A Study on the system in the Theory of 'Syndrome Differentiation' from the Viewpoint of Yoon Gilyeong (윤길영의 변증체계 고찰)

  • Objectives Syndrome differentiation and treatment (辨證論治) was one of the core theories in Korean medicine and syndrome differentiation (辨證) constitutes a branch of disease diagnosis in Korean medicine. Yoon Gil-Young, one of the modern outstanding scholar of basic medical science in Korean medicine, wrote on basic theories of Korean medicine such as physiology, pathology, formula science, etc. Hereby we will analyze and discuss his works to understand his recognition of historical changes in the syndrome differentiation. Methods We conducted researches into the two works of Yoon Gil-Young's, which are "The Clinical Formula Science of Eastern Medicine (東醫臨床方劑學)" and "The theory of Four-Constitution Medicine (四象體質醫學論)". From Yoon's academic standpoint which connects the basic medical science with the clinical medicine, we analyzed his opinion about the system in the Theory of 'Syndrome Differentiation'. Results According to Yoon's research work on the Theory of 'Syndrome Differentiation', the system of syndrome differentiation, which had its deep root in the theory of Yin and Yang (陰陽) & the theory of abbreviation of the five circuit phases (五運) and the six atomspheric influences (六氣) of the "Huangdi's Internal Classic (黃帝內經)". Conclusions Yoon Gil-Young's theory of differentiation of syndromes and treatment is widespread so much that he studied on the learning field of Traditional Korean Mediciine and ingenious as well. He explain on the main principles of differentiation of syndromes based on "Huang Di Nei Jing" and the system of differentiation of syndromes is composed of Traditional Korean Medical Physiology.

  • Music therapy is a treatment for a range of disorders (physical, emotional, mental, and social) by using music. This article introduces traditional medical books, which mentioned relation between sounds and emotions or internal organs based on the five phases (五行) theory. And, authors provide the Korean traditional musical instruments related with five phases and Six Healing Sounds (六字訣).

  • The theory of Hwa Acupuncture Therapy (HAT), called Hwa Chim Therapy, is a part of the Five Elements Theory unique to Korea. HAT, created and developed by Jaehoon Song, integrates the victor-vanquished as well as the son-mother relationship of the Five Elements of breakdown and restoration of balance between yin and yang. And also, it provides resources and data on The seventy fifth Difficulty(75難) of Classic on Difficulty - Nan Jin 75. HAT establishes objectiveness and accuracy of diagnosis based upon the traditional method and procedure of pulse taking. In HAT, a person's state of illness is diagnosed by applying the comparative examination of the palpitation of the pulse. It is fact that the pulse varies according to the state, and that HAT treatment has proven the positive results by using the victor-vanquished relationship of Classic on Difficulty-Nan Jin 75. Despite a concise and simple theory, Hwa Chim is very effective in treating a wide range of illnesses, and thus it has gained an increasing attention of many scholars and practitioners in the field of traditional Korean oriental medicine. However, it is the first theoretical attempt to the clinical research and scientific methodology of Ohaeng Hwa Chim, and more active Hwa Acupuncture R&D is being conducted nationwide.

  • Because of the changes of a variety of aspects of disease theory alternative to standard medicine, demand for traditional medicine has increased. This study was undertaken to grasp and compare traditional-medicine administrative structure, functions and role between Korea and China. According to our research, both nations promote policy related to traditional-medicine, yet there are various differences because of each nation's scale, political structure, medical system, etc. Especially, compare to the Korean government, the Chinese government not only has paid more attention to traditional medicine but also established and promoted a synthesis development strategy for a long time. Additionally, there is weakness in Korea's traditional medicine policy compared to the Chinese's Synthetic development-strategy, which is the Oriental-policy Department and Korea Food&Drug administration. Therefore, to make up for the weakness, they will have to organize their role better and make a long-term development-strategy plan for traditional medicine.

  • In Korea and China, traditional medicine's holistic approaches, based on the views of whole-body and whole-person, have been applied to make the solution of health problem. However, these holistic approaches of traditional korea or chinese medicine have been limited in interpreting their theories in a view of modern scientific aspects of medicine. This limitation seems to be mainly due to the reductionism approaches of modern scientific medicine. Traditionally, science has taken a reductionism approach; dissecting biological systems into their constituent parts and studying them in isolation. However, systems biology based on omics technologies is providing a new thought and method for traditional medicine's research and interpretation. Systems biology uses integrity study as the characteristic and bioinformatic technology as the key method for connecting reductionism and holism. Therefore, it has much in common with the theory of traditional medicine. It was reviewed that how systems biology is applied to traditional medicine in Korea and China. Also it was suggested that more future researches on interpretation between traditional medicine and systems biology must be focused on personalized medicine since systems biology will have a major impact on future personalized therapeutic approaches.

The Review on the Traditional Medicine Concepts in the UMLS (UMLS내 전통의학 용어에 대한 고찰)

  • Objective This is a previous study for including traditional Korean medical terms into the UMLS(The Unified Medical Language System) and achieving the interoperability between various medical systems. Method First, the traditional medical terms were divided into 4 categories : basic theory, acupuncture, herb and formulae. And then, searching these terms through metathesaurus in UMLSKS(UMLS Knowledge Source Server), terminology information was investigated and analyzed. Results In the case of TM title, traditional Korean medicine was categorized as different semantic type from Traditional medicine, Traditional Chinese medicine and Kampo medicine. Acupuncture points were described only as abbreviation and herbs were conceptualized inconsistently, as some belonged to scientific name and some belonged to Chinese pronunciation. Formulaes are described as Chinese, Japanese and Korean pronunciations. Conclusions More research is needed on diagnosis/disease terms and semantic types for the unique concepts in traditional Korean medicine in order to including the international standard.

