Objective The purpose of this study is to learn the anti-inflammatory effect of Douchi on ulcerative colitis (UC). Methods Three-month-old mice were divided into 4 groups as follows: control group (Ctrl), UC induced group (UCEG), Pentasa treated group after inducing UC (OPTG), and Douchi treated group after inducing UC (FGTG). NF-κB, p-IκB, iNOS, COX-2 were observed by immunohistochemistry and Masson trichrome, PAS, and Phloxine-tartrazine staining were used to observe histochemical changes. Results Inflammation indicators of the large intestine were significantly lower in FGTG than in the UCEG and OPTG. Also, indicators involved in pulmonary alveolar formation were significantly higher in the FGTG than in the UCEG and OPTG. Conclusions The result of this study suggests that Douchi extract was effective in ulcerative colitis and helped in the formation of alveolar. This result suggests that the lungs and colon are correlated.

This study was examined the effects of Daesenggi-Tang on intestinal mucosa and gastrointestinal transit time and plasma lipids in rats. Adult male rats were fed four weeks on diets containing no addition(basal diet group), 5% cellulose(cellulose group) and Daesenggi-Tang group(Daesenggi-Tang group). The results were as follows; 1. The gastrointestinal transit times was significantly decreased in Daesenggi-Tang administered group compare to basal diet and cellulose groups. Carmine red mixed with Daesenggi-Tang, as a marker, was administered through a gastric tube for stomach or intracecally by a chronically implanted catheter for colon transit. Small intestinal transit and large intestinal transit time were significantly decreased in Daesenggi-Tang administered group compare to basal diet. 3. The height of jejunal villi was developed in Daesenggi-Tang administered group compare to basal diet. The thickness of mucosa and muscle layer of colonic mucosa were significantly developed in Daesenggi-Tang administered group compare to basal diet group. 4. The numerical change of goblet cell in colonic mucosa was increased acid mucin stained alcian blue in Daesenggi-Tang administered group compare to basal diet and cellulose group. 6. HDL-cholesterol of plasma lipid was increased in Daesenggi-Tang administered group compare to basal diet and cellulose groups. Theses results suggests that Daesenggi-Tang may be used in prevention and treatment of constipation resulting in increase of fecal weight, decrease of gastrointestinal transit time, development of intestinal villi, intensify of stainability of acid mucin in colon.
Ninety-six crossbred (Large White${\times}$Landrace${\times}$Duroc) pigs, weaned at 35 days of age, were assigned to four dietary treatments in order to investigate the effects of oral antibiotics on the performance and the intestinal microflora of weanling pigs. Pigs were fed either a basal diet, without antibiotics, or the basal diet plus either 50 ppm acetylspiramycin, 50 ppm olaquindox, or 100 ppm bacitracin zinc. The pigs were housed eight per pen with three pens per treatment in an environmentally controlled nursery. Ten days after weaning, three pigs from each treatment were slaughtered and intestinal pH, microflora, and volatile fatty acid concentration were determined. At the end of the 4 week trial, the remaining pigs were weighed and feed consumption was measured. Average daily gains for pigs fed acetylspiramycin, olaquindox, bacitracin zinc and the control diet were 0.43, 0.40, 0.37, and 0.34 kg per day (p=0.001), respectively. Antibiotic addition did not modify feed intake, but acetylspiramycin improved feed conversion (p=0.003). In comparison with the control, acetylspiramycin significantly increased Bifidobacteria numbers in the jejunum (p=0.082) and ileum (p=0.014) and decreased total bacterial counts throughout the intestine (p<0.01 except for the ileum where p=0.079). Acetate production was significantly lower in the cecum (p=0.028) and colon (p=0.079) of pigs fed acetylspiramycin. In addition to increasing numbers of Bifidobacteria in the jejunum (p=0.082) and ileum (p=0.014), olaquindox increased Lactobacillus in the jejunum (p=0.004) and decreased E. coli in the colon (p=0.022). Bacitracin zinc increased Lactobacillus numbers in the jejunum (p=0.004) and Bifidobacterium concentrations in the jejunum (p=0.082) and ileum (p=0.014).
