소아 알레르기 결막염의 한의학적 병인, 변증, 치법 및 한약재 선택에 대한 문헌적 고찰 (A Review of Korean Medical Pathogenesis, Pattern Identification, Treatment Principle and Herbal Medicines for Allergic Conjunctivitis in Pediatric Patients)

  • Objectives We aimed to understand etiology, pattern differentiation, treatment principle, and the role of selected herbs in the treatment of Korean herbal medicine for allergic conjunctivitis (AC) in children. Methods We searched relevant literature published up to February 20, 2021 through CNKI, using search formula of (SU= '結膜炎'+'conjunctivitis') AND (SU='儿童'+'小儿'+'少儿'+'幼年') AND (SU='治療'+'中医治療'+'中藥'+'中医藥'+'顆粒'+'胶囊'+'自擬'+'湯'+'丸'+'散'+'方'). Results Among 81 searched studies, 9 randomized controlled trials and 1 case report were selected and analyzed. In all studies included, the efficacy of herbal treatment for AC was confirmed, and there was no evidence that herbal medicine treatment has higher risk of developing side effects compared to conventional eye drop treatment. As an etiology, wind (風邪) was observed the most frequently, and Saposhnikoviae Radix (防風) and Schizonepetae Spica (荊芥) were in frequent use to dispel wind (祛風). As visceral pattern identification, spleen (脾) and lung (肺) were two important keywords, and spleen deficiency (脾虛), dampness-heat in the spleen and stomach (脾胃濕熱), lung qi deficiency (肺氣虛) were main pathologic mechanisms. Else, treatments related to liver (肝) and removing dampness-heat (濕熱) were mentioned next. Conclusions This study is significant in that AC in children, which has a lack of research compared to other allergic diseases, has been studied in the contents of Korean medicine and laid foundation that can be used as basic data in the actual clinical field. Based on this study, it is hoped that clinical data of Korean herbal medicine treatment for childhood AC will be further accumulated.

조선중기(朝鮮中期) 의학(醫學)의 계통(系統)에 관(關)한 연구(硏究) (A Study on the Lineage of Medicine in the Middle Period of the Chosun Dynasty)

  • The Korean Medicine in the middle period of the Chosun Dynasty, with its clear independence, has made an excellent display of originality and superiority in the world medicine. By pinpointing its merits, classifying its lineal descent, and examining its relation with present Korean Medicine we can draw the following conclusions. First, the study on the history of Korean Medicine should, in terms of time, grasp the generational stream and, in terms of space, examine the relation with medicines of neighbouring countries, with both an inner and outer way of observing. Secondly, it is but a manifestation of the colonial view of history to classify the 460 years from Tae-jo(太祖) to Cheol-jong(哲宗) as Mordern Period of Korean Medicine. Therefore, the 250-year period between the mid-16th century of Myung-jong(明宗) to the late-18th century of later Jeong-jo(正祖), which saw a pivotal development of the national medicine as a practical medicine based on experiences, should be established as the period of Korean Medicine in the mid-Chosun dynasty. It is required from both nationalistic view of history and history as a positive science. Thirdly, Korean Medicine in the mid-Chosun period was developed with an emphasis on an independent development and creative succession of the national medicine, which was succeeded to the late-Chosun period. Fourthly, medical thought in the mid-Chosun period was much influenced by, and based on the practicality of, Shil-hak(or practical science : 實學) as was the current of the times. Fifthly, though medicine in the mid-Chosun period was generally developed, the recipes for the measles, epidemic and smallpox were especially developed owing to rage of epidemics, and the development of the acupunture as a military medicine was the most c lear because of the two major wars against the Japanese and Chinese, respectively in 1592 and 1636. Sixthly, Whang doh-yeon(黃道淵)'s Bangyak-happyeon(方藥合編), Lee je-mah(李濟馬)'s Tongui-susebawon(東醫壽世保元) and Lee kyu-jun(李奎晙)'s Uigam-jungma(醫鑑重磨) are representative medical books, in the late-Chosun period, which creatively succeeded national medicine. Seventhly, the lineage of national medicine flows from Hyangyak-jipseongbang(鄕樂集成方) Uibangryuchui(醫方類聚) Gugeupbang(救急方) to Uirimchualyo(醫林撮要) Tonguibogam(東醫寶鑑) Uimunbogam(醫門寶鑑) Jejung-sinpyeon(濟衆新編) and to Bangyak-happyeon(方藥合編) Tongui-susebawon(東醫壽世保元) Uigam-jungma(醫鑑重磨). These books are representative medical books in the early middle and late Chosun period respectively. On the basis of the above facts, it is clear that the orthodoxy of national medicine is in the direction of a creative succession and that is the best way to keep the spirit of Korean medicine today.

