횡행결장간막의 림프절을 침범한 고래회충유충증의 장외감염 1례 (A case of extragastrointestinal anisakiasis involving a mesocolic Iymph node)

  • 고래회충의 유충감염증은 주로 위장관을 침범하여 육아종을 형성하고 급성복통 등 임상증상을 유발한다. 그러나 감염 일부에서 유충은 위장관 벽을 통과하고 복강내 기관에서 육아종을 형성하는 경우가 있으며 일본에서는 이러한 장외감염이 전체 고래회충중의 0.4%를 차지한다고 보고된 바 있다. 우리나라에서는 이와 같은 증례가 보고된 바 없어 이를 기록하고자 한다. 환자는 43세된 남자로 서울에 거주하며 입원 3주일 전부터 피곤감과 식욕녁진 증상이 나타나고 3일전부터는 혈 변이 있어서 복부 전산화 단층촬영을 시행한 결과 위 중간부 전벽에서 종양이 발견되었다. 종괴를 수술하는 과정에서 횡행결장간막의 림프절이 커진 것이 발견되어 생검하였다. 림프절의 크기는 $0.8{\;}{\times}{\;}0.6{\;}cm$였으며, 조직병리학적 검색 결과 호산구의 침윤이 심한 육아종이었고 육아종 중심에서 선충의 절편이 발견되었다. Y-자 모양의 측선 근육세포의 수와 모양, 배설세포의 모양과 크기, 전체 절편의 크기 등을 기초로 고래회충(Anisakis species) 유충으로 진단하였다. 그러나 이 환자의 주병변과 고래회충의 장외감염과는 관련이 없다고 생각하였다.

한국산 해산어류의 Anisakid유충 감염률 및 형태학적 분류 (Morphological Classification and Infection Rate of Anisakid Larvae in Marine Fishes)

  • Anisakiasis occur after the ingestion of raw marine fish and squid containg anisakid larvae. In recent years about 40 cases of anisakiasis have been recorded in Korea. Considering, however, the Korean style of eating raw fish, many more cases would exist and prevention is necessary. We describe the infection rate of anisakid larvae in 13 species of marine fish and squid which were caught in the Korean sea. And each extracted larva is classified according to morphological characters. The results are following ; Scomber japonicus, Pseudosciaena manchurica, Trichiurus haumela showed high infection rate of anisakid larvae. Although Sepia esculenta showed low infection rate, most of anisakid larvae found in Sepia edulis are embedded in muscles. So relative high frequent rate of anisakiasis may developed by Sepia esculenta. Five type (Anisakis Type I, Terranova Type B, Raphidascaris sp., Contracaecum Type A, Contracaecum Type D) of anisakid larvae are classified according to their morphological characters.

  • Pseudoterranova decipiens larva is a rare cause of anisakiasis. Indeed, prior to the present study, there had been only 12 reports of larval P. decipiens infection in the Republic of Korea. In June 2011, an anisakid larva, 32.1 mm in length and 0.88 mm in width, and finally identified as the third stage larva of P. decipiens owing to the presence of an intestinal cecum but lacking ventricular appendage, was discovered in a 61-year-old woman during the course of endoscopy executed as a part of routine physical examinations. The patient had eaten raw a rockfish 13 hr prior to the endoscopy, but showed no symptoms of anisakiasis. This paper is the 13th report of P. decipiens infection in Korea.

  • The third-stage larvae (L3) of the parasitic nematode, Anisakis simplex, have been implicated in the induction of hyperimmune allergic reactions in orally infected humans. In this work, we have conducted a review of an investigation into immune reactions occurring in animals experimentally infected with A. simplex L3. The patterns of serum antibody productions if the experimental animals against excretory-secretory products (ESP) of A. simplex L3 contributed to our current knowledge regarding specific humoral immune reactions in humans. In our review, we were able to determine that L3 infection of experimental animals may constitute a good model system for further exploration of immune mechanisms and allergy in anisakiasis of humans.

시험관내에서 아니사키스 유충의 운동성에 대한 고련피, 관중, 사군자의 억제효과 (Inhibitory effects of herbal extracts (Meliae ezadarach, Dryopteris crassirhizoma, Quisqualis indica var villosa) on larval migration of Anisakis spp. in vitro)

