심혈관질환 예측을 위한 저밀도 대 고밀도 지단백 콜레스테롤 비(LDL : HDL-cholesterol ratio)의 적정기준에 관한 연구 (Assessment of the Optimum LDL : HDL-cholesterol(LDL : HDL-C) Ratio for Predicting CHD)

  • Purpose: This study was aimed to determine the optimum low-density lipoprotein : high-density lipoprotein-cholesterol (LDL : HDL-C) ratio for predicting coronary heart disease(CHD) in Korean people. Methods: It was analyzed this data of 5,431 adults who had undergone health examinations in a hospital in Gyeonggi-do between January 2006 and December 2007. The covariation of the coronary risk factors such as age, HbA1C, systolic blood pressure(SBP), and waist-to-stature ratio(WSR) were analyzed by using logistic regression analysis. Results: The LDL : HDL-C ratio in the male and female groups was mostly distributed between 1.5 and 4.0. The LDL : HDL-C ratio was the most significant cholesterol-related parameter influencing CHD (male: B = .306, p = .054, female : B = .940, p = .010), followed by LDL-C and total cholesterol. It was observed a sharp increase in the odds ratios for LDL : HDL-C ratios of 2.25 - 2.50(male) and 2.00 - 2.25(female). A significant difference was observed in both male(2.25 : $x^2$ = 2.494, p = .072) and female(2.00 : $x^2$ = 413.742, p = .000) groups. Conclusion: The risk level of CHD was set to 2.25 for males and 2.00 for females. Therefore, the optimum LDL : HDL-C ratio for Koreans should be far lower than that for the people in western countries.

Correlations among Anthropometric Measurements, Serum Lipid Levels and Nutrient Intake in Female University Students

  • The purpose of this study was to investigate the correlations among the anthropometry, serum lipid levels and nutrient intake in Korean female university students. The subjects were 119 female students at a university located in Incheon. This study was conducted using a self-administered questionnaire. Anthropometric data were measured and blood lipid levels were analyzed. Nutrient intake collected from 3 day-recalls was analyzed by the Computer Aided Nutritional Analysis Program. The data were analyzed by SPSS 10.0 program. Average age, height and weight of the subjects were 20.9 years, 160.1cm and 54.3kg, respectively. Average serum TG (triglyceride), total cholesterol, HDL-C (high density lipoprotein-cholesterol) and LDL-C (low density lipoprotein-cholesterol) levels of the subjects were 69.47mg/dl, 146.85 mg/dl, 50.49mg/dl and 82.52mg/dl, respectively. Average AI (atherogenic index) of the subjects was 2.03, which was in the normal range based on risk values. Average intake of most nutrients except protein, vitamin B$_1$, vitamin C and phosphorus were lower than the Korean RDA. Especially calcium and iron intakes of the subjects were under 65% of the Korean RDA. Serum TG, total cholesterol and LDL-C levels were negatively correlated with DBP (diastolic blood pressure). HDL-C/LDL-C and HDL-C/total cholesterol were positively correlated with height. Age was positively correlated with phosphorus intake. DBP of the subjects was positively correlated with calcium and iron intakes. Serum TG level was positively correlated with total cholesterol, HDL-C, LDL-C and AI, while negatively correlated with HDL-C/total cholesterol. Total cholesterol level was positively correlated with HDL-C, LDL-C and AI, while negatively correlated with HDL-C/LDL-C, HDL-C/total cholesterol. HDL-C level was positively correlated with LDL-C, HDL-C/LDL-C and HDL-C/total cholesterol, while negatively correlated with AI. LDL-C level was negatively correlated with HDL-C/LDL-C and HDL-C/total cholesterol, while positively correlated with AI HDL-C/LDL-C ratio was positively correlated with HDL-C/total cholesterol and AI. HDL-C/total cholesterol was negatively correlated with AI. Fat intake was positively correlated with total cholesterol, HDL-C level, and vitamin B$_2$ intake was positively correlated with TG, HDL-C/LDL-C. Therefore, nutrition education is necessary for female university students to promote the lipid profile and to optimize the nutritional status. (J Community Nutrition 4(3) : 151∼158, 2002)

