성장장애 한의표준임상진료지침 개발을 위한 한의사 인식조사 (A Survey on Korean Medicine Doctors' Recognition for Developing Korean Medicine Clinical Practice Guideline for Growth Disorders)

  • Objectives The purpose of this study is to provide data for development of the korean medicine (KM) clinical practice guidelines (CPG) for growth disorder (GD) by identifying the awareness and knowledge needs of KM doctors (KMD) through online survey. Methods Survey questionnaire was produced by referring to the previous recognition survey studies for clinical KMDs. The survey questionnaire was composed 18 questions regarding the current status of clinical care for GD, clinician's knowledge level about GD, and other details that clinicians use during practice. The survey was conducted from January 2021 to March 2021. An online survey was conducted on 101 KMDs from the association of pediatrics of KM. Results According to the survey, 96 respondents (40.3%) said the causation of GD without growth hormone deficiency needs to be included in CPG. 96 (23.5%) of the respondents wishes to utilize percentile in diagnosis and evaluation of the growth assessment. 24.7% of the clinicians were using the herbal medicine treatment. Currently, when treating with KM for GD, herbal medicine (100%) is the most widely used, followed by acupuncture (77.6%), and moxibustion (36.7%). In terms of a complex treatment, growth therapy efficacy (26.8%) is shown to be the most important factor to consider and needed to be included in CPG for the complex treatment, and diet (22.3%) is also considered to be important in GD. Conclusions In this study, we were able to understand the clinical KMDs' perception of GD, knowledge level, and the requirements in the CPG. The results of this study will provide the basic data for development of CPG for GD.

만성 소화불량증 환자에서 설 색상과 심박변이도의 경향성 파악 (Trends in Tongue Color and Heart Rate Variability in Chronic Dyspepsia Patients)

  • Objectives From observing the tongue of a patient, one can assess the health status; this method has been frequently used in traditional Korean Medicine (KM) clinics. In particular, KM posits that the color of the tongue is highly related to digestive functions. In this study, the color of tongue and heart rate variability (HRV) were compared between chronic dyspepsia (CD) patients and healthy subjects. Methods Healthy subjects and CD patients with functional dyspepsia (FD), gastroesophageal reflux disease (GERD), or chronic gastritis (CG) were enrolled for the study. Profile view images of the tongue were acquired by using a computerized tongue image acquisition system (CTIS). The color of the tongue body was extracted from the non-coated region on the tongue images. Results Color differences in CIE L*a*b* color space between the three sub-types of CD patients and healthy subjects were analyzed by using multiple linear regression analysis with age and sex as the factors. The variable b* was significantly lower in GERD patients than in the controls (p=0.017). Variable a* was significantly lower in CG than in the controls (p=0.03). No significant difference was seen between FD and controls. In GERD, the tongue body seems to be intense red in color; in CG, pale red. Frequency domain analysis showed that HF was significantly lower in GERD patients than in the controls (p=0.041). Conclusions The color of the tongue body and HF of HRV can be used for diagnosing digestive functions in health care.

노인 인구에 대한 이침치료와 이압요법의 국내 임상연구 동향 (Ear Acupuncture and Ear Acupressure in Elderly Population: A Scoping Review on Domestic Clinical Research)

  • Objectives: The objective of this study was to review and analyze domestic research trends of ear acupuncture and ear acupressure (EAs) in elderly population. Methods: We searched five domestic databases to collect relevant original clinical studies up to July 2, 2021. Conditions of elderly participants and results reported in included studies were analyzed. Methods of EAs used were analyzed referring to STandards for Reporting Interventions in Clinical Trials of Acupuncture. Results: A total of 17 studies including nine randomized controlled trials were analyzed. As for the target condition, sleep disorder was the most common (n=9), followed by pain condition (n=6). There were three studies on participants with specific Sasang constitution. Procedures of EAs used in nursing studies were generally similar to those of Korean medicine (KM). Conclusions: Domestic studies have reported that EAs might be helpful in improving the health of the elderly. In the future, more research studies using EAs for the elderly are needed in KM field. Collaborative research with KM nurses is recommended. EAs should be further investigated as a promising KM intervention in terms of health and welfare for the elderly.

