A Comparison of Acoustic Parameters between Vocal Fold Bowing and Vocal Fold Polyp (궁형성대와 성대폴립 간의 음성 비교)

  • Background and Objectives : Vocal fold bowing is an organic voice disorder that is associated with an abnormal structure of the vocal folds whereas vocal fold polyp is a functional voice disorder caused by an abnormal use of the vocal folds. Both types of vocal folds share a common property in that they make one's voice breathy or strained. The purpose of this study is to compare voice from two types of vocal folds and to offer information of clinical importance. Materials and Method: Vocal fold bowing and vocal fold polyp groups consisted of 7 male subjects, respectively. All subjects recorded /a/ in the state of measuring MPT (maximum phonation time), repeating 3 times, by a voice recorder (48 kHz sampling rate; 24 bit quantization). They answered the questions of K-VHI. Time domain parameters (such as perturbation parameters including HNR, Jitter, etc.) were calculated for the whole duration of /a/ and those of the frequency domain were measured in initial 40 ms and stable 40 ms of /a/, respectively. Mann-Whitney V-test was used for the time domain parameters and K-VHI survey, and Wilcoxon signed rank test was applied to the frequency domain parameters (H1, H2, H1-H2). Results: For K-VHI survey and the time domain analysis, there was no significant difference between bowing and polyp group. For frequency domain analysis, H1 and H2 showed a significantly different result between two groups. Vocal fold bowing group has longer duration and lower intensity than that of vocal fold polyp group in the 'aspirated interval', which could be observable prior to ordinary vowel oscillation. Conclusion: Both groups seem to show breathy voice. This could be referred on the basis of the value of H1-H2. The K-VHI survey says that subjects with vocal fold bowing feel more uncomfortable than subjects with vocal fold polyp.

Gene Expression Profile in Epididymal Adipose Tissue from High-fat Diet Fed Mice (고지방식이를 섭취한 mouse에서의 유전자군의 발현양상 변화)

  • Obesity can be defined as a metabolic disease due to a increased state of fat tissue caused by an imbalance of calorie intake and use. To define genes that affected by different nutrient, we study gene expression from mice which were fed different nutrient. Epididymal and retro-peritineal adipose tissue were increase in high fat diet feeding mice compared with control, but liver and spleen were not. In serum, total cholesterol were differently increase in high fat diet feeding mice but total triglyceride and free fatty acid were not. That was maybe result of energy balance regulation in vivo system. aP2, PPART2 and FAS genes that were increased during adipogenesis were inclosed in high fat diet fed mice compared with control. In microarray assay, 1.4% of total genes were affected in epididymal adipose tissue by different nutrient. 1.1% of total genes were decreased down 0.5 fold and 0.3% were increased over 2 fold. These results indicated that many genes are affected in adipose tissue by nutrient.

Studies on medicinal protection with KH-19 against side effect of 5-fIurouracil and its mechanism (KH-19의 5-flurouracil 부작용 억제 효력 및 기전 연구)

  • The objective on this study was to investigate medicinal protection with KH-19, which was composed of 9 kinds of oriental herbs tonifying the blood, against side effect of chemotherapy in mice and its mechanism through microarray. Not only WBC (white blood cell), and PLT (platelet) as a hematopoiesis toxicity indicator but also spleen weight as an immune toxicity indicator was reduced significantly 7 day after. 5-flurouracil (FU) treatment. However, reduction of WBC, PLT, and spleen weight after 5-FU treatment was significantly recovered by KH-19. For mechanic study on KH-19 action, gene expression in mouse spleen treated 5-FU was compared with gene expression in mouse treated 5-FU and KH-19. The result indicates that 23 genes were increased. in expression level over 2 fold and 41 genes were decreased in expression level more than 2 fold at mouse spleen by KH-19 treatment. KH-19 mechanism may be complicated more than other drug which of mechanism were composed of single compound because KH-19 was composed of various compounds.

