한국표준질병사인분류의 개정에 관하여 (The 5th revision of the Korean Standard Classification of Diseases)

  • The 5th revision of Korean Classification of Diseases(KCD) became effective on January 1, 2008. It has reflected the changes made to the tenth revision of International Classification of Diseases (ICD-10) between 1998 and 2005 and the suggestions of academic and related societies in Korea. Two important alterations seem to have a major implication in the insurance industry. One would be the official introduction of a Korean version of International Classification of Diseases for Oncology, third edition(ICD-O-3). The borderline ovarian tumor is classified as a borderline neoplasm, which was classified as a malignant neoplasm in the previous edition of International Classification of Diseases for Oncology. The other would be the appearance of non-C-code malignant neoplasm for the diseases, such as polycythemia vera, newly classified as a malignant neoplasm by the current edition of International Classification of Diseases for Oncology. The National Office of Statistics(NSO) adopted the way of implementation used in the Australian Modification of International Classification of Diseases(ICD-10-AM), instead of assigning them into corresponding C code. Overall, the changes made in this revision doesn't seem to have a serious impact on the insurance industry since it has only reflected updates made to ICD-10.

KCD 7과 OIICS의 분류기준을 활용한 국내 연구실 사고의 통계적 분석 (Statistical Analysis of Domestic Laboratory Accidents using Classification Criteria of KCD 7 and OIICS)

  • This study statistically analyzed the laboratory accidents by investigating 806 laboratory accident survey reports which were officially submitted to government from 2013 to June 2017. After comparing domestic and foreign accident classification criteria, the laboratory accidents were classified using KCD7(Korean Standard Classification of Diseases) and OIICS(Occupational Injury and Illness Classification System) criteria. For the type and part of injury, KCD7 classification criteria was adopted. And, for the cause and occurrence type of accidents, OIICS was adopted to analyze the laboratory accidents. Most of injuries happened to the wrist and hand caused by sharp materials or chemical materials. The analysis of accident cause showed that accidents resulted in medical practice and accidents from handtools and chemical materials such as acid and alkali frequently occurred. The major occurrence types of laboratory accidents was body exposure to the chemical materials such as hydrochloric acid and sulfuric acid. In addition, the accidents resulted in destroy of grasped object or falling object were frequently reported.

  • Carcinoid tumor is called as neuroendocrine tumor and is classified into neuroendocrine tumor Grade 1, neuroendocrine tumor Grade 2, and neuroendocrine carcinoma based on the differentiation of tumors. Recently, the incidence of rectal carcinoid tumor has been increasing probably due to the increased interest on screening colonoscopy and the advancement of endoscopic imaging technology. As the rectal carcinoid shows a wide range of clinical characteristics such as metastasis and long-term prognosis depending on the size and histologic features, it is a challenge to give a consistent diagnostic code in patients with the rectal carcinoid. If the rectal carcinoid tumor is less than 1 cm in size, it can be given as the code of definite malignancy or the code of uncertain malignant potential according to International Classification of Diseases for Oncology (ICD-O) by World Health Organization (WHO). Because patients get different amount of benefit from the insurance company based on different diagnostic codes, this inconsistent coding system has caused a significant confusion in the clinical practice. In 2019, WHO updated ICD-O and Statistics Korea subsequently changed Korean Standard Classification of Diseases (KCD) including the code of rectal carcinoid tumors. This review will summarize what has been changed in recent ICD-O and KCD system regarding the rectal carcinoid tumor and surmise its clinical implication.

소시호탕 systemic review 및 KCD 코드 대조 (Systemic review of Xiaochaihutang and its correlation with KCD)

  • Objectives : Xiaochaihutang(小柴胡湯) is a famous herbal prescription in the Traditional Korean Medicine, and its components include Bupleuri Radix(柴胡), Scutellariae Radix(黃芩), Ginseng Radix(人蔘), Pinelliae Tuber(半夏), Glycyrrhizae Radix et Rhizoma(甘草), Zingiberis Rhizoma Crudus(生薑), Zizyphi Fructus(大棗). Methods : In this study, we reviewed the effects of xiaochaihutang by searching Pubmed. And we connected with Korean Standard Classification of Diseases (KCD) and the effects of xiaochaihutang. Results and Conclusion : The effects of xiaochaihutang was related with B18, C22, C34, C56, J30, J45, K70, K74, K76, F32, F33, F34, F41 as indicated by KCD. In clinics, the medical doctors almost use the KCD code for insurance. This review give us the scientific clue of Xiaochaihutang's effect in KCS system.

