The Clinical Significance of Bioelectrical Impedance Analysis for The Diagnosis of Obesity on Elementary Students (초등학교 아동의 비만 진단에 있어서 생체 전기저항 분석법의 의의)

  • Purpose: The purpose of our study is to provide useful information for me-asurement methods of childhood obesity and to provide clinical significance of bioelectrical impedance analysis. Methods: We measured height, weight, waist to hip ratio, skinfold thickness and bioelectrical impedance from 206 elementary students in Kwangju from May to July 1997. Results: In males; 1) There were statistically high correlations between tri-ceps skinfold thickness and body mass index (BMI), percentage of body fat (%BF), weight, Rohler index (RI), total fat weight (TFW), waist to hip (W/H) ratio, height, lean body mass (LBM) (all, p<0.001). 2) There were statistically high correlations between subscapular skinfold thickness and BMI, RI, weig-ht, %BF, TBW (all, p<0.001). 3) There were statistically high correlations between %BF and BMI, triceps skinfold thickness, RI, TFW, subscapular skinfold thickness, weight (all, p<0.001). In females; 4) There were statistically high correlations between triceps skinfold thickness and BMI, weight, LBM, height, %BF (all, p<0.001). 5) There were statistically significant correlations between subscapular skinfold thickness and BMI, weight, LBM, RI, height. 6) There were statistically high correlations between percentage of body fat and BMI, RI, triceps skinfold thickness. The percentage of body fat according to the bioelectrical impedance analysis highly correlated with that calculated by skinfold thickness in males (r=0.57, p<0.01) and in females (r=0.30, p<0.01). Conclusion: The results of our study suggests that bioelectrical impedance analysis is a useful method of estimating body fat in the childhood obesity.

Interobserver and Intraobserver Reproducibility of SUL Measurements in Reference Organs on FDG PET/CT (FDG PET/CT 검사 시 참고장기에서 측정한, 제지방체중으로 표준화한 표준화 섭취계수의 관찰자 사이 및 관찰자 내 재현성에 대한 연구)

  • Purpose: The use of SUV which should be normalized by lean body mass (LBM) is recommended for PET response criteria in solid tumors. LBM which was determined by whole body CT was used for SUV normalization (SUL) in this study. The purpose of the present study was to assess interobserver and intraobserver reproducibility of SUL measurements in reference organs. Materials and Methods: F-18 FDG PET/CT was conducted on 52 subjects and LBMs were directly determine by whole body CT for normalization of SUV. The 3 cm diameter spherical VOI, $1\times2$ cm cylindrical VOI, 2 cm diameter spherical VOI were placed in the liver, descending aorta and spleen, respectively. Experienced two observers measured SULmax and SULmean in each organ. Repeated measurements were conducted two weeks apart by observer 1 blind to previous results. Similarly, measurements were conducted on the same patients by observer 2. For assessing reproducibility(or repeatability), the paired t-test, Pearson's correlation coefficients (CC), and technical error of measurement (TEM) were calculated. Results: For interobserver reproducibility in liver SULmax and SULmean, no significant differences were found between observers(paired t-test, P=0.536, 0.293, respectively). CC and TEM for liver SULmean were 0.909 (P=0.000) and 0.067 SUL unit, respectively. Corresponding figures for liver SULmax were 0.882 (P=0.000) and 0.117 SUL unit, respectively. For intraobserver reproducibility in liver SULmax and SULmean, no significant differences were observed within observer1 (paired t-test, P=0.374, 0.268, respectively). CC and TEM for liver SULmean were 0.924 (P=0.000) and 0.061 SUL, respectively. Corresponding figures for liver SULmax were 0.908 (P=0.000) and 0.104 SUL, respectively. Similarly, no significant differences were found in SULmax and SULmean of the spleen and aorta between observers. Conclusion: The current study demonstrated that both SULmean and SULmax measurements in normal reference organs are highly reproducible. Reproducibility of SULmean in reference organs were slightly better than SULmax. Interobsever technical error of measurement was less than 0.10 SUL unit for liver SULmean, and 0.12 SUL unit for liver SULmax. Intraobsever technical error of measurement was less than 0.07 SUL unit for liver SULmean, and 0.11 SUL unit for liver SULmax.

