한방 의료기관 내 의료인 대상 폭력 피해에 대한 실태 조사 (Investigation on the situation of violence against medical staffs occurring in Korean medicine clinics)

  • Objectives : The purpose of this survey is to investigate the current status of violence that occurred in Korean medicine clinics targeting medical staff. Methods : First, questionnaires were developed to investigate the actual situation of violence against medical personnel occurring within Korean medicine clinics. The questions consist of five items on the characteristics of the respondent, ten items on identifying the actual state of violence against Korean medical workers, two items on relevance to COVID-19, two items on related legal issues, and three items on policies. The survey was conducted online from 16 November 2021 to 30 November 2021. Results : Total 519 Korean medical doctors responded to the questionnaire, and 314 (60.50%) responded that they had experienced violence demage while working. While the female staffs experience significantly more violence (𝜒2=18.398, p <0.001) and sexual violence (𝜒2=74.855, p <0.001) than the males, the males tend to be more exposed to physical assault than the female (𝜒2=3.900, p = 0.048). Conclusion : Through this study, we identified the situation of violence against medical staffs in Korean medicine clinics, and concluded that it should be necessary to establish strategies for the prevention and management of violence in a social and institutional way.

일부 지역사회 주민의 의료이용행태에 관한 연구 -양산시 주민의 shopping-around 현상을 중심으로- (A Study on Health Seeking Behaviors of Local Residents -Focused on Shopping Around Phenomenon in Yangsan City, Korea-)

  • Objectives : This study aimed to investigate the use and patterns of health care resources of local patients and to analyze the health seeking behaviors focusing on shopping around phenomenon. Methods : Questionnaires on the use of health institution were developed and administered to the residents in Yangsan city, Korea. We selected patients who were suffering from any diseases two months and over. Patients' health seeking behaviors were categorized, and the factors for selecting health institutions in each visiting stages were analyzed. Results : 139 local patients participated in the survey, and 130 completed questionnaires were analyzed. We found that there were sixty eight different types of medical utilization paths. The most frequent path was 'western clinics ${\rightarrow}$ Korean medicine hospitals or clinics ${\rightarrow}$ western clinics'. Only 6 patients started at Korean medicine, however, 16 patients(12.3%) finalized their treatment at Korean medicine hospitals or clinics. In most health seeking stages, patients tended to select health resources by their own decisions, and the geographic factor was the key reason to select health resources. Conclusions : Patients had not any stable forms to treat with their diseases, and we observed diversified patterns in the health seeking behaviors of patients, a typical shopping around phenomenon.

20년간(1987년~2007년) 한의원 경영수지의 경향 연구 (Trend analysis of financial balance of Korean medicine clinics during 20years(1987~2007))

  • Objectives : This study aimed to analyze the trend of financial balance of Korean medicine clinics during 20 years, and to provide basic information for adjusting the fee schedule of Korean medicine procedures in national health insurance(NHI). Methods : We collected 6 financial analysis reports for Korean medicine clinics from the Association of Korean Oriental Medicine(AKOM). The data on incomes, costs and EBIT(earning before interests and taxes) of subject clinics were abstracted, and their long-term trends were evaluated. Results : The proportion of insurance income in total income increased from 23% to 56% during 20 years. Among 5 treatment groups, 'non-insurance medication' took up 65~67% of total incomes in 1997, but its proportion decreased to 42.4% in 2007. 'medical procedure, physical therapy and others' increased from 12.4% in 1987 to 29.2% in 2007. The labor cost was major part of total cost and its proportion maintained from 52% to 54%. Cost of 'non-insurance medication' was on the decline from 41% in 1996 to 31.6% in 2007. EBIT were -17.4 million won in 1996, and 18.4 million won in 2007. Conclusions : Financial balance of Korean medicine clinics improved until early 2000, but it became worse in 2007. Though deficits from NHI procedures has been covered up by profit from 'non-insurance medication', health insurance became a major source of income during last two decades.

산학공동연구를 위한 기초조사 연구 (A Study of Exploration for the Educational-Industrial Joint Research Projects on Oriental Medicine)

  • I. PURPOSE AND SIGNIFICANCE OF RESEARCH The presentation of research direction for compositive study and application of oriental medicine is urgently demanded. Since there is few investigation for the interest in basic and clinical field of oriental medicine, and we hope that compositive study and concentrative research between instruction in college and local clinics is established by this research. II. RESEARCH CONTENT ${\cdot}$ Analysis of survey in basic field of oriental medicine as instruction curriculum. ${\cdot}$ Analysis of survey in clinical field of oriental medicine as the classification of disease. III. RESULT The result of investigation shows that the most interested field is acupuncture and moxibustion in clinical field and diagnostics in basic field of oriental medicine. And oriental medicinal clinic doctors are concerned about prescription and a newly-developed techniques such as new acupuncture therapy, new machines. They hope that a study of oriental medicine in clinical field be accomplished with the special clinics as leader. According to the previous results, we can work out the foundation of compositive study and concentrative research between instruction in college and local clinics, and on this ground it is considered that we can grope for the presentation of research direction of local clinics.

우리나라 일부 농촌지역의 모자보건 실태조사 (Survey for the Current Status of MCH Service in Rural Area)

  • The maternal and child health is a basis of national health, and indicates the level of social welfare and health of the country, because it is related with community welfare status, general cultural conditions, and medical and health sciences. This is a study carried out to identify the present practices of maternal and child health care programs implemented by the private clinics located in Guns(counties ; rural area) in Kyungsangnam Province and to propose alternatives to improve their current programs through a self-administrative questionnaire. The subjects were 90 private physicians who operated their own clinics since 1990 and were general practitioners, Obstertrician/Gynecologists or pediatricians: This survey was conducted by mail from 15 January to 25 February 1992. The response rate was 94.4 percent. 1) The major manpower for MCH programs of the studied clinics was physicians and nurseaids. 70.3% of physicians were general practitioners, 81.1% of nursing manpower were nurseaids. 31.1% of the studied clinics employed lab-technicians. 89.2% of them had MCH room whatever the size and the setting, and 84.4% of Ob/Gyn clinics installed laboratory equipments. 2) 55.4% and 63.5% of the studied clinics provided 151 or above consulting services and curative services of MCH per physician a month respectively and 33.8% and 25.7% of them provided 10 or less consulting services and curative services per physician a month. 91.9% of lab-technicians had 10 or less laboratory tests per technician a month. 3) There was a difference between Ob/Gyn and pediatric clinics in terms of services delivered : for example, 80% of Ob/Gyn clinics provided pre- and post- natal care services, while 84.6% of pediatric clinics provided vaccinations for children. It was also found that only a few of general practitioners involved pre-and post- natal care services. 4) There were no clinics which had opened regular health education session but 24.3% of them had opened the sessions irregularly. Ob/Gyn clinics put emphasis on maternity and pediatric clinics did on child health, but general practitioners touched with both maternal and child health. 21.6% of the studied clinics had some kind of educational materials for MCH programs. Most of the materials were pamphlets or small booklets. 5) Proteinuria/glucosuria, blood pressure and blood type were tested in 48.6~69% of the studied clinics ; tests for blood sugar and hepatitis B were provided in 39.2~41.9% of them, most of them were done in Ob/Gyn clinics. 6) 41.9% of physicians, 29.7% of nurses and 45.9% of nurses-aids wanted to receive on-job-training for MCH programs.
