Recurrent respiratory papillomatosis (RRP) is a benign tumor that occurs in the respiratory tract, especially in the larynx. The etiology of RRP is human papillomavirus (HPV). According to the age of occurrence, RRP is divided into childhood-onset and adult-onset types. Generally, childhood-onset RRP shows a high recurrence rate and diffuse involvement in the respiratory tract. Adult-onset RRP is more localized and appears more frequently as a solitary lesion. It may be the result of sexual transmission or the proliferation of latent HPV infections. At present, the treatment of choice for RRP is CO2 laser ablation. In addition, pulse dye laser or KTP (KTiOPO: potassium-titanyl-phosphate) lasers are also used. Non-surgical adjuvant therapies can be applied in cases requiring repetitive surgery or with diffuse extensions. This review will introduce the clinical features of RRP and various treatment options including lasers.
In this work, head-disk interactions are studied when flying height becomes lower than laser bump height on the landing zone of a disk. With the reduction of the spinning speed in a spin stand, the flying height is decreased under the height of laser bumps. Conventional and padded pico sliders sweep between landing Bone and data zone and, then, the dynamic behavior of the pico sliders and head-disk impacts are investigated using AE and stiction/friction signals. After 200n cycle-sweep tests, bearing analysis and AFM analysis indicate that there are some signs of wear and plastic deformation in the landing zone of a disk, although AE and stiction/friction signals are not significantly changed during the sweep tests. The experimental results of this paper suggest that in CSS tests at component level, more rigorous examination methods of wear and plastic deformation might be necessary as flying height becomes getting lower.
Introduction Energy-based devices allow for a more rapid and efficient ligation of blood vessels during operations. In the present study, we evaluated the feasibility of a laser as an alternative energy source for the vessel sealing system and determined the optimal condition of laser for an effective vessel sealing through histologic examination. Materials and Methods The arteries (5 mm diameter) harvested from porcine legs were compressed between two glass-slides to eliminate its luminal space and were irradiated with 1,470-nm diode laser under various sealing conditions, including laser power (5-30 W), irradiation time (5 or 10 seconds), and focus mode (focus or defocus). Subsequently, the irradiated vessels were fixed in 4% formaldehyde and then processed to paraffin block. The paraffinized sample was sectioned and stained with hematoxylin and eosin for histological evaluation. Results The extent of tissue change was positively correlated with duration and power of laser. In defocus mode, the irradiated vessels showed sufficient tissue denaturation for sealing effect without severe tissue destruction. Moreover, among the various conditions of irradiation, laser power between 15 and 20 W, as well as exposure time of 5 seconds were appropriate for sealing the blood vessels. Conclusion Adequate power and irradiation duration of laser can render blood vessels to be sealed effectively, although the higher power of laser may be required to cut the vessels.
Transoral $CO_2$ Laser Microsurgery of glottic Carcinoma is replacing external approaches. The qualify of life after surgery for laryngeal cancer may be as important as complete resection of the tumor for patient. Transoral $CO_2$ laser cordectomy for the management of early laryngeal cancer has advantages with regard to oncological results, preservation of laryngeal functions, morbidity and cost in comparison to those of open surgery or radiation therapy. Moreover, transoral laser surgery can be a useful choice as a salvage surgery in radiation therapy failed early glottic cancer. A classification of laryngeal endoscopic cordectomies which included eight different types was described by the European laryngological Society in 2000. We will also introduce type VI which was newly proposed recently.
Five-axis laser cutting has great advantages when it is applied to 3-dimensional machining requiring high cutting quality. For developing 5-axis CNC laser cutting systems, however, many problems such as rotating a laser head or a working table, 5-axis servo-control mechanism, tool path generation and post processing, and collision avoidance between a laser head and a work-piece should be solved. In this paper, we deal with developing a dedicated CAM system based on UG-NX3 for 5-axis laser cutting machine. Two essential modules such as post-processor and cutting motion simulation was developed. The developed system was applied to cutting curve defined on 3-D workpiece in order to show the validity of the proposed methods.
For developing 5-axis laser cutting systems, many problems such as rotating of laser head or table, 5-axis tool path generation and collision avoidance between laser head and product should be solved. In this paper, a five-axis laser cutting machine with table swivel and rotary type configuration is developed. The five axes (X,Y,Z,A,B) are controlled and interfaced to PC via MMC board. Two kinds of CAM S/W such as commercial 5-axis CAM S/W(Euclid) and UG-API are engaged to generate NC code for the developed 5-axis laser cutting machine.
Transoral $CO_2$ laser microsurgery has been widely used for various laryngeal diseases. Laser cordectomy for the management of early laryngeal carcinoma has advantages with regards to oncologic results, preservation of laryngeal functions, morbidity and cost in comparison to those of open surgery or radiation therapy. $CO_2$ laser is also applicable to benign vocal fold lesions such as nodules, polyps, cysts, reinke's edema, granulomas, papillomas, hemangiomas and glottis webs. $CO_2$ laser is suitable for microsurgery because it has small focus size and hemostatic effect. We should also recognize the risks of $CO_2$ laser for safe use.

The plume produced by vaporizing tissue with a laser contains a variety of contaminants called laser-generated air pollutants (LGACs). LGACs consist of a mixture of toxic gas components, biomicroparticles, dead and living cells, and viruses. Toxic odors and thick smoke from surgical incisions and the coagulation of tissues can irritate eyes and airways, as well as cause bronchial and pulmonary congestion. Because of the potential risk of the smoke, it is advisable to appropriately remove it from the surgical site. We recommend using a smoke evacuator to remove the smoke. Suction nozzles should be placed as close as possible to the surgical site in a range of 2 cm or less. In-line filters should be used between the inlet and outlet of the surgical site. All air filtration devices should be capable of removing particles below 0.1 microns in size. The filter pack should be handled according to infection control procedures in the operating room. The laser mask can be an auxiliary protective device if it is properly worn. Some smoke inhaled under the nose wrap or over the side of the mask will not be filtered. As in electrosurgical operations, a suitable mask should be worn while smoke is present.

Bronchial foreign body aspiration (BFA) is a common but emergent condition in infants and children. Furthermore, it can result in various complications such as atelectasis, pneumonia, bronchiectasis, and pneumothorax. Among these, pneumothorax is a very rare complication. However, it can be fatal without the swift implementation of appropriate treatment. We experienced a case of 16-month-old girl with an aspirated peanut. The foreign body was fixed in her left main bronchus. A CO2 laser was used to safely cut and break the foreign body. Removal was successful after breaking it. But after the process, inflammatory tissue of the tracheal mucosa was ruptured. Bilateral tension pneumothorax followed after the rupture. The patient was treated with bilateral chest tube insertion. Here we present this BFA case with a rare and unexpected complication. We also review the appropriate literature.
The cure rates for early stage laryngeal cancer are similar between laser cordectomy and radiation therapy. As well as the survival outcome, one of the main measures of success in treatment of early laryngeal cancer is voice outcome. Many studies have demonstrated that laser cordectomy and radiation therapy to be equivalent with regard to vocal outcome, whereas others favor radiation. Although such as somewhat disadvantages of voice outcome, laser cordectomy still remains a valid option. Since the patients who treated with laser may benefit from additional phonosurgery to improve postoperative vocal outcome. In this article, we reviewed the techniques of phonosurgery which can be used for laryngeal reconstruction after laser cordectomy. The indications for using each technique are discussed, with particular attention paid to functional outcomes following these reconstructive efforts.
