Subcutaneous Cysticercosis at Abdominal Wall: Case Report (복벽에 발생한 낭미충증 1례)

  • Human cysticercosis is an infestation with C. cellulosae, the larval stage of the tapeworm T. solium. It prevails in regions of poverty and where personal hygiene is poor. They can lodge in almost any tissue, but cysticerci are most frequently found in brain, skeletal muscle, subcutaneous tissues. We experienced a 41-year old male with $5{\times}12cm$ sized movable non tender brownish hard mass at lateral abdominal wall. The laboratory study didn't show any specific symptoms except peripheral blood eosinophilia and positive parasite ELISA screen for cysticercosis. The ultrasonogram revealed multiple ill-defined mixed echoic inflamatory lesion. Excisied cyst showed multiple severe foul-odor gelatinous subcutaneous mass. Microscopic examination disclosed necrotic body with foreign body reaction, massive eosinophilia and dead parasitic organism, compatible with cysticercosis. There was no evidence of recurrence during 12 months follow-up.

  • Enterotoxemia caused by Clostridium perfringens(Cl. perfringens)has occurred sporadically with devastating effect in Korean cattle. The disease is confirmed by detecting bacterial toxin the interstinal contents. however diagnosis has been challenging since the toxin is short lived and it must be demonstrated within one hour following death. The purpose of this paper is to provide additional clinicall and pathological information derived from observation of cattle administered with Cl. perfringens type A and/or its toxins isolated from natural case. Clinically cattle died suddenly in lateral recumbency with dyspnea tachycardia and muscle tremor without increased rectal temperature. hematology showed leukopenia with absolute decrease of all leukocytes. Blood glucose AST and ALT were increased. Grossly lungs were hyperemic haemorrhagic and edematous. Small intestine especially ileum was hyperemic and hemorrhagic with diffuse areas of necrosis. This study demonstrated that death without increased rectal temperature combined with leukopenia increased serum glucose pulmonary haemorrhage and edema plus necrotizing ileitis are important features associated with enterotoxemia due to Cl. perfringens type. Pediatric Orbital Medial Wall Trapdoor Fracture with Normal Computed Tomography Findings

  • With advances in diagnostic technology, radiologic diagnostic methods have been used more frequently, and physical examination may be neglected. The authors report a case of pediatric medial orbital trapdoor fracture in which the surgery was delayed because computed tomography (CT) findings did not indicate bone displacement, incarceration of rectus muscle, or soft tissue herniation. A healthy 6-year-old boy was admitted to the emergency room for right eyebrow laceration. We could not check eyeball movement or diplopia, because the patient was irritable. Thus, we performed facial CT under sedation, but there was normal CT finding. Seven days later, the patient visited our hospital due to persistent nausea and dizziness. We were able to perform a physical examination this time. Lateral gaze of right eye was limited. CT still did not show any findings suggestive of fracture, but we decided to perform exploratory surgery. We performed exploration, and found no bone displacement, but discovered entrapped soft tissue. We returned the soft tissue to its original position. The patient fully recovered six weeks later. To enable early detection and treatment, thorough physical examination and CT reading are especially needed when the patient shows poor compliance, and frequent follow-up observations are also necessary.

Endoscopic Endonasal Transsphenoidal Resection of Solitary Extramedullary Plasmacytoma in the Sphenoid Sinus with Destruction of Skull Base

  • Solitary extramedullary plasmacytomas are isolated plasma cell tumors of soft tissue that typically do not metastasize. They are rare and account for 4% of all plasma cell tumors. To our knowledge, only 14 cases of solitary extramedullary plasmacytomas in the sphenoid sinus have been reported. A 32-year-old man presented to our department with complaint of ocular pain in the right eyeball and diplopia. Physical and neurological examinations revealed intact and prompt direct and indirect light reflexes in both pupils and limitation of extraocular muscle movement seen with the lateral gaze of the right eyeball. Magnetic resonance imaging suggested the presence of mucocele or mycetoma, therefore surgical resection was performed with endoscopic endonasal transsphenoidal approach. Histopathology was consistent with plasmacytoma. Systemic work-up did not show any evidence of metastasis and the sphenoid sinus was the sole tumor site, and therefore the diagnosis of solitary extramedullary plasmacytoma was confirmed. We report a rare case of solitary extramedullary plasmacytoma in the sphenoid sinus with successful treatment using the endoscopic endonasal transsphenoidal resection and adjuvant radiotherapy.

  • The Temporomandibural joint(TMJ) is one of the most frequently used joint in the body as $1,500{\sim}2,000$ times per day for the activities of chewing, swallowing, talking, yawing and sneezing. The TMJ are formed by condylar process of mandible and mandible fossa of temporal bone, separated by an articular disc. This articular disc divides into two cavities as upper cavity and lower cavity. The gliding movement occurs in the upper cavity of the joint, whereas hinge movement occurs in the lower cavity. The movements that are allowed at the TMJ are opening, closing, protrusion, retraction and lateral movement. A cause of TMJ dysfunction are capsulitis, internal derangement, osteoarthritis, rheumatoid arthritis, infection and inflammation near the joint, trauma on joint, ankylosis, subluxation or dislocation of joint, injury of articular disc, myositis, muscle contracture or spasm, myofascial pain dysfunction syndrome, dyskinesia of masticatory muscles, developmental abnormality, tumor, connective tissue disease, fibrosis, malocclusion, swallowing abnormality, wrong habits such as bite nail or hair, bruxism, psycological stress and Costen syndrome etc. Assessment of TMJ dysfunction consist of interview, observation, functional examination, palpation, reflex test, joint play test, electromyography and radiologic examination and behavioral and psycological assessment etc.