Chemopreventive Potential of Lonicerae flos Aqua-Acupuncture Solution (금은화 약침액의 암예방 효과)

  • Lonicerae flos aqua-acupuncture solution (LFAS) and Lonicerae flos water-extracted solution (LFWS) were prepared and tested for chemopreventive potentials. Three biomarkers [quinone reductase (QR), ornithine decarboxylase (ODC), glutathione(GSH)] were used to test chemopreventive potential of LFAS. LFAS was potent inducer of QR activity in Hepa1c1c7 murine hepatoma cells, whereas LFWS is less potent. LFAS and LFWS were also induced QR activities in cultured human hepatoma Hep3B cells. The effect of LFAS and LFWS were tested on the growth of Acanthamoeba castellanii. Proliferation of Acanthamoeba castellanii was inhibited by LFAS and LFWS at concentrations of $0.1{\times},\;0.5{\times}\;1{\times},\;and\;3{\times}.$ In addition, GSH levels were increased about 2-fold with LFAS and 1.5-fold with LFWS in cultured murine hepa1c1c7 cells. LFAS and LFWS were also shown to increase GSH levels in human Hep3B cells. These results suggest that LFAS has chemopreventive potential by inducing QR activity, inhibition of ODC activity and increasing GSH levels.

  • It is generally known that many cases of pathological rough voice are characterized not by simple random perturbations but by quasi-periodic perturbations in the speech wave. However, there are few studies on the characteristics of perturbations in vocal fold vibrations associated with this type of voice. We have been conducting studies of pathological vocal fold vibration using a high-speed digital image recording system developed by our institute, Compared to the ordinary high-speed-motion picture system, the present system is compact and simple to operate and thus, it suited for pathological data collection. In vitro combination effects of natural substances and antimicrobials against Edwardsiella tarda and Streptococcus iniae (In vitro에서 Edwardsiella tarda와 Streptococcus iniae에 대한 항균제와 천연 유래 항균물질의 병용효과)

  • The purpose of this study is to explore the possibility of essential oils and flavonoids improving the performance of antibiotics, proving a higher efficacy in the presence of natural substances against Edwardsiella tarda and Streptococcus iniae. Increased efficacy was observed in several cases: both essential oils and flavonoids increased antibacterial efficacy of oxytetracycline and flumequine against E. tarda by 2-fold; essential oils and flavonoids increased antibacterial efficacy of nalidixic acid against E. tarda by 4-fold; flavonoids increased antibacterial efficacy of josamycin against S. iniae by 4-fold. The results obtained in this study suggest that essential oils and flavonoids might be especially useful in increasing the antimicrobial activity of nalidixic acid and josamycin against E. tarda and S. iniae in fish. Natural compounds examined in this study could also be useful to help decrease the overuse of antibiotics in fish.

Treatment Outcomes and Prognosis of Benign Vocal Fold Lesions (양성 성대 병변의 치료 결과 및 예후인자)

  • There are no standard consensus about treatment results and prognostic factors based on randomized trials for benign vocal fold lesion. Currently, voice therapy is the treatment of choice for vocal nodules, and laryngomicroscopic surgery is for vocal polyps. There are no strong evidences to support it, based on randomized controlled trials, But, it's just a consensus among laryngologist. Considering the pathophysiology of benign vocal cord lesions, cognitive behavioral therapy that corrects the patient's bad voice habits and improves their vocal hygiene could be most important factor for treatment outcomes.

  • With recent developments in medical technology and the introduction of various types of lasers, the role of fiberoptic laryngeal laser surgery (FLS) in laryngology has been significantly expanded. FLS are typically performed under local anesthesia, and patients may return to normal activities immediately after the procedure. This corresponds to the current trend of minimally invasive surgery and may limit unnecessary general anesthesia, reduce medical costs, and increase patient compliances. Main indications of FLS procedure were vocal polyp, recurrent respiratory papillomatosis, vocal fold granuloma and vocal fold dysplasia. In this review, we discuss practical tips and unique value of FLS.

Anesthesia for Office Based Vocal Fold Injection (외래 시행 성대주입술을 위한 마취 방법)

  • Vocal fold injections are usually performed with a patient wake in an office under local anesthesia. For comfortable and safe office-based procedures, thorough anesthesia and premedication should be provided to the following three regions; nasal cavity, oropharynx, and larynx. Topical lidocaine is most widely used anesthetics on office based procedure. Lidocaine has a low to intermediate potency, 45 minutes to 60 minutes' duration of action, and onset of sufficient anesthesia within 90 seconds of topical administration. Tetracaine, prilocaine, ropivacaine, and bupivacaine also have been used in the office-based procedures. Nasal decongestant, oxymetazoline, is also used for widening nasal cavity by constriction of nasal mucosa. The amount of topical and local anesthetics used in vocal fold injection rarely exceeds toxic doses. The physician should know proper anesthesia techniques and must be familiar with the safe dose and complication of all anesthetics used.
