The Correction of a Secondary Bilateral Cleft Lip Nasal Deformity Using Refined Open Rhinoplasty with Reverse-U Incision, V-Y Plasty, and Selective Combination with Composite Grafting: Long-term Results

  • Background : This article presents long-term outcomes after correcting secondary bilateral cleft lip nasal deformities using a refined reverse-U incision and V-Y plasty or in combination with a composite graft in order to elongate the short columella. Methods : A total of forty-six patients underwent surgery between September 1996 and December 2008. The age of the patients ranged from 3 to 19 years of age. A bilateral reverse-U incision and V-Y plasty were used in 24 patients. A composite graft from the helical root was combined with a bilateral reverse-U incision in the 22 patients who possessed a severely shortened columella. The follow-up period ranged between 2 and 10 years. Results : A total of 32 patients out of 46 were evaluated postoperatively. The average columella length was significantly improved from an average of 3.7 mm preoperatively to 8.5 mm postoperatively. The average ratio of the columella height to the alar base width was 0.18 preoperatively and 0.29 postoperatively. The postoperative basal and lateral views revealed a better shape of the nostrils and columella. The elongated columella, combined with a composite graft, presented good maintenance of the corrected position with no growth disturbance. A composite graft showed color mismatching in several patients. Twenty-six patients demonstrated no alar-columella web deformity and satisfactory symmetry of the nostrils. Four patients experienced a drooping and overhanging of the corrected alar-columella web. Conclusions : A bilateral reverse-U incision with V-Y plasty or in combination with a composite graft was effective in correcting secondary bilateral cleft lip nasal deformity.

Periodontal treatment of a patient with aplastic anemia (재생 불량성 빈혈(Aplastic anemias) 환자의 치주 치료 증례)

  • Aplastic anemia is a disease characterized by general lack of bone marrow activity; It may affect not only the red blood cells but also the white blood cells and platelets, resulting in pancytopenia. Spontaneous gingival hemorrhage is present in some cases and it is related to the blood platelet deficiency. This case report presents the periodontal treatment of a patient with aplastic anemia. A 43-year-old female was referred for continuous gingival bleeding after periodontal treatment. Periodontal findings revealed generalized gingival imflammation, oozing of blood from gingival crevice, and it was diagnosed as adult periodontitis. Root planing and extraction of the upper left third molar with poor prognosis were put into operation after elevation of the platelet count with platelet transfusion. The extraction socket was sutured with 3-0 silk. Bleeding continued even after digital compression at the upper right second premolar, second molar, and left canine areas, which presented severe inflammation. Although platelets were transfused repeatedly, platelet count did not stay elevated since survival rate of the transfused platelets were low due to alloimmunization. Thrombin gauze packing was not effective. Bleeding ceased 3 days after treatment with transfusion of donor platelets. 20 days after the treatment, the gingiva was generally healthy except upper right second premolar and lateral incisor areas. The result of periodontal treatment was good, but bleeding control after treatment was troublesome. In the periodontal treatment of patient with aplastic anemia, elevation of the platelet count with platelet transfusion seems to be the best method for hemorrhage control.

Changes in Acceleration at the Upper Thigh and Ankle with Variations in Gait Speed and Walkway Slope (보행 속도와 보행로 경사에 따른 대퇴상부와 발목상부에서의 가속도의 변화)

  • The purpose of this study was to investigate the effect of gait speed and walkway slope on the body acceleration, for the future validation of using an accelerometer in the estimation of energy consumption. Ten young healthy subjects with accelerometers on the upper thigh and ankle walked on a treadmill at 9 conditions(three speeds ${\times}$ three slopes) for 5 minutes. Acceleration signals of four directions, i.e. anterior-posterior(AP), medio-lateral(ML), superior-inferior(SI) and vector sum(VS) directions, of each sensor were measured, and root means squared(RMS) values of them were used as analysis variables. As statistical analysis, repeated measure two-way ANOVA was performed for RMS accelerations at each attachment sites, with slope and velocity as independent factors. At both the upper thigh and ankle, RMS acceleration of all directions were affected by gait velocities(p<.001) showing greater accelerations for higher velocities. Contrary to expectations, no slope effect existed in RMS accelerations at hip. Moreover, RMS acceleraion at ankle decreased with slope in SI and VS directions(p<.01). These results suggests that RMS acceleration cannot reflect the change in physical activity due to the change in walkway slope.

