Analysis of Transplantation of 99 Free Flaps (유리조직 이식술 99례 분석)

  • Free flap transplantation demands meticulous microsurgical technique to cover the exposed vital structures which is important to restore and maintain functions of the extremities. From July 1992 through December 2000, 99 patients were received reconstructive microsurgery in the upper and lower extremity at Department of Orthopedic Surgery, Chonbuk National University Hospital. The most common cause in the upper extremity was industrial accident, 8 cases of total 15 cases and in the lower extremity was traffic accident, 66 cases of total 84 cases. The most commonly involved site was thumb and finger, 8 cases of total 15 cases and in the lower extremity was leg, 65 cases of total 84 cases. In upper extremity, the wrap around free flap was carried out in 4 cases(4.0%), first dorsal metatarsal artery flap and lateral arm flap were 3 cases(3.0%) each in 15 cases and in lower extremity, latissimus dorsi myocutaneous flap were 23 cases(23.2%), gracilis 20cases(20.2%), and rectus abdominis muscle flap 18(18.2%) in 84 cases. Overall 89 cases(89.9%) of 99 cases were survived and maintained and revealed good cosmetic results.

Key-hole Technique in Treatment of A-C Dislocation - Preliminary Report - (Key-hole 술식을 이용한 급성 견봉쇄골관절 탈구의 치료-예비보고-)

  • The results of the operative treatment of the Grade III acromioclavicular joint injury is defined by the durability of the reduced joint and free of exertional pain. Several surgical techniques have been applied to reduce and stabilize the joints effectively. Resection of clavicular lateral end and subacromial decompression also could be applied to prevent post-operative arthritic change. Biomechanical studies reveals the role of clavicular elevation and rotation to achieve more than 90 degrees of elevation. It also serves as a attachment site of deltoid and trapezius muscle. The stability and mobility of the both acromioclavicular and coracoclavicular joint are important to get full functional recovery. We modified the methods of coracoacromial ligament transfer described by Weaver-Dunn and by Shoji et a!. to pre­vent pullout of the transferred ligament and to get more improved functional results. Main technical point was harvesting full thickness bone block and fix it through the key-hole to reduce pull out angle.

Activation of the Triceps Surae During Heel Raising Depend on the Knee Joint Flexion Angles (무릎관절 굽힘 각도에 따른 뒤꿈치 들기 동안 종아리 세갈래근의 활성도)

  • PURPOSE: The purpose of this study was to investigate the change of triceps surae activation during heel raise test in standing among knee flexion angles($0^{\circ}C$, $30^{\circ}C$, $45^{\circ}C$, $60^{\circ}C$). METHODS: Twenty healthy individuals performed unilateral plantarflexion in standing with $0^{\circ}C$, $30^{\circ}C$, $45^{\circ}C$, $60^{\circ}C$ knee flexion. Activity of medial gastrocnemius(MG), lateral gastrocnemius(LG), soleus(Sol) was recorded with surface electromyography(EMG). RESULT: The muscle activations induced the four different positions were compared and results showed that was significant difference MG and LG while the angle increase from $0^{\circ}C$ to $30^{\circ}C$, $45^{\circ}C$, $60^{\circ}C$ but Sol did not show significant differences in every angle. CONCLUSION: This study suggest that $30^{\circ}C$ knee flexion is required to induce a significant mechanical disadvantage of gastrocnemius.

Berlin standard activator in the treatment of growing patients with mandibular deficiency: Case report (성장기 하악골 열성장 환자의 Berlin standard activator를 이용한 부정교합 치료: 증례보고)

  • Activator is a removable functional appliance used for correcting the skeletal Class II malocclusion in children with the mandibular deficiency. Berlin standard activator modified from Andresen activator has following characters; do not cover the palatal surface for tongue space, relief on lingual surface of mandibular incisors and resin capping 1/3-1/2 of crown height on mandibular incisors for preventing labioversion of mandibular incisors, L-hook between maxillary lateral incisor and canine for anterior high pull headgear, relief on mandibular posterior bite block for differential eruption of posterior teeth. Two cases presented here had a mandibular deficiency and slight maxillary protrusion. First case (an 11-year-old girl) treated with Berlin standard activator and anterior high pull headgear for 13 months followed by fixed orthodontic appliance for another 29 months. Second case (a 12-year-old boy) treated with Berlin standard activator for 6 months followed by fixed appliance for another 24 months. Treatment results showed a significant improvement in sagittal skeletal and occlusal relationship without premolar extraction. Mandibular condyles were concentric in TMJ [ossa, and masticatory muscle activities were normalized after treatment. In the retention period facial harmony and occlusal stability was maintained.

