Effects of Wet Cupping (Al-Hijamah) on Cholesterol in a Sudanese Population

  • Background: Wet cupping (WC) is an efficient and cost-effective technique for removing metabolic waste from the bloodstream via the skin. The study aimed to examine the effect of WC on cholesterol levels including total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) in a Sudanese population. Methods: In this prospective cross-sectional study, 30 participants undergoing regular WC therapy were enrolled. Blood samples collected twice: pre-WC therapy (case group) and 10-14 days afterward (controls). Results: Of the participants, 56.67% were male and 43.33% were female, aged 24-69. Pre-WC TC and LDL-C levels were significantly higher than the post-WC control group (p = 0.001). Conversely, HDL-C levels decreased significantly in the pre-WC cases compared to controls (p = 0.001). No significant sex-based difference in mean cholesterol levels (p > 0.05). Conclusions: After WC, males and females experienced significant reductions in TC and LDL-C, and significant increase in HDL-C.

  • The principal objective of this study was to assess the effects of chitosan oligosaccharide supplementation on the improvement of blood glucose, lipid components and enzyme activities in the serum of streptozotocin(STZ, 55 mg/kg B.W., I.P. injection)-induced hyperglycemic rats fed on experimental diets for 5 weeks. The concentrations of blood glucose, total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free cholesterol, cholesteryl ester ratio, triglycerides(TG) and phospholipids(PL) in serum were remarkably higher in the hyperglycemic group(group BSW) and STZ(I.P.)+chitosan oligosaccharide supplementation group(group ECW) than those in the control group(group BW, basal diet+water). However the concentrations of blood glucose, total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, tree cholesterol, cholesteryl ester ratio, TG and PL in serum were lower in the ECW group than in the BSW group, whereas the ratio of HDL-cholesterol concentration to total cholesterol and HDL-cholesterol concentration in the ECW group were higher than in the BSW group. The activities of alkaline phosphatase(ALP) and aminotransferase(AST, ALT) in serum were lower in the ECW group than in the hyperglycemic BSW group. The results shown above suggested that chitosan oligosaccharide supplementation effectively improves blood glucose, lipid composition and enzyme activities in the sera of STZ-induced hyperglycemic rats.

가미제습순기탕(加味除濕順氣湯)이 고지혈증(高脂血症)을 유발시킨 고혈압(高血壓) 백서(白鼠)에서의 혈액학적(血液學的) 변화(變化)에 미치는 영향(影響) (A study on the Effect of Kamijesubsungitang on Hyperlipidemia)

  • KJST is the oriental herbal medicine of hyperlipemia which consisted of a herb of 17. We induced hyperlipemia to Spontaneous Hypertensive Rat(SHR) and studied the treatment effect by KJST. Hyperlipidemia was induced by hyper-lipidemic diet fed for 4weeks. Total cholesterol, LDL-cholesterol, HDL-cholesterol, Triglyceride, Total bilirubin, Lactic dehydrogenase(LDH), Glucose and Total protein were measured on the serum after an oral administration of KJST. Total cholesterol, LDL-cholesterol, Glucose and LDH was significantly lower in the KJST treated animals, and HDL was significantly raised in the KJST treated animals. These results suggest that KJST is effective for the treatment of hyperlipidemia.

한국인에서 유산균 발효유의 섭취가 혈중 콜레스테롤에 미치는 영향 (Effect of Fermented Milk on the Blood Cholesterol Level of Korean)

  • This study showed the effect of fermented milk on the cholesterol level of Korean. 130 persons among the teachers of elementary, middle and high schools, professors and graduate school students were divided into two groups according to their cholesterol level and provided with fermented milk 300 $m\ell$ dairly from Sep.25 to No. V3. They were given blood test at 20 days and 40 days after drinking fermented milk, and surveyed about their ordinary life style and the change of fecal condition. Cholesterol, LDL and the ratio of LDL and HDL in blood were significantly decreased in both high cholesterol group and normal group while HDL cholesterol was notably increased. TG showed slightly tendency of decrease in normal group while no changed in high group. There is no significant change in cholesterol level after drinking fermented milk among groups which are divided by factors of dringking, smoking, caffeine, B.M.I. and family history. The research also showed that drinking fermented milk improved the fecal frequency and fecal condition.

