Morphological Characteristics and Pathogenicity of Streptococcus iniae (Streptococcus iniae의 형태학적 특성과 병원성)

  • Streptococcus iniae is one of the major bacterial pathogens of cultured olive flounder, Paralichthys olivaceus in Korea. This study was carried out to investigate the interaction between morphological characteristics and immune response such as phagocytosis and serum killing activity. They were differentiated phenotypically into two groups, the viscous colonies and the non-viscous colonies, but the strains in both groups were similar in physiological and biochemical characteristics. Electron microscopic examination of the viscous form revealed thick, electron-dense exopolysaccharide materials surrounding polycationic ferritin-stained cells, while the exopolysaccharide material was absent around the non-viscous form of S. iniae. The viscous type strains were disclosed more virulent than those of non-viscous type for olive flounder. The viscous strains were resistant to normal serum killing activity, while the non-viscous strains were susceptible to bactericidal activity of normal serum from olive flounder. The viscous strains were more resistant to opsonophagocytosis and decreased the chemiluminescence response of head kidney macrophages of olive flounder. All of the tested non-viscous S. iniae strains were efficiently destroyed by the macrophage after phagocytosis. Both the viscous and the non-viscous strains invaded and replicated in cultured fish cell line (CHSE-2l4). This cellular invasion may contribute to the pathogenesis of invasive S. iniae infection.

Nutritional Assessment and the Effectiveness of Dietary Counseling in Infants and Young Children with Iron Deficiency Anemia (철결핍성 빈혈을 가진 영유아에서 영양학적 평가 및 영양상담 효과)

  • Purpose : Iron deficiency is still the most common nutrient deficient disorder despite the improvement in general health and nutrition. This study is designed to evaluate the dietary history of infants and young children with iron deficiency anemia(IDA) and the effects of nutritional counseling. Methods : This study was conducted on 120 children from 6 to 36 months of age with IDA. Their parents completed a questionnaire and took counsel for nutrition. IDA was defined as Hb <11.0 g/dL, ferritin <10 ng.mL or transferrin saturation <15%, or Hb increase >1 g/dL after iron preparation. The questionnaire consisted of their feeding patterns, weaning time and kinds of food. Results : In the 120 infants and young children aged from 6 to 36 months, the parents of 82 cases was counseled about nutrition. Fifty six infants among 82 cases have started weaning and the main foods of weaning were rice and/or rice gruel. Nutritional problems in weaning were that some children over one year of age were using a bottle, and parents restricted weaning food at will because of allergic disease or chronic disease. Most parents were satisfied with the nutritional counseling given from a clinical dietitian and showed good compliance. Conclusion : Many infants and young children with IDA were provided with non iron-fortified foods and made an inadequate wean. Most parents were satisfied with the nutritional counseling and showed good compliance. The need of dietary counseling was required for prevention and treatment of iron deficiency anemia because of inadequate weaning.

Efficacy and Safety of Intraveous Iron Sucrose in the Perioperatively Anemic Patients of Gastrectomy (위 절제 수술 전후 빈혈 환자에 있어 철수크로오스의 유효성 및 안전성)

  • Purpose: This study was aimed to evaluate the efficacy and safety of of intravenous iron sucrose ($Venoferrum^{(R)}$) for treating the perioperative anemic gastrectomy patients. Materials and Methods: From September 2006 to February 2007 at Seoul National University Hospital, the gastrectomy patients who displayed perioperative anemia (7.0 g/dl $\leq$ hemoglobin levels (Hb) < 11.0 g/dl) and who were admitted or visited the outpatient clinic of the Department of surgery, were divided into two groups. The preoperative (${\leq}\;2{\sim}3$ weeks before gastrectomy) or postoperative ($\geq$ 1 month after gastrectomy) patients without evidence of acute bleeding were included into Group 1. The immediate postoperative (< 1 month after gastrectomy) patients with stable vital signs were included into Group 2. The age, gender, diagnosis, Hb, hematocrit (Hct), mean corpuscular volume (MCV), serum ferritin (SF), total iron binding capacity (TIBC), serum iron and reticulocyte counts (RC) were evaluated before and after intravenous iron sucrose administration. The adverse effects of drugs were investigated. Results: The number of patients of group 1 and group 2 was 79 and 46, respectively. In group 1, there was a statistically significant difference in the Hb, Hct, MCV, SF, RC and TIBC with each mean change of 1.3 g/dl, 4.1%, 3.1ft, 195 ng/ml, 0.2% and -86.4 ug/dl, respectively. In group 2, there was a statistically significant difference in the Hb, Hct, MCV, SF and RC with each mean change of 1.8 g/dl, 6.1%, 3.4fl, 260 ng/ml and 0.3%, respectively. Two patients (1.6%) suffered local thrombophlebitis as an adverse effect. Conclusion: Intravenous iron sucrose for the perioperative anemia of gastrectomy patients was efficacious in the short period without significant adverse effects.
