후두근전적출술 후 기관공 밸브를 사용한 환자 1례 (A Case of Using a Tracheostoma Valve after Near Total Laryngectomy)

  • Previously, laryngectomized patients can achieve speech with tracheoesophageal device. Tracheoesophageal speech is currently one of the best methods of postlaryngectomy voice rehabilitation. But, tracheoesophageal speakers have some troublesome with digital occlusion of tracheostoma in every speaking time. Therefore, the tracheostoma valve has been introduced since 1982, and this valve could eliminate the need of tracheostoma digital occlusion. Currently, couple of tracheostoma valves has been introduced and developed. In this article, we introduced the 3 representative tracheostoma valves. Also, we discussed their use and characteristics. Traditionally the tracheostoma valve is used in total laryngectomized patients. We introduced one case using a tracheostoma valve after near total laryngectomy. Advantage and disadvantage of Blom-Singer valve and BIVONA II valve during using in this patients are discussed.

후두전절제술 후 음성재활 (Voice Rehabilitation after Total Laryngectomy)

  • Total laryngectomy remains as an important treatment option in selected patients with laryngopharyngeal cancers, which inevitably sacrifices naturally produced voice. Much effort has been devoted to voice restoration for these laryngectomized patients. Several ways of voice rehabilitation after total laryngectomy have been developed and utilized thus far, including tracheoesophageal shunt speech, esophageal speech, pneumatic speech aid, and electrolarynx. Of these, tracheoesophageal shunt speech appears to be the most effective voice restoration method, while other trials might also be useful in special situations. Nevertheless, each method has its own unique mechanisms of voice production, thus has its advantages and drawbacks in clinical setting. In this review, we discuss the currently available management options for the rehabilitation of laryngectomized voice.

방사선 치료 환자에서의 Provox 사용 (The effect of Radiation Therapy on the Use of Proven Prosthesis in Laryngectomees)

  • Provox is now widely used for voice rehabilitation for total laryngectomized patient because of its low airway resistance and easiness for phonation. This study was designed to reveal the influence of radiation therapy on Proven complications. Forty-four patients who underwent total laryngectomy were grouped into group A (no radiation), group B(radiation and then Provox insertion), group C(Proven insertion and then radiation). Provox complications were leakage, granulation tissue formation, malfunction and infection. The average survival time of Provox was longer in group C (9.2 me) than group A(8.6 m) or group B (7.3 me), but no statistical significance was found. The first time of Provox change was 10.2, 8.6 and 9.7 months respectively. The incidence of complication was not significantly different among groups. The cases of Provox remeval due to shunt failure were 5, 4 and 2 respectively. Even though a significant statistically difference was not found partialy due to the small numbers of patients, a special caution should be exercised in inserting Provox to prevent a serious complication for the patients who had a history of previous radiation.

  • Background : Electrolarynx, Esophageal voice, and Silicone voice prosthesis with tracheoesophageal(T-E) fistula have been used as vocal rehabilitating methods for the post-laryngectomized patients. Prosthetic rehabilitation of voice after total laryngectomy has gained wide acceptance and has become a common practice in many clinics since the pioneering works of Singer and Blom In 1979. Since the introduction of tracheo-esophageal puncture and application of Blom Singer$\circledR$ voice prosthesis in 1980, several reliable voice prostheses have been developed and are successfully being used. Objectives : Even though quality of voice produced by Silicone voice prosthesis with T-E fistula is superior to other modalities, it still has some disadvantages. We devised a new cannulatyped silicone voice prosthesis. Methods : 1) Devising a new prototype of cannula-typed silicone voice prosthesis. 2) Application of the prototype using canine animal model(laryngectormized dog) and fitting trial on human patient whose previously inserted Silicone voice prosthesis is not functioning due to presumed fungal infection. Discussion : Final form of prototype was made after several times of major and minor modifications. Insertion of the newly developed Cannula-typed Silicone voice prosthesis on canine animal model and human trial were done without any difficulty. There were no serious leakage of saliva or food during swallowing. Conclusion : The newly developed Cannula-typed Silicone voice prosthesis(So-Mang$\circledR$) and the modified replacement method will further improve the results of post-laryngectomized prosthetic voice rehabilitation. Long-term animal study and human trial are planned in the near future.

Multi-Speech를 통한 후두적출자의 발성에 대한 음향학적 분석 (An acoustic study on the alaryngeal voice using the Multi-Speech)

  • The purpose of this study was to provide acoustic data on the voice of the laryngectomized patients for more scientific and efficient voice rehabilitation. The phonation of prolonged /a/ of 9 electronic artificial larynx(AL) users, 5 esophageal(EP) speech users, and 2 tracheo-esophageal(TEP) voice users were recorded and analyzed using Multi-Speech. Habitual f0, mean f0, sd f0, max f0, min f0, jitter, shimmer, and NHR were compared among groups of subjects using t-test. The EP and TEP groups exhibited higher f0 compared to the AL group. The AL and TEP groups showed more stable f0 than the EP group. In addition, the quality of TEP and EP voices were comparatively better in terms of jitter, shimmer, and NHR.

후두전적출술 52 례에 대한 임상통계학적 고찰 (Clinical Analysis of Laryngectomized Patients)

  • A retrospective investigation of 52 cases of carcinoma of the larynx, who underwent total laryngectomy $\bar{c}\;or\;\bar{s}$ neck dissection at Pusan National University Hospital from 1978 to 1985, was performed. The results obtained were as follows: 1) There were 32 glottic(62.7%), 18 supraglottic(35.3%) and 1 subglottic(2.0%) carcinoma. 2) In stage grouping, stage ill was the most(64.7%) and then stage II, stage IV, stage I in order. 3) Overall rate of cervical metastasis was 29.4%. In glottic carcinoma, 0% of $T_1,\;40%\;of\;T_2,\;18%\;of\;T_3\;and\;25%\;of\;T_4$. In supraglottic carcinoma, there was 0% of $T_1,\;29%\;of\;T_2,\;56%\;of\;T_3\;and\;50%\;of\;T_4$. 4) The incidence of postoperative complication was 31.4% and stomal stenosis was the most(13.7%) 5) There were 8 cases of local recurrence and 3 cases of distant metastasis(2 cases in lung, 1 case in esophagus) among 40 cases which were able to follow up. 6) 3 year estimated survival rate for glottic and supraglottic carcinoma were 73.3 % and 85.7% respectively.

경부형 전기인공후두 Blueton의 개발과 음향학적 성능 분석 (Development of Neck-Type Electrolarynx Blueton and Acoustic Characteristic Analysis)

  • Electrolarynx(EL), battery operated vibrators which are held against the neck by on-off button, has been widely used as a verbal communication method among post-laryngectomized patients. EL speech can produce easily without need of any additional surgery or special training and be used with any other methods. This institute developed a neck-typed EL named "Blueton" in commperation with EL Company Linkus, which consists of 3 parts : Vibrator part, Control part, Battery part. In this study we evaluated the acoustic characteristics of the produced voices by Blueton compared with Servox-inton using MDVP. Three EL users (2 full time users, 1 part time user) were participated. The results revelaed that NHR higher in Servox than Blueton and intensity is higher in Blueton than Servox. The spectra for vowels produced by EL speakers are mixed signals combined with talkers' vocal output and electrolarynx noise. The spectra pattern is similar with two ELs. High, SPI index and vowel spectra from MDVP demonstrated characteristics of both electrolarynxes related to noise signal. This finding suggests that Blueton helps to provide one of useful rehabilitation options in the post laryngectomy patients.
