자리공 뿌리 섭취로 발생한 급성 중독 1례 : 증례보고 (Phytolacca Radix Poisoning: A Case Report)

  • Objectives: This study aims to present the case of poisoning associated with Phytolacca radix. Case Presentation: A 61-year-old male with nausea, vomiting, dizziness, and abdominal pain after ingesting about 10 centimeters of Phytolacca radix visited Korean medicine hospital. He began to show symptoms one hour after eating the plants. Methods: The patient was diagnosed with poisoning by Phytolacca radix. The patient received acupuncture, moxibustion, and herbal medicine treatment including Glycyrrhizae radix. laboratory test including complete blood count, biochemistry was also conducted. Results: Treatment, including acupuncture, moxibustion, and herbal medicine, improved his symptoms. His symptoms subsided within two hours. The laboratory test conducted on the next day show a slight increase in his white blood cell, blood urea nitrogen, urine ketone levels. There was no damage to the liver or kidneys. Conclusions: This report demonstrates the need for caution when consuming wild plants, which could lead to serious adverse effects. Patients should be alerted to the indiscriminate ingestion of wild plants. Further research on how to treat poisoning in Korean medicine needs to be considered to confirm these findings.

오장(五臟)의 본(本)에 대한 연구 (Study on the fundamentals of the Five Viscera)

  • In oriental medicine, there is the fundamentals to lead a stirring life, and it is an important matter that make a study of fundamentals for treatment. The su-wen(素問) mentions the fundamentals of sheng(生之本), the fundamentals of qi(氣之本), the fundamentals of feng-cang(封藏之本), the fundamentals of ba-ji(罷極之本) and the fundamentals of cang-lin(倉廩之本). They are terminologies expressing physiological functions of the five viscera - heart, lungs, kidneys, liver and spleen. The five viscera are expressed through face, fur, hair, nail and lips. And the five viscera are closely connected with blood, skin, bone, muscle and flesh. Through the changes of personal appearance, we can recognize those of their internal organs. In oriental medicine, they are called as the cang-xiang(藏象). Heart is the fundamentals of sheng(生), and it is related to spirit, blood and blood vessel. Lungs are the fundamentals of qi(氣), because they master the extensive meaning of qi(氣). Kidneys are the fundamentals of feng-cang(封藏) which means seclusion. Liver is the fundamentals of ba-ji(罷極) that have several view by concept. Spleen, stomach, large and small intestines, paunch and bladder are the fundamentals of cang-lin(倉廩), that is to say, gastrointestinal tract that ingest diet and digest it.

식이내 섬유질의 종류가 성장기 흰쥐의 납 흡수 및 체내대사에 미치는 영향 (Effect of dietary fiber on lead absorption and metabolic changes in growing rats)

  • This study was performed to investigate nutritional effect of various dietary fibers on lead absorption and metabolism of protein and lipid in growing rats. Forty eight male rats of Sprague-Dawley strain weighing 75.7$\pm$0.7g were blocked into six groups according to body weight and fed six kinds of diet different with fiber source(non-fiber, cellulose, pectin) and lead level(0%, 1% ) for 4 weeks. Results are summerized as follows: 1) Food intake, weight gain, FER and PER were remarkably decreased in lead(Pb) added groups, and FER and PER in Pb-added pectin group were significantly lower than those in Pb-added non-fiber group. 2) Weight of liver, kidney and epididymal fat pad, bone weight and length, hematocrit, and hemoglobin content were decreased in Pb-added groups. 3) Total protein content in serum was tended to be decreased in Pb-added groups, but total lipid and cholesterol contents in serum were not different with dietary Pb level and fiber source. 4) Nitrogen, lipid and cholesterol content in liver were tended to be deceased in Pb-added groups, and especially those of the Pb-added pectin group were the lowest among groups. 5) Daily urinary and fecal excretions of nitrogen, lipid and cholesterol were decreased in Pb-added groups. Especially fecal excretions of nitrogen, lipid and cholesterol in Pb-free groups were significantly increased by dietary cellulose and pectin. 6) Pb content in blood was significantly increased in Pb-added pectin group. There was no significant decrease in Pb contents of liver, kidney and tibia, and increase in excretion of Pb by feeding dietary fibers. In conculsion, dietary fibers had no effect on the absorption of Pb, and dietary pectin seemed to increase Pb poisoning by decreasing bioavailibility of protein, lipid and other nutrients in the diet.