세계보건기구 국제질병분류 11판 베타버전 중 한의학 고유 상병의 로마자 표기 및 영문표현 검토연구 (Review and proposed improvements for Romanization and English expressions of rubrics in the WHO ICD-11 beta version traditional medicine chapter)

  • Objectives : The purpose of this study is to review and propose improvements for the Romanization and English expressions in the WHO international classification of diseases 11th revision beta version (ICD-11b) traditional medicine chapter. Methods : ICD-11b as of October 5, 2015, was reviewed. Romanization and English expressions were analyzed with reference to existing standards such as the Basic Principles of Romanization stipulated by the National Institute of Korean Language, and the Korean Standard Classification of Diseases (KCD), suggested improvements followed. Results : Following the Basic Principles of Romanization, 131 ICD-11b rubrics need improvement in the Romanization of Korean. When compared to KCD-6 comparable rubrics, 161 ICD-11b rubrics are the same and 64 are different. When compared to KCD-7 comparable rubrics, 118 ICD-11b rubrics are the same, and 51 are different. In KCD-6, there are 127 rubrics that do not match with items in ICD-11b. In KCD-7, there are 123 rubrics that do not match with items in ICD-11b. Conclusions : ICD-11b may be improved by correcting the Romanization and consideration of English expressions suggested in this study.

계층적 분류체계를 적용한 한국질병사인분류 예측 모델의 개선 (The improvement of Korean Standard Classification of Diseases prediction model by applying the hierarchical classification system)

  • 한국표준질병사인분류(KCD)는 사람의 질병과 사망 원인을 유사성에 따라 체계적으로 유형화한 분류체계이다. KCD는 계층적 분류체계로 구성되어 있어 분류마다 연관성이 존재하지만, 일반적인 텍스트 분류 모델은 각각의 분류를 독립적으로 예측하기 때문에 계층적 정보를 반영하는 데 한계가 있다. 본 논문은 계층적 분류체계를 적용한 KCD 예측 모델을 제안한다. 제안 방법의 효과를 입증하기 위해 비교 실험을 진행한 결과 F1-score 기준 최대 0.5%p의 성능 향상을 확인할 수 있었다. 특히 비교 모델이 잘 예측하지 못했던 저빈도의 KCD에 대해서 제안 모델은 F1-score 기준 최대 1.1%p의 성능이 향상되었다.

혈허증(血虛證)의 임상 질환 범위에 대한 고찰 (Study on Clinical Diseases of Blood Deficiency Pattern)

  • This article is a study on to which categories of modern diseases blood deficiency pattern types are assigned by reference to modern clinical papers. Clinical papers were searched in China National Knowledge Infrastructure(CNKI) from 1994 to 2013. Results are as follows. First, diverse diseases classified in qi-blood depletion pattern and pattern of blood deficiency and wind-dryness are reported and pattern types designated by the name of viscera are the minority. Second, among pattern types in Korean Standard Classification of Diseases(KCD), diseases classified in heart blood deficiency pattern, liver blood deficiency pattern and heart-liver blood deficiency pattern are a few. Third, the level of designation by the combined patterns such as qi deficiency, fluid deficiency, yin deficiency, kidney deficiency, essence deficiency, wind-cold, cold-dampness, dampness-heat, liver hyperactivity, liver depression and static blood is more specific than KCD, which makes pattern types more useful to clinical application. The detailed relation between modern diseases and pattern types can be an another topic.

황금탕(黃芩湯) 표준화 및 KCD 질병코드 연구 - 동의보감 및 PUBMED를 중심으로 (A literature study of Huangqintang for standardization and KCD code)

  • Objectives : Huangqintang(⿈芩湯) is a famous herbal prescription in the Traditional Korean Medicine (e.g. Shanghanlun) to treat various chronic disease such as neurological disease and digestive system disease. Its components include Scutellariae Radix(⿈芩), Paeonia lactiflora(芍藥), Glycyrrhizae Radix et Rhizoma(⽢草), Zizyphi Fructus(⼤棗). Methods : In this study, we standardize the name, composition and medical uses of Huangqintang by literature studying and paper searching. Based on the comparison between oriental medicine and modern medicine pharmacology, we linked the medical uses of Huangqintang to Korean Standard Classification of Diseases (KCD). Results and Conclusion : The name of the prescription is unified into "Huangqintang", and its composition ratio between Scutellariae Radix, Paeonia lactiflora, Glycyrrhizae Radix et Rhizoma, and Zizyphi Fructus is 3:2:2:3. Huangqintang is used for enteritis diseases due to its antipyretic and antinflammatory effects. Its medical application was relatied with 'A'code, 'C'code, and 'K'code as indicated by KCD. By standardizing the mixed conceptions of Huangqintang, this review will facilitate the coding of Huangqintang prescription. Therefore, it will make Huangqintang prescription more useful in clinics.