The Effects of Trunk Strengthening Program in Gait and Spinal Segment with Spastic Cerebral Palsy (청소년기 경직형 뇌성마비아동의 체간강화 운동이 척추분절 및 보행에 미치는 영향)

  • Purpose : This study aims to examine effects of trunk strengthening exercise in three rigid cerebral palsy adolescents aged between 11 and 13 on their spinal segments and gait. Methods : 2A trunk strengthening program for proprioceptive neuromuscular facilitation was made focusing on improvement in their school life and mental and social activities during their school years, and then trunk strengthening exercise was applied to the three subjects for 30 minutes five times per week for four weeks. With the subjects in a static state, each group's gait analysis was made and maximum anterior flexion, maximum extension, maximum left and right lateral flexion, and cross rotation of the trunk were measured using Sonosens (Germany) prior to and after the intervention. Results : Assessment of spinal segment mobility with the subjects in a static state showed that the trunk muscle strengthening exercise increased their maximum joint movement angles from the right side to the center and rotational abilities of the cervical and lumbar spines. Gait analysis indicated increased movements in the thoracic and lumbar spines and relatively decreased anterior-posterior movement of the cervical spine. Conclusion : Trunk strengthening program is effective in enhancing spinal segment mobility and balance ability of cerebral palsy adolescents and considered able to be used together with diverse treatment interventions.

Development of a Rehabilitation Robot for Mckenzie Cervical Exercise (경부 맥킨지 운동용 재활로봇의 개발)

  • In this paper a cervical rehabilitation robot for Mckenzie exercises to be effective to neck pain relief is proposed. The robot has two degrees of freedom (DOF) for Lateral flexion and extension, Dorsal and Vental flexion which enable user to perform cervical stretching and isometric exercises for neck muscles. The mechanical parts of the cervical rehabilitation robot can be mounted on a back- or head-rest of chair, and user can perform the Mckenzie exercise with seated. In experiments we measured the range of motion of cervical part, EMG signals from neck muscles and the contact forces of a head bracket fixing the head part of user, and then evaluated their performances. From the experimental results, we showed a feasibility of the cervical rehabilitation robot proposed in this study.

Effects of Plantar sole Vibration using Various Frequencies on Postural Response During Standing (기립상태에서 발바닥에 인가한 진동자극의 주파수에 따른 자세균형 응답)

  • We studied the postural response induced by plantar sole vibration with various frequencies(20, 60, 100Hz) and vibration zone(the anterior and posterior foot zone) of both soles during standing. Eight healthy young adults were exposed to 15s periods of plantar sole vibration while blindfolded. Body sway(COM, center of mass), the angle of neck, trunk, hip, knee, ankle and EMG of four lower limb muscles(tibialis anterior, lateral and medial gastrocnemial, soleus muscle) were recorded during 15s plantar sole vibration using 3D motion analysis system. Simulating each zone separately resulted in spatially oriented body tilts; oppositely directed backward and forward, respectively, the amplitude of which was proportional to the vibration frequency. EMG activity of lower limb muscles also varied according to the direction of the vibration zone and linearly according to the frequency. These findings led us to consider the plantar sole vibration as useful method of postural balance control and adjustment.

  • Microvascular free tissue transfer technique is widely accepted for reconstruction of extensive soft tissue defects on the extremities. The system of flap based on the subscapular artery and vein provides the widest ways of composite free flaps. The possible flaps that can be harvested based on this single vascular pedicle include the scapular and parascapular skin flaps, the serratus anterior and latissimus dorsi muscular flaps, the lateral scapular bone flap, the latissimus dorsi-rib flap, and the serratus anterior-rib flap. This combined flap is available to mutiple tissue defects or complex defects because it can incorporated with skin, muscle and bone flaps. A strikig advantage is the independent vascular pedicles of each components, which allow freedom in orientation of each components. So, it can be freely applied to any forms of three demensional defects on the upper and lower extremities. The combination of scapular cutaneous flap and latissimus dorsi musculocutaneous flap can be resurfaced for massive cutaneous defects on the extremities. We report the use of the combined scapular and latissimus dorsi free flap in seven patients to reconstruct massive deefcts on the extremities. There was no flap failure and little complications and disadvantages. The anatomy of this flap is reviewed and the indication and advantages are discussed.

An Intrathyroidal Branchial Cleft Cyst and a Perithyroidal Branchial Cleft Cyst (갑상선 내부 및 갑상선 주위에 발생한 새열낭종)

  • Branchial cleft cysts typically are characterized as lateral swellings anterior to sternocleidomastoid muscle in upper third of the neck. However, cysts have been reported in unusual location such as the thymus, oral cavity, parotid gland, pancreas, and thyroid. Perithyroidal branchial cleft cysts are also rare and preoperative diagnosis is very difficult. Recently we have experienced a case of intrathyroidal branchial cleft cys and a case of perithyroidal branchial cleft cyst, which were diagnosed preoperatively as the parathyroid cyst. So, we report these two cases with review of the literatures.