Ulcerative colitis (UC) is a chronic inflammatory disorder of the gastrointestinal tract that affects the large bowel. Its etiology remains controversial. However, an infectious or immunologic origin is considered the primary cause. The onset of UC is typically slow and insidious, but some patients may present acutely with symptoms mimicking infectious colitis. We report a case of ulcerative colitis mimicking acute hemorrhagic colitis at initial presentation. A 60-year-old man was referred to Yeungnam University Hospital for bloody diarrhea and abdominal pain. Sigmoidoscopy revealed mildly edematous mucosa in the rectum and hyperemic mucosa with petechiae in the sigmoid colon. The patient was treated with antibiotics for several days, and his symptoms improved. However, after one month, his bloody diarrhea relapsed. Follow-up sigmoidoscopy revealed mucosal friability in the rectum and sigmoid colon. He was diagnosed with ulcerative colitis, and his symptoms were improved with mesalazine and a steroid enema.
Cytodiagnosis of pleural and ascitic fluid is a commonly performed laboratory examination. Especially, positivity for malignant cells in effusion cytology is very effective and also presents the first sign of malignancy in unknown primary site of the tumor. We examined each 34 cases of pleural and ascitic fluid cytologic specimen diagnosed as metastatic tumor, which was selected among 964 pleural fluid cytology cases and 662 ascitic fluid cytology cases from September 1989 to June 1995. Among the pleural fluid cytology specimens examined, 34 specimens were positive in 27 patients. The lung was the most frequent primary site(44%), followed by the stomach (12%), lymphoreticular neoplasm(12%), pancreas(3%) and colon(3%). And the cases of unknown primary site with positive pleural biopsy alone were 24%. Among trio ascitic fluid cytology specimens examined, 34 specimens were positive in 29 patients. The most common primary neoplasms. were carcinomas of ovary(32%), stomach(22%), colon(6%), breast(3%), pancreas(3%), and lung(3%) and lymphoreticular neoplasms(3%) The metastatic tumor was predominantly adenocarcinoma type in both pleural(82%) and ascitic(91%) fluid. The study of metastatic adeno- carcinoma in effusion from lung, ovary, and stomach was undertaken to find distinctive features for the identification of the primary site. The smears of metastatic pulmonary adenocarcinoma had a tendency to show high grade pleomorphism and many large tight cell clusters, whereas that of the ovarian adenocarcinoma showed low grade pleomorphism with abundant intracytoplasmic vacuoles in relatively clear background. That of the stomach revealed the intermediate features.
The effects of kimchis intake on Helicobacter pylori infection in the stomach, the counts of lactic acid bacteria in the large intestine, and bacterial enzymes ($\beta$-glucosidase, $\beta$-glucuronidase) and pH in feces were examined. A total of 20 participants (age range 34 ∼ 57) were assessed for H. pylori infection status by Be urea breath test. Fourteen participants were eliminated because they were H. pylori-negative. This study consisted of 4 consecutive phase, each of which lasted 4 weeks. Three hundred grams of kimchi were administered to H. pylori-infected subjects during the kimchi phase, followed by 4 weeks of control phase. During the control phase, subjects consumed 60 g of kimchi, the minimum amount in their customary diets. All participants were found to be H. pylori-positive during all experimental periods. During the kimchi phase, delta over baseline (DOB) level was lower than during the control phase, although significant difference between the kimchi and control phases were not found (p=0.9439). However, the counts of Lactobacillus sp. and Leuconostoc sp. significantly (p < 0.0005) increased during the kimchi phase. $\beta$-Glucosidase and $\beta$-glucuronidase activities and pH were significantly decreased by kimchi intake compared to control (p=0.000l). These results suggested that kimchi consumption did not show any therapeutic effect on H. pylori in the stomach. However, kimchi seemed to be a good food for colon health, since it increased the beneficial bacteria such as lactobacillus and decreased toxic enzyme ($\beta$-glucosidase and $\beta$-glucuronidase) activity and pH.

