Robotic assisted surgery in adult patient with congenital ventricular septal defect Since December in 2005, we have done minimally invasive surgeries in selected cases of mitral valve diseases, tricuspid valve diseases, atrial septal defects and atrial fibrillations with the $AESOP^{TM}$ robotic arm. We have had a better surgical view and skill, according to accumulation of the experience of this procedure. Recently, we performed robotic assisted surgery in a 47-year-old female with congenital perimembranous ventricular septal defect.

We herein present a case of a successful correction of cor triatriatum associated with thrombotic pulmonary hypertension diagnosed in an adult female patient. We confirmed diagnosis using transthoracic and transesophageal echocardiography in addition to cardiac computed tomography and magnetic resonance imaging. Surgical repair comprised excision of the fibromuscular membranous septum in the left atrium, patch closure of an atrial septal defect, and reconstruction of the pulmonary arteries with a vascular graft. Cor triatriatum complicated pulmonary thrombotic hypertension with atrial septal defect is amenable to surgical correction with satisfactory results.
Known as a female hormone, estrogen, performs important functions, and the activities of the hormone are mediated via the estrogen receptor. The principal objective of the present study was to assess the effects of a estrogen receptor agonist in male reproductive organs. In this study, the estrogen receptor alpha agonist, PPT, was injected subcutaneously into adult male mice. The effects of PPT on the murine reproductive system were histologically assessed at 3,5, and 8 weeks after treatment. In the treatment group, reductions were observed in the weight of the body, testis and epididymis. Microscopic examination revealed a reduction in seminiferous tubular diameter in the testis, and epithelial cell height in the epididymis during the experiment. 8 weeks after treatment, spermatogenesis was not detected, nor was the lumen of the seminiferous tubules. In the fertility test, 1 week after PPT injection, the fertilizing ability of males was decreased, and on the 2nd and 3rd weeks, complete infertility was observed. In conclusion, the injection of high concentrations of PPT into adult males induced physiological changes, including infertility, and also induced morphological changes, including a reduction in the height of epithelial cells within the reproductive system.
The role of cytochrome P-450 2E1 (P450 2E1) in the early phase of alcoholic fatty liver was examined. Female rats were pretreated with either allyl sulfide (200 mg/kg, po), disulfiram (500 mg/kg, po), YH 439 (250 mg/kg, po) or pyrazine (200 mg/kg/day$\times$2 days, ip). Marked changes in carbon tetrachloride-induced hepatotoxicity and caboxyhemoglobin (COHb) elevation due to dichloromethane administration were observed in rats treated with one of the P450 2E1 modulators. A single dose of ethanol (6 g/kg, po) increased the hepatic triglyceride contents approximately 2 fold, which was inhibited completely by YH 439 pretreatment. However, the other P450 2E1 modulators failed to alter the ethanol-induced hepatic triglyceride accumulation. In vitro hepatic microsomal enzyme activity was determined in 4 week old premature and 12 week old adult rats. Aminopyrine-N demethylation was not different, but p-nitrophenol hydroxylation and p-nitroanisole O-demethylation were significantly higher in premature rats. However, no difference in the triglyceride accumulation induced by an intraperitoneal dose of ethanol (3 g/kg) was noted between premature and adult rats. The results suggest that the P450 2E1 activity dose not play an important role in the induction of acute alcoholic fatty liver.
To understand the developmental process of the gait pattern from infancy to the adult, a gait analysis for a 14 months female infant has been performed for ten weeks. The gait parameters has been measured every other week since the week when she was able to walk without aid. The infant gait data at the tenth week of the test shows a similar pattern to the adult gait in many aspects including initial knee flexion, initial ankle dorsiflexion, the push-off pattern, etc. There is not a significant change in the hip flexion angle from week to week from the two weeks after the test started. To check the individuality in the collected data, more infant subjects are being tested presently.
To determine the effect of newly developed insecticide on two spotted spider mite, Tetranychus Urticae, populations, ten leaf discs were sprayed with three different concentrations (0.5%, 1.0%, and 1.5%) of the tested insecticide solution, and with a water-sprayed control. The tested insecticide solution was toxic to adult female and immature of two spotted spider mite, but was relatively non-toxic to eggs. For equivalent concentrations, more adult females were killed than immature stages. There was no marked difference between the hatch of treated and untreated eggs, although a weak trend between the numbers of unhatched eggs and the insecticide concentration after 7 days is shown.
