A Study on Actual Status of Acupuncture Therapy Pediatric Practice (임상 진료에서 소아질환(小兒疾患)에 대한 침술치료(鍼術治療)의 현황 연구)

  • Objectives : This study was performed to evaluate a recent status of acupunctural treatment in pediatric practice. Methods: 660,893 children, who visited HAMSOA oriental clinic from January 2004 to December 2005 was screened to decide the number treated with acupuncture, the diagnosis and the type of acupuncture. In addition, the questionnaire for clinician about acupuncture therapy was performed and analysed to understand their applications. Results: The number treated with acupuncture was 69,797, 10.56%. Classified with ages, 3 months and below was 729 (1.7%), 1 year - 3 months was 5,035 (11.80%), 2 year - 1 year was 6,875 (16.10%), 3 year - 2 year was 5,540 (12.98%), 6 year - 3 year was 12.636 (31.94%) and 6 year and above was 10,876 (25.48%). The most frequent diagnosis taken acupuncture was atopic dermatitis (14.93%) and the next was rhinitis (13.80%). The most frequent type of acupuncture was laser acupuncture. The clinicians answered questionnaire agreed that acupuncture therapy is effective for pediatric disease treatment and fear about acupuncture of young patients was the most important obstacle. Conclusion : The acupuncture therapy in pediatric practice is not so frequent treatment but in increasing stage. Futhermore, in-depth research about acupuncture therapy in pediatrics is demanded.

Effects of Acupuncture, Electro-acupuncture, Low Level He-Ne Laser Therapy at Oe-gwan($TE_5$) ${\cdot}$ Chogimup ($GB_{41}$) on L5 Spinal Nerve Ligation Model in Rats (건측(健測) 취혈(取穴) 다종(多種) 침자법(鍼刺法)이 백서(白鼠)의 신경병리성(神經病理性) 동통(疼痛)에 미치는 영향)

  • Objectives : To find effects of acupuncture, electro-acupuncture, low level He-Ne laser therapy(LLLT) at $TE_5$, $GB_{41}$ in the neuropathic pain. We made experiment on rats ligated L5 spinal nerve like general herniation of nucleus pulposus(HNP). Methods : A model of neuropathic pain was made by isolating Left 5th lumbar spinal nerve of rats. Three days after the neuropathic surgery, acupuncture and LLLT, electro-acupuncture was injected at $TE_5$, $GB_{41}$ one time a day for a week. Each group was divided two. one is opposite side performed the surgery which is right, another is left side performed the surgery. After that, the author examined the withdrawal response of neuropathic rats' legs by van Frey filament and acetone stimulation. And also the author examined c-Fos, Nociceptin and KOR-3 in the midbrain central gray of neuropathic rats. Results : As we have observed the effect of mechanical allodynia, LT-R group were diminished on 6th day compared with control group, EA-L group, EA-R group and LT-L group were diminished on 7th day compared with control group. As we have observed the effect of cold allodynia, EA-R group were diminished on 6th day, 7th day compared with control group. As we have observed the effect of activity of c-Fos in the central gray part, EA-R group and LT-R group were diminished compared with control group. As we have observed the effect of activity of Nociceptin in the central gray part, EA-R group were a little increased compared with control group but it is not reliability. As we have observed the effect of activity of KOR-3 in the central gray part, EA-R group were significantly increased compared with control group. Conclusions : We have noticed that effect of acupuncture at opposite side of sickness and powerful stimulation could be more effective, because of EA-R group have more controllable effect all test we have done on the other hand EA-L group have only effect on mechanical allodynia. This study can be used in clinical therapy for neuropathic pain. But it is not reliability that Nociceptin have effectively to control pain. Therefore We have to follow up about that.

  • Background and Objectives This study aimed at examining the effects of photobiomodulation therapy (PBMT) on glucose, cholesterol, triglycerides, and low- and high-density lipoprotein (LDL and HDL, respectively) levels in vitro. Materials and Methods A total of 38 serum samples collected in plain (n=10) and heparinized tubes (n=28) were subjected to PBMT at 60 Joules (J)/cm2 for 2 min at 810 nm. The glucose and lipid profiles, cholesterol, triglycerides, LDL, and HDL of each sample was measured before and after PBMT. Results A statistically significant increase in glucose levels was observed in the PBMT-sera in 8 out of 10 samples in plain tubes. However, only two samples that were prepared in heparinized tubes showed an increase in glucose levels. The remaining heparinized samples that were exposed to PBMT presented lower glucose values. The treated sera exhibited a fluctuation in the lipid profiles after PBMT. However, high cholesterol levels were evident following PBMT. Similar trends with HDL and LDL in heparinized tubes were evident. Conclusion Together, the findings suggest that photobiomodulation exhibits an effect on glycemic and lipid profiles in vitro. Hence, the use of low-level laser therapy could have therapeutic potential. However, the differences between individual responses appear to indicate that the impact of PBMT may not always be beneficial.