Constipation is a common clinical problem that comprises of symptoms that include excessive straining, hard feces, feeling of incomplete evacuation and infrequent defecation. Although many conditions, such as metabolic problems, fiber deficiency, anorectal problem, an drug, can cause constipation. Background: The purpose of this study was to identify role ambiguity of comprehensive nursing care unit nurses. Methods: A concept analysis method by Walker and Avant was used to understand role ambiguity of comprehensive nursing care unit nurses. Results: The antecedents of role ambiguity of nurses at comprehensive nursing units were shortage of nurses, unclear admission criteria, and demands for customized nursing care according to severity. Attributes include ambiguity in role delegation, patient placement ambiguity, and professional ambiguity among nursing staff. The consequences were diminished job satisfaction due to excessive workload, difficulty in resolving role ambiguity due to the lack of work analysis studies, and poor outcome of nursing indicators. Conclusion: Improvement of nationwide awareness for comprehensive nursing care unit is required. Clear division at scope of practice for nursing staff in accordance of each medical institution's characteristics is essential. Nurses at comprehensive nursing care unit should understand nature of role ambiguity that occurs as they work in large groups. Nurses should promote communications between nursing staff and they must have volition to improve status quo. An additional research of comprehensive nursing care on the causes of role ambiguity in the practice of nursing care for ward nurses is needed, and management measures should be sought at the organizational level.
Background : Surgical site infection(SSI) is one of the important nosocomial infections with pneumonia, urinary tract infection. SSI increases mortality, morbidity, length of stay, and costs for postoperative patients. The purpose of this study was to estimate length of stay(LOS) and health care costs from SSI using the large observational data. The ultimate objective was to show the effect of prevention of SSI. Method : This study used antibiotic prophylaxis evaluation data and claims data of the HIRA(Health Insurance Review and Assessment Service). The study population included 18,361 patients who underwent gastric surgery, endoscopic cholecystectomy, colon surgery, hysterectomy, cesarean section in nationwide hospitals from August to October 2007. SSI group and non-SSI group were matched according to propensity score resulted from logistic regression. The paired t-test was used to compare the difference of the LOS and health care costs between SSI group and non-SSI group. Results : The 598 cases of SSI were detected of total subjects, and the crude SSI rate was 3.3%. For each surgery, SSI rates were 5.5% for gastric surgery, 4.7% for cholecystectomy, 6.6% for colon surgery, 2.6% for hysterectomy, and 1.6% for cesarean section. The 596 cases of SSI and the 596 cases of non-SSI were matched by propensity score. The LOS of SSI group was longer than that of non-SSI group, and the difference was statistically significant. Health care costs of SSI group was more than that of non-SSI group which was significant. Conclusions : SSI increased apparently the LOS and healthcare costs. The economic loss might affect the cost of national healthcare as well as patients and hospitals. This study provided the evidence that the healthcare expenditure could be reduced by preventing SSI.
This study was conducted to assess the chemopreventive effects of sodium selenite in the rat medium-term multi-organ bioassay using a DMBDD model (DEN+MNU+BBN+DMH+DHPN). Seventy five,6-week-old, male SD rats were divided into 3 groups. The animals in group 1 received DEN(diethylnitrosamine,100 mg/kg bw, single i.p., in saline), MNU (N-methyl-nitrosourea,20 mg/kg bw, i.p.,4 times for 2 weeks), BBN (N-butyl-N-(4-hydroxybutyl) nitrosamine, 0.2% in drinking water for 2 weeks), DMH (1,2-dimethylhydrazine, 40 mg/kg bw, s.c., in saline.4 times (or 2 weeds), and DHPN (N-bis(2-hydroxy-pro-pal)nitrosamine,0.1% in drinking water for 2 weeks), then were placed on sodium selenite (4 ppm in drinking water) for 22 weeks from weeks 4 to 26. The animals in group 2 were given DMBDD alone. The animals in group 3 were given sodium selenite alone. Animals were sacrificed at week 12 for ACF quantitative analysis and at week 26 for tumor induction. The body weights in the group 1 were significantly decreased compared with those of group 2. The tumor multiplicities of large intestine in the group 1 were significantly decreased compared with those of group 2 (P<0.05). These results indicate that sodium selenite may have a potential as chemopreventive agents of colon carcinogenesis.
