A convergence of association between menopause, breastfeeding and first birth age and metabolic syndrome in Women (여성의 폐경과 모유수유 기간 및 첫출산 연령과 대사증후군의 연관성에 관한 융합적 고찰)

  • The purpose of this study was investigate the association between birth age related health factors and metabolic syndrome in women. The data of 6743 women from Korean National Health and Nutrition Examination Survey(2012-2014 KNHANES) were analyzed. Dependent variable was metabolic syndrom related variables were collected. Menopause status was associated with metabolic syndrome [odds ratio of with post-menopause 1.44(95% CI: 1.09-1.90)]. First birth age was associated with metabolic syndrome [odds ratio of women with 26-30 year : 0.81 (95% CI: 0.68-0.98); odds ratio of women 31 year over: 0.62(95% CI: 0.45-0.87)] after adjusted for age, region, household income, smoking, occupation. In conclusion, the metabolic syndrome is closely related to menopause and first birth age, and will be used as a basic data in developing future educational programs for women.

A Study on Nutritional Anemia of Pre-school Children in Korea (한국 미취학 아동의 영양성 빈혈에 관한 연구)

  • Nutritional anemia is an important nutritional problem affecting large population groups in most developing countries. Nutritional anemia is caused by the absence of any dietary essential involed in hemoglobin formation or by poor absorption of these dietary components. The most likely causes are lack of dietary iron, and folate, vitamin $B_{12}$ and high qualify protein. Anemia is considered to be a late mainfeastation of nutritional deficiencies, and even mild anemia is not the earilest sign of such a deficiency. Therefore, the object of therapy is to correct underlying deficiency rather than merely its manifestation. Iron deficiency anemia is generally much the most common form of anemia. And it is very prevalent particularly in pregnant women and young children, especially under five year of life. According to the rapid growth rate of infants, dietary iron should he provided for infants over three months of age in adequate amounts for the synthesis of hemoglobin required by the increasing blood volume and for the demands of newly formed cells. The principal causes of iron deficiency anemia are an inadequate dietary iron content, interference with absorption of iron from the intestine, excessive losses of iron from the body, disturbance of iron metabolism by infection, and social and cultural environments. The present study is planned to obtain informations concerning nutritional anemia through anthropometric and biochemical determinations for the assessment of nutriture in pre-school children. Determination was taken in 226 pre-school children in ruraI arae in 1968, 122 pre-school children in 1970, and 1526 hospitalized pre-school children in 1970. The results of this study are as follows; (1) According to Iowa Malnutrition Borderline (85 percentile) for weight, the proportions of underweighed pre-school boys and girls in rural area were 47.2% and 46.2% in1968, and were 36.1% and 51.8% in 1970. According to Iowa Malnutrition Borderline for height, the proportions of underheight boys and girls in rural area were 30.5% and 33.7%, and were 26.2% and 21.8% in 1970. Malnutrition scores of underweight for height values of boys and girls in rural area were 19.3 and 17.3 in 1968, and the scores of boys and girls were 15.6 and 15.5 in 1970. (2) The mean hemoglobin values of boys and girls in rural area were $11.2{\pm}1.8g/100ml\;and\;11.4{\pm}1.6g/100ml$ in 1968. In 1970, the mean values of boys and girls in rural area were $11.3{\pm}1.3g/100ml\;and\;11.7{\pm}2.4g/100ml$. The mean hemoglobin values of hospitalized boys and girls were $11.9{\pm}2.2g/100ml\;and\;11.7{\pm}2.4g/100ml$ in 1970. It is found that 92 of 215 children (42.7%) in rural area had concentrations of hemoglobin less than 11.0g/100ml in 1968. In 1970, 55 of 121 children (45.4%) in rural area and 559 of 1526 hospitalized children (36.6%) had concentrations of hemoglobin less than 11.0g/100ml. (3) The mean hematocrit levels of hospitalized boys and girls were $35{\pm}26.8%\;and\;35.4{\pm}6.4%$ in 1970. And 443 of 1334 hospitalized children (33.2%) had hematocrit values below 33%. (4) The average mean corpuscular hemoglobin concentration levels of hospitalized boys and girls were $32.4{\pm}2.2\;and\;32.3{\pm}2.2$ in 1970. And 1016 of 1352 hospitalized children (75.1%) had the mean corpuscular hemoglobin values below 34. (5) The mean iron values of young children in rural area and hospitalized children were $62.0{\pm}6.3{\mu}g/100ml\;and\;60.7{\pm}22.8{\mu}g/100ml$. The proportions of anemia cases below $50{\mu}g/100ml$ in rural area was 37.9%, and 34.3% in hospitalized children. (6) The mean total iron binding capacity of young children in rural area was $376{\pm}57.88{\mu}g/100ml,\;and\;342.2{\pm}6.15{\mu}g/100ml$ in hospitalized children. (7) The average transferrin saturation percentage of young children in rural area was $16.9{\pm}4.7%,\;and\;18.0{\pm}8.4%$ in hospitalized children. The proportions of anemia cases below 15% of young chi1dren in rural area and hospitalized children were 48.3% and 41.2%. Therefore, authors wish to recommend that the following further studies should be undertaken: (1) Standardization of simplied laboratory examination of nutritional anemia. (2) The prevalence of nutritional anemia and the requirements of iron, folate, and vitamin $B_{12}$ of pre-school children. (3) The content and absorption of iron in Korean food. (4) The pathogenesis of nutritional anemia and prevention of parasitic disease. (5) Maternal health and nutrition education.

