadcast of aircraft position, identity, velocity and other messages over unencrypted data links to generate a common situational awareness picture for ATM. However, since ADS-B messages are unauthenticated, it is easy to insert fake aircrafts into the system via spoofing or insertion of false messages. Unfortunately, the authentication for ADS-B messages has not yet been well studied. In this paper, we propose an efficient broadcast authentication scheme with batch verification for ADS-B messages which employs an identity-based signature (IBS). Security analysis indicates that our scheme can achieve integrity and authenticity of ADS-B messages, batch verification, and resilience to key leakage. Performance evaluation demonstrates that our scheme is computationally efficient for the typical avionics devices with limited resources, and it has low communication overhead well suitable for low-bandwidth ADS-B data link.

Quantifying Architectural Impact of Liquid Cooling for 3D Multi-Core Processors

  • For future multi-core processors, 3D integration is regarded as one of the most promising techniques since it improves performance and reduces power consumption by decreasing global wire length. However, 3D integration causes serious thermal problems since the closer proximity of heat generating dies makes existing thermal hotspots more severe. Conventional air cooling schemes are not enough for 3D multi-core processors due to the limit of the heat dissipation capability. Without more efficient cooling methods such as liquid cooling, the performance of 3D multi-core processors should be degraded by dynamic thermal management. In this paper, we examine the architectural impact of cooling methods on the 3D multi-core processor to find potential benefits of liquid cooling. We first investigate the thermal behavior and compare the performance of two different cooling schemes. We also evaluate the leakage power consumption and lifetime reliability depending on the temperature in the 3D multi-core processor.

Evaluation of Pressure Reducing Valves performance using Statistical Approach in Water Distribution System : Case Study (통계적 기법을 이용한 배·급수 관망 내 감압 밸브 성능 평가에 관한 사례 연구)

  • It has been widely accepted that the pressure management of water distribution systems using pressure reducing valves(PRVs) would be an effective method for controlling leakages. A pressure reducing valve (PRV) regulates outlet pressure regardless of fluctuating flow and varying inlet pressure, thereby reducing leakage and mitigating the stress on the water distribution system. However, the operation of a PRV is vulnerable to its mechanical condition and hydraulic operability. In this research, the effect of PRVs installed in water distribution system are evaluated in terms of hydraulic pressure reduction and mechanical performance by analyzing measured pressure data with statistical approach. A statistical approach using the moving average filter and frequency analysis based on fourier transform is presented to detect abnormally operated PRVs that have been densely installed in water distribution system. The result shows that the proposed approach can be a good performance evaluation method by simply measuring pressures for the PRVs.

  • This study was performed to analyze the vulnerability of the situation in which combat system is shot by external projectile impacts. In developing combat system, it is vital to consider the survivability as well as its mission capability because it is directly connected with loss of lives. Especially, when the parts which are susceptible to fire are shot under battle situation, the system is exposed to the dangerousness and the situation when the parts such as fuel tanks are impacted by external projectile impacts can lead to flash fire as a result of the leakage of fuel. Therefore, in this study the possibility of flash fire was calculated by analyzing a variety of variables supposing that fuel tank in the combat system is shot. The aim of this study is to suggest effective methods in the basic steps when combat system is designed.

  • This paper provided a counter measures against the troubles and accidents that are likely to take place in the power plant using hydrogen gas as a coolant for the cooling system of the generator. Because of the extremely wide flammability limits of hydrogen in comparison to the other flammable gases, the safety measures against the hydrogen accidents is very important to ensure the normal operation of electric-power facility. This study's purpose was a presentation of standard model of safety management of hydrogen equipments in the coal firing power plant such as following items: 1) providing the technical prevention manual of the hydrogen explosions and hydrogen fires occurring in the cooling system of power generator; 2) the selection of explosion-proof equipments in terms of the risk level of operating environment; 3) the establishment of regulations and counter measures, such as the incorporation of gas leakage alarm device, for preventing the accidents from arising; 4) the establishment of safety management system to ensure the normal operation of the power plant.

  • Fintech, which means the convergence of finance and information technology, becomes a hot topic in the financial sector. Through innovative activities on financial services, ICT(Information and Communication Technology) is integrated into the overall financial industry, and a new form of financial services could be expected to improve the existing financial system. On the other hand, fintech services are relatively vulnerable to security issues. Due to the process simplication and the channel fusion, the leakage of personal and financial informations, authentication bypass, phishing, and pharming are getting more concerned. In this study we investigated the security risk of fintech services in the viewpoints of service provider, technology adoption, and security policy. The possible countermeasures to reduce those risks are suggested because security is an important criterion for selecting financial services. This study basically offers quantification of the potential security risks and step-by-step control measures about business processes in the fintech services. The suggested security model includes user authentication, terminal security, payment information protection, API(Application Programming Interface) security, and abnormal transaction monitoring. This study might contribute to an understanding of the security risks and some possible measures for mitigating those risks on the practical perspective.

Development of Impact Evaluation and Diagnostic Indicators for Sinkholes

  • Based on the previous studies on sinkholes and ground subsidence conducted until date, the factors affecting the occurrence of sinkholes can be divided into natural environmental factors and human environmental factors in accordance with the purpose of the study. Furthermore, to be more specific, the human environment can be classified into the artificial type and the social type. In this study, the assessment indices for assessing risks of sinkholes and ground subsidence were developed by performing AHP analysis based on the results of the study by Lee et al. (2016), who selected the risk factors for the occurrence of sinkholes by performing Delphi analysis targeting relevant experts. Analysis showed that the artificial environmental factors were of significance in affecting the occurrence of sinkholes. Explicitly, the underground factors were found to be of importance in the natural environment, and among them, the level of underground water turned out to be an imperative influencing factor. In the artificial environment, the underground and subterranean structures exhibited similar importance, and in the underground structures, the excessive use of the underground space was found to be an important influencing factor. In the subterranean ones, the level of water leakage and the erosion of the water supply and sewage piping system were the influential factors, and in the surface, compaction failure was observed as an imperative factor. In the social environment, the regional development, and above all, the groundwater overuse were found to be important factors. In the managemental and institutional environment, the improper construction management proved to be the most important influencing factor.

Real-time Network Middleware Supporting Fault-Tolerant Operations for Personal Robot System

  • By development of a robot technology, personal robot is being developed very actively. Various infra-technologies are accumulated in hardware and software how by internal a lot of research and development efforts, and it is circumstance that actual commodity is announced. But, personal robot is applied to be acting near human, and takes charge of safety and connected directly a lot of works of home security, gas-leakage, fire-warning facilities, or/and etc. So personal robot must do safe and stable action even if any unexpected accidents are happened, important functions are always operated. In this paper, we are wished to show design structures for supporting fault-tolerant operation from our real-time robot middleware viewpoint. Personal robot, in being developed, was designed by module structure to do to interconnect and to interoperate among each module that is mutually implemented by each research facilities or company. Also, each modules can use appreciate network system that is fit for handling and communicating its data. To guarantee this, we have being developed a real-time network middleware, for especially personal robot. Recent our working is to add and to adjust some functions like connection management, distributed routing mechanism, remote object management, and making platform independent robot application execution environment with self-moving of robot application, for fault-tolerant personal robot.
