A Case of Anaplastic Carcinoma Arising Ectopic Thyroid (이소성 갑상선에 발생한 역형성 암종 1예)

  • Ectopic thyroid glands generally occur in the midline as a result of abnormal median migration, and their presence lateral to the midline is extremly rare. We report a 83-year-old male with anaplastic carcinoma admix papillary carcinoma in the extrathyroid area. We suspected left lateral neck metastasis on preoperative fine needle aspiration result and computed tomography. the patient underwent total thyroidectomy and left selective neck dissection(level II, III, IV, V). The patient was diagnosed as having an ectopic thyroid gland on the lateral neck with anaplastic carcinoma admix papillry carcinoma. The patient is alive without incident of tumor recurrence at 5month after surgery and radiotherapy.

A Case of Well-Differentiated Liposarcoma on the Anterior and Posterior Neck (전, 후경부에 동시 발생한 호분화성 지방육종 1예)

  • Liposarcoma is a malignant tumor originated from adipose tissue, accounting for 20% of all soft tissue sarcomas. A 69-year-old man with anterior and posterior neck mass visited to our department and we performed an excisional biopsy in level I. Histopathological examination revealed liposarcoma, so we performed wide excision in anterior and posterior neck, and adjuvant radiotherapy. We report a unique case of well-differentiated liposarcoma on anterior and posterior neck with literature review. To our best knowledge, such case has not been reported earlier in Korea previously.

  • Salivary gland tumors take possession of almost 5% in head and neck malignancies. Among these, mucoepidermoid carcinoma(MEC) is most common malignany in major salivary glands(12-29%) and the parotid gland is most predilection site. Intra-oral MEC has a tendency to various locations, and the predilection sites are palate, cheek, mandible, lip and tongue in order of frequency. A few cases of MEC are occurred in with retromolar trigone, oropharynx, and ectopic salivary gland. Recently, we experienced a-65-year old woman with retromolar trigonal mass, and she was finally diagnosed as MEC. We report it with review of literature.

  • Warthin's tumor (WT) is second most common neoplasm in the parotid gland and it can be accompanied by inflammation and necrosis. The chest wall inflammation may present a rapid and fatal clinical course and secondary to parotid abscess is extremely rare. An 81-year-old man came to emergency room complained of rapidly enlarged left parotid mass and inflammatory symptoms and signs around the upper lateral neck. We performed incision and drainage with adequate infection control. He was pathologically diagnosed as abscess. We report the unique and instructive clinical case with a literature review.

  • We experienced a case of 63 years old male patient who had synchronous rectus abdominis intramuscular schwannoma and chest wall lipoma. Schwannoma is rare benign tumor which derived from nerve sheath and mainly peripheral nerve of flexor part. The authors report rare synchronous schwannoma and lipoma development.

  • Cemento-osseous dysplasias are a group of disorders known to originate from periodontal ligament tissue and involve, essentially, the same pathological process. They are usually classified into three main groups: periapical, florid, and focal cemental dysplasias depending on their extent and radiographic appearances. Radiographically, florid cemento-osseous dysplasia (FCOD) appears as dense, lobulated masses, often symmetrically located in various regions of the jaws. The best management for the asymptomatic FCOD patient consists of regular recall examinations with prophylaxis. The management of the symptomatic patient is more difficult. A case of FCOD occurring in a 52-year-old edentulous Korean female is reported which is rare with regard to race and sex.

  • Paratracheal air cysts are rare lesions and detected incidentally during CT scan or autopsy. Histopathologic diagnoses of paratracheal air cysts include trachocele, tracheal diverticulum and lymphoepithelial cyst. The cysts are lined by ciliated columnar epithelium and have communication with trachea. Previous reports suggested an association with obstructive lung disease because of increased expiratory pressures in these patients. Most of these cysts are asymptomatic but rarely cause productive cough, wheezing, recurrent laryngeal nerve paralysis and difficult intubation. We report a case of paratracheal air cyst causing dyspnea with a review of literature.

  • In general, reverse shoulder arthroplasty revision is performed using the same implant for both the humerus and glenoid components. However, the authors of the present case used different implants from what was used previously for treating instability with scapular notching and glenoid aseptic loosening and report the case.

  • Objective : Several reports have found abnormal levels of androgen in post-traumatic stress disorder (PTSD) patients. This abnormality in androgen is hypothesized to due to chronic psychological stress effects on the hypothalamic-pituitary-gonadal (HPG) system. The present study was conducted to estimate serum testosterone levels in PTSD patients in comparison with normal subjects. Methods : Seventy-five male Korean veterans of the Vietnam War volunteered for the study, of which eleven were excluded because of incomplete psychological assessment. To measure basal serum testosterone, blood samples were collected between 8.00 and 9.30 a.m. The clinician administered PTSD scale (CAPS), the structured clinical interview for DSM-IV (SCID), Hamilton Anxiety Rating Scale (HAM-A), Hamilton Rating Scale for Depression (HAM-D), Mini International Neuropsychiatric Interview Plus (Korean version of MINI-Plus), CES-K (Korean version of combat exposure scale). Results : The serum testosterone level of PTSD patients ($5.4{\pm}2.5ng/mL$) was higher than that of a control group ($3.1{\pm}1.7ng/mL$, p<0.001). Testosterone levels were significantly correlated with CAPS (r=.38, p<.01), HAM-A (r=.35, p<.01) and HAM-D (r=.28, p<.01) in all subjects. Conclusion : The results of the present study suggest that chronic psychological stress affects the HPG system.