Overexpression of Anti-apoptotic Molecules and Sax Translocation to Mitochondria by Pharbitis Nil Extracts in AGS

  • Conventional medicines have usually sorted to a number of treatments such asoperation, radiotherapy, and chemotherapy. The existing anti-cancer agents, designed to eradicate cancer cells, have strong toxicities, also with leading to harmful side effects. Recently, a number of researches on natural products have been actively carried out in efforts to develop new treatments that can decrease side effects or increase anti-cancer effects. We performed this study to understand the molecular basis underlying the antitumor effects of Pharbitis nil, and Plantago asiatica, which have been used for herbal medicinal treatments against cancers in East Asia. We analyzed the effects of these medicinal herbs on proliferation and on expression of cell growth/apoptosis related molecules, with using an AGS gastric cancer cell line. The treatment of Pharbitis nil dramatically reduced cell viabilities in a dose and time-dependent manner, but Plantago asiatica didn't. FACS analysis and Annexin V staining assay also showed that Pharbitis nil induce apoptotic cell death of AGS. Expression analyses via RT-PCR and Western blots revealed that Pharbitis nil didn't increase expression of the p53 and its downstream effector p21/sup wafl/, and that the both increased expression of apoptosis related Sax and cleavage of active caspase-3 protein. We also confirmed the translocation of Sax to mitochondria. Collectively, our data demonstrate that Pharbitis nilinduce growth inhibition and apoptosis of human gastric cancer cells, and these effects are correlated with down- and up-regulation of growth-regulating apoptotic and tumor suppressor genes, respectively.

여드름 한약 치료의 무작위 대조군 임상연구에 관한 분석 (Analysis of Randomized Controlled Trial Study of Korean Herbal Medicine Treatment for Acne)

  • Objectives : The review was conducted to validate the effectiveness of herbal medicines on acne. Methods : Randomized controlled trials(RCTs) reporting the effects of herbal medicine treatment on acne were searched through seven electronic databases from the time of application of the material to October 2021. Cochrane Collaboration's Risk of Bias was used to assess the risk of bias. Analysis was performed using Review Manager 5.4 and R 4.1.0 Meta, Metafor program. Results : 34 RCTs was selected and meta-analysis was performed with 30 studies. The inflammatory lesion count of the herbal medicine(MD=0.29, 95% CI:-0.59-0.01, p<0.01, I2=80%) and the non inflammatory lesion count of herbal medicine(MD=-0.30, 95% CI:-0.70-0.10, p<0.01, I2=78%) were seen. The score of IGA(RR=1.43, 95% CI:0.90-2.27, p<0.91, I2=0%), VISIA(MD=0.36, 95% CI:0.21-0.51, p=0.07, I2=40%), effective rate(RR=1.23, 95% CI:1.05-1.44, p<0.01, I2=98%), DLQI(MD : 0.59, 95% CI:0.14-1.04, p=0.07, I2=63%) and recurrence rate(RR=0.36, 95% CI:0.23-0.58, p=0.73, I2=0%) were seen. The effective rate of herbal medicine were statistically higher that of the control group(WM)(RR=1.19, 95% CI:1.04-1.37, p<0.01, I2=97%). As a result of dividing the treatment groups into OHM, EHM, and OEHM, the EHM group(RR=1.17, 95% CI:0.79-1.72, p<0.01, I2=99%) showed the most statistically significant effect. The overall risk of bias of the included studies was some concerns. No serious adverse effects were observed. Conclusions : This review found the effectiveness of herbal medicine for acne.

우리나라 암환자가 이용하는 보완·대체요법에 관한 연구 (A Study of Complementary and Alternative Medicine used by Cancer Patients in Korea)

  • Purpose: The purpose of this study was to study complementary and alternative medicines (CAM) used among cancer patients in Korea. Method: This study was a descriptive survey. Study participants were 1,150 cancer patients selected from 28 representative hospitals in Korea. Data were collected between December 1, 2004 and March 30, 2005. Results: Their diagnosis were as follows : 20.0% for breast cancer, 15.6% for stomach cancer, 14.4% for liver cancer, 10.8% for lung cancer, 10.0% for colon cancer, and 10.0% for uterine cancer. 75.0% of cancer patients had used CAM. The most popular categories of CAM use were diet with 153.2%, non- prescription of oriental medical doctor 47.9% and mind-body control 20.3%. CAM use gender, age, cancer type, cancer treatment method, and current treatment type was significantly different in. Conclusion: CAM use among cancer patients in Korea was very popular(75.0%) and so systemic guidelines of CAM use are needed.

고삼 Trifolirhizin의 항위염 및 항 H. pylori 균 효과 (Antigastritic and Anti Helicobacter pylori of Trifolirhizin from Sophora Radix)

  • Sophorae Radix, the dried roots of Sophora flavescens Aiton (Leguminosae), has been used in oriental traditional medicine for treatment of skin and mucosal ulcers, sores, gastrointestinal hemorrhage, diarrhea, inflammation and arrhythmia. Present study was carried out for the gastroprotective effect of trifolirhizin from Sophora flavescens. This reports were evaluated antibacterial activity against Helicobacter pylori and HCl ethanol-induced gastric lesion in rats and showed the significant effectiveness. In pylorus ligated rats, the treatments of trifolirhizin showed decrease in the volume of gastric secretion and acid output. Also we evaluated the antibacterial activity against H. pylori treated with methanol extract and trifolirhizin from Sophora flavescens had a equivalent antibacterial activity with ampicillin against H. pylori at the dose of $100{\mu}g/mL$. It may be regarded that the antigastritic effects and antibacterial activity of trifolirhizin from Sophora flavescens are originated from reduction of total acid output identified by gastric secretion reduce, free radical scavenging effects and the antibacterial activity against H. pylori.