LDL-콜레스테롤의 Friedewald 계산값과 실측값 비교: 국민건강영양조사 2009-2010 (Friedewald-Estimated Versus Directly Measured LDL-Cholesterol: KNHANES 2009-2010)

  • LDL-콜레스테롤(LDL-C)은 심뇌혈관질환의 주된 교정 가능한 위험인자로서, 정확한 측정값을 임상에 적용하는 것이 중요하다. 하지만 LDL-C의 측정은 실제 측정이 아닌 Friedewald 공식에 의한 계산방법이 널리 이용되고 있다. 본 연구의 목적은 LDL-C의 Friedewald-추정값과 실측값을 비교하고, 두 방법의 LDL-C 위험수준 분류 일치도를 평가하는 것이다. 표본은 국민건강영양조사 2개년(2009년과 2010년)의 공개된 자료에서 추출되었고, 혈액 검사에서 총 콜레스테롤, HDL-콜레스테롤, 직접 측정한 LDL-C, 그리고 중성지방 중 어느 한 결측치도 없는 4,319명을 연구대상으로 하였다. 중성지방 400 mg/dL 미만일 때, Friedewald-추정값과 실측값은 높은 상관관계를 보였고 (r = 0.958, p < 0.001), 위험수준 분류 일치 백분율은 82.7%이었다. 중성지방 수준이 높을수록, 일치 백분율은 낮았다. 중성지방 수준 150 mg/dL 미만, 150-200 mg/dL, 그리고 200-399 mg/dL일 때, 일치 백분율은 각각 85.4%, 78.2%, 그리고 71.4%이었다. Friedewald 공식은 중성지방 농도 150 mg/dL 미만에서는 LDL-C를 과대평가하는 반면, 중성지방 농도 150 mg/dL 이상에서는 과소평가하는 경향이 있었다. 이에 따라 LDL-C 위험수준 분류에 있어 그 범주가 과대평가된 사람은 382명 (9.1%)인 반면, 과소평가된 사람은 348명 (8.3%)이었다. 이러한 결과는 Friedewald-추정값의 LDL-C 과소평가뿐만 아니라, 과대평가도 심각한 문제일 수 있음을 제시한다.

저분자화알긴산이 콜레스테롤식이 흰쥐의 혈청과 간장지질의 콜레스테롤 수준 및 지방산 조성에 미치는 영향 (Effect of Low Molecular Alginates on Cholesterol Levels and Fatty Acid Compositions of Serum and Liver Lipids in Cholesterol-Fed Rats)

  • 미역과 큰다시마에서 추출한 수용성 알긴산 및 산$\cdot$알칼리가용성 알긴산과 그 염산부분가수분해에 의하여 얻어진 저분자화 알긴산 (저분자화 범위 : 미역 약 4,000 kDa$\to$약 38kDa; 큰다시마 약 1.283kDa$\to$약 341 kDa과 약 45kDa)을 함유한 실험사료를 횐쥐 (SD계, 4주령)에 4주간 급이했을 때의 혈청 및 간장지질 콜레스테롤 수준 및 지방산 조성에 미치는 영향을 실험$\cdot$검토하였다. 알긴산의 급이는 식이효율이 $0.37\~0.44$로서 기초식이군에 비하여 $0.03\~0.05$ 정도 낮은 값을 보였고, 콜레스테롤 급이에 의한 간의 중량 증가도 유의적으로 억제하였다. 수용성 알긴산은 산$\cdot$알칼리가용성 알긴산에 비하여 혈청 및 간장지질 중의 총콜레스테롤, 유리콜레스테롤, LDL-콜레스테롤, triglyceride및 인지질에 대하여 현저한저감효과를 보였다. 그리고 각각의 효과는 저분자화에 의하여 현저히 증대되었다(저분자화에 의한 저감효과 수용성 알긴산 - 혈청지질: 총콜레스테롤 $59\%$, 유리콜레스테롤 $65\%$, LDL-콜레스테롤 $96\%$, triglyceride $50\%$, 인지질 $36\%$, 간장지질: 총콜레스테롤 $42\%$, 유리콜레스테롤 $62\%$, LDL-콜레스테롤 $44\%$, triglyceride $33\%$, 인지질 $44\%$$\cdot$알칼리가용성 알긴산 - 혈청지질: 총콜레스테롤 $52\%$, 유리콜레스테롤 $97\%$, LDL-콜레스테롤 $78\%$, triglyceride $32\%$, 인지질 $64\%$, 간장지질. 총콜레스테롤 $11\%$, 유리콜레스테롤 $12\%$, LDL-콜레스테롤 $10\%$, triglyceride $27\%$, 인지질 $21\%$) 저분자화 알긴산의 급이가 혈청 및 간장지질 구성지방산의 조성에 미치는 효과는 콜레스테롤 급이군에 비하여 polyene산의 함유비율을 혈청지질에 있어서는 약 $44\%$이상, 그리고, 간장지질에 있어서는 약 $70\%$ 이상으로 각각 증가시켰다. 이상의 결과에 비추어 저분자화 알긴산의 급이는 실험동물의 혈청 및 간장지질 조성을 생리적으로 개선하는데 그 효과가 현저함을 뒷받침하였다.

