의학교육에서 의과대학생들의 개인 책무성 (Personal Accountability of Medical Students in Medical Education)

  • Producing graduates with sufficient practical competency is the main mission of every educational institution. Following the accreditation of the Korean Institute of Medical Education and Evaluation, medical schools have been stepping up efforts to establish curriculum that reflects the practical value of medical education and the importance of adapting to the practice of graduates in order to increase the accountability of medical education in Korea each year. To this end, all medical schools have recently made efforts to develop diverse policies to strengthen the social accountability of medical education along with the transition to a competency-based curriculum. In line with this trend, the institutional accountability of medical education as well as the personal accountability of students, the main subjects of learning, should be highlighted, and educational activities to foster accountability need to be specified. Personal accountability in medical students involves recognizing their social accountability as future doctors and understanding and practicing student accountability. To achieve this, medical schools should provide programs that support and teach practical application of skills, and students need to define and attempt specific activities to strengthen their accountability.

청소년의 음주·흡연에 따른 구강지식평가 및 구강보건교육의 필요성에 관한 연구 (Study of necessity of teenagers' drinking, mouth knowledge estimation by smoking and dental health education)

  • This study is to enforce to ready basic data of effective dental education program and no smoking and no drinking program for high school students who do smoking and drinking, executed investigation by own recording way using smoking and questionnaire consisted of actual conditions and caries prevention awareness and dental health knowledge degree of drinking to some man high school. Analyzes result that total 487 people respond in SPSS WIN 13.0 programs and got following sequence. Result of this study is as following. 1. Mans high school students had more students who do not than a drinking student and main current received most soju. Third-year student appeared by thing which receive a lot of soju than 1 or 2 class student but different other difference was not seen by class. 2. Mans high school students had more students who do not than a smoking student and a student who smoke more than 3 years was the most. I can know was old since third-year student smokes than 1 or 2 class student. 3. It was not difference that keep in mind by drinking, smoking availability although there were many students who recognize that all of the on-time dental medical examination, sugar intake limitation, fluorine inclusion toothpaste, teeth fluorine application are seldom effective on caries prevention. 4. Recognized that right brushing is effective relatively on caries prevention. I was shown difference that recognized that righter brushing is effective on bad tooth courtesy call than a student who a student who do not smoke does, and keeps in mind according to smoking availability ($x^2$=6.78, p<0.05). 5. I knew best about question of 'A caries may not treat if is painless' among that question ask knowledge for a caries, and appeared by thing which do not know best about question of 'Know that method that supply fluorine in water'. 6. A student who do not drink general dental know-how for a caries was high knowledge degree for a caries than a drunk student but it was no difference that keep in mind by drinking availability. Also, a student who do not smoke by smoking availability was high knowledge degree for a caries than a done student but difference that keep in mind statistically was not seen. 7. It was no difference that keep in mind by drinking availability, but a student who do not smoke by smoking availability was high relation knowledge degree with food and a caries than a smoked student, and I was shown difference that keep in mind according to smoking availability(t=-2.03, p<0.05). 8. Awareness and synthetic estimation degree of knowledge degree a student who do not smoke than a smoked student mouth knowledge degree high, and I was shown difference that keep in mind according to smoking availability(t=-2.03, p<0.05). When see as result such as singularity, family, school, all social institutions may have to help by national business coming and try so that can become national business based on special know-how for development of effective and epoch-making education program as quickly as possible.

외국 의과대학에서의 장기추적통합임상실습 개발과 운영사례 (Development and Operation of Longitudinal Integrated Clerkship Programs in Overseas Medical Schools)

  • In recent years, Korean medical schools have shown a growing interest in the longitudinal integrated clerkship (LIC) as a means of improving clinical education. Some overseas medical schools have tried the LIC since the 1970s. In the 1990s, several universities in the United States, Canada, and Australia introduced LICs. A variety of studies have confirmed positive results of the LIC. Traditional block-rotation is a discipline-based, inpatient-centered practice. Instead, under the LIC system, a medical student responsible for a panel of longitudinal patients observes developments of their diseases, serving several departments simultaneously. Overseas medical schools have different LIC models depending on their affiliate hospitals' sizes and characteristics. The purpose of this study is to introduce LIC practices in Harvard Medical School and University of California San Francisco, which could be applied by Korean medical schools. This paper also aims to find out the strategies that have been able to help the two US medical schools implement the LIC successfully.