Validity for Use of Non-HDL Cholesterol Rather than LDL Cholesterol

  • NonHDL cholesterol values have been suggested as a risk marker for cardiovascular disease. NonHDL cholesterol values were calculated, using a very simple measurement [nonHDL cholesterol=serum total cholesterol-HDL cholesterol]. This formula is very useful as a screening tool for identifying dyslipoproteinemias, risk assessment, and assessing the results of hypolipidemic therapy. The data from the 2009 Korean National Health and Nutrition Examination Survey were used. Analysis was done for 1,992 subjects with lipid panels (Cholesterol, HDL, LDLdirect and Triglycerides) results. We studied the relationship between nonHDL cholesterol and LDL cholesterol. As a result, nonHDL cholesterol values were plotted against the LDL direct and calculated values. The linear regression equation for nonHDL cholesterol and direct LDL cholesterol was $nonHDLchol=23.60+1.03{\times}LDLdirect$ (p<0.0001, $r^2=0.80$) in all subjects. The subjects were classified into triglyceride values. When triglycerides are below 400 mg/dL, the linear fit to LDL direct is found to be $[nonHDLchol=17.34+1.07{\times}LDLdirect]$ (p<0.0001, $r^2=0.88$) and to the Friedewald LDL calculation is $[nonHDLchol=23.10+1.02{\times}LDLcalc]$ (p<0.0001, $r^2=0.82$). For triglycerides above 400 mg/dL, the linear fit equation is $[nonHDLchol=87.57+0.92{\times}LDLdirect]$ (p<0.0001, $r^2=0.50$) and to the LDL calculated, it is $[nonHDLchol=142.70+0.50{\times}LDLcalc]$ (p<0.0001, $r^2=0.32$). This study provides examples of the utility of nonHDL cholesterol concentrations in clinical medicine.

Cholesteryl Ester Transfer Protein (CETP) Deficiency and CETP Inhibitors

  • Epidemiologic studies have shown that low-density lipoprotein cholesterol (LDL-C) is a strong risk factor, whilst high-density lipoprotein cholesterol (HDL-C) reduces the risk of coronary heart disease (CHD). Therefore, strategies to manage dyslipidemia in an effort to prevent or treat CHD have primarily attempted at decreasing LDL-C and raising HDL-C levels. Cholesteryl ester transfer protein (CETP) mediates the exchange of cholesteryl ester for triglycerides between HDL and VLDL and LDL. We have published the first report indicating that a group of Japanese patients who were lacking CETP had extremely high HDL-C levels, low LDL-C levels and a low incidence of CHD. Animal studies, as well as clinical and epidemiologic evidences, have suggested that inhibition of CETP provides an effective strategy to raise HDL-C and reduce LDL-C levels. Four CETP inhibitors have substantially increased HDL-C levels in dyslipidemic patients. This review will discuss the current status and future prospects of CETP inhibitors in the treatment of CHD. At present anacetrapib by Merck and evacetrapib by Eli Lilly are under development. By 100mg of anacetrapib HDL-C increased by 138%, and LDL-C decreased by 40%. Evacetrapib 500 mg also showed dramatic 132% increase of HDL-C, while LDL-C decreased by 40%. If larger, long-term, randomized, clinical end point trials could corroborate other findings in reducing atherosclerosis, CETP inhibitors could have a significant impact in the management of dyslipidemic CHD patients. Inhibition of CETP synthesis by antisense oligonucleotide or small molecules will produce more similar conditions to human CETP deficiency and may be effective in reducing atherosclerosis and cardiovascular events. We are expecting the final data of prospective clinical trials by CETP inhibitors in 2015.

  • Although an elevated plasma level of high density lipoprotein (HDL) is known as a protective component against the development of atherosclerosis and ensuing coronary heart diseases, the related mechanisms are still not established . It has been clearly demonstrated in the early stages of atherogenesis that adhesion of monocytes and lymphocytes to the vascular endothelium is enhanced via adhesion molecules, and that monocytes and macrophages accumulate in the subendothelial space. The present study has investigated whether isolated plasma HDL plays a role in protection against atherogenesis by inhibiting the expression of vascular cell adhesioin molecule-1(VCAM-1) on the endothelial cells. Effects of plasma native low density lipoprotein (LDL) and ac ethylated LDL(AcLDL) on VCAM-1 expression were also examined by using an immunocytochemical technique. While plasma HDL did not alter the basal expression of VCAM-1 , lipopolysaccharide(LPS) induction of this adhesion modlecule was markedly inhibited at a phyaiological concentration of HDL. In contrast, 30$\mu\textrm{g}$ protein/ml AcLDL increased sifnificantly both basal VCAM-1 expression and its LPD induction , suggesting that this modified LDL enhances leukocyte adhesiion to endothelial cells. Unlike AcLDL , plasma native LDL inhibited significantly VCAM-1 expression. This indicates that LDL did not undergo oxidative modificantion while incubated with endothelial cells. These results suggest that plasam HDL may inhibit atherogenesis by reducing the expression of adhesion molecules, which is a protective mechanism independent of tis reverse cholesterol transport function . Modified LDL is a potent iducer for adhesion molecules in vascular endothelical cells and could play a role in the pathogenesis of atherosclerosis by adhering to blood cells.