일개 한방병원에 내원한 산욕초기 산모의 한국표준질병·사인분류 분석 (Korean Standard Classification of Diseases of Early Postpartum Women in a Korean Medicine Hospital)

  • Objectives: The purpose of this study is to collect and analyze the KCD codes applied to the treatment of 27 postpartum women who had been treated with Korean traditional medicine in a Korean medicine hospital, so that this study may be used as a basic data for setting the direction of postpartum Korean medical treatment research. Methods: It was approved by the Institutional Review Board (IRB) of ${\bigcirc}{\bigcirc}$ University medical center (IRB approval number : WSOH IRB H1708-02-01). Twenty-seven postpartum women who had been treated at ${\bigcirc}{\bigcirc}$ University medical center were received outpatient treatment for two weeks (from September 27, 2017 to January 5, 2018), and the KCD codes applied to the mothers were collected after obtaining the consent. On the day of registration of the study, the fertility, obstetric history and high-risk pregnancies were identified through an interview. Results: 1. The mean age of the 27 subjects was $33.33{\pm}3.99\;years$ old. Among the subjects, 17 mothers (63.0%) were high-risk pregnancy and 10 mothers (37.0%) were normal. 2. Among the 22 major disease categories, 8 categories were used. M code (musculoskeletal system) was used 243 times (70.85%), followed by R code (unclassified symptom) of 51 times (14.87%) and U code (special purpose code) of 23 times (6.71%). 3. The most commonly used code among the ten frequently used codes was M25.57 (joint pain, ankle and foot), a total of 47 times. Of the remaining nine codes, except for R60.1 (systemic edema) and U68.4 (The deficiency of yang in Bi), all codes were M codes (musculoskeletal system). 4. The M code (musculoskeletal system) was the most used major disease category in high-risk group, a total of 159 times. But in specific categories, the most commonly used code was R60.1 (systemic edema), a total of 28 times. 5. In normal group, the M code (musculoskeletal system) was the most used major disease category, a total of 84 times. Also, in specific categories, the most commonly used code was M25.57 (joint pain, ankle and foot), total 29 times. 6. The U code, corresponding to 'the diagnosis of childbirth and other obstetrical medical use', was used 23 times (6.71%), O code three times (0.87%) and Z code two times (0.58%), which was less than 10% of the total number of codes used. Conclusion: When analyzing KCD codes related to Korean medicine treatment for postpartum diseases, it is important to select the KCD codes that reflect the actual clinical state.

대상질환군으로 본 의·한 협진연구 경향 분석 (Trend Analysis of Korean-Western Medicine Collaboration Studies by Disease Group)

  • Objectives: The purpose of this study was to analyze trends in Korean-Western Collaborative Medicine studies by disease group. Methods: Articles published until May 2020 were searched in domestic databases (NDSL, RISS, KISS, DBPIA, and KoreanTK) using the keywords '한양방', '양한방', '협진', '협의진료', '의한의', '의·한의', and '통합의학'. The articles were analyzed according to the publication year, journal type, research methodology, and disease group using the major and intermediate classifications of the Korean Standard Classification of Diseases (KCD-7). Results: In total, 112 articles were selected. The most frequent major disease groups were disease codes G, C (D), I, and M, in that order. The medium disease groups numbered 41 and most commonly involved collaborative studies on cerebrovascular diseases. Among the three top KCD-7 major group diseases, publications on Group C (D) were increasing significantly, publications on Group G were slightly increasing, but, publications on Group I decreased in the 2010s compared to the 2000s. Conclusions: In this study, we analyzed the frequent disease groups treated with domestic Korean-Western medicine in collaborative studies using the major and medium classifications of KCD-7. Well-designed clinical trials and economic evaluation research are needed, and this study can provide directions for future collaborative studies using Korean-Western medicine.