A Qualitative Inquiry Into the Challenges of Medical Education for Retention of General Practitioners in Rural and Underserved Areas of Iran

  • Objectives: General practitioners (GPs) retention in rural and underserved areas highly effects on accessibility of healthcare facilities across the country. Education seems to be a critical factor that affects GPs retention. Thus, the present study aimed at inquiry into medical education challenges that limit their retention in rural and underserved areas. Methods: A qualitative approach was applied for the aim of this study. Data were gathered via 28 semi-structured interviews with experts at different levels of Iran's health system as well as GPs who retained and refused to retain working in rural settings. Interviews mainly were performed face-to-face and in some cases via telephone during 2015 and then coded and analyzed using content analysis approach. Results: Iran's medical education is faced with several challenges that were categorized in four main themes including student selection, medical students' perception about their field of study, education setting and approach, curriculum of medical education. According to experts this challenges could results in making GP graduates disinterested for practicing in rural and underserved areas. Conclusions: Challenges that were found could have negative effects on retention. Modification in student's perception about rural practice could be done via changing education setting and approach and curriculum. These modifications could improve GPs retention in rural and underserved areas.

Excess Deaths During the COVID-19 Pandemic in Southern Iran: Estimating the Absolute Count and Relative Risk Using Ecological Data

  • Objectives: The coronavirus disease 2019 (COVID-19) pandemic led to increased mortality rates. To assess this impact, this ecological study aimed to estimate the excess death counts in southern Iran. Methods: The study obtained weekly death counts by linking the National Death Registry and Medical Care Monitoring Center repositories. The P-score was initially estimated using a simple method that involved calculating the difference between the observed and expected death counts. The interrupted time series analysis was then used to calculate the mean relative risk (RR) of death during the first year of the pandemic. Results: Our study found that there were 5571 excess deaths from all causes (P-score=33.29%) during the first year of the COVID-19 pandemic, with 48.03% of these deaths directly related to COVID-19. The pandemic was found to increase the risk of death from all causes (RR, 1.26; 95% confidence interval [CI], 1.19 to 1.33), as well as in specific age groups such as those aged 35-49 (RR, 1.21; 95% CI, 1.12 to 1.32), 50-64 (RR, 1.38; 95% CI, 1.28 to 1.49), and ≥65 (RR, 1.29; 95% CI, 1.12 to 1.32) years old. Furthermore, there was an increased risk of death from cardiovascular diseases (RR, 1.17; 95% CI, 1.11 to 1.22). Conclusions: There was a 26% increase in the death count in southern Iran during the COVID-19 pandemic. More than half of these excess deaths were not directly related to COVID-19, but rather other causes, with cardiovascular diseases being a major contributor.

연극인표준화환자와 학생표준화환자를 사용한 일개 한의과대학 진료수행시험(CPX)에 대한 임상실습 학생 인식 조사 (A Survey on Clinical Practice Students' Perception of Clinical Performance Examination in a College of Korean Medicine Using the Actor Standardized Patient and Student Standardized Patient)

  • Objectives : This study compares clinical practice students' perception of Clinical Performance Examination (CPX) in a College of Korean Medicine using actors as Actor Standardized Patient (ASP) and enrolled students as Student Standardized Patient (SSP). Methods : Targeting the clinical practice students who completed the clinical practice education in the first semester of the fourth year in the Department of Korean Medicine, a survey was conducted on total six categories such as satisfaction with CPX using ASP and SSP, prior knowledge about clinical practice education, self-evaluation, difficulties in the process of performing the CPX, satisfaction with ASP/SSP, and familiarity with them. Results : In the results of the survey, the mean score of CPX program using SSP was higher than that of CPX program using ASP in every area. Among them, in the satisfaction with CPX itself and prior knowledge about various clinical practice education, the mean score of CPX program using SSP was statistically significantly higher than that of CPX program using ASP. Conclusions : Overall, the clinical practice students' satisfaction with CPX program using SSP was higher than that of CPX using ASP, and statistically significantly higher in some areas.