The impact of head repositioning accuracy and proprioception on cervical stabilization exercise in healthy adults

  • Objective: Cervical stabilization exercises are frequently to improve strength and endurance of cervical muscles. The purpose of this study was to identify changes in head repositioning accuracy (HRA) and neck proprioception through cervical stabilization exercises in healthy adults. Design: One group pretest-posttest design. Methods: Thirteen participants with no previous history of neck pain or injury to the cervical spine were recruited. HRA was measured by equipment including laser pointer, helmet, eye patch and marking pens. The distance between the spot where the beam had stopped and the center of the graph paper was measured three times with the averaged value used as the head repositioning accuracy. Neck proprioception was measured by a cervical range of motion device (CROM). Subjects wore the CROM tester and were to look straight ahead while bending his/her neck. Subjects were instructed to perform extension, lateral flexion and rotation, and the values were then measured and recorded. The measurements were performed pre-intervention, and after cervical stabilization exercise. Results: There was no significant difference on HRA after intervention. In addition, there was no significant difference on neck proprioception compared with pre-intervention. Conclusions: The present study did not identify any effect on HRA and neck proprioception of cervical stabilization exercise. Further investigations are required to elucidate this in old aged participants and patients with neck pain.

Low-level laser therapy affects osseointegration in titanium implants: resonance frequency, removal torque, and histomorphometric analysis in rabbits

  • Objectives: The purpose of this study was to investigate the effects of low-level laser therapy (LLLT) with a diode gallium-aluminum-arsenide (Ga-Al-As) low-level laser device on the healing and attachment of titanium implants in bone. Materials and Methods: Thirteen New Zealand white male rabbits weighing $3.0{\pm}0.5kg$ were used for this study. Dental titanium implants (3.75 mm in diameter and 8.5 mm in length, US II RBM plus fixture; Osstem, Seoul, Korea) were implanted into both femurs of each rabbit. The rabbits were randomly divided into a LLLT group and a control group. The LLLT was initiated immediately after surgery and then repeated daily for 7 consecutive days in the LLLT group. Six weeks and 12 weeks after implantation, we evaluated and compared the osseointegration of the LLLT group and control group, using histomorphometric analysis, removal torque testing, and resonance frequency analysis (RFA). The results were statistically significant when the level of probability was 0.05 or less based on a non-parametric Mann-Whitney U-test. Results: The implant survival rate was about 96%. Histologically and histomorphometrically, we observed that the titanium implants were more strongly attached in LLLT group than in control group. However, there was no significant difference between the LLLT group and control group in removal torque or RFA. Conclusion: Histologically, LLLT might promote cell-level osseointegration of titanium implants, but there was no statistically significant effects.

AN EFFECT OF Nd : VAG LASER IRRADIATION ON THE MICROHARDNESS OF ROOT SURFACE (Nd : YAG layer 조사가 치근면의 미세경도에 미치는 영향)

  • Root caries is very frequently developed on exposed root surface after periodontal surgical treatment. In order to determine the anti-caries effect of Nd : YAG laser irradiation on periodontally exposed root surface, 40 mandibular molar teeth that had been extracted due to excessive periodontal destruction were used as the experimental teeth. All teeth were treated by the same procedure as conventional periodontal root treatment, ie thorough scaling, root planing and root conditioning with tetracycline HCl(100mg/ml, 5min.). Within middle one third of root, mesial half surface(20) or distal half surface(20) was randomly irradiated at various power of 1.0W, 2.0W, 3.0W and 4.0W for 60 seconds by non-contact(5mm) delivery of a pulsed Nd : YAG laser(EN.EL.EN060, Italy). The microhardness was measured by Vikers microhardness tester(Wilson, USA) at 2mm/second of jog speed under 100gm load. The difference of microhardness between irradiated side and non-irradiated side was statistically analyzed ANOVA and Duncan's method. Following results were obtained ; 1. The microhardness(Knoop hardness number) was significantly higher in laser irradiated surface than non-irradiated surface(p<0.05). 2. There was no significant difference in microhardness between experimental groups classified by different laser power(p>0.1). The results suggest that Nd : YAG laser irradiation on exposed root suface after periodontal therapy may inhibit the root caries development by enhancing surface microhardness.