  • A simple and reliable reverse phase HPLC method was developed to determine pharmacologically active marker compounds of Ephedrae Herba, Phytolaccae Radix and Polyporus. The stability test of water-extract of three natural medicines were examined for six months. However, no significant change in the content of the marker compounds of each extract observed during the time of investigation.

예비동결 온도에 따른 숙지황 분말의 이화학적 특성 (Physicochemical Characteristics of Rehmanniae Radix Preparata Powder by Milling with Pre-freezing Temperatures)

  • Background: Rehmanniae radix preparata (RRP) has been used as a traditional medicine and is one of the most important oriental herbal medicines. However, the physical characteristics of RRP are not suitable for use in industry. The present study was under-taken to determined the preparation method of RRP powder and the physicochemical characteristics of RRP powder by milling under different pre-freezing temperatures. Methods and Results: Moisture content, powder yield, particle size, bulk density, compressive stress, extraction yield, and 5-HMF content of PRR powders by milling with pre-freezing temperatures (-20, -40, -60, and $-80^{\circ}C$) were analyzed, and correlation among these factors was determined. Powder yield increased and particle size decreased in a pre-freezing temperature-dependent manner from -20 to $-60^{\circ}C$. The Hausner ratio increased from 1.186 to 1.225 with decreasing temperature from -20 to $-80^{\circ}C$, whereas compressive stress showed the opposite trend. Extraction yield and 5-HMF content were not significantly different between RRP powder. Significant correlations were observed among pre-freezing temperature and physical characteristics (e.g., yield, particle size, Hausner ratio, and compressive stress). Conclusions: These results suggest that the pre-freezing temperature is an important factor affecting the physical characteristics of PRR powder and applicable to the industrial production of RRP powder.

『동의보감(東醫寶鑑)』에 수록된 돼지 활용(活用)에 관한 고찰(考察) - 단방처방(單方處方)을 중심으로 - (The Medicinal Effects of Pork in 『Donguibogam』)

  • Objectives : "Donguibogam(東醫寶鑑)" is a medical book of the Joseon Dynasty compiled by Heo Jun and was first published in 1613. It recorded a great deal of prescriptions which contained pork and gave detailed explanations of parts of pork's natural properties and effects. Pork is widely used as a food ingredient in the world but the traditional medicine community has a contraindication of pork and traditional herbal medicines taken together. The aim of this study was to find the supports to develop pork functional foods for the treatment of diseases in "Donguibogam". Methods : Therefore we investigated the documents recorded in "Donguibogam" about usefulness of pork which were used for not only a food ingredient but also a medicine. Results : "Donguibogam" introduced 25 parts of pork including liver, kidney, stomach, bladder, heart etc, and a variety of taking ways i.e. common usages which are internal, external, common use, and aid use which cure a variety of diseases not a simple food but a therapies food. Conclusions : We confirmed that pork has been used to treat many diseases, so we propose to develop pork functional foods available for prevention and cure of certain diseases.

뇌경색 환자의 Warfarin($Coumadin^{\circledR},{\;}Warfar^{\circledR}$) 복용시 한약물이 INR에 미치는 영향 (Influence of Herbal Medicine Coadministration on the INR in Stroke Patients Taking Warfarin)

  • Objectives: Warfarin is a representative anticoagulant, and it has been the preferred drug for treating thromboembolic diseases and preventing ischemic stroke. It should be administered in a delicate manner, because combined therapy with other drugs could affect the INR (International Normalized Ratio), thus bleeding tendency might occur. Interaction with herbal medicines still remains controversial. We aimed to examine the effect of herbal medicine on INR in patients being treated with warfarin. We aimed to examine the effect of herbal medicine on INR in the patients being treated with warfarin. Methods: We enrolled subjects being treated with a combined therapy of warfarin and herbal medicine from 1 October, 2002 to 20 November, 2003 at the Department of Cardiovascular & Neurologic Diseases (Stroke Center), Hospital of Oriental Medicine, Kyung Hee Medical Center. INR was monitored per 1 to 2 weeks and the significance of change was examined by Wilcoxon signed rank test or repeated measure analysis Results: Eighty-three subjects were included in the final analysis. As a whole, their INR was not significantly changed. There were only 9 cases in which INR changed more than 1.0; 7 showed increasing tendency, and 2 showed decreasing tendency of INR. Among the 9 cases, only 5 were suspected to have a relation to herbal medicine. Conclusions: We suggest that there was no strong evidence that herbal medicine has an influence on INR, but there is a need to perform further evaluation on larger samples to reach a concrete conclusion.
