Purpose: The purpose of this study was to investigate the balance confidence and balance ability among community-residing stroke patients. Methods: The sample for this study was a convenience sample obtained from patients who visited welfare centers for the disabled in three cities from October to December in 2009. The Activities-specific Balance Confidence Scale and Berg Balance Scale were used to measure confidence in task performance and balance ability. Results: The level of balance confidence and balance ability were low in these subjects. There was a significant relationship between balance confidence and balance ability (r=.424, p=.000). Subjects with less balance ability felt less balance confidence. Conclusion: Among community-residing stroke patients, falls are common. Falls prevention which may contribute to the incidence of fall and fall related injury should be part of stroke rehabilitation plan. Also, it is necessary that an intervention for falls prevention after stroke should take into account factors such as the balance confidence and balance ability.
PURPOSE: The initiation of the trunk muscles in stroke patients is delayed because the muscles involved in reach arm are activated earlier than the trunk muscles. The objective of this study was to examine the effects of mobility, balance, and trunk control ability through selective trunk exercise (STE) in patients with chronic stroke. METHODS: A randomized pre-test and post-test control group design was initially used, with subjects randomly assigned to the STE group (n=15) and a control group (n=14). All groups underwent physical therapy based on the neuro-developmental therapy (NDT) for 30 minutes a day, five times per week for four weeks. Additionally, the STE group did the trunk exercise for 30 minutes a day, three times per week for four weeks. The timed up and go test (TUG), Berg balance scale (BBS), and trunk impairment scale (TIS) were used for assessment. RESULTS: The scores of the TUG, BBS, dynamic sitting balance subscale, and coordination subscale of TIS improved significantly in both groups but the improvement was more pronounced in the STE group (p<.05). This study showed a large effect on the scores of the TIS coordination subscale (d=.93) (p<.05), TIS dynamic balance subscale (d=.81) (p<.05), TUG (d=.75) (p<.05), and BBS (d=.73) (p<.05). CONCLUSION: The combined STE and NDT program showed improvements in measures of mobility, balance, and trunk control in chronic stroke patients. These results suggest that STE should be considered to be included in the treatment program for patients with chronic stroke.
Objective : The purpose of this study was to describe the Progressive Intervention strategy applied ICF (International Classification of Functioning, Disability and Health) Tool about Gait for TBI(Traumatic Brain Injury) patient. Methods : The data was collected by TBI patient. We applied the progressive Intervention strategy applied ICF Tool to TBI patient. Parameters of result were collected for using the Berg balance scale, TETRAX, Timed up and go test, Sit to stand test and ICF Evaluation Display Results : Significant differences were observed the TBI patient for Berg balance scale, TETRAX, Timed up and go test, Sit to stand test and ICF Evaluation. TBI patient improved all test. Conclusion : Progressive Intervention strategy applied ICF Tool is very useful and effective. It is effective in clinical practice.
Background: Hemispatial neglect is defined as the failure to attend, report, respond, or orient toward meaningful stimuli provided in the contralateral side of a brain lesion. Objects: This study was conducted to find out the effect of dynamic trunk equilibirum exercise for stroke patients with hemi-spatial neglect. Methods: This study included 21 stroke subjects, randomly assigned to either the experimental group or the control group. The exercise program consisted of 5 sessions of 20 minutes per week during 4 weeks. The line-bisection test, the Albert test, the balance function score, the Berg balance scale, the postural assessment scale for stroke and the modified Barthel index were measured before and after training. All data were analyzed using SPSS 12.0 for Windows. Between-group and within-group comparison was analyzed by using Independent t-test and Paired t-test respectively. Results: The results of study were as follows: There were significant differences between before and after intervention in both group (p<.05). There were significant differences in the line-bisection test, Albert test, balance function score, Berg balance scale, postural assessment scale for stroke and modified Barthel index between the experimental group and the control group (p<.05). Conclusion: Dynamic trunk equilibrium exercise had a positive effect on patients' neglect, balance ability and activities of daily living. Further studies are required to generalize the results of this study.