Synergistic Effects of Cisplatin-epigel and Interstitial KTP Laser Treatment on a Xenografted Squamous Cell Carcinoma

  • Background and Objectives Cisplatin is an important chemotherapy drug for the treatment of head and neck cancer. Interstitial laser treatment (ILT) has cosmetic utility and is very important for maintaining the function of the head and neck after cancer treatment. This study examined the synergistic effects of locally injected cisplatin-epigel and high fever induced by an interstitial potassium titanyl phosphate (KTP) laser treatment on a xenografted human Heinz squamous cell carcinoma. Materials and Methods SNU-1041 (107 cells/0.1 ml) cells were xenografted into the back of nude mice by subcutaneous injection. The ILT group (n = 10) was treated with a KTP laser (1 J/mm3) through a cylindrical diffuser tip inserted into the tumor, monitoring the temperature at 43-45℃. In the combined treatment group (n = 10), local hyperthermia was induced by intratumoral injection of 100-200 ㎍ of cisplatin into a collagen-based gel carrier (cisplatin-epigel), which was released slowly four hours before ILT. After four weeks of follow-up, the treated tumors were evaluated for tumor remission and volume change. Results Eight (80%) of the combined group showed complete tumor remission at the four-week follow-up, whereas only three (30%) of the ILT group showed remission (30%) (p < 0.01). Conclusion The current study has shown the synergistic effects of a local cisplatin injection and high fever from ILT on a xenografted human Heinz squamous cell carcinoma.

Photodynamic Inactivation of Moraxella catarrhalis (Moraxella catarrhalis의 광역학적 비활성화)

  • The aim of this study was to evaluate the bacterial effects of Moraxella catarrhalis in otitis media with effusion (OME) by photodynamic therapy (PDT). Bacterial suspensions (10000 CFU/mL) were prepared. The colony forming units (CFU) of Moraxella catarrhalis have been measured after an application of photogem plus 632 nm diode laser irradiation. One ml of the bacterial suspensions have been incubated in the dark for 3h with various concentrations of photogem ($0.625{\sim}5.0_{\mu}g/mL$) and then irradiated with 632 nm diode laser ($15J/cm^2$). After, the PDT Moraxella catarrhalis suspensions ($50{\mu}L$) were inoculated on chocolate agar plate and cultured in the dark at $37^{\circ}C$, 5% $CO_2$ condition for 18h. The colony forming units off the bacteria were measured. Also transmission electron microscopy (TEM) was employed to evaluate the effect of otitis media pathogens by PDT. The nucleus of Moraxella catarrhalis was stained using green fluorescent nucleic acid dye thiazole orange and the fluorescence intensity of the nucleus was measured by flow cytometry. The PDT was effective in killing Moraxella catarrhalis at the photogem dose of $5.0_{\mu}g/mL$, respectively, As assessed by flow cytometry analysis the fluorescence intensity of the nucleus got lower after PDT. TEM result appeared to able to cause damage to the bacterial membranes. On the basis of these findings, bacterial photodynamic therapy with photogem can be considered to be a promising new therapeutic approach for OME.

  • Objectives : This research was performed to investigate the effect of acupuncture using invasive low level laser therapy (LLLT) at Yolgyol (LU7) + Yogu (LR5) on weight gain, food intake, food efficiency, lipid metabolism, atherogenic index, HTR (HDL-cholesterol to total cholesterol ratio) and morphological change of hepatic tissue in hyperlipdemia rats. Methods : Experimental groups were divided into high fat diet group (Control group), high fat diet and acupuncture therapy group at LU7 + LR5 (AT group), high fat diet and acupuncture group using 10 mW LLLT at LU7 +LR5 (LA10 group), high fat diet and acupuncture group using 20 mW LLLT at LU7+LR5(LA20 group), high fat diet and acupuncture group using 60 mW LLLT at LU7 + LR5 (LA60 group), once per 3 days during 9 weeks. Results : Body weight was decreased significantly in AT and LA20 groups compared with Control group. Food intake was increased significantly in LA60 group compared with Control group. Food efficiency was decreased significantly in LA10, LA20 and LA60 groups compared with control group. In the lipid metabolism, total cholesterol was decreased significantly in AT, LA10, LA20 and LA60 groups, triglyceride was decreased significantly in LA10, LA20 and LA60 groups, TG/HDL-cholesterol ratio was decreased significantly in LA 60 group compared with control group. In the morphological change, hepatic tissue were not showed balloning degeneration and irregular arrangement of hepatic cell in LA10 and LA20 groups with control group. Conclusions : Acupuncture using LLLT at LU7+LR5 can manage hyperlipemia by controlling body weight, food intake, food efficiency ratio and lipid metabolism.