Correlation between oral frailty and health-related quality of life (HINT-8) among older adults in Korea (한국 노인의 구강노쇠와 건강관련 삶의 질(HINT-8)의 관련성)

  • Objectives: This study aimed to confirm the correlation between oral frailty and health-related quality of life (HINT-8) among older adults in Korea. Methods: The data of 1,318 individuals aged ≥65 years who participated in the eighth Korean National Health and Nutrition Examination Survey (2019) were analyzed using complex sample statistical analysis. Results: Chewing discomfort was found to decrease the HINT-8 scores by 1.246, 1.324, and 1.089 times in the physical, social, and mental domains, respectively. Speech discomfort was found to decrease the HINT-8 scores by 1.275, 1.449, and 1.175 times in the physical, social, and mental domains, respectively. The HINT-8 scores of participants with ≤19 natural teeth were lower in the physical and social domains. Similarly, the HINT-8 scores of participants with brushing frequency of ≤2 were lower in the positive health domain. Non-use of oral hygiene products led to a reduction in the HINT-8 score in the social health domain. Conclusions: Oral frailty in older adults reduces the health-related quality of life. Thus, it is necessary to formulate policies to manage oral frailty in this population and develop specialized programs for the management of oral frailty.

The Effects of Quality of Life in the Elderly's Health Condition (노인의 건강상태가 삶의 질에 미치는 영향)

  • This study conducted a secondary analysis by using original data of performed by Korea Institute for Health and Social Affairs to determine factors affecting health-related quality of life for the elderly aged over 65 years living in Korea. The survey was conducted in 2005 and it evaluated totally 34,152 cases. Among them 3074 cases investigating the elderly aged over 65 years were selected for this study and finally 2036 cases were included in it by excluding cases with no answer or a wrong answer. The results were as follows. In the illness days, the average of the whole subjects was 11.05 days and it was longer when subjects were female and older, had lower educational background and lower family income, did not have spouses and jobs and were covered by medical aid as medical security. In the years of activity restriction, the average of the total subjects was 3.48 years and it was increased when they were older, had lower educational background and lower family income, did not have spouses, lived in detached houses and were covered by medical aid as medical security. Subjective health condition of the total subjects was 2.64 points out of 5 points in average. The scores were higher when subjects were male, younger, had better educational background, spouses, jobs and more family income, lived in multi-family living houses and were covered by corporate insurance as medical security. The average of total quality of life of the subjects was 2.61 points out of 3 points and it was found to be higher when subjects were male, younger, had better educational background, spouses, jobs and more family income and were covered by corporate insurance as medical security. It was decreased with higher illness days, higher years of activity restriction and lower subjective health condition. For the effects of the factors related to quality of life, subjective health condition showed the largest influence.