Changes in the Laboratory Data for Cancer Patients Treated with Korean-medicine-based Inpatient Care

  • Objectives: The study aimed to determine changes in laboratory data for cancer patients receiving Korean medicine (KM) care, with a focus on patients' functional status, cancer-coagulation factors and cancer immunity. Methods: We conducted an observational study of various cancer patients in all stages admitted to the East-West Cancer Center (EWCC), Dunsan Korean Hospital of Daejeon University, from Mar. 2011 to Aug. 2011. All patients were under the center's multi-modality Korean-medicine-based inpatient cancer care program. The hospitalization stay at EWCC ranged from 9 to 34 days. A total of 80 patients were followed in their routine hematologic laboratory screenings performed before and after hospitalization. Patients were divided into three groups depending on the status of their treatment: prevention of recurrence and metastasis group, KM treatment only group, and combination of conventional and KM treatment group. The lab reports included natural killer cell count (CD16 + CD56), fibrinogen, white blood cell (WBC), lymphocytes, monocytes, neutrophil, red blood cell (RBC), hemoglobin, platelet, Erythrocyte Sedimentation Rate (ESR), and Eastern Cooperative Oncology Group (ECOG) performance status. Results: With a Focus on patients' functional status, cancer-coagulation factors and cancer immunity, emphasis was placed on the NK cell count, fibrinogen count, and ECOG scores. Data generally revealed decreased fibrinogen count, fluctuating NK cell count and decreased ECOG, meaning improved performance status in all groups. The KM treatment only group showed the largest decrease in mean fibrinogen count and the largest increase in mean NK cell count. However, the group's ECOG score showed the smallest decrease, which may be due to the concentration of late-cancer-stage patients in that particular group. Conclusions: Multi-modality KM inpatient care may have positive effect on lowering the cancer coagulation factor fibrinogen, but its correlation with the change in the NK cell count is not clear.

일상 증상 기록과 활용 방안 연구 (Study on Daily Living Symptom Record and Utilization)

  • Bian-zheng(辨證) of Korean Medicine(KM) is based on four examinations(四診) of Korean medical doctor. The interrogation or questioning(問診) provides the most information of four examinations. The symptom obtained from the interrogation or questioning is the main basis of the Bian-zheng. KM is understood in the whole state of the body of a specified time without seeing the disease exist. So the observable symptom is disease itself. Symptom in KM is used as an important basis for the diagnosis. But if the interview when memories are not sure of the correct answer does not get much easier to find exactly the symptoms. So when recording original symptom(素證) and daily subjective symptom can be helpful for care. In this paper, we propose daily living symptom record system as a method that can be applied to the health care according to the importance of collecting the symptom in the KM. Daily living symptom record system can record the symptom in the individual to awaken daily. The system stores the symptom in structure and provides an open shared services. So it can be used as a symptom of other systems, such as PHR, EMR, CDSS. In addition, Doctor may be able to help in the treatment determined by reference to shared symptom.

알레르기 비염 환자의 의과·한의과 의료 이용 현황 분석 : 2010-2016년 청구 데이터 분석 (Analysis of Medical Care Utilization of Allergic Rhinitis Patients in Western Medicine and Korean Medicine between 2010 and 2016 : A Study of the Health Insurance Review and Assessment Service National Patients Sample Database)

  • Objectives : Allergic rhinitis(AR) is a common chronic disease that accounts for 10-40% of the world's population. This study aims to analyze the status of claims, prevalence, and medical utilization of allergic rhinitis patients using representative patients sample data. Methods : This study used the National Patients Sample(HIRA-NPS) of the Health Insurance Review and Assessment Service for 7 years(2010-2016). And we defined AR patients as all statements including J30 or a subcategory of J30 as the main disease, using the Korean Standard Classification of Diseases(KCD-7). The trend of AR patients by year was divided into Western medicine(WM) and Korean medicine(KM), and analyzed by subgroup analysis such as inpatient/outpatient, gender, age, insurance type, and care institution. Results : Patients with AR were mainly claimed for first sub diagnosis in WM and major diagnosis in KM, and the number of claims increased about 1.3 times and 1.4 times compared to 2010 in WM and KM, respectively. In addition, the total annual medical expenses in 2016 increased 1.3 times and 1.7 times compared to 2010, respectively. Conclusions : Both WM and KM are showing a steadily increasing trend in medical use due to allergic rhinitis. Further research is needed by considering genetic and environmental factors and individual characteristics, and linking with additional data.

Tolosa-Hunt Syndrome으로 인한 통증성 안구운동 장애 치험 1례 (A Case of Painful Ophthalmoplegia Patient Diagnosed as Tolosa-Hunt Syndrome)

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(KM) on a patient with Tolosa-Hunt syndrome. ■ Methods We treated a 58 years old male patien t with Tolosa-Hunt syndrome who had several clinical symptoms such as ocular pain, limitaion of ocular movement and unilateral ptosis. We used herbal medication, acupuncture, electro-acupuncture(EA), cupping, moxibustion, bee-venom injection. Then we evaluated the improvement by Numerical Rating scale(NRS), Distance of eye opening and ratio of pupil movement. ■ Results We demonstrated the decrease of pain score and improvement of equal ocular movement(EOM) and ptosis after the KM treatment. ■ Conclusion This study proved the effect of KM treatment on Tolosa-Hunt syndrome. It could cooperative treatment would be more effective than mono treatment of western medicine.