  • A high incidence of Anisakiasis has been reported in many countries where people eat frequently raw or undercooked seafood. Anisakis spp. larvae were obtained from the mackerel acquired from a fish market of Cheongju city. They were divided into several groups and placed in culture dishes containing RPMI-1640 (culture media), in the presence or absence of different concentrations of herbal extracts (Meliae ezadarach, Dryopteris crassirhizoma, Quisqualis indica var villosa). The objective of the present study was to investigate the activity of larval migration inhibition in vitro. Meliae ezadarach at the concentrations of 7.5, 15, and 30 mg/ml effectively inhibited the larvae migration in time-dependent manner during experimental period of 0-24 h. Treatment of Meliae ezadarach at the three concentrations completely inhibited the larvae migration in vitro. Dryopteris crassirhizoma at the concentrations of 5, 10, and 20 mg/ml also effectively inhibited the larvae migration in a time-dependent manner. The treatment of Dryopteris crassirhizoma for 12 h completely inhibited the larvae migration. The inhibitory effect of Dryopteris crassirhizoma was stronger than that of Meliae ezadarach. Although Quisqualis indica var villosa also showed the inhibitory effect on larvae migration, its inhibitory efficacy was the weakest among tested herbal extracts. These results indicated that some herbal extracts may be useful in controlling human anisakiasis.

Molecular Identification of Anisakis Larvae Extracted by Gastrointestinal Endoscopy from Health Check-up Patients in Korea

  • Anisakiasis is a zoonotic disease induced by anisakid nematodes, and endoscopic inspection is used for a diagnosis or remedy for it. Anisakis simplex, Anisakis physeteris, and Pseudoterranova decipiens had been reported to be the major species causing human infections, particularly, in Japan. However, in Korea, recent studies strongly suggested that Anisakis pegreffii is the major species of human infections. To support this suggestion, we collected anisakid larvae (n=20) from 20 human patients who were undergone gastrointestinal endoscopy at a health check-up center in Korea, and molecular identification was performed on the larvae using PCR-RFLP analysis and gene sequencing of rDNA ITS regions and mtDNA cox2. In addition, anisakid larvae (n=53) collected from the sea eel (Astroconger myriaster) were also examined for comparison with those extracted from humans. The results showed that all human samples (100%) were identified as A. pegreffii, whereas 90.7% of the samples from the sea eel were A. pegreffii with the remaining 9.3% being Hysterothylacium aduncum. Our study confirmed that A. pegreffii is the predominant species causing human anisakiasis in Korea, and this seems to be due to the predominance of this larval type in the fish (sea eels) popularly consumed by the Korean people. The possibility of human infection with H. aduncum in Korea is also suggested.

Parasitic Infections Based on 320 Clinical Samples Submitted to Hanyang University, Korea (2004-2011)

  • We analyzed 320 clinical samples of parasitic infections submitted to the Department of Environmental Biology and Medical Parasitology, Hanyang University from January 2004 to June 2011. They consisted of 211 nematode infections, 64 trematode or cestode infections, 32 protozoan infections, and 13 infections with arthropods. The nematode infections included 67 cases of trichuriasis, 62 of anisakiasis (Anisakis sp. and Pseudoterranova decipiens), 40 of enterobiasis, and 24 of ascariasis, as well as other infections including strongyloidiasis, thelaziasis, loiasis, and hookworm infecions. Among the cestode or trematode infections, we observed 27 cases of diphyllobothriasis, 14 of sparganosis, 9 of clonorchiasis, and 5 of paragonimiasis together with a few cases of taeniasis saginata, cysticercosis cellulosae, hymenolepiasis, and echinostomiasis. The protozoan infections included 14 cases of malaria, 4 of cryptosporidiosis, and 3 of trichomoniasis, in addition to infections with Entamoeba histolytica, Entamoeba dispar, Entamoeba coli, Endolimax nana, Giardia lamblia, and Toxoplasma gondii. Among the arthropods, we detected 6 cases of Ixodes sp., 5 of Phthirus pubis, 1 of Sarcoptes scabiei, and 1 of fly larva. The results revealed that trichuriasis, anisakiasis, enterobiasis, and diphyllobothriasis were the most frequently found parasitosis among the clinical samples.

Analysis of Parasitic Diseases Diagnosed by Tissue Biopsy Specimens at KyungHee Medical Center (1984-2005) in Seoul, Korea