  • The most fundamental and important medical treatment is science of acupuncture and moxibution, which is based on twelve channels theory. Meridian is a pathway that conveys material and energy in a human body. Twelve channels are divided into channels of hand & foot, channels of yin & yang. Yang channels are divided into taiyang, yangming, shaoyang, yin channels are divided into taiyin, shaoyin, jueyin. These are referred to twelve channels, and this theory is being used for diagnosis and test in oriental medicine. Meridian-doin-tajiquan is born, combining taijiquan which is recently handed down from China and Korean traditional method for health protection and treatment in ancient times and twelve channels, three yin & yang theory. I report this because meridian-doin-tajiquan which is non-medical and non-invasive way can be used in the treatment of disease, just like three yin & yang theory, the heart of the meridian theory, and Gehapchu theory are adjusted in the clinical science of acupuncture and moxibution. And I report this because I could mater the appropriateness of the traditional theory and I believed this corresponded with it, training myself by meridian-din-tajiquan. It is considered that this will be used in the treatment of pain disease of muscles and joints system and the diabetes, hypertension, obesity caused by stress in the near future.

  • Rhinitis, inflammation of the nasal cavity caused by the disease in our country that a recent trend is the rapidly increasing number of patients. It causes multiple complications and interfere with daily life and social life in case of being viewed as a mild cold. The rhinitis treatment for symptomatic therapy in modern medicine is staying because the situation is exposing the limitations. so traditional medicine is trying to find a solution but that is not easy. In modern medicine, rhinitis is defined as a set of histological lesions and clinical symptoms, but traditional medicine has developed in dealing with each symptoms in disease. The treatment that doesn't distinguish any treatment in growing numbers of patients does not seem to validate, because doctors treat without considering the difference of recognition. Looking at the symptoms for each, I found that each symptoms and causes are varied according to doctors and era. In spite of considering all of that in treatment, it was ignored or underestimated in modern medicine. Furthermore, in traditional medicine, they just conjugated certain period theory and only had limited treatment by a lack of integrated awareness about definition and mechanism. In addition, in case of mentioning about the causes of prescription in each medical books, there were not enough explain about pathological states. Therefore, these causes should be made to further the research about the validity, pathogenesis and treatment. We can see that theory could be established and modified by the accumulation of clinical experience, observations through classification by each of the causes, prescription and treatment. In traditional medicine simply does not pay attention only to nasal inflammation, such as the impact of Meridian and organs considered to have focused on the fundamental treatment. This increased immune resistance to the modern rhinitis patients may offer new therapeutic approaches. I hope this paper would be helpful to find the treatment with uncovered theory in modern medicine.

Yoon Gil-Young's "A Study on the Methodology of Traditional Korean Medical Physiology" : Review from an Insider's Viewpoint (현곡(玄谷) 윤길영(尹吉榮)의 "한방생리학(漢方生理學)의 방법론(方法論) 연구(硏究)" 재 조명)

  • Yoon Gil-Young(1911-1987) was a frontier of the field of Traditional Korean Medical Physiology(TKMP) who was academically active since mid 1950s until early 1970s. He wrote several important papers and books. Among his writings, "A Study on the Methodology of Traditional Korean Medical Physiology", which was published serially in the "Korean Oriental Medical Journal" from June 1966 until March 1967, played an important role in establishing the contemporary TKMP and showed his academic stance on Traditional Korean Medicine(TKM). This review will evaluate this paper's contribution to the establishment of TKMP in historical context, summarize the unique characteristics of his understanding of TKMP by analyzing the text, and then based on those information, examine his view on TKMP and TKM. Historically, this paper was written in transition period from the traditional TKM knowledge system to the modernized one. Aim of this paper was to provide a methodology for establishing TKMP as a scientific knowledge system like that of the western medicine. Based on his study of Yin-Yang theory and theory of five circuits and six qi in Huangdi's Internal Classics, he suggested those theories as methods to observing life phenomena and systematizing the observations. And he regarded these methods as of great value in determining the unique characteristics of TKM compared to those of western medicine. Through re-reading this paper, it was found that he had pride and confidence in the methodology of TKM, and also that he thought in scientizing and modernizing TKM it was very important to understand and efficiently put in use the methodology of TKM. It was also found that his view on TKMP and TKM was pro modernization.

Study on Body Constitution and Disease Symptoms and Signs (논체질여병증(论体质与病证))

  • Body constitution research in Korean traditional medicine adopt the diagnosis and treatment theory of "image-differentiation[body constitution differentiation, disease differentiation] in combination with syndrome differentiation diagnostic model and symptoms and signs of herbal property belong to image" as its core, which is key national medical science research project of State Administration of Traditional Chinese Medicine, the project brought up 4 key scientific problem ? body constitution differentiation theory, correlation theory of body constitution and disease, body constitution adjustable theory and symptoms and signs of herbal property belong to image theory. In body constitution pathology, it brought up "correlation between body constitution and symptoms", "differentiation between body constitution and symptoms" which increase the diagnostic level and diagnostic accuracy rate. In the condition of pathology, it obviously has low reliability according to body constitution differentiation, sometimes happen the description not comply with body constitution and disease symptoms, which lead to decrease the clinic diagnostic and treatment level, treatment effect not satisfying too. Now taking 4 key scientific achievement as criterion to illustrate the body constitution and disease symptoms.