수부냉증 진단을 위한 조갑모세혈관 혈류속도 측정 예비연구 (The Role of Nail-fold Capillary Blood Velocity in Evaluating Cold Hypersensitivity in the Hands: A Pilot Study)

  • This study aims to compare nail-fold capillary blood velocity (CBV) according to the presence or absence of cold hypersensitivity in the hands (CHH) using nail-fold capillaroscopy under cold stress conditions. Eleven participants were taken from the cohort study for observing long-term health status of the faculty of Semyung university. Seven of the participants displayed CHH while four showed signs of non-CHH. CBV between 0 and 60 seconds (S1) and between 240 and 300 seconds (S2) was measured under cold stress for 5 minutes, respectively, and the average value was used to compare non-CHH group and CHH group. Body mass index, Nogung (PC8) temperature and temperature difference between Nogung (PC8) and Hyeopbaek (LU4) were significantly lower in CHH group compared to non-CHH group. The S2-S1 CBV difference was negatively correlated with PC8 temperature and PC8-LU4 temperature, and positively correlated with cold pattern score and cold hypersensitivity visual analog scale. The CHH group had a lower CBV overall than non-CHH group, and the S2-S1 CBV difference was significantly higher in the CHH group than in non-CHH group. This study suggests that CBV measured by nail-fold capillarosopy might be a useful indicator of cold hypersensitivity properties.

Use of Herbal Decoction and Pharmacopuncture in Individuals with Chronic Disease: findings from a nationally representative panel

  • Objectives: This study analyzed the Korea Health Panel Annual Data 2019 to investigate factors related to the use of non-insured Korean medicine (KM) treatment in individuals with chronic diseases. The non-insured KM treatments of interest were herbal decoction (HD) and pharmacopuncture (PA). Methods: Among adults aged 19 or older, 6,159 individuals with chronic diseases who received outpatient KM treatment at least once in 2019 were included. They were divided into three groups according to the KM treatment used: (1) basic insured KM non-pharmacological treatment (BT) group (n = 629); (2) HD group (n = 256); (3) PA group (n = 184). Logistic regression analysis was used to explore factors associated with favoring HD or PA use over BT. Potentially relevant candidate factors were classified using the Andersen Behavior Model. Results: Compared to BT, the 1st to 3rd quartiles of income compared to the 4th quartile (odds ratio: 1.50 to 2.06 for HD; 2.03 to 2.83 for PA), health insurance subscribers compared to medical aid (odds ratio: 2.51; 13.43), and presence of musculoskeletal diseases (odds ratio: 1.66; 1.91) were significantly positively associated with HD and PA use. Moreover, the presence of cardiovascular disease (odds ratio: 1.46) and neuropsychiatric disease (odds ratio: 1.97) were also significantly positively associated with HD use. Conclusion: The presence of some chronic diseases, especially musculoskeletal diseases, was significantly positively associated with HD and PA use, while low economic status was significantly negatively associated with HD and PA use, indicating the potential existence of unmet medical needs in this population. Since chronic diseases impose a considerable health burden, the results of this study can be used for reference for future health insurance coverage policies in South Korea.

한의학의 음양론적 인체관과 음양개념의 은유적 이해 (Understanding the Yin-Yang Doctrine of Korean Medicine As a Metaphor)

  • In Korean Medicine (KM), the Yin-Yang doctrine is still used as a theoretical tool for understanding and explaining the clinical experiences. However, as the traditional culture declined in East Asia and the scientific culture took over, there was an increased negative view on the Yin-Yang doctrine, and thus a heightened distrust over KM. For KM to survive in an unfamiliar culture of science, a novel outlook on the Yin-Yang doctrine is needed. In this sense, I consider a thoroughly medical take on the Yin-Yang doctrine to be most important. The focus needs to be on the goals of medicine: this includes riddance of any discourses on Yin-Yang that cannot contribute to the goals, and an enhancement of the Yin-Yang concept as a rational and scientific terminology. One way to achieve this is by understanding Yin-Yang as a type of metaphor. The Yin-Yang doctrine that is utilized in KM corresponds well to the conceptual metaphor suggested by Lakoff and Johnson. As a metaphor, the Yin-Yang concept plays a role in structuring the target domain, that is life phenomena, metaphorically. Through the Yin-Yang metaphors, the life phenomena are understood as the Yin-Yang phenomena, and are systematically organized by the subcategories contained in the Yin-Yang doctrine. Understanding Yin-Yang as a metaphor is a good way to enhance the Yin-Yang concept and doctrine as a rational terminology and method.