Tocopherol 투여가 사람의 HDL Subfractions 와 Lipoprotein Pattern에 미치는 영향 (Effect of Tocopherol Supplementation on Serum HDL Subfractions and Lipoprotein Pattern in College Women)

  • Twenty three healty women on self - selected diet were given 800IU of tocopherol daily for 4 weeks. The levels of tocopherol in plasma and red blood cells( RBC ) and total choles terol, TG, HDL- chol, HDL subfractions and lipoprotein pattern in serum were determined pre-and postregimen at 2 and 4 weeks. No significant change was noted in VLDL, LDL, HDL fraction and LDL/HDL ratio separated by electrophoresis, even though HDL fraction was decreased at 2 wk but slightly increased at 4 wk. There were also no significant changes in the relative amount of HDL-chol and VLDL-chol when cholesterol content of each lipoprotein fraction was assayed. A transient increase in LDL-chol was observed at 2 wk but returned to the pretreatment level. Plasma and RBC tocopherol levels were significantly ( p ^lt;0.05 ) increased and decreased respectively, at both 2 and 4 wk, and LDL-chol was positively correlated to plasma tocopherol level ( p<0.05 ) but not to RBC tocopherol. However HDL-chol fractionated by heparin-Mn was increased at 2 and 4 wk by a significant increase in $HDL_{2}$-chol but no change in $HDL_{3}$-chol, which resulted in a significant reduction of $HDL_{3}$/HDL ratio and increase of $HDL_{2}/HDL$ and $HDL_{2}/HDL_{3}$ ratios. HDL- chol was negatively correlated to the levels of LDL-chol ( p<0.05), VLDL-chol (p<0.01), and T-chol/HDL ratio ( p<0.01 ). Serum TG was significantly decreased ( p <0.05 ) but total cholesterol was decreased only at 4 weeks.

Fluorescence Immunoassy of HDL and LDL Using Protein A LB Film

  • A fluorometric detection technique for HDL (High Density Lipoprotein) and LDL (Low Density Lipoprotein) was developed for application in a fiber-optic immunosensor using a protein A Langmuir-Blodgget (LB) film. For the fluorescence immunoassay, antibodies specific to HDL or LDL were imobilied on the protein A LB film, and a fluorescence amplification method was developed to overcome their weak fluorescence. The deposition of protein A using the LB technique was monitored using a surface pressure-are $({\pi}-A)$ curve, and the antibody immobilization of the protein A LB film was experimentally verified. The immobilized antibody was used to separate only HDL and LDL from a sample, then the fluorescence of he separated HDL or LDL was amplified. The amount of LDL or HDL was measured using the developed fiber optic fluorescence detection system. The optical properties resulting from the reaction of HDL or LDL with o-phtaldialdehyde, detection range, response time, and stability of the immunoassay were all investigated. The respective detection ranges for HDL and LDL were sufficient to diagnose the risk of coronary heart disease. The amplification step increased the sensitivity, while selective separation using the immobilized antibody led to linearity in the sensor signal. The regeneration of the antibody-immobilized substrate could produce a stable and reproducible immunosensor.

