Proposal for the Design of the Oriental Medical Doctor's Gown (한의사 가운 디자인 제안)

  • Recently the status of oriental medicine in Korea has been more increased as the alternative medicine which overcome the limitations of western medicine. With the increased status, the development of the oriental medical doctor's gown which is distinguished from that of western medical doctor has been brought up among the oriental medical doctors' group since the middle of 1990. We carried out the former study to know the development's direction of the present oriental medical doctor's gown by the survey the requisites for the oriental medical doctor's gown which they want(OK Suh, 2002). We aim to propose the developed design of the oriental medical doctor's gown which meet their request and has honored tradition according to the data of the former survey. As the result of the production designing of the oriental medical doctor's gown considering tradition, practicality, appreciation of the beautiful etc., in our study, we could lead to distribute 800 gowns through the Association of Korean Oriental Medicine supporting by the brand of Amorepacific's Sulhasoo.

A Study of Clinical Applications of Leeches (Hirudo) (수질의 임상적 활용에 대한 문헌적 고찰)

  • Traditionally, the Leech (Hirudo) has been used for the treatment of cerebral vascular diseases, cardiovascular diseases, women's diseases, etc. In Chinese medicine, usually Chinese medical doctors use Whitmania pigra Whitman among several species in their clinics. Because it was considered as toxic animal medicine according to related books, they used it in a parched form with Talcum powder, and this is the most general type for medication. Nowadays, however, many clinical doctors regard it as secure, so they tend to use it naturally in a capsule, in large quantities, and more frequently. We have no doubt that it is useful for diseases connected with blood stasis, especially including pulmonary diseases and allergic diseases, so we urge that many Korean oriental medical doctors use it practically in their clinics.

  • Development of artificial intelligence is expected to revolutionize today's medicine. In fact, medicine was one of the areas to which advances in artificial intelligence technology were first applied. Recently, state-of-the-art artificial intelligence, especially deep learning technology, has been actively utilized to treat cancer patients and analyze medical image data. Application of artificial intelligence has the potential to fundamentally change various aspects of medicine, including the role of human doctors, the clinical decision-making process, and even overall healthcare systems. Facing such fundamental changes is unavoidable, and we need to prepare to effectively integrate artificial intelligence into our medical system. We should re-define the role of human doctors, and accordingly, medical education should also be altered. In this article, we will discuss the current status of artificial intelligence in medicine and how we can prepare for such changes.

The Levels of Psychosocial Stress, Job Stress and Related Factors of Medical Doctors Practicing at Local Clinics (일부 개원의사들의 사회심리적 스트레스 및 직무 스트레스 관련 요인)

  • Objectives : This study was conducted to investigate the levels of psychosocial stress, job stress and their related factors among medical doctors practicing at local clinics. Methods : A survey using a self administered questionnaire was administered to 1,456 doctors practicing at private clinics via post for 2 months (2006. 1 - 2006. 3). Psychosocial stress, job stress, demographic factors, job related factors and health related behaviors were investigated. Among the eligible study population, the respondents were 428 doctors (29.4%). Results : The average scores of psychosocial stress and job stress were 2.19 and 3.13, respectively. The levels of psychosocial stress and job stress were statistically lower in older respondents, those who worked shorter or who were more satisfied with their job, and those with higher socioeconomic status. The level of psychosocial stress was related with smoking status, drinking status and exercise. The level of job stress was related with smoking status and exercise. In multiple linear regression analysis using psychosocial stress as a dependent variable, age, working hours per day, job satisfaction and perception on socioeconomic status were significant independent variables. In analysis using job stress as a dependent variable, age, working hours per day and job satisfaction were significant independent variables. Conclusions : Stress affects the doctor-patient relationship, productivity and overall health level of people. Therefore, it is important to manage and relieve the stress of doctors. It is suggested that more advanced studies on stress level and related factors and ways to improve the stress and health related behaviors of medical doctors should be conducted.

A Study on the Recognition and Actual Condition of Korea Medical Doctors in Oriental Medical Care of Hypertension (국내 임상한의사의 고혈압 한방치료 인식 및 실태조사)

  • Objectives : The study was accomplished to find out how korean medical doctor take oriental medical service for hypertension patients in clinical practice Methods : Survey questions were developed based on consensus of 2 clinician, 2 oriental internal medicine specialist and 1 acupuncture specialist. The list of the Korean oriental medical doctors is provided by the Association of the Korean Oriental Medicine. The questionnaire were distributed via E-mail to 9,465 members of Korean oriental medical doctors from 19th August 2008 to 11th September 2008. One thousand sixty three(11.24% of 9,465) members completed answer and the computerized data were analyzed by SAS statistical program Results : Fifty-one percent of Korean oriental medical doctors has experienced hypertension treatment. The most common medical diagnosis method was pattern diagnosis(64.2%). Saam and five element acupuncture were as frequent as 32.3% of acupuncture prescription principle. The most common acupuncture points were region of shoulder and back acupoints inclued $GB_{21}$, $GV_{14}$(25%). The most common herbs prescription principle were 'constitution prescription'(35.4%), 'pattern prescription'(12.6%). The most common opinion for the revitalization of oriental medical service were 'medical insurance system of herbal medicine'(28.8%) and 'research of oriental medical treatments in hypertensive patients'(26.8%). Conclusions : This survey provides unique insight into the perception, treatment method of the Korea medical doctor at oriental medical service for hypertension patient. This study provides a wealth of information and a fresh raft of questions which will inform future research and policy-making.