납(Pb)과 단백질 수준을 달리한 식이로 사육한 성장기 흰쥐의 체내대사 변화 (Metabolism Changes in Growing Rats Fed Diets with Different Levels of Lead and Protein)

  • This study was performed to see the effects of lead poisoning and dietary protein levels(6, 15 and 40 % casein diets) on growth, protein and lipid metabolisms in growing rats. It was also investigated whether the high protein intake would alleviate lead toxicity by decreasing Pb absorption and/or increasing Pb excretion. The results obtained were summarized as follows ; 1) Weight gain, F.E.R liver weight, weight and length of bone in Pb-administered groups were lower than in Pb-free groups. However, these values in the 40% casein diet group with Pb were increased to the level in 15% casein diet group without Pb. 2) Hematocrt and hemoglobin content in blood were lower in Pb -adminstered groups than in Pb free groups. Especially, these levels were lower in 6% casein diet group with Pb than in any other group. 3) Plasma protein level in th e 40% casein casein diet group was the highest of all groups and those of Pb-administered groups tended to be lower than those of Pb-free groups. Plasma lipid and cholesterol levels were increased with decreasing dietary protein level, and these levels were higher in the animals exposed to Pb than in free groups.4) Total liver protein, lipid and cholesterol contents were increased with increasing dietary protein level, and these contents were lower in Pb-administered groups than in Pb free groups. 5) Fecal Pb excretion was not different between 6 and 40% casein diet groups. However, urinary Pb excretion was higher in the 40% casein diet group than in the 6% casein diet group. Above results suggest that, in exposing to the Pb pollution, sufficient protien intake must be recommended. High protein intake seemed to alleviate lead toxicity by increasing urinary Pb excretion.

Excessive dietary lead reduces growth performance and increases lead accumulation in pigs

  • Objective: The objective of this study was to investigate the influence of dietary lead (Pb) supplementation and feeding period on growth performance, organ weight, and Pb accumulation in pigs. Methods: In a 56-day feeding experiment, a total of 48 barrows with initial body weight 10.4±0.6 kg were allotted to 2 dietary treatments (0 and 200 mg/kg of supplemental Pb) in a completely randomized design with 6 replicates. Body weight and feed intake were recorded to calculate growth performance. At the end of each 14 day-period (on days 14, 28, 42, and 56), an animal was randomly selected from each pen and slaughtered to collect blood samples, hair samples, left 5th rib, heart, liver, kidneys, lungs, and longissimus dorsi muscle samples. Results: Average daily gain and average daily feed intake were reduced (p<0.05) by supplemental Pb during the day 42 to 56. Relative kidney weight to body weight was linearly increased with increasing feeding period in pigs fed the Pb-supplemented diet, but not in pigs fed the control diet (p<0.05). The Pb concentrations in hair, left 5th rib, kidneys, and lungs were linearly increased with longer feeding period in pigs fed the Pb-supplemented diet, but not in pigs fed the control diet (p<0.01). Conclusion: Dietary Pb supplementation caused growth retardation and Pb accumulation in most organs, particularly in hair, bone, and kidneys in a time-dependent manner.

亞鉛이 鉛中毒 白鼠에서 肝 및 腎臟의 鉛蓄積과 血中 $\sigma$-Aminolevulinic Acid Dehydrase 活性度에 미치는 영향 (The Effects of Zinc on $\sigma$-Aminolevulinic Acid Dehydrase Activities in Blood and Lead Accumulation in Livers and Kidneys in Rats)

  • The purpose of the study is to determine the effects of Zn in lead poisoning rats by way of examining interaction mechanism of Pb and Zn. The fifty-five rats were Pb divided into four groups such as Zn group, group, Pb and Zn group, and control group. The rats of Zn group and of Zn and Pb group were subdivided into four groups by dose of Zn respectively 250mg/l, 500mg/l, 1, 000mg/l and 2, 000 mg/l. The rats having been fed the above mentioned chemicals, were weighed every five days for fifty-five days, and the subjects were slaughtered for measuring $\delta$-ALAD activities in blood and the accumulation amount of the chemicals in livers and kidneys. The results of the study are summarized as following 1. As for body weight gains, those of the control group rats were the highest, and those of Pb group the lowest. 2. $\delta$-ALAD activities of Pb group showed the tendency of decrease in comparison with those of control group. In Zn group, the subgroups of 250mg/l and 500mg/l showed higher activities than control group, whereas the subgroups of 1, 000mg/l and 2.000mg/l showed lower. 3. Hb value of Pb group was lower than that of control group. In Zn group, Hb value of the groups of 250mg/l and 500mg/l was a little higher than that of control group, while that of the groups of 1, 000mg/l and 2, 000mg/l was lower. 4. The amount of Pb and Zn accumulated in liver was much higher than in kidney. The amount of Pb accumulated in organs of Zn and Pb group decreased gradually in contrast to high concentration of Zn.