한국인의 일부 도시인에서 비만, 이상혈당, 이상지질혈증의 집락과 고혈압의 관련성 (Association of Hypertension with Cluster of Obesity, Abnormal glucose and Dyslipidemia in Korean Urban Population)

  • To examine the association of hypertension with cluster of obesity, abnormal glucose and dyslipidemia in Korean urban population, we conducted this cross-sectional study among 3027 men and 2127 women age 20-85 years who visited a prevention center between May 1991 and June 1995 for a multiphasic health check at St. Mary's Hospital, Seoul. By the self-administered questionnaire, the informations of educational attainments, monthly income, alcohol consumption, cigarette smoking, and physical excercise level were obtained. Height, weight, and blood pressure were measured by a trained nurse. The fasting blood sugar (FBS), total cholesterol, high density lipoprotein (HDL) cholesterol and triglyceride were tested by enzyme method. Low density lipoprotein (LDL) cholesterol was calculated by 'total cholesterol - HDL cholesterol - triglyceride/5'. For testing the differences of cardiovascular risk factors between hypertension and normotension group, 1-test and $\chi^2$-test were performed. For the age adjusted odds ratios of hypertension in persons with obesity, abnormal glucose, and dyslipidemia compared with normal, logistic regression was performed by using SAS pakageprograme. The results obtained were as follows: 1. Age, weight, body mass index(BMI), blood glucose, total cholesterol, LDL cholesterol, and triglyceride of hypertension group in men and women were significantly higher than normotension group, but height and HDL cholesterol of hypertension group only in women significantly lower than normotension group. The frequency of obesity $(BMI\geq25kg/m^2)$, abnormal glucose $(\geq\;120mg/dl)$, hypercholesterolemia $(\geq\;240mg/dl)$, lower HDL cholesterol (<45 mg/dl in women only), higher LDL cholesterol $(\geq\;160mg/dl)$, and hyper hypertriglyceridemia $(\geq\;250mg/dl)$ in hypertension group of men and women were significantly higher than normotension group. 2. Systolic and diastolic blood pressure were negatively correlated with hight, but positively with age, weight, BMI, total cholesterol, LDL cholesterol, and triglyceride in men and women. BMI was positively correlated with fasting blood sugar, total cholesterol, LDL cholesterol and triglyceride but negatively with HDL cholesterol. 3. The age adjusted odds ratios of hypertension were as follows in men and women : among persons who were obese compared with those nonobese, 2.53 (95% Confidence Intervals [C.I.] 2.08-3.07) and 2.22 (95%C.I. 1.71-2.87): among persons who were abnormal glucose compared with those normoglycemic, 1.43 (95%C.I 1.13-1.82) and 2.01 (95%C.I 1.36-2.94): and among persons who were dyslipidemia (hypercholesterolemia or lower HDL cholesterol or higher LDL cholesterol or hypertriglyceridemia) compared with those normal lipid, 1.59 (95%C.I 1.30-1.95) and 1.51 (95%C.I 1.16-1.96). After combined more than one risk factor, the odds ratios were increased. Among persons with cluster of obesity, abnormal glucose, and dyslipidemia, the odds ratio of hypertension was 2.25 (95%C.I 1.47-3.37) in men and 3.02 (95%C.I 1.71-5.30) in women. In conclusion, it was suggested that hypertension was associated with cluster of obesity, abnormal glucose, dyslipidemia in this Korean urban population.

Effects of Soy Isoflavones on Lipid Profiles and Hepatic LDL Receptor mRNA Level in Growing Female Rats

  • The present study examined the effect of soy isoflavones on lipid metabolism in growing female rats. Rats were randomly assigned to three different groups and provided experimental diets for 9 weeks. The experimental groups were classified into 1) a control group, 2) a soy protein isolate group: soy (+)) group and 3) a soy protein concentration group: soy (-)) group. Diets contained either casein or one of two soy proteins with (soy (+)) or without isoflavones (soy (-)). Serum triglyceride concentration showed no significant differences among the experimental groups. Serum total cholesterol concentration was significantly lower in both the soy (+) and soy (-) groups than in the control group and LDL-cholesterol concentration was significantly lower in the soy (+). Serum HDL-cholesterol concentration was significantly higher in the control group than in the soy protein groups but the HDL-cholesterol share rate in total cholesterol tended to be lower in the control group than in the soy protein groups, insignificant as it was. Hepatic IDL receptor mRNA level was significantly increased in the soy (+) group when compared to the other two groups to be 20% higher than the control group. In conclusion, soy protein isolate, soy protein rich with isoflavones reduced serum total cholesterol and LDL-cholesterol concentration and increased hepatic IDL receptor mRNA expression in growing female rats. Therefore, it is considered that the intake of soy isoflvones during puberty can be advantageous in terms of the long-tenn control of serum lipid.

