Purpose: The purpose of this study is to analyze the clinical characteristics of anaphylaxis and anaphylactic shock caused by bee venom. Methods: We retrospectively collected the data of the patients who experienced anaphylaxis caused by natural bee sting or acupuncture using bee venom from January 1999 to December 2008. Seventy subjects were divided into the shock and non-shock groups. The clinical characteristics, sources of bee venom, treatments and outcomes were compared between the two groups. Results: The mean age of the subjects was $45.5{\pm}16.3$ years old and the number of males was 44 (62.9%). There were 25 patients in the shock group and 45 in the non-shock group. The age was older (p=0.001) and females (p=0.003) were more frequent in the shock group. Transportation to the hospital via ambulance was more frequent in the shock group (p<0.001). No difference was found in species of bee between the two groups. The cephalic area, including the face, was the most common area of bee venom in both groups. Anaphylaxis caused by bee sting commonly occurred between July and October. Cutaneous and respiratory symptoms were the most frequent symptoms related to anaphylaxis. Cardiovascular and neurologic symptoms were more frequent in the shock group. The amount of intravenously administered fluid and subcutaneous injection of epinephrine were much more in the shock group than that in the non-shock group. Conclusion: Older age was the factors related to anaphylactic shock caused by bee venom. Further validation is needed to evaluate the gender factor associated with shock.
Background and Objective : Increasing interest in anti-aging and anti-wrinkling agents for the skin has triggered the recent outflow of researches and studies in this field. This study was designed to investigate the effects of bee venom on skin wrinkling and skin aging by testing the skin wrinkling, skin elasticity, trans-epidermal water loss (TEWL), free radical level, anti-oxidative agent level, and skin tissue after infusion of bee venom on hairless mouse. Materials and Methods : Fifteen hairless mice aged between 36~40 weeks were divided randomly into 3 Group; the Bee Venom Syringe Group, the Bee Venom Needle Group, and the control group. The Bee Venom Syringe Group were injected subcutaneously with bee venom (0.1cc in total) using an insulin syringe on three spots in the lumbar spine (one spot on the center and two spots 1~2cm to the side bilaterally). The Bee Venom Needle Group were pricked with bee venom-smeared acupuncture needles on three longitudinal spots in the lumbar spine each 1cm apart, after which the needles were removed 10 minutes later. The Control Group did not receive any form of intervention. All procedures took place thrice a week for four weeks, during which the mice were allowed free access to water and fodder. The mice were measured and compared in the weight, skin wrinkling scale, skin elasticity, and TEWL before and after the experiment. After the experiment, blood samples were taken to measure the free radical and anti-oxidative agent level, and the skin tissue was sliced for examination. Data was analyzed using the SPSS program (ver 12.0). The ANOVA analysis was used to compare and contrast the three groups, and t-test for paired samples was used to evaluate skin-wrinkling before and after experiment. The cut-off p-value of significance was set at p<0.05. Results : 1. Administration of bee venom did not cause serious weight loss or gain. 2. Compared to the control group, the Bee Venom Syringe Group and the Bee Venom Needle Group both showed a decrease in skin wrinkling scale after intervention. Especially, the Bee Venom Syringe Group showed a significant decrease (p<0.05). 3. Compared to the control group, the Bee Venom Syringe Group and the Bee Venom Needle Group both showed an increase in skin elasticity. Especially, the Bee Venom Syringe Group showed a significant increase (p<0.05). 4. No significant change in TEWL was found in the mice in all the three groups before and after experiment. 5. Free radical level was normal in all 15 mice in all the three groups, and anti-oxidative agent was not significantly different across the three groups. 6. The Bee Venom Syringe Group, the Bee Venom Needle Group, and the control group did not show any significant difference in the thickness of epidermis and dermis, infiltration of inflammatory cells, and skin wrinkling. The epidermis layer was relatively better preserved in the Bee Venom Syringe Group as compared to the Bee Venom Needle Group and the control group. Conclusion : Direct injection of bee venom on the hairless mouse using a syringe was found to improve wrinkling of the skin and increase skin elasticity but did not show effectiveness on skin dryness due to water loss. The bee venom appears to have suppressive effects on skin wrinkling, one of the symptoms of skin aging, through a process independent of suppression of free radicals or increase of anti-oxidative agent.