AN EXPLORATORY STUDY COMPARING BLOOD METAL CONCENTRATIONS BETWEEN STROKE AND NON-STROKE PATIENTS IN KOREANS

  • Results from previous studies revealed that metal level in the body is related to certain types of diseases. For example. serum copper level with chronic heart failure, iron and transferrin in the blood serum with acute cerebral vascular diseases, Zn in the CNS, lead with neurotoxicity, hypertension, genetic damage, arsenic with cancer skin lesion, Al with neurobehavioral function (cognitive impairment and memory disorder), and etc. The rate of stroke has increased in recent years and several metals were found to be responsible for causing stroke. This study compared several blood metal concentrations between stroke and non-stroke patients. Patients with stroke (116 samples) and non-stroke (111 samples including lowback pain and others) participated in this study. Total of 227 blood samples were collected and participants completed questionnaires regarding age, gender, occupation, residence, alcohol, smoking, and etc. To be qualified into the stroke group, patients have never experienced stroke previously. Subjects only included ischemic stroke and intracerebral hemorrhage patients diagnosed by brain CT and brain MRI. Patients with high risk of metal exposure such as herbal intake and job related exposure were excluded. 10ml of blood samples were analyzed by ICP-MS method at the Center of Nature and Science at Sangji University. Metal geometric mean (SD) concentrations in blood of study subjects showed higher values, 2.64-36.12%, than WHO reference values in Mn, Ni, Hg, Se, and As. Metal concentration in blood of stroke patients non-adjusted for potential confounders was higher except for Hg and also higher except for Ni in adjusted for potential confounders. Co was significantly higher in stroke patients (p=0.002) than non-stroke patients adjusted for potential confounders. Regression coefficient values of stroke patients was 0.17-8.25 in each metals. Odd ratio of stroke patients had 0.96 (Ni)-2.68 (Co) compared to non-stroke cases. This result means that Co increase of 1 raises the risk ratio of stroke by 2.86 times. Based on the results, metal concentration in blood seems to affect incidence of stroke.

녹차가 납중독된 흰쥐의 신장에 미치는 조직학적 및 생화학적 연구 (Histological and Biochemical Study on the Effects of the Green-Tea in Rat Kidney Toxicated by Lead)

  • 흰쥐에서 납 독성에 대한 녹차의 방어효과를 알아보기 위하여 150g 내외의 흰쥐를 대조군, 납 단독투여군, 납-녹차 투여군으로 나누어 4주, 8주간 lead acetate (500 ppm)를 1주에 2번씩 복강 투여하였고, 녹차침수액(3g of tea/100m1 of water)은 식수로 공급하였다. 생화학적 및 형태학적 실험을 통해 다음과 같은 결론을 얻었다. 1. ALP값은 대조군에 비해 실험군 모두 감소하였고, 납-녹차투여군은 납 단독투여군보다 감소가 완화되었다. BUN는 대조군에 비해 실험군 모두 증가하였고, 납 녹차투여군은 납 단독투여군보다 증가가 완화되었다. 2. 신장의 전자현미경적 결과 B-1군은 미세융모, 사립체가 변형되었다. 다수의 리소좀이 관찰되었다. 핵의 모양은 불규칙하게 나타났다. B-2군은 미세융모가 잘 발달되었다. 다수의 사립체와 리소좀이 관찰되었다. rER의 수조는 팽창되고 수가 감소하였고 핵은 정상적으로 관찰되었다. C-1군은 대부분의 미세융모가 탈락, 변성되었고, 다수의 리소좀을 관찰할 수 있었다. 사립체는 감소, 변형되었다. 핵은 함입되어 불규칙하였다. C-2군은 미세융모의 일부가 팽창되거나 변화되었고, 다른 세포소기관들은 B-2군과 비슷한 양상을 나타냈다.