고콜레스테롤 조건하에 배양된 HepG2에서의 ginsenoside-Rb2에 의한 LDL receptor 억제 완화 기전 (The Mechanism of LDL Receptor Up-regulation by Ginsenoside-Rb2 in HepG2 Cultured under Enriched Cholesterol Condition)

  • 인삼성분 중 ginsenoside-Rb$_2$에 의한 LDL receptor발현 증가의 기전을 HepG$_2$세포에서 관찰하였고 이를 lovastatin과 비교하였다. 콜레스테롤 투여에 의하여 억제된 LDL receptor mRNA발현이 ginsenoside-Rb$_2$에 의하여 다시 증가하였고 이는 lovastatin에 의한 증가 효과보다 뛰어났다. SREBP mRNA의 발현 또한 콜레스테롤 투여에 의하여 억제되나 ginseonside-Rb$_2$에 의하여 발현이 증가하였고 이는 lovastatin에 의한 효과와 비슷하였다. 세포에 투여한 ginsenoside-Rb$_2$의 농도에 비례하여 SREBP-1 mRNA의 발현이 증가하였으며 ginsenoside-Rb$_2$의 대사체인 compound K를 투여한 경우에도 SREBP-1 mRNA가 비슷한 양상으로 혹은 더 많이 발현되었다. 따라서 ginsenoside-Rb$_2$에 의한 LDL receptor의 발현 증가는 SREBP의 발현 증가 때문이라고 설명할 수 있다. 즉 ginsenoside-Rb$_2$에 의한 SREBP 발현 증가는 콜레스테롤 투여에 의하여 억제된 LDL receptor발현을 증가시켜 결과적으로 혈중의 콜레스테롤을 효과적으로 제거하는 것으로 판단된다.

Lovastatin과 Simvastatin의 고지혈증 치료 비교 (A Comparison of Lovastatin and Simvastatin in Treatment of Hyperlipidemia)

  • Hypercholesterolemia is one of main causes of coronary heart disease(CHD). Clinical trials demonstrated that lowering serum cholesterol levels would reduce incidence of new cardiovascular events and mortality by primary or secondary preventions. The objective of this retrospective study was to compare efficacy and side effects of lovartatin and simvastatin in treatement of hypercholesterolemia. In Boramae Hospital, patients were included when they have taken lovastatin 20 mg or simvastatin 10 mg for 52 weeks with laboratory monitoring for cholesterol at baseline, 3, 6 and 12 month period. As results, total 128 outpatients were included with their total cholesterol level <240 mg/dl and triglyceride level <400 mg/dl at baseline. Total cholesterol and LDL cholesterol of lovastatin group (n=60) and simvastatin group (n=68) were significantly reduced from baseline (p=0.001). Lovastatin maximally reduced total cholesterol by $23.9\%,\;triglyceride\;by\;12.3\%$, LDL cholesterol by $36.1\;\%$ and increased HDL cholerterol by $7.8\%$ and simvastatin reduced by $24.1\%,\;20.5\%,\;34.3\%\;respectively$ and HDL increased by $11.2\%$. There were no significant differences between lovastatin and simvastatin in mean percent change of lipid levels at 12, 24 and 52 weeks from baseline. Cumulative percentage of patients reaching the target LDL cholesterol concentration by 24 weeks was $61.7\%$ in lovastatin and $64.7\%$ in simvastatin. Average time to reach the target LDL goal was 100.1 days in lovastatin and 99.8 days in simvastatin. Both lovastatin and simvastatin also significantly reduced total cholesterol and LDL cholesterol in all subgroups (diabetes mellitus, hypertension, and coronary heart disease). In this study, treatment efficacy in patients with coronary heart disease was lower than other patients. Considering clinical importance of secondary prevention, more intensive treatment is necessary to decrease LDL cholesterol level of 100 mg/dl or lower in patients with coronary heart disease or other clinical atherosclerotic disease. There were no serious side effects during the study period. Digestive side effects were most frequently reported (lovastatin $8.3\%\;vs\;simvastatin\;8.8\%$). In conclusion, both lovastatin and simvastatin were similar in lipid lowering effects and there was no difference in incidence of side effects.