  • We analyzed parasitic diseases diagnosed by tissue biopsy specimens at KyungHee Medical Center (KMC) from 1984 to 2005. The total number of parasite infection cases was 150 (0.07%) out of the total 211,859 biopsy specimens submitted for histopathological examinations. They consisted of 62 cysticercosis, 23 sparganosis, 16 paragonimiasis, 15 amebiasis, 11 anisakiasis, 11 clonorchiasis, 3 ascariasis, 2 scabies, 2 enterobiasis, 2 trichuriasis, 1 leishmaniasis, 1 taeniasis, and 1 thelaziasis. Out of 62 cysticercosis cases, 55 were detected in subcutaneous tissues or the central nerve system. Eighteen out of 23 sparganosis cases were involved in muscular and subcutaneous tissues. In most anisakiasis cases, the involved organ was the stomach. The lung and the pleura were the most common site of paragonimiasis. The incidence of parasitic diseases during the first 5 years (1984-1988) was the highest of all observed periods. After 1989, similar incidences were shown throughout the period. Whereas cysticercosis was diagnosed in 34 cases during 1984-1988, no case has been diagnosed since 2000. In the case of sparganosis, the chronological incidence was almost uniform throughout the period 1984-2005. Paragonimiasis showed a similar tendency to cysticercosis. In gender and age distribution of parasitic diseases, men showed higher incidence rates than females, and the age groups of the 40s or older indicated higher infection frequencies than other age groups. Therefore, these results are a significant report to appear the tendency of human parasitic disease diagnosed by tissue biopsy in association with parasitosis at KMC in Seoul.

ELISA와 SDS-PAGE/immunoblot을 이용한 실험적 토끼 아니사키스증에서 혈청 항체가의 변화 (Changes of IgM and IgG antibody levels in experimental rabbit anisakiasis as observed by ELISA and SDS-PAGE/immunoblot)

  • 인체아니사키스증은 우리나라에서도 드물지 않게 발생하나 과거에는 수술에 의한 충체의 확인만이 진단이자 치료이어서 진단에 많은 어려움을 겪었다. 그러나 위내시경이 진단과 치료에 이용되면서 많은 중례의 발견을 가져오게 되었다. 그러나 증상이 심하지 않을 경우나 위내시경의 삽입이 어려운 장(장) 아니사키중 또는 만성 아니사키스중을 진단하기 위해서는 혈청학적 방법이 필요하다. 이 연구는 아니사키스중의 혈청학적 진단연구를 위해 붕장어에서 채집한 유충을 토끼에 10마리섹 감염시키고 IsM, IgG 항체의 출현시기와 변화양상을 ELISA와 SDS-PAGE/immunoblot으로 관찰하였다. 그 결과는 다음과 같다. 1. ELISA에 의하여 측정한 19M 항체는 감염 6일부터 증가하여 11일째 최고치에 도달한 후 35일 이후에는 감 염 전의 수준으로 저하하였고 IgG 항체가는 감염 6일부터 증가하여 26일째 최고치에 도달한 후 95일까지 감소하는 양상을 보였다. 2. 7.5∼15% SDS-PAGE에서 아니사키스추출액은 최소 41개 이상의 단백분회 을 나타냈으며 195, 145, 110, 104, 73, 69, 49, 42.5, 40, 34, 30, 24, 20, 16, 14 kDa가 주 분획이었다. 3. Immunoblot에 의해 IgM 항체에 반응하는 주 band는 168, 95, 74, 64, 51, 47, 34 kDa의 5개 band이었다. 4. IgG 항체와 반응하는 항원 분획은 11개 이상 관찰되었고 그 중 168, 92, 85, 64, 58, 52, 42, 40 kDa가 강한 반응을 보였다. 5. 아니사키스추출액 항원과 다른 기생충성 질환자 혈청과의 교차반응은 관찰할 수 없었다.

Pseudoterranova decipiens의 유충에 의한 위 아니사키스증 1례 (A human case of gastric anisakiasis by Pseudotewcnova decipiens larva)

  • 1991년 4월 23일에 심한 상복부통을 주소로 부산 백병원 내과를 찾아온 42세 가정주부에 대한 내시경 검사에서 위벽을 뚫고 있는 아니사키스 유충 1마리를 검출하였다. 환자는 내원 약 15시간 전에 우럭 회를 먹은 경험이 있었으며 회를 먹고 약 6시간 후부터 심한 상복부통이 나타났다고 하였다. 각종 검사상 특이 소견은 없었고 위 내시경 검사에서 위체 대만부 중간부위의 점막을 뚫고 있는 아니사키스 유충 1마리를 발견하여 제거하였다. 제거한 충페는 $29.73{\;}{\times}{\;}0.94{\;}mm$ 크기이었고 ventriculus 전방 115 이상 부위까지 올라오는 맹장을 가지고 있었으며 ventricular appendage는 관찰되지 않았다. 충체의 전단에서 윤곽이 뚜렷한 3개의 구순은 관찰되었으나 boring tooth는 존재하지 않았고 후단에서 mucron도 관찰되지 않았다. 이상의 형태학적 특징 및 계측치를 토대로 하여 Pseudntewonon decipien (= Terranova type A) 제4기 유충인 것으로 동정하였다. 이 기생충의 국명을 물개회충으로 제안한다.