CPs(단삼, 삼칠, 빙편 복합제)가 고지혈증에 미치는 영향 (Effects of CPs (Combined Preparations) of Radix Salvia Miltiorrhiza, Radix Notoginseng, and Borneolum on Hyperlipidemia)

  • Objectives: In order to determine the effect of CPs, experimental studies were performed in hyperlipidemia humans. The contents of serum total cholesterol, TG (triglyceride), LDL cholesterol, HDL cholesterol, AST/ALT and BUN/creatinine were measured. Methods: We made a comparative study of total cholesterol, TG (triglyceride), LDL cholesterol, HDL cholesterol, AST/ALT and BUN/creatinine during one month off and on for twenty patients with hyperlipidemia who visited Gunpo Oriental Medical Center of Wonkwang University from December 2002 to July 2003. Results: The following result were obtained: 1. The serum TG and total cholesterol of patients who took CPs for one month showed significant (p<0.01) decrease. 2. The serum HDL-cholesterol and LDL-cholesterol of patients who took CPs for one month showed significantly increase in HDL, and decrease in LDL (p<0.05). 3. The serum AST/ALT and BUN/creatinine of patients who took CPs for one month showed no significant effects. Conclusions: According to the results mentioned above, the effects of CPs on serum triglyceride and total cholesterol of hyperlipidemia patients were significantly confirmed for decreasing effects.

우리나라 성인에서 혈청 지질성분비가 허혈성 심장질환 발생에 미치는 위험도 평가: 코호트 연구 (Associations of Serum Lipid Profiles with Incidence of Ischemic Heart Diseases in Korean Adults: Retrospective Cohort Study)

  • 본 연구에서는 우리나라 30세 이상 성인들의 허혈성심장질환(ischemic heart disease, IHD) 발생률을 계산하고, 지질지표(콜레스테롤, 중성지방, 고밀도지단백콜레스테롤, 저밀도지단백콜레스테롤)가 허혈성심장질환 발생에 미치는 위험도를 파악하고자 19개 대학 및 종합병원의 건강검진센터에서 검진을 받은 417,642명을 대상으로 1993년 9월부터 2009년 6월까지 평균 8.5년동안 허혈성심장질환의 발생을 추적관찰하였다. 자료수집은 검진자들에 대한 설문조사지를 이용하였고, 허혈성심장질환의 발생여부는 국민건강보험공단의 데이터베이스에서 확인하였다. 발생률은 발생밀도로 계산하였고, 혈청지질 지표에 따른 허혈성심장질환의 발생 위험도는 콕스의 비례위험 회귀모형을 이용하여 연령, BMI, 생활양식을 보정한 상태에서 성별에 따른 위험요인별 위험비와 95% 신뢰구간을 계산하였다. 연구결과 TC/HDL 비의 증가에 따라 IHD의 발생 위험비는 남자에서 1.21배에서 1.84배까지, 여자는 1.26배에서 1.86배까지 증가하였으며, TG/HDL 비의 증가에 따른 IHD의 발생 위험비는 남자에서 1.17배에서 1.49배까지, 여자는 1.42배에서 1.97배까지, LDL/HDL 비에 따라 IHD의 발생 위험비는 남자에서 1.26배에서 1.82배까지, 여자는 1.26배에서 1.68배까지 증가하였다. 결론적으로 혈청지질지표는 심혈관질환의 중요한 위험요인으로 총콜레스테롤, 저밀도지단백콜레스테롤, 중성지방은 혈중 농도가 높을수록, 고밀도지단백콜레스테롤은 낮을수록 IHD의 위험이 높아지는 것으로 나타났고, TC/HDL 비, TG/HDL 비, LDL/HDL 비에서 단독의 지질지표보다 위험도가 더 높게 나타나는 경향이 있었다. 따라서 추후 허혈성 심장질환의 예방 및 관리에는 혈청지질지표의 비도 감안하여야 한다.

인삼 사포닌 분획이 혈청 지단백질 분포에 미치는 영향 (Effect of Saponin Fraction of Panax ginseng C.A. Meyer on Blood Serum Lipoprotein Distribution of Cholesterol Fed Rabbits)

  • 고려 인삼 뿌리에서 얻은 사포닌 분획이 고cholesterol 식이로 2주내지 4주간 사육한 토끼의 혈청 지단백질 분포에 미치는 영향을 조사하였다. 고cholesterol 식이를 인삼과 함께(시험군) 또는 단독(대조군)으로 2~4주간 투여한 토끼의 혈청 극저밀도 지단백질(VLDL)과 저밀도 지단백질(LDL)이 다같이 크게 증가하는 반면 고밀도 지단백질(HDL)의 농도는 저하 하였다. 그러나 VLDL과 LDL 농도의 증가율이나 HDL 농도의 저하율은 인삼을 투여한 토끼군에서 현저히 적었으며 인삼사포닌의 고cholesterol 혈증 개선작용이 VLDL의 LDL 농도를 낮추고 HDL의 농도를 높이기 때문인 것으로 생각된다.
