Polarization Spectral Imaging System for Quantitative Evaluation of Port Wine Stain Blanching Following Laser Treatment

  • Objective methods to assess quantitatively port wine stain (PWS) blanching in response to laser therapy are needed to improve laser therapeutic outcome. Previous studies have attempted to assess objectively PWS color based on point measurement devices. To date, these approaches have typically been limited by a number of factors such as small test area and need for contact. To address these issues, a polarization spectral imaging system and an image analysis method have been developed to evaluate quantitatively erythema and melanin content distribution in skin. The developed polarization spectral imaging system minimizes artifacts such as glaring, shadowing, and non-uniform illumination that interfere with image fidelity. Furthermore, the image analysis method has been employed to get images of skin melanin and erythema indices from the acquired color images for quantitative analysis. Finally, using PWS patient color image, the effectiveness in laser treatment of PWS was evaluated by calculating relative erythema index image that is the relative erythema index of PWS region to the normal region. The developed device and analysis method appears to be a simple and effective method for quantitative analysis of PWS blanching.

Post Treatment Application of Jaungo after a Combined Therapy of Carbon Dioxide Laser and Trichloroacetic Acid in a Case of Vulvar Syringoma

  • Syringoma is a benign eccrine sweat gland tumor that predominantly appears in females during puberty with multiple smooth papules measuring 1-2 mm in diameter. Common locations are on lower eyelids and cheeks. Vulvar syringoma is quite a rare, occurring condition with only a few cases reported. Here, we are reporting a case of 31-year-old female with vulvar syringoma associated with pruritus. The lesion was treated with carbon dioxide (CO2) laser ablation and 50% trichloroacetic acid (TCA) chemical peeling. Jaungo was used for wound care after laser abrasion. The combination treatment was effective for removing syringoma lesions. Post laser management with fusidic acid cream and jaungo cream was sufficient to prevent infection and promote wound healing.

  • A 38-year-old man visited the hospital for a voice change that began two months previously. There were no other accompanying symptoms; the patient had a history of 20 years of smoking. Carbon dioxide (CO2) laser cordectomy was performed on the left side of the vocal cord, and the biopsy results showed squamous cell carcinoma. Radiation therapy was not performed, and CO2 laser cordectomy was performed again because of a recurrence of left vocal cord mass three years after a progress observation.

  • Tracheal granuloma, the most commonly reported sequela of pediatric tracheotomy. A variety of techniques are available for the management of tracheal granuloma. Potassium-titanyl-phosphate (KTP) laser has been previously established as an acceptable technique for removal of laryngeal surgery, which emits a green light with a wave length of 532 nm, which is well-absorbed by hemoglobin and can coagulate and vaporize tissue. The ability to deliver laser energy through a flexible glass fiber makes the technique convenient for use with a rigid bronchoscope, overcoming problems with intraluminal access encountered with earlier attempts at CO2 laser therapy for this problem. Another advantage of KTP laser is the avoidance of the risks and morbidity associated with an open procedure. We report our surgical technique KTP laser in the management of tracheal granuloma removal into the tracheostomy site. KTP laser is good tool for management of tracheal granuloma with low incidence of complications.

The Effect of Heat Therapy on Cutaneous Blood Flow and Skin Temperature at Pre-auricular Region (온열요법이 전이부의 표층부 혈류량과 피부 온도에 미치는 영향)

  • The purpose of this study was to assess the effect of heat therapy on cutaneous blood flow and skin temperature at pre-auricular region. Moist heat therapy and ultrasound therapy were applied to 20 healthy subjects(male: 10, female: 10). Cutaneous blood flow and skin temperature before and after heat therapy were measured with laser doppler flowmetry and thermocouple. The results were as follows ; 1. Cutaneous blood flow and skin temperature were significantly increased after moist heat therapy and ultrasound therapy. 2. In application of moist heat therapy, cutaneous blood flow and skin temperature were more increased and maintained longer than in ultrasound therapy. 3. Before heat therapy, cutaneous blood flow and skin temperature were higher in male. 4. There was no significant gender difference in changes of cutaneous blood flow and skin temperature after heat therapy. In conclusion, both moist heat therapy and ultrasound therapy increased cutaneous blood flow and skin temperature significantly, and moist heat therapy was more effective to increase cutaneous blood flow and skin temperature and to maintain increased cutaneous blood flow and skin temperature. There was no significant gender difference in the effect of heat therapy on cutaneous blood flow and skin temperature.

A Clinical Study of InGaAlP Laser Transcutaneous Blood Irradiation on Heart Rate Variability in Healthy Adults (II) (InGaAlP 레이저 경피혈액조사가 정상성인의 심박변이도에 미치는 영향에 대한 임상적 연구(II))

  • Objective : The heart rate variability is very useful indicator to study the function of the autonomic nervous system(ANS), and the physiologic signals can be observed based on the changes of the ANS of the heart. In order to assay the effects of the laser exposing to healthy subjects, the double blind test has been performed. Methods : This study included 62 healthy adults who have not any ANS disease and had normal sinus rhythm in electrocardiogram. The control group consisted of 31 subjects, laser group consisted of 31 subject. HRV was measured for 5 minutes before laser irradiation, sham and real laser irradiated for 30 minutes and than HRV remeasured for 5 minutes. Statistical significance was evaluated by independent T-test. Results : Mean HRV, Ln(VLF), Ln(HF), Ln(TP) of both groups at post-laser period decreased compared with that of the pre-laser period. Ln(LF) of both groups at post-laser period increased compared with that of the pre-laser period. LF/HF, SDNN of real laser group decreased and sham group decreased. Conclusions : There is no difference between two groups. The reason is presumed that all the studied subjects are healthy adults, and also the short and single transcutaneous laser irradiation would not influence upon changes of the ANS. The further study must be followed.