Objectives: We investigated the current status of the consent form for bee-venom pharmacopuncture therapy, which is using in Korean medicine hospitals. We suggest the development of a standard informed consent form. Method: Through the questionnaire survey, status of using informed consent form was identified at 24 Korean medicine hospitals. We analyze different types of informed consent form, which was developed by each hospitals. We investigated the types of informed consent forms for various medical procedures through electronic searches. A standard informed consent form for bee-venom pharmacopuncture therapy was developed based on the medical law and the standard informed consent form for medical procedures developed by Korea Fair Trade Mediation Agency. Result: In our survey, 65% of the hospitals do not use consent well, only 35% of the hospitals use informed consent form, and the most hospitals use self-developed informed consent form. As a result of analyzing the contents of informed consent form used in each hospitals, the explanation of diagnosis, treatment precautions, suggestions for other treatments, consequences of not performing the scheduled procedure, possibility of treatment change was insufficient. 48% of hospitals manage consent in recording on a chart, 39% in scanned documents, and 13% in digital electronic consent form. Conclusion: A standard informed consent form for Bee-venom pharmacopuncture therapy include diagnosis, effectivness, necessity, indications, method, skin reaction test, hypersensitivity questionnaire, treatment precautions, possible hypersensitivity reactions and countermeasures, suggestions for other treatments, consequences of not performing the scheduled procedure, possibility of treatment change and the name of doctor.
Objectives: The purpose of this report is to find out how Mountain Ginseng Pharmacopuncture(MGP) and Sweet Bee Venom(SBV) treatments are effective on prostate cancer patients by monitoring Prostate specific antigen(PSA) values. Methods: We treated two prostate cancer patients with MGP and SBV from October 2008 to April 2011. One patient had localized prostate cancer, the other was in the terminal stage of prostate cancer with lung and bone metastasis and both had been receiving western anticancer therapy. We had monitored the changes of PSA value. Results: In case 1, MGP and SBV treatments seemed to be helpful in preventing the recurrence of localized prostate cancer. In case 2, PSA value was decreased by MGP treatment. Conclusions: It is conceivable that MGP and SBV are effective treatments for patients with prostate cancer.
Objectives: This study aims to evaluate the efficacy of pharmacopuncture in the treatment of acute peripheral facial paralysis (APFP) through a meta-analysis of randomized controlled trials to assess the clinical evidence for pharmacopuncture as a treatment for PFP. Methods: Literature was searched using databases. The search terms used were "pharmacopuncture," "sweet BV," "apipuncture," "apitoxin," "injection acupuncture," and "herbal acupuncture" in English and "bongchim," "bongyakchim," and "yakchim" in Korean. The RevMan 5.2 program was used for meta-analysis. Results: In all, 1902 studies were screened and the full texts of 219 articles were reviewed. Of these, nine studies were included in the systematic review and nine studies were included in the meta-analysis. Treatments utilizing bee venom, sweet bee venom (SBV), Soyeom, Calculus Bovis Fel Ursi Moschus (BUM), Hominis Placenta, and Suseunghwagang (SSHG) were observed in nine RCTs. In the forest plot, the diamond favors the PA treatment against the control treatment. (95% CI: 0.34 [0.08, 0.60], Z = 2.57, P = 0.01), which Our meta-analysis in RevMan 5.2 confirmed the significant efficacy of PA in treating APFP. Conclusion: Our systematic review and meta-analysis suggest that treatments using Soyeom, bee venom, and BUM are effective in APFP and they reduce the duration of postauricular pain occurring at the acute stage of APFP. However, due to small effect size, further investigation is needed.

This report presents the clinical progress of a 50-year-old male diagnosed with a left acetabular fracture and left rib fracture after falling from a height of 3 m. The patient was treated nonsurgically and underwent complex traditional Korean medicine (TKM) treatment and rehabilitation, including acupuncture, electroacupuncture, pharmacopuncture, bee venom, herbal medicine, physiotherapy, and moxibustion. Patient progress was assessed using the numerical rating scale (NRS), manual muscle test (MMT), and modified Harris Hip Score (mHHS). During 113 days of hospitalization, the NRS score decreased from 7 to 3, and the MMT grade increased to normal in the hip, knee, and ankle joints. The mHHS score increased from 26 to 99 points. Radiographs and computed tomography scans of the left acetabulum were obtained periodically, and fracture union was successfully achieved. To our knowledge, this is the first case report of complex TKM treatment and rehabilitation of acetabular fractures.
