A Case of Dysplastic Epiglottis in Elderly Patient (노령 환자에서 발견된 후두개 이형성증 1예)

  • Dysplastic epiglottis is extremely rare congenital malformation, which usually occurs in association with other laryngeal anomalies. Hypoplasia is the most common type in epiglottic malfomations. Other abnormalities include rudimentary, aplasia and bifid etc. Mostly, they are found in infancy and early childhood, and diagnosis at adulthood is extremely rare. A 69-year-old man with chronic cough and globus sense visited our clinic. Laryngoscopic findings revealed a unique form of epiglottis. He had no history of laryngeal trauma, tumors, head and neck surgery, and radiation. There was no another anomaly in the laryngo-pharynx. Because of a mass-like lesion at the apex of epiglottis, we performed the laryngeal microsurgery. The pathology revealed as granulation tissue. We report a rare and unique case of dysplastic epiglottis in elderly patient with a brief literature review.

Two Cases of Extraparotid Warthin's Tumor in Lateral Cervical Region (측경부에 발생한 이하선외 Warthin씨 종양 2예)

  • Extraparotid Warthin's tumor(EPWT) is a rare disease entity; its incidence is about 8% of Warthin's tumor(WT). The periparotid and upper cervical lymph nodes are the most predilection sites. The lymphoid tissue of WT can act like a regional lymph node, and the necrosis and inflammation within the tumor are well known. In our cases, both 81-year-old man and 58-year-old man were visited our clinic, presented with upper lateral neck mass that had been present for the last few months. We operated the excisional biopsy in level ll of neck. The pathologic examination was proven to be EPWT. We report the unique two cases of extraparotid Warthin's tumor with literature review.

A Study on Patients' satisfaction for quality improvement of Physical Therapy service (물리치료실의 질 향상(QI)을 위한 내원 환자의 만족도에 관한 연구)

  • The purpose of this study was to contribute to quality improvement with physical therapy service and determining the factors affecting patients' satisfaction. The study subjects were ambulatory and admitted patients treated with physical therapy at B general hospital located in Taejon from July 15, 2000 to July 21. Authors developed structured questionnaire, and distributed it to each physical therapist of B general hospital. Total number of distributed questionnaire was 164, and 157 questionnaire were collected and analysed finally. Demand of physical therapy for quality improvement(QI) related to treatment time(r =0.746, P<01), treatment method(r =.664, P<.01). treatment effect(r=0.648, P<.01), equipments(r=0.620, P<.01), cleaning status(r =0.619, P<.01). Willingness to revisit and recommendation of this physical therapy room related to treatment method(r=0.489, P<.01), treatment time(r=0.469, P<.01), cleaning status(r=0.432, P<.01).

  • The hand is a very specialized organ that functions to obtain information and to execute motor acts essential to human interaction with the environment. Loss of hand function through infections affects the mechanical tasks that the hand performs and psychological adjustments to their disability. Infection is a disastrous complication of hand injuries and adequacy of circulation is of greatest importance to prevention of infection. Careful debridement, incision, and adequate drainage and antibacterial treatment are of great importance. Optimal care of the infected hand demands that carefully surgical care, early postoperative exercises and other therapy. Hand rehabilitation has grown as a specialty area of both physical and occupational therapy. It is essential that the surgeon and therapist work together, and communicate freely-all of which generally require daily contact. Treating the psychological loss suffered by the patient with a hand infections is an integral part of the rehabilitation therapy as well. Treatment techniques, Whether thermal modalities or specifically designed exercises, are used as a bridge to reach a further goal of returning to functional performance.

A Case of Protrusion of False Vocal Fold Resulting from the Deformed Thyroid Cartilage (변형된 갑상연골에서 기인한 가성대 돌출 1예)

  • Trauma, congenital malformation and aging process can be a cause of the deviation of laryngeal prominence in the thyroid cartilage. Among these, the senility is the most common cause. Usually, ossification in the thyroid cartilage has occurred symmetrically, but the asymmetrical event leads to the shift of laryngeal prominence. Also, such deformity can provoke protrusion of false vocal fold. A 75-year-old man with hoarseness and globus sense in throat visited our clinic. Five years ago, he experienced a blunt trauma on left midline neck and had a concave deformity in the left thyroid cartilage lamina. Laryngoscopic findings revealed a marked protrusion in the left false vocal fold. We performed the laryngeal microsurgery to discriminate the tumorous condition. The pathology revealed non-pathologic mucosa. We report a unique and didactic case with a brief literature review.

A Case of CRPS Treated with Implantable Port System -A case report- (Implantable Port System을 이용한 복합부위통증증후군 치험 1예 -증례 보고-)

  • The complex regional pain syndrome(CRPS) exhibit symptoms such as: abnormal skin color, temperature change, abnormal pseudomotor activity, edema. If CRPS is not treated appropriately at acute stage, then the affected extremity may become a useless, painful appendage. Treatment of CRPS by sympathetic blockade may be achieved by repeated intravenous regional guanethidine blocks, repeated anesthetic sympathetic blocks, surgical sympathectomy or oral sympatholytic therapy. We treated 29-year-old male patient with CRPS of left upper extremity by continuous cervical epidural blockade. Due to wound infection and dislocation of the epidural catheter, we inserted an implantable port system to inject the mixture of local anesthetics and small amount of morphine. After 10 months of treatment, patient was cured of symptoms and signs of CRPS and was able to resume a normal life.

  • Herpes zoster is a relatively common disease caused by Varicella-Zoster virus and characterized by a vesicular dermatomal rash and a variety of neurologic manifestations. These neurologic symptoms include herpes zoster neuralgia, myelitis, encephalitis, cranial arteritis, segmental and rarely polyradiculitis. This report is a case of a 57-year-old female with herpes zoster paresis affecting the arm and hand. Herpetic pain was much relieved after continuous cervical epidural blockade. However, fifty days after onset of ailment, she complained motor paralysis. EMG of muscles innervated by the left $C_8$ and $T_1$ roots revealed high frequency denervation potentials and large amplitude polyphasic motor units. We recommended physiotherapy and aggressive exercise therapy of the hand. After eighty days of therapy, denervated spontaneous activities disappeared on EMG. Clinically, strength of muscles had nearly recovered to 75% of normal strength in left hand intrinsics.

  • Stellate ganglion block is a well established method for the management of certain pain syndromes (e.g., chronic regional pain syndrome, facial pain) in the cervicothoracic region and upper extremity. The stellate ganglion resides between the C7 transverse process and the head of the first rib. Anesthetic injections for the stellate ganglion block are typically made at the level of the transverse process of either the C6 or C7 vertebrae to avoid the pleura, vessels, and nerve roots. Method of positioning the needle tip directly at the ganglion has been described, but are problematic because of the risk of injury to or injection into adjacent structures. It is necessary to know the exact anatomic position of the stellate ganglion when permanent blockade is required by means of radiofrequency thermocoagulation. Whereas fluroscopy shows only bony feature, computerized tomography also images nerves, vessels, and lung, allowing accruate needle placement. We report a case of the percutaneous radiofrequency thermocoagulation of the stellate ganglion after computerized tomography-guided localization.