Do We Really Need More Medical Schools or Doctors in Korea? (과연 의과대학 증원이 필요한가?)

  • Recently, several social issues have raised public concerns about a deficiency of doctors. The government says it will solve the problem by increasing the number of doctors, saying essential medical service is collapsing. However, the medical community argues that there is a fundamental problem with Korea's health care delivery system. The government needs to take advice from the medical community to make policies in the right direction. And the press should no longer provide the public with misinformation that doctors and the public are opposed to. In this article, I would like to mention problems among current medical services and suggest healthcare policies for them.

  • The purpose of this paper is to infer the effects on the separation of pharmacy and clinic in the oriental medicine, the unification of the medical system and the medical and pharmaceutical system, etc. from the conflict between the oriental medicine doctors and the pharmacists in 1990s. The results are as follows. 1. The oriental medicine doctors, the pharmacists and a private organization(Citizen's Coalition for Economic Justice) found the solution in the conflict nongovernmently as the government failure. 2. The conflict brought about directly the separation of pharmacy and clinic in the western medicine but the opposite effect in the oriental medicine. 3. The conflict brought about the continuance of the dual medical system. 4. In the part of the medicine, citizen's organizations stated to play their part in the dispute. This is a positive side in the conflict. 5. The conflict contributed to democratization in the medical market and the medical world.

Validity Study of the Questionnaire for Doctors for Sasang Constituion Diagnosis (의사용 체질진단지의 타당화 연구)

  • This study is validity study of the questionnaire for doctors for Sasang constituion diagnosis. (an objects of this study : 314 patients) We analyzed the results of questionnaires about 314 patients who were diagnosed in Department of Sasang Constitutional Medicine from June to November, 2004. The questionnaire for doctors consists of objective valuation about the patients for Sasang constituion diagnosis. In the analysis about all variables, we obtain the diagnosis accuracy of 92%. In the analysis about body type variable, we obtain the diagnosis accuracy of 44-48%. In the analysis about external appearance, mental characteristics, physiology & pathologic symptoms variables, we obtain the diagnosis accuracy of 46-72%. After all, in order to increase of the diagnosis accuracy, we have to analysis total variables(body type, external appearance, mental characteristics, physiology & pathologic symptoms) for sasang constituion diagnosis. Hereafter, We need many data and standardization for sasang constituion diagnosis.

Validity of Self-reported Stroke and Myocardial Infarction in Korea: The Health Examinees (HEXA) Study

  • Objectives: Self-reported disease history is often used in epidemiological studies. In this study, we acquired the hospital records of subjects who self-reported stroke or myocardial infarction (MI) and evaluated the validity of the participants' self-reported disease history. We also determined the level of agreement between specialists and non-specialists. Methods: Among the participants in the Health Examinees study, 1488 subjects self-reported stroke or MI during 2012-2017, and medical records were acquired for the 429 subjects (28.8%) who agreed to share their medical information. Each record was independently assigned to 2 medical doctors for review. The records were classified as 'definite,' 'possible,' or 'not' stroke or MI. If the doctors did not agree, a third doctor made the final decision. The positive predictive value (PPV) of self-reporting was calculated with the doctors' review as the gold standard. Kappa statistics were used to compare the results between general doctors and neurologists or cardiologists. Results: Medical records from 208 patients with self-reported stroke and 221 patients with self-reported MI were reviewed. The PPV of self-reported disease history was 51.4% for stroke and 32.6% for MI. If cases classified as 'possible' were counted as positive diagnoses, the PPV was 59.1% for stroke and 33.5% for MI. Kappa statistics showed moderate levels of agreement between specialists and nonspecialists for both stroke and MI. Conclusions: The validity of self-reported disease was lower than expected, especially in those who reported having been diagnosed with MI. Proper consideration is needed when using these self-reported data in further studies.

A Clinical Study on the Protective Effects of Gamdu-tang on Intoxication of Organophosphate Insecticides (감두탕(甘豆湯)의 농약중독 예방효과에 대한 임상적 연구 (I) - 살충제(유기인제)를 중심으로 -)

  • Objectives: We have accomplished a clinical research about counteracting the pesticides intoxication using oriental medical method and treatment. And we observed the preventive effect of Gamdu-tang(甘豆湯) to intoxication of Organophosphorus Insecticides. Material and Methods: The subjects were 8 volunteers who were healthy and not taking medications. we observed change of cholinesterase(ChE) activity in serum of them after exposed to pesticides Results and Conclusions : In this study of the preventive effects of Gamdu-tang(甘豆湯) on the intoxication of Organophosphate Insecticides, we evaluated that the decreasing value of ChE activity after spraying insecticides of Gamdu-tang medication group was smaller than non-medication group, but it was less significant(P>0.05 P<0.1). We had a difficult to gather participant in this study, so it's necessary for us to get together more larger group in the next study.