The Effect of a Piezoelectric Ultrasonic Scaler with Curette Tip on Casting Gold Removal in Vitro (큐렛팁을 장착한 압전방식 초음파치석제거기의 작업조건에 따른 치과주조용 합금의 삭제에 관한 연구)

  • Periodontal debridement is most important procedure in periodontal treatment, because periodontal disease is the biofilm infection. The use of ultrasonic instrument has many clinical advantages compared to classical hand instrument. The introduction of newly developed ultrasonic scaler tips made the use of ultrasonic scaler popular. However the study of tooth substance removal according to the working parameters of ultrasonic scaler with newly developed tips is not sufficient. The purpose of this study is to evaluate the effects of working parameters of piezoelectric ultrasonic scaler with curette tip on casting gold removal. The working parameters was standardized by the sledge device which controls lateral force(0.5 N, 1.0 N, 2.0 N) and power setting was adjusted 2, 4, 8 in P mode and S mode and instrumentation time was 5 seconds. The defect depth and width were measured with profile meter and defect surface was examined by SME. The depth of defect was significantly large in S mode( $39.58{\pm}19.35{\mu}m$) compared to P mode( $8.37{\pm}6.98{\mu}m$). There was significant decrease of depth of defect between 1.0N($32.87{\pm}27.18{\mu}m$) and 2.0N( $14.86{\pm}15.04{\mu}m$). The area of defect was also significantly large in S mode($4482.42{\pm}3551.71{\mu}m^2$) compared to P mode( $922.06{\pm}960.32{\mu}m^2$). There was significant decrease of area of defect between 1.0N($3889.12{\pm}3936.00{\mu}m$) and 2.0N( $974.66{\pm}986.01{\mu}m$). The change of mode did not effect on the width of the defect. The change of power setting did not effect on the depth, width, and area of defect. In spite of limitation of this study it could be concluded that the use of piezoelectric ultrasonic scaler with curette tip on S mode could make significant tooth substance loss.

  • Tooth impaction is defined as a cessation of the eruption of a tooth at the level of the oral mucosa or alveolar bone. Maxillary canines are the most frequently impacted teeth next to the third molar. Maxillary canine impaction is associated with congenital missing of lateral incisors, peg lateralis and genetic factors such as ectopic positioning of a tooth germ. The clinicians have an important role in early detection of tooth impaction for prevention of esthetic and functional problems. There are specific methods to treat impacted tooth for different conditions. In this case, an 11-year-old girl with a horizontally impacted maxillary right canine in a palatal position was treated through orthodontic traction along with surgical button attachment procedure. On regaining of eruption space, canine traction was performed. At the completion of treatment, the canine was positioned fairly within the arch with proper keratinized gingiva and complications such as root resorption were not observed.

Immediate implant placement and provisionalization of single implant in the esthetic region : Cases report (심미적 영역에서 즉시 임플란트 식립 및 임시 수복을 통한 단일치의 수복에 대한 증례 보고)

  • In the anterior maxillary area, dental implants for tooth replacement are challenging due to the need to satisfy high esthetic level as well as functionality. Immediate implant placement and provisionalization can dramatically reduce the edentulous period, and then fulfill patient's demand for esthetics. The aim of present case report is to demonstrate two cases that successfully restored single tooth with immediate implant placement and provisionalization in the anterior maxillary area. A 47 years old female was scheduled to replace her maxillary right central incisor due to crown-root fracture by trauma. Another 54-year-old female was planned to place dental implant following tooth extraction of maxillary right lateral incisor owing to continuous pus discharge despite repetitive treatments including apicoectomy. In these two cases, surgical and prosthetic procedures progressed in a similar way. After minimal flap elevation, atraumatic tooth extraction was performed. Implant was placed in proper 3-dimensional position and angulation with primary stability. Bone graft or guided bone regeneration for peri-implant bone defect was conducted simultaneously. Provisionalization without occlusal loading was carried out at the same day. Each definitive crown was delivered at 7 and 5 months after the surgery. Two cases have been followed uneventfully for 2 to 5 years of loading time. In conclusion, Immediate implant placement and provisionalization could lead to esthetic outcome for single tooth replacement with dental implant under proper case selection.

Supplementary Blue and Red Radiation at Sunrise and Sunset Influences Growth of Ageratum, African Marigold, and Salvia Plants

  • BACKGROUND: Light-emitting diodes (LEDs) with lower electric cost and the specific wavelength have been considering as a novel light source for plant production in greenhouse conditions as well as in a closed culture system. Supplementary lighting for day-length extension was considered as light intensity, light quality, and/or photoperiod control on plant growth and development. Effects of supplementary blue or red LED radiation with lower light intensity on growth of Ageratum (Ageratum houstonianum Mill., cv. Blue Field), African marigold (Tagetes erecta L., cv. Orange Boy), and Salvia (Salvia splendens F. Sello ex Ruem & Schult., cv. Red Vista) were discussed during sunrise and sunset twilight in the experiment. METHODS AND RESULTS: Supplementary lighting by blue and red LEDs for 30 (Treatment B30; R30) or 60 (Treatment B60; R60) min. per day were established in greenhouse conditions. Photosynthetic photon flux for supplementary radiation was kept at $15{\mu}mol\;m^{-2}\;s^{-1}$ on the culture bed. Natural condition without supplementary light was considered as a control. The highest shoot and root dry weights were shown in African marigold exposed by red light for 60 min. per day. Supplementary blue and red lighting regardless of the radiation time significantly stimulated development of lateral branches in African marigold. Stem growth in Ageratum and Salvia seedlings was significantly promoted by red radiation as well as natural light. CONCLUSIONS: Extending of the radiation time at sunrise and sunset twilight using LEDs stimulated reproductive growth of flowering plant species. Different characteristics on growth under supplementary blue or red lighting conditions were also observed in the seedlings during supplementary radiation.