  • Rectus abdominis syndrome is the abdominal pain which occurs in the distribution of the medial or lateral cutaneous branch of the 7~12th intercostal nerves. It is frequently cause that results in unnecessary pain and expense to patient. The physical examination is difficult because of severe abdominal pain. We must have attention to the possibility that patients with abdominal pain, in whom no intra- abdominal cause is founded, may suffer from this presumed nerve entrapment syndrome. If we can find the cause of pain in the abdominal rectus muscle, no evaluation and surgery are required and therapy can be simple.

  • Atlantoaxial rotatory fixation (AARF) in adult is a rare disorder that occurs followed by a trauma. The patients were presented with painful torticollis and a typical 'cock robin' position of the head. The clinical diagnosis is generally difficult and often made in the late stage. In some cases, an irreducible or chronic fixation develops. We reported a case of AARF in adult patient which was treated by immobilization with conservative treatment. A 25-year-old female was presented with a posterior neck pain and limitation of motion of cervical spine after a traffic accident. She had no neurological deficit but suffered from severe defect on the scalp and multiple thoracic compression fractures. Plain radiographs demonstrated torticollis, lateral shift of odontoid process to one side and widening of one side of C1-C2 joint space. Immobilization with a Holter traction were performed and analgesics and muscle relaxants were given. Posterior neck pain and limitation of the cervical spine' motion were resolved. Plain cervical radiographs taken at one month after the injury showed that torticollis disappeared and the dens were in the midline position. The authors reported a case of type I post-traumatic AARF that was successfully treated by immobilization alone.

  • New chords formation of mitral valve using expended-polytetrafluoroethylene sutures was performed in six patients with mitral regurgitation (MR) from April, 1994 to March, 1995. There were 3 men and 3 women whose mean age was 65 years, ranged from 55 to 75. A double-armed pledget suture was passed through the lateral portion of papillary muscle head and tied. Each arm of the suture was brought up to the free margin of the leaflet and passed through the area where the native chorda was attached. At leaflet surface, one more weaving was made. fter the lengths of the two arms were adjusted, the ends were tied. There were no operative deaths. Patients have been followed up from 2 to 13 months, mean 8. Echocardiographic sutudies were performed at 10 days after operation and revealed normal mitral valve function in all patients.

  • Objective: This report was designed to investigate characteristic of ankle sprain due to cause, degree of injury, treatment process, sex, age etc Methods: We selected 36 patients who visited our clinic at last over two times complaining of ankle-sprain since 8th, March, 2002. Results: The results were summarized as follows. 1. The major cause of ankle-sprain was injury of lateral collateral ligament. 2. Ankle-sprain was distinguished three degree, and generally presented pain, regional tenderness, swelling, limited movement, muscle spasm, redness, deformity. 3. In early stage of ankle-sprain, ice massage, compression, elevation, rest were very important. 4. In oriental medicine, the principle of treatment were promoting blood circulation to remove blood stasis, relaxing muscles and tendons and activating the flow of Gi and blood in the channels and collaterals, reducing edema or swelling, and alleviating pain. 5. There were more effective result to using three-edged needle with acupuncture. Conclusions: Ankle-sprain were treated successfully using acupuncture and three-edged needle.

  • There are many causes of trismus. Aetiology can be roughly divided into muscle spasm, mechanical interference, extracapsular ankylosis, intracapsular ankylosis. Trismus caused by mechanical interference between postero-lateral wall of zygoma-maxillary complex and coronoid process following reduction of fractured facial bone is rare. Especially on maxillary bone fracture, when we faced the trismus following removal of intermaxillary fixation, we got used to solve that problem by physical exercise. We obtained good results by coronoidectomy on patients with limited mouth opening who were referred from department of plastic surgery, St. Marys' hospital, the Catholic university of Korea. We report our experience with literature review.

Response of masticatory muscles to passive stretch stimulus - from perspectives of functional appliances

  • Objective: The aims of this study were to examine whether a passive stretch stimulus by means of a functional appliance induces changes in the fiber composition of masticatory muscles and whether these changes are similar to the changes in stretched limb muscle fibers by using RT-PCR, western blot, and immunohistochemical assays. Methods: Five male New Zealand White rabbits were fitted with a prefabricated inclined plane on the maxillary central incisors to force the mandible forward (- 2 mm) and downward (- 4 mm). Further, 1 hind limb was extended and constrained with a cast so that the extensor digitorum longus (EDL) was stretched when the animal used the limb. The animals were sacrificed aft er 1 week and the masseter, lateral pterygoid, and EDL were processed and compared with those from control animals (n = 3). Results: The stretched EDL had a significantly higher percentage of slow fibers, whereas the stretched masticatory muscles did not show changes in the composition of the major contractile proteins aft er 7 days. Conclusions: The transition of fiber phenotypes in response to a stretch stimulus may take longer in the masticatory muscles than in the limb muscles.