Urine Cotinine Should Be Involved in Initial Evaluation of Tinnitus in Adolescents

  • Objectives. Smoking is associated with hearing loss, while the correlation between tinnitus and smoking is not fully elucidated. This study aimed to evaluate risk factors of tinnitus in adolescents in terms of smoking, and we identified a rectifiable parameter that can be serially monitored. Methods. A cross-sectional study was conducted using data from the Korea National Health and Nutrition Examination Survey, with 2,782 participants aged 12 to 18 years, from 2008 through 2011. Participants with history of ear disease, hearing loss, and inadequate responses to questionnaires were excluded. We investigated the prevalence of tinnitus and tinnitus-related annoyance by questionnaire and sought potential risk factors in blood and urine tests and smoking history. Results. The prevalence of tinnitus in the 12- to 18-year-old population was 17.5%, with 3.3% reporting tinnitus-related annoyance. On univariate analysis, the prevalence of tinnitus increased with age (P<0.001) and was higher among girls (P=0.012). Blood tests and urinalysis showed significant correlation between tinnitus and red blood cell count, alkaline phosphatase levels, and urine cotinine (P=0.002, P<0.001, P=0.018, respectively). In multivariate analysis, the urine cotinine level was the only parameter associated with tinnitus (odds ratio, 1.000; 95% confidence interval, 0.999 to 1.000; P=0.038). Smoking was also significantly correlated with tinnitus (P=0.043), and amount of smoking with tinnitus-related annoyance (P=0.045). However, current smoking and past smoking were not correlated with tinnitus. Conclusion. Urine cotinine may be a rectifiable marker for management of tinnitus in adolescents. This suggests that smoking cessation should be incorporated in the management of tinnitus in adolescents.

Relationship between Mental Health Counseling and Depression Prevalence in the Korean Adult Population (한국 성인 인구집단의 정신건강 상담과 우울증 유병과의 관계)

  • This study was a secondary data analysis study to determine the relationship between mental health counseling and the current prevalence of depression in the Korean adult population. Data were extracted from raw data of the 7th 3rd year (2018) of the National Health and Nutrition Examination Survey (NHANES) conducted by the Korea Centers for Disease Control and Prevention. The subjects of this study were 6,238 adults aged 19 years or older who participated in the NHANES conducted from January to December 2018. The collected data were analyzed by frequency analysis and logistic regression analysis of the complex sample by the SPSS/WIN 27.0 version program. The mental health counseling statistically significant associated with variables such as sex, marital status, economic activities, and family income level, respectively (p<0.05). The prevalence of depression statistically significant associated with age group (p<0.0001). The odds-ratio of current prevalence of depression was 5.09 times (OR, 5.085; p<0.0001) lower in subjects who had undergone mental health counseling for the past year compared to subjects who did not. These findings could be useful as evidence-based data for the prevention and management of depression by predicting the vulnerable factors for mental health in the Korean adult population.

Group Classification on Management Behavior of Diabetic Mellitus (당뇨 환자의 관리행태에 대한 군집 분류)

  • The purpose of this study is to provide informative statistics which can be used for effective Diabetes Management Programs. We collected and analyzed the data of 666 diabetic people who had participated in Korean National Health and Nutrition Examination Survey in 2007 and 2008. Group classification on management behavior of Diabetic Mellitus is based on the K-means clustering method. The Decision Tree method and Multiple Regression Analysis were used to study factors of the management behavior of Diabetic Mellitus. Diabetic people were largely classified into three categories: Health Behavior Program Group, Focused Management Program Group, and Complication Test Program Group. First, Health Behavior Program Group means that even though drug therapy and complication test are being well performed, people should still need to improve their health behavior such as exercising regularly and avoid drinking and smoking. Second, Focused Management Program Group means that they show an uncooperative attitude about treatment and complication test and also take a passive action to improve their health behavior. Third, Complication Test Program Group means that they take a positive attitude about treatment and improving their health behavior but they pay no attention to complication test to detect acute and chronic disease early. The main factor for group classification was to prove whether they have hyperlipidemia or not. This varied widely with an individual's gender, income, age, occupation, and self rated health. To improve the rate of diabetic management, specialized diabetic management programs should be applied depending on each group's character.