임상실습 내실화를 위한 일례 연구: 학생 경험 중심으로 (Evaluating Methods of Reinforcing the Clinical Clerkship)

  • This study evaluated the meaning of the clinical clerkship through in-depth interviews with students who had experienced a clinical clerkship. The clinical clerkship is one of the most important steps in the curriculum for becoming a doctor. While students perform actual clinical practice, the experience also reinforces a great deal of medical knowledge and competence as a reserve doctor. However, departments that operate inefficiently have caused some problems. Therefore, this study analyzed the data from interviews of 25 students who finished the clinical clerkship in terms of the meaning of the clinical clerkship, the attitude of the students toward work in clinical practice, the attitude of professors toward the clinical clerkship, benefits, and drawbacks. Comments are feedback from a student's experience in the clinical clerkship, and they may be an important resource illuminating the current status of clinical practice. This study also discusses how to run an effective clinical clerkship by checking in with students on the problems they face in clinical practice.

성남지역 대학생 건강검진에서 대사증후군의 유병율 및 영양상태의 관련성 연구 (The Association between Prevalence of Metabolic Syndrome and Nutritional Status on university student medical examination in Sungnam Metropolitan City)

  • Objectives: Some papers have raised a lot of concerns about relation among the prevalence of metabolic syndrome, serum ferritin and metabolic syndrome. Accordingly, we researched relation between the prevalence of metabolic syndrome and iron state. Methods: A group of 1304 test subjects were gathered in university student medical examination in Sungnam metropolitan city. BIA for body position, body size and blood test were estimated. Results & conclusions: The study represents an adult population of young people in their twenties. Therefore there is so little level for prevalence of metabolic syndrome than have ever known things. And actually, metabolic syndrome was closely connected with so high iron state.

학부 성형외과학 과목평가를 위한 짧은 사례문항의 개발과 시행: 3년간의 경험 (Development and Enforcement of Short Case Test Items for Evaluation in Undergraduate Course of Plastic Surgery: 3 Years Experience)

  • The aim of this study is to evaluate the clinical competence of medical student using short case test items. Development of short case test items with definition of an assessment of subject, choice of clinical stations sampling, identification of components of clinical competence to be evaluated, and assessment of practicability and results exploitation. For the last three years 28, 48 and 30 topics were developed annually. Among them 20, 23 and 23 topics were selected. About 50 students were examined annually. The mean score was 72.7, 57.1, 64.3 respectively. The mean preparation time of the students for the examination was 9.8 hours. About 60% of the students prepared the test in small group activity. Our experience suggests that short case test items be feasible for evaluation of student learning in Plastic Surgery. This method should be considered as a useful tool to assess medical student's competence or even an examination for board certification.

의과대학 대학원생의 학술정보 검색엔진 이용 동기 및 이용자 만족도에 관한 연구 (A Study on the Motivation and User Satisfaction of Medical Students Using the Academic Search Engines)

  • 본 연구는 의과대학 대학원생들이 특정 학술정보 검색엔진을 사용하는 이용 동기와 이용자 만족도를 조사하고, 이용 동기 요인이 이용자 만족도에 미치는 영향에 대해 살펴보았다. 본 연구는 서울의 한 대규모 의과대학 대학원생을 대상으로 2018년 9월 10일부터 2018년 9월 21일까지 설문을 실시하였으며, 총 135부의 유효한 응답을 분석하였다. 분석 결과, 의과대학 대학원생들이 학술정보 검색엔진을 이용하는데 있어 펍메드(PubMed)의 신뢰성 정도와 상호작용성 정도가 구글 스칼라(Google Scholar)보다 높게 나타났으며, 편리성과 전문성이라는 두 가지 이용 동기 요인이 학술정보 검색엔진의 이용자 만족도에는 유의미한 영향을 미치는 것으로 나타났다. 특히 편리성은 이용자들이 학술정보 검색엔진을 선택하는데 있어서도 가장 큰 영향을 미치는 것으로 나타났다. 연구 결과는 웹 사이트를 통한 학술정보서비스에 대한 논의를 의학정보의 맥락에서 이용과 충족이론으로 설명했다는 점에서 학문적 의의가 있다. 한편, 구체적인 연구 결과는 의학 관련 학술정보 검색엔진의 서비스 개선을 위한 실질적인 함의를 제공한다.