PURPOSE: This study examined the effects of balance exercises accompanied by ankle fixation on the healthy side on the balance control ability and gait of patients with subacute stroke. METHODS: The study was conducted on 23 patients with subacute stroke. Eleven people were assigned randomly to the experimental group who performed balance exercises with ankle fixation on the healthy side, and 12 were assigned to the control group who performed balance exercises without ankle fixation. The intervention was conducted for 30 minutes three times a week for four weeks. The Berg Balance Scale, a weight-bearing ratio measurement, was performed to evaluate the balance ability before and after intervention. The gait symmetry, walking speed, cadence, step length, and 10-meter walk test were conducted to evaluate the walking ability. RESULTS: A significant difference in the Berg Balance Scale was observed between before and after the intervention in the experimental group. A comparison of the two groups also revealed a significant difference. Significant differences in the gait symmetry, walking speed, and step length measurements were observed before and after the intervention in the experimental group, and significant differences in the gait symmetry and step length measurements were observed between the two groups. CONCLUSION: Through this study, balance exercises with the healthy side ankle fixed showed qualitative improvement in the balance and walking ability, suggesting future directions for the rehabilitation treatment of stroke patients.
PURPOSE: This study compared the effectiveness of trunk control exercise performed on an unstable surface with that of general balance exercise on dynamic balance in the patients with chronic stroke. METHODS: The persons of this study were thirty-seven chronic stroke patients were recruited and randomly divided into 2 groups; (1) those who performed trunk control exercise on a foam roll and (2) those who performed general balance exercise. The exercises were performed 5 times a week for 4 weeks. To determine the effectiveness of the 2 types of exercises, we measured dynamic balance at the beginning of the exercises and again after 4 weeks at the completion of exercises program. RESULTS: After 4 weeks of exercise, both the groups showed increased Berg's balance scale and timed-up-and-go test (p<.001) scores. However, Trunk control exercise group was more effective than general balance exercise group was in increasing the Berg's balance scale (p<.01) and timed-up-and-go test (p<.05) scores. CONCLUSION: We suggest that trunk control exercise may be effective in increasing the balance ability of patients with chronic stroke than general balance exercise. Thus, trunk control exercise is important for such patients. Further studies are needed for better understanding of the effectiveness of trunk control exercise in chronic stroke patients.
Purpose: This study was intended to discover the effect of the turning training on figure of 8 tract on stroke patients' static and dynamic balance, as well as walking. Methods: A total of 42 stroke patients participated in this study. The training group was trained on the figure of 8 tract, while the control group was trained on the straight path for 30 minutes per day, for 4 weeks. Berg balance scale was used to measure the balance of the patients, before and after the training, and Good balance system was used to measure the sway speed and the distance of COG while standing. To measure the ability of walking, TUG and FSST were also used. Results: There were significant increases in the average score of Berg balance scale, in both groups after the training, and also significant difference between both groups were observed. The training group showed significant differences in the static balance ability, as well as anteroposterior and mediolateral sway speed. Further, there were significant increases in the dynamic balance ability, COP total distance, and TUG in both groups. The results showed more differences in comparing the control group with that of the training group. In FSST, there was no change in the control group, but there was a significant increase in the training group. Conclusion: Turning training on figure of 8 tract in stroke patients significantly increased the static and dynamic balance and walking ability. Based on the results, it can be seen that the training on the figure of 8 tract can influence the ability of balance and walking, which can lead to appropriate reactions to the change of environment and various tasks. Thus, it is assumed that turning training on figure of 8 tract as a means of improving the condition of stroke patients can be a meaningful program.