Stimulatory Effect of Ga-As Infrared Laser on the Regeneration of Injured Sciatic Nerves (손상된 좌골신경의 재생에 미치는 Ga-As 적외선 레이저의 효과)

  • The purpose of this study was to examine the effect of Ga-As(Gallium-Arsenide, wave length; 904 nm) infrared laser irradiation on healing of the experimentally crush injured rat sciatic nerves. The bilateral sciatic nerves of 43 adult male Sprague-Dawley rats were compressed surgically with a straight hemostat (1 mm width). The right legs of all the rats were irradiated using a 27 mW Ga-As infrared laser (laser irradiated group). The radiation procedure was administered for 3 minutes every day for 1, 3, 5, and 7 weeks in each group. Left legs were not irradiated and served as the control group. The numbers of total myelinated axon and degenerated myelin in the sciatic nerves of bilateral legs were measured and analyzed with mage analysis system in order to make a morphological analysis of the effect of the Ga-As infrared laser on injured nerves. Total number of myelinated axons was increased with time interval, especially in the 1, 3. and 5 week of irradiated group. Conversely, the number of degenerated myelin was decreased with time interval, especially in the irradiated group. The effects in the irradiated group were more pronounced than those of the control group. In conclusion, the Ga-As infrared laser irradiation is a useful adjuvant therapy to the regeneration of the peripheral nerve injury.

Efficacy of 694-nm Fractional Toning Ruby Laser in the Treatment of Malar Melasma

  • Background and Objectives Among the numerous available therapies, the usefulness of laser treatment has been proven in melasma, especially for refractory to topical agents or chemical peels. This study evaluated the effectiveness of 694-nm wavelength fractional toning ruby laser in melasma presenting on the malar area. Materials and Methods This was a retrospective study of 40 melasma patients treated with 694-nm wavelength ruby fractional toning laser for a total of 8 cycles at intervals of 2 to 3 weeks. Two independent investigators assessed the photographic findings and evaluated the severity of melasma by applying the Melasma Severity Scale (MSS). Subject satisfaction score was also surveyed at every visit. Results After the 4th treatment, subjects were continuously satisfied with their results. At the 8th visit and 1-month after the last treatment, MSS was determined to be significantly improved as compared to the first visit. One case of minor hypopigmentation was reported, but was resolved after one month. Conclusion Application of 694-nm wavelength fractional toning ruby laser for treating melasma on the malar area showed significant improvement in the MSS. Moreover, the treatment could be repeatedly applied, with low complication rate and significant patient satisfaction.

GafChromic RTQA Film Dosimetry for Laser Beam with Photodynamic Therapy (GafChromic RTQA Film을 이용한 광역학적 치료용 레이저의 선질 측정)

  • Purpose: The purposes of this study were to measure the dose distribution of Photodynamic therapy(PDT) laser with 635 nm wavelength using GafChromic film. Method & Result: We made each output 300 J by changing mW and sec using the laser beam radiation mode such as C.W(Continuous Wave) mode, Pulse mode and Burst Pulse mode and measured the does at 0 mm and 5 mm of distance from optic fiber catheter end to the film, and at 5 mm distance by changing the angle of the end of the optic fiber catheter as $0^{\circ}$ and $0.5^{\circ}$. The radiated film was scanned and OD(Optical Density) was compared. And two-dimensional isodose curves were obtained and the consistency of shapes was compared. It was confirmed that there was consistency between optic density and the dose radiated on the film when we radiated GafChromic film by changing distance and angle of 300 J output in each radiation mode coordinating mW and sec. Conclusion: In this study, we could identify the stability according to changes in laser beam modes, changes in output according to distance, changes in uniformity according to angle, and beam profiles using GafChromic film, and we could also get two-dimensional isodose curve. It was found that small change in the distance and angle that is made when optic fiber catheter was contacted on the treatment area did not make big effects on the output of beam and the uniformity of dose, and it was also found that GafChromic film could be utilized for the purpose of QA of PDT laser beam.

A Case of Tracheal Papillomas Treated With Bronchofibroscopic Nd-YAG Laser Therapy (기관지 내시경하 Nd-YAG Laser소각으로 치료 성공한 기관내 유두종)

  • In contrast to juvenile laryngeal papillomatosis, which occurs most often in children and teenagers and is usually self limited, soliatry papillomas in adults are one of uncommon tumors of airway, and have a higher incidence of cancer. They are caused by the respiratory infection of human papilloma virus(HPV). They could spread to more distal airways and have a tendency of recurrence after limited surgical excision. Recently endoscopic therapies such as Nd-YAG laser, electrocautery, and cryotherapy provide extremely effective treatment modalities. We report a case of solitary tracheal papillomas in a 48 year-old man who presented with cough, scanty hemoptysis, and functional evidence of central airway obstruction. He was successfully treated by a Nd-YAG laser therapy via fiberoptic bronchoscopy under the local anesthesia.