Objectives This study was carried out to investigate the experimental effects of Lonicerae Caulis and Bee-venom Acupuncture in Yanglingquan (陽陵泉, GB34) that have clinical efficacy in the Rheumatoid Arthritis. Methods Materials of present study are Lonicerae Caulis Extracts (LCE), Bee-Venom Acupuncture (BVP), Sprague-dawley rats (250 g or so, ♂), and various kinds of needing experimental studies. We measured several experimental items of the rats with the arthritis induced by Freund's complete adjuvant (0.2 ml/kg), such as body weight, rate of paw edema, analgesic effect by hot plate method, WBC, TNF-${\alpha}$ cytokine and IL-10 cytokine. Rats were divided into four groups; Normal group that was treated with normal saline 1.0 ml (o.p) and $15{\mu}l/kg$ (GB34) to normal rats, Control group that was treated with normal saline 1.0 ml (o.p) and $15{\mu}l/kg$ (GB34) to pathologic model rats induced by Freund's complete adjuvant 0.2 ml/kg, Experimental group A that was treated with LCE 1.0 ml (o.p) and normal saline $15{\mu}l/kg$ (GB34) to pathologic model rats induced by Freund's complete adjuvant 0.2 ml/kg and Experimental group B that was treated with LCE 1.0 ml (o.p) and BVP $15{\mu}l/kg$ (GB34) to pathologic model rats induced by Freund's complete adjuvant 0.2 ml/kg, and the experiment took over after 28 days. The results were analysed using SPSS for windows 12.0. Results Experimental group A showed the increase in body weight, paw licking times and IL-10 cytokine compared to Control group. Also it was decreased in rate of paw edema, WBC, and TNF-${\alpha}$ cytokine compared with Control group. Experimental group B showed the increase in body weight, paw licking times, and IL-10 cytokine compared to Control group, and showed the decrease in rate of paw edema, WBC and TNF-${\alpha}$ cytokine compared to Control group. Especially TNF-${\alpha}$ cytokine and rate of paw edema were accepted statistical significance compared with Control group. Conclusions It is suggested that Lonicerae Caulis (o.p) and Bee-venom Acupuncture (GB34) can be used in the treatment of rheumatoid arthritis.
Objectives : This study was performed to report the effect of oriental medical treatments on the 4 patients with lateral epicondylitis of the elbow. Methods : The 4 patients were treated by Bee Venom acupuncture, Dong-Qi acupuncture, acupuncture on tender points of the origin of the extensor carpi radialis and taping on the extensor carpi radialis. The state of patients was measured by visual analog scale(VAS), elbow range of motion(ROM) and patient's subjective symptom grade(PSSG). Results : After mean 9.5 times of treatments, the recovery of elbow ROM was measured and significant pain relief was seen as the average of VAS changed from 10 to 1.25 and average of PSSG changed from 2.75 to 0.5 grade. Conclusions : These results suggest that oriental medical treatments including Bee Venom acupuncture, Dong-Qi acupuncture, acupuncture, taping are effective to improve the lateral epicondylitis of the elbow.
Objectives: This study was performed to analyze a 13-week repeated dose toxicity test of Sweet Bee Venom (SBV) extracted from bee venom and administered in Sprague-Dawley (SD) rats. Methods: Male and female 5-week-old SD rats were treated once daily with SBV (high-dosage group: 0.28 mg/kg; medium-dosage group: 0.14 mg/kg; or low-dosage group: 0.07 mg/kg) for 13 weeks. Normal saline was administered to the control group in a similar manner (0.2 mL/kg). We conducted clinical observations, body weight measurements, ophthalmic examinations, urinalyses, hematology and biochemistry tests, and histological observations using hematoxylin and eosin (H&E) staining to identify any abnormalities caused by the SBV treatment. Results: During this study, no mortality was observed in any of the experimental groups. Hyperemia and a movement disorder were observed around the area of in all groups that received SBV treatment, with a higher occurrence in rats treated with a higher dosage. Male rats receiving in the high-dosage group showed a significant decrease in weight during the treatment period. Compared to the control group, no significant changes in the ophthalmic parameters, the urine analyses, the complete blood cell count (CBC), and the biochemistry in the groups treated with SBV. Compared to the control group, some changes in organ weights were observed in the medium-and the high-dosage groups, but the low-dosage group showed no significant changes. Histological examination of thigh muscle indicated cell infiltration, inflammation, degeneration, and necrosis of muscle fiber, as well as fibrosis, in both the medium- and the high-dosage groups. Fatty liver change was observed in the periportal area of rats receiving medium and high dosages of SBV. No other organ abnormalities were observed. Conclusion: Our findings suggest that the No Observed Adverse Effect Level (NOAEL) of SBV is approximately 0.07 mg/kg in male and female SD rats.
Methods : This study was conducted to carry out quantitative evaluation using HPLC Content analysis was done using HPLC Results : According to HPLC analysis, each BVA-1 contained approximately $0.36{\mu}g$ melittin, and BVA-2 contained approximately $0.54{\mu}g$ melittin. But the volume of coating was so minute, slight difference exists between each needle. Conclusions : Above results indicate that the bee venom acupuncture can complement shortcomings of syringe usage as a part of Oriental medicine treatment, but extensive researches should be done for further verification.