Patients over 15 years of age who have undergone a surgical correction of a congenital cardiac malformation during period of January 1958 through January 1986 have been reviewed. During this period there were 3957 congenital cardiac lesions consisting of 2712 acyanotic and 1245 cyanotic cases. Among them, a total of 725 adults [18.3%] with a variety of congenital heart lesions, 548 acyanotic group and 177 cyanotic group were operated on. 372 patients were male and 252 patients were female. There were 280 patients under 20 years of age, 206 between 20-24 years, 102 between 25-29 years, 48 between 30-34 and 89 over 35 years. The most common defects were atrial septal defect which accounted for 207 cases [28.6%] and other common anomalies were VSD [140 cases, 19.3%], TOF [136 cases, 18.6%], PDA [120 cases, 16.6%] and PS [33 cases, 4.6%] in order of incidence. Overall operative mortality for this series was 3.6% [1.8% of a cyanotic group and 9.0% of cyanotic group] compared with 2.8% of total cases of congenital heart disease [acyanotic group 1.1% and cyanotic group 6.5%]. This reviewed series reveals the incidence of operable congenital heart defects appearing in any adult life and demonstrates that surgical repair can be accomplished with a satisfactory low mortality rate.
Purpose: Epidermal nevi are hamartomas of the skin with multiple clinical variants. Normally, epidermal nevi present at an early age, are found on the head and neck area, and have a female predominance. Only very rarely do epidermal nevi have an adult onset. Most lesions are small and giant Epidermal nevi (> 5 cm at its greatest diameter) are quite rare. Huge verrucous plaque was observed on the scalp of a 70-year-old man. He had been reclusive for a long time, and this problem might have played a crucial role in the development of the giant epidermal nevi. Methods: A punch biopsy and MRI imaging were performed. The huge mass was resected. A local flap and split thickness skin graft was used for reconstruction. Results: The histology examination revealed the characteristic findings of epidermal nevus. Postoperatively, the patient did not suffer from complications or recurrence. Conclusion: A rare case of a huge epidermal nevus on the scalp of an elderly man was treated with a surgical excision. This is the oldest patient reported with an adult onset epidermal nevus in Korea.
This study investigated the impact of parentification on ambivalence over the emotional expression of early adults who had a sibling with disabilities and examined if rejection sensitivity moderated the relationship of parentification and ambivalence over emotional expression. Participants consisted of 116 siblings in early adulthood who had a sibling with disabilities (45 male; 71 female). The data were collected from 13 centers for family disability, four community rehabilitation centers, three parent societies for people with disabilities, and three self-help groups in Seoul, Busan, and Gyeonggi province. The levels of ambivalence over emotional expression, parentification, and rejecton sensitivity were measured by the Ambivalence over Emotional Expression Questionnaire (King & Emmons, 1990), the Filial Responsibility Scale-Adult (Jurkovic, Thirkield, & Morrell, 2001), and the Rejection Sensitivity Questionnaire (Downey & Feldman, 1996), respectively. The PROCESS Macro program examined the moderating model. The Results indicated that both levels of parentification and rejection sensitivity increased the level of ambivalence over emotional expression of non-disabled siblings. In addition, rejection sensitivity moderated the effect of parentification on ambivalence over emotional expression. The influence of parentification on ambivalence over emotional expression was greater when the level of rejection sensitivity was high, compared to when it was low.
Background: This study was conducted to investigate factors related to unmet medical needs of medical care in adult diabetes patients and to suggest factors related to unmet medical in Korea. Methods: This study used data from the Korea National Health and Nutrition Examination Survey (KNHNES) 2014-2017. The subjects of the study were conducted on patients with unmet medical needs experience among the patients and analyzed using the IBM SPSS ver. 25.0 program (IBM Corp., Armonk, NY, USA). Results: Overall, 10.9% of patients had unmet medical needs. Being female, less educated, and lower medical aid were related to unmet medical needs. And subjects with poor subjective health and higher stress level were more likely to report unmet medical needs. Conclusion: Although comprehensive health insurance coverage, 10.9% of people with diabetes experienced unmet healthcare needs. The results of this study suggest that socioeconomic factors such as low education and medical aid were associated with unmet medical needs.