Effect of Aerobic Exercise on Factors Relative to the Brain Nerve Growth in Girls (유산소운동이 여자 아동의 뇌세포 생성 관련인자에 미치는 영향)

  • Exercise can favorably influence brain plasticity by facilitating neurogeneration, neuroadaptivity, and neuroprotection. Aerobic exercise has been reported to change brain nerve growth factors (growth hormone, insulin like factor-1, estrogen and serotonin). The purpose of this study was to demonstrate the effects of aerobic exercise for 12 weeks on brain nerve growth factors in girls. Fourteen female participants in elementary school grades 1 through 3 were randomly allocated to the exercise group (EG, n=6) and control group (CG, n=8). The EG participated in 60 minutes of modified ballet exercise as aerobic training three days a week for 12 weeks. Based on comparison between groups by two-way ANOVA with repeated measures, aerobic exercise program participants experienced decreased weight (p<0.01), BMI (p<0.01), fat mass (p<0.001), fat percent (p<0.001) and increased LBM (lean body mass) percent (p<0.001). In addition, we detected that aerobic exercise decreased the level of serotonin (p<0.05) and increased the level of GH (p<0.05) and IGF-1 (p<0.05). These findings suggest that aerobic exercise programs can be an efficient intervention to change body composition, alleviate central fatigue, improve brain function, and induce brain cell proliferation in girls.

Correlation of SUV According to the Body Weight and Body Surface Area in PET/CT Imaging (PET/CT 검사 시 체중과 체표면적에 따른 SUV의 상관관계)

  • Purpose: There are several methods of measuring SUV in PET/CT imaging including $SUV_{bw}$ which uses the body weight, $SUV_{bsa}$ which that uses body surface area and $SUV_{lbm}$ which uses lean body mass. Currently, Seoul National University Hospital uses $SUV_{bw}$ method which minimizes the variability. In this study, we compared and analyzed the correlation between $SUV_{bw}$ and $SUV_{bsa}$ according to patients' body mass index. Materials and Methods: Using Biograph mCT40 (Siemens, Germany), we conducted $^{18}F-FDG$ PET/CT imaging on 70 patients (41 males, 29 females; ages $58.04{\pm}12.44$). We classified the patients as underweight (BMI<20), normal weight (20$${\leq_-}$$BMI<25), overweight (25$${\leq_-}$$BMI<30), obese (30$${\leq_-}$$BMI<35) and severely obese (35$${\leq_-}$$BMI) according to the patient's sex, age and BIM. Then, bone, liver and lungs were set as ROI for calculation of maximum values of $SUV_{bw}$ and $SUV_{bsa}$, through Syngo.via VA11A analysis program. Results: Comparing the five groups divided according to the BMI by the standard differences between $SUV_{bw}$ to $SUV_{bsa}$, $SUV_{max}$ was measured to be $0.66{\pm}0.15$, $0.78{\pm}0.35$, $0.77{\pm}0.21$, $1.00{\pm}0.44$, $1.53{\pm}0.38$ for bones in underweight, normal weight, overweight, obese and severely obese groups, respectively. For liver, values of $SUV_{max}$ were $1.64{\pm}0.16$, $2.06{\pm}0.34$, $2.19{\pm}0.21$, $2.52{\pm}0.21$ and $2.74{\pm}0.40$ in the same order. And for lung, values of $SUV_{max}$ were $0.69{\pm}0.33$, $0.54{\pm}0.17$, $0.62{\pm}0.23$, $0.83{\pm0.29}$, $1.03{\pm}0.30$. Conclusion: By comparing and analyzing the differences between $SUV_{bw}$ and $SUV_{bsa}$ in this study, it was found that the differences between $SUV_{bw}$ and $SUV_{bsa}$ increased as patient's BMI increased. Thus, there is room for error in the values of SUV depending on the methods of calculations, and appropriate methods must be applied according to the circumstances in clinical settings.

Effects of a Weight Control Program on Anthropometric Measurements and Serum Lipid Profiles of Obese Elementary Students (초등학교 비만관리 프로그램이 비만 아동의 체구성 성분과 혈청 지질 수준에 미치는 영향)

  • The purpose of this study was to investigate the effects of a 12 week weight control program, which included nutrition education and regular physical fitness, on body weight, degree of obesity, BMI, and lowering of blood lipids in 16 obese male elementary school students. Changes in waist and hip circumferences and their ratios were measured at the conclusion of the 12 week program conducted from September 9 to November 25, 2005. Body weight after 12 weeks on the program slightly reduced from 55.4${\pm}$10.9 kg to 54.4${\pm}$9.7 kg. Degree of obesity and BMI decreased from 121.3${\pm}$8.1%, and 25.9${\pm}$3.3 to 120.0${\pm}$8.2% and, 25.6${\pm}$2.7, respectively. Body fat percentage decreased slightly from 38.3${\pm}$4.7% to 37.9${\pm}$5.2%. Waist to hip ratio did not change after 12 weeks. There were slight changes in weight of body fat and lean body mass from 22.12${\pm}$6.53 kg and 34.56${\pm}$4.75 kg to 21.68${\pm}$6.05 kg and 34.70${\pm}$4.54 kg, respectively. Also, after 12 weeks on the weight control program, body, waist, and hip circumferences decreased. However, there were no significant differences among the blood lipid components, including triglycerides, total cholesterol, and HDL-cholesterol. Finally, body weight, degree of obesity, BMI, and body fat showed more significant correlations to waist, hip circumference and their ratio than to blood lipid levels.