Estimating Irrigation Requirement for Rice Cropping under Flooding Condition using BUDGET Model

  • This study explored the effect of rainfall pattern and soil characteristics on water management in rice paddy fields, using a soil water balance model, BUDGET. In two sites with different soil textural group, coarse loamy soil (Gangseo series) and fine soil (Hwadong series), respectively, we have monitored daily decrease of water depth, percolation rate, and groundwater table. The observed evapotranspiration (ET) was obtained from differences between water depth decrease and percolation rate. The root mean square difference values between observed and BUDGET-estimated ET ranged between 10% and 20% of the average observed ET. This is comparable to the measurement uncertainty, suggesting that the BUDGET model can provide reliable ET estimation for rice fields. In BUDGET model of this study, irrigation requirement was determined as minimum water need for maintaining water-saturated soil surface, assuming 100 mm of bund height and no lateral loss of water. The model results showed different water balance and irrigation requirement with the different soil profile and indicated that minimum percolation rate by plow pan could determine the irrigation requirement of rice paddy field. For the condition of different rainfall distribution, the results presented different irrigation period and amounts, representing the importance of securing water for irrigation against different rainfall pattern.

The effect of Glomus intraradices on the physiological properties of Panax ginseng and on rhizospheric microbial diversity

  • Background: Glomus intraradices is a species of arbuscular mycorrhizal fungi that, as an obligate endomycorrhiza, can form mutually beneficial associations with plants. Panax ginseng is a popular traditional Chinese medicine; however, problems associated with ginseng planting, such as pesticide residues, reduce the ginseng quality. Methods: In this experiment, we studied the effect of inoculating G. intraradices on several physiological properties and microbial communities of ginseng. UV-Visible Spectrum method was used to detect physical properties. Denaturing gradient gel electrophoresis method was used to analyze microbial communities. Results: The results indicated that inoculation with G. intraradices can improve the colonization rate of lateral ginseng roots, increase the levels of monomeric and total ginsenosides, and improve root activity as well as polyphenol oxidase and catalase activities. We also studied the bacterial and fungal communities in ginseng rhizospheric soil. In our study, G. intraradices inoculation improved the abundance and Shannon diversity of bacteria, whereas fungi showed a reciprocal effect. Furthermore, we found that G. intraradices inoculation might increase some beneficial bacterial species and decreased pathogenic fungi in rhizospheric soil of ginseng. Conclusion: Our results showed that G. intraradices can benefit ginseng planting which may have some instructive and practical significance for planting ginseng in farmland.

Differences in dentoskeletal and soft tissue changes due to rapid maxillary expansion using a tooth-borne expander between adolescents and adults: A retrospective observational study

  • Objective: The purpose of this study was to compare the differences in dentoskeletal and soft tissue changes following conventional tooth-borne rapid maxillary expansion (RME) between adolescents and adults. Methods: Dentoskeletal and soft tissue variables of 17 adolescents and 17 adults were analyzed on posteroanterior and lateral cephalograms and frontal photographs at pretreatment (T1) and after conventional RME using tooth-borne expanders (T2). Changes in variables within each group between T1 and T2 were analyzed using Wilcoxon signed-rank test. Mann-Whitney U test was used to determine the differences in the pretreatment age, expansion and post-expansion durations, and dentoskeletal and soft tissue changes after RME between the groups. Spearman's correlation between pretreatment age and transverse dentoskeletal changes in the adolescent group was calculated. Results: Despite similar amounts of expansion at the crown level in both groups, the adult group underwent less skeletal expansion with less intermolar root expansion after RME than the adolescent group. The skeletal vertical dimension increased significantly in both groups without significant intergroup difference. The anteroposterior position of the maxilla was maintained in both groups, while a greater backward displacement of the mandible was evident in the adult group than that in the adolescent group after RME. The soft tissue alar width increased in both groups without a significant intergroup difference. In the adolescent group, pretreatment age was not significantly correlated with transverse dentoskeletal changes. Conclusions: Conventional RME may induce similar soft tissue changes but different dentoskeletal changes between adolescents and adults.