The effect of restrictions on oral health-related activities of adults in Korea on quality of life: Using the 8th Korean National Health and Nutrition Examination Survey (우리나라 성인의 구강건강 관련 활동 제한이 삶의 질에 미치는 영향: 국민건강영양조사 제8기 1차년도(2019)자료 활용)

  • Background: This study aims to investigate the effect of restrictions on oral health-related activities of young adults, middle-aged, and elderly in Korea on the quality of life and contribute to the development of intervention strategies to improve the quality of life by improving the importance of oral health care and healthy lifestyle habits of adults. Methods: The 8th National Health and Nutrition Survey was used, targeting adults categorized into three age groups: young adults aged 19 to 29, middle-aged adults aged 30 to 49, and prime-aged adults aged 50 to 64. Demographic characteristics and EQ-5D, HINT-8 and oral-related toothache experience, chewing problems, speaking problems, and complaint of discomfort to chew analyzed. T-test and one-way ANOVA were performed to find out the difference in quality of life according to the restrictions on oral activities of adults, and linear regression analysis was performed to investigate the factors affecting the quality of life of adults. Results: The differences between EQ-5D and HINT-8 according to the restrictions on oral health-related activities of young, middle-aged, and prime-aged were statistically significant in all oral activity restriction variables(p ≦0.05). Factors affecting EQ-5D of all adults were statistically significant in all variables such as region, gender, household monthly income, education level, basic living status, economic activity, subjective oral health status, toothache experience, chewing problem, speaking problem, and complaint of discomfort to chew(p ≦0.05). Factors affecting HINT-8 of all adults were statistically significant in variables such as gender, household monthly income, education level, basic living status, economic activity, toothache experience, chewing, speaking, and complaint of discomfort to chew(p ≦0.05). Conclusions: Various measures are needed to improve the quality of life in old age by allowing adults to face physically, mentally, and socially prepared old age. Based on the results of this study, an adult oral health program should be developed to improve the oral health and quality of life of adults.

Diabetes Risk Analysis Model with Personalized Food Intake Preference (개인 식품섭취 선호도에 따른 당뇨병 발생 위험도 분석 모델)

  • The need of continuous management for diseases came to the fore as a chronic disease has increased, however, research related to personalized food intake analysis are insufficient. In diabetes risk analysis model of this study, food preferences are calculated by Pearson correlation coefficient that is proven method to assess the similarity, and diabetes risk is computed as a Logistic regression that was used in prevalence studies. For the Significance evaluation of this model, it was verified through t-test at 0.05 level of 52 comparison subjects and 52 control subjects. Both groups were significantly independent (p=0.046 <0.05). This model is a new way to personalized health management, through the application to healthcare system based on web and mobile.

Convergent Factors Related to Depression of Wage Workers in Korea: Focusing on Gender Differences (한국 임금근로자의 우울과 관련된 융복합적 요인: 성별 차이를 중심으로)

  • This research was conducted to investigate the prevalence of depression and convergent factors related to depression in male and female wage workers. Using data from the 2014 and 2016 Korea National Health and Nutrition Examination Survey, 3,763 adults (1,888 males, 1,875 females) between the ages of 19 and under 65 were analyzed. Depression was measured by the Patient Health Questionnaire-9 (PHQ-9), and the depression (PHQ-9≥10) of all workers was 4.1% (3.2% for men, 5.0% for women). Multiple logistic regression analysis was performed to evaluate the depressive factors of male and female wage workers. In male workers, work-related characteristics such as employment type and working hours per week were found to be significantly related to depression even after adjusting for socio-demographic characteristics, health-related characteristics, and life style characteristics. However, in female workers, work-related characteristics did not expose a significant relationship, showing a difference from male workers. Therefore it is necessary to develop and implement workplace counselling or mental health promotion programs that take into account the gender characteristics of depression risk factors. Also, as health-related characteristics (diagnosis of depression, suicide plan, perceived stress level, unmet medical services, self-rated health status) were found to be related to depression for both male and female workers, continuous management of these factors is required.