Purpose : The purpose of this study was to investigate the effects of curl-up and proprioceptive neuromuscular facilitation (PNF) neck flexion exercises on stroke patients' trunk muscle activity, ability to control the trunk, and balance by comparing two exercise methods. Methods : The study involved patients who had been diagnosed with stroke based on the results of computerized tomography or magnetic resonance imaging at O Hospital in Daegu, South Korea, between January and July 2020. In total, 30 subjects were selected and by flipping a coin, randomly assigned to an experimental group (n=15) that performed PNF neck flexion exercises and a control group (n=15) that performed curl-up exercises. Both groups received traditional rehabilitation therapy for 30 minutes a day five times a week for a six-week period. In addition, the experimental group performed PNF neck flexion exercises and the control group curl-up exercises for 15 minutes a day five times a week over the same period. The subjects' muscle activity in the rectus abdominis (RA), external oblique (EO), and internal oblique (IO) were measured before and after the experiment. The subjects' trunk impairment scale (TIS) and Berg balance scale (BBS) scores were also assessed. Paired t test was performed to measure the amount of statistical change before and after intervention in both groups. An independent sample T test was performed to measure the amount of statistical change between the two groups. Results : Both groups experienced statistically significant increases in their RA, IO and EO muscle activity, total TIS scores, and total BBS scores after the intervention. No statistically significant differences in the changes before and after the intervention were found for any of the resulting values between the two groups. Conclusion : A comprehensive review of the study's results suggested that neck flexion exercises using the PNF irradiation concept and curl-up exercise are effective in increasing stroke patients' abdominal muscle activity and improving trunk control ability and balance in chronic stroke patients.
Purpose : We studied the correlation between reference voluntary contraction (% RVC) of vastus lateralis (VL) and vastus medialis (VM), Berg balance scale (BBS), and timed-up & go (TUG) test. Methods : We recruited 30 stroke patients from a rehabilitation center at a hospital.? All subjects could walk with or without an assisting device. Subjects were evaluated with % RVC of VL and VM, BBS, and TUG. The data were analyzed using a Pearson correlation coefficient. Results : The % RVC of VL and VM and BBS (p<0.01) showed a significant positive correlation. TUG negatively correlated with % RVC of VL and VM and BBS (p<0.01). Conclusion : Lower extremity muscle activity increases balance and walking ability. We recommend the implementation of lower extremity strength exercises in the rehabilitation of stroke patient.
The purpose of the current study was to investigate the effect of repetitive transcranial magnetic stimulation (rTMS) in conjunction with mirror therapy on the balance function of patients with sub-acute stroke hemiparesis. This study was conducted with 36 subjects who were diagnosed with a hemiparesis due to stroke. Participants in the experimental (19 members) and control groups (17 members) received rTMS and sham rTMS during 10 minute sessions each, which were carried out five days per week for four weeks. This was followed by the mirror therapy over 30 minute sessions, which were carried out five days per week for four weeks. Motor recovery was assessed by balance index, dynamic limits of stability, Berg balance scale, and time up go test. The change values of the balance index ($-2.06{\pm}1.99$ versus $-0.41{\pm}1.11$), dynamic limits of stability ($3.68{\pm}2.71$ versus $1.17{\pm}2.38$), and time up go test ($-7.05{\pm}5.64$ score versus $-3.35{\pm}5.30$ score) were significantly higher in the experimental group than in the control group (p < 0.05). At post-test, balance index ($4.08{\pm}1.14$ versus $5.09{\pm}1.04$), dynamic limits of stability ($13.75{\pm}0.60$ versus $11.73{\pm}3.53$), and time up go test ($23.89{\pm}4.51$ versus $28.82{\pm}3.07$) were significantly higher in the experimental group than in the control group (p < 0.05). In the experimental group, significant differences were found in the pre- and post-test scores for the balance index, dynamic limits of stability, Berg balance scale, and time up go test (p < 0.01). In the control group, a significant difference was observed between the pre- and post-test only for the Berg balance scale and time up go test (p < 0.05). These findings demonstrate that the application of 1Hz rTMS in conjunction with mirror therapy can be helpful in improving the balance function of patients with sub-acute stroke hemiparesis, and this may be used as a practical adjunct to routine rehabilitation therapy.
