An Example of Systematic Searching for Guidelines to Prevent Catheter-associated Urinary Tract Infections - Part I: Using the PubMed Database (유치도뇨관 감염예방 가이드라인에 관한 체계적 문헌검색 사례 - Part I: PubMed 검색데이터베이스 이용)

  • Purpose: Effective literature searching is essential to support evidence-based nursing. The aim of this study was to present our recent systematic search experience to identify guidelines in PubMed for prevention of catheter-associated urinary tract infections. Methods: Five approaches to the literature search via PubMed were employed. The searches were restricted to items published from 1980 to 2010, for patients admitted to hospital, and in the English language. The search results were compared using the number of records and relevant items, and the sensitivity and precision of each search strategy. Results: The individual approaches retrieved 19-141 of records and 3-6 of relevant items. Sensitivity ranged from 37.5% to 75.0% with the highest values for simple searches and a search combining MeSH terms and free textwords with a methodological search filter. Precision varied from 4.3% to 21.7% and the highest precision was found for MeSH terms with limits feature. Conclusion: The simple search in PubMed is an appropriate way for nurses in a busy clinical practice to search the literature for evidence. However, several approaches using MeSH terms, free textwords, limits feature or methodological search filters are also required to have more efficient and better informed search results.

  • Recent reseach in the field of medical information systems has paid much attention to an ontology based medical terminology system to support clinical study and effective information search. This study aims to conduct research for further application or construction of ontology systems in Korea. This research reviews the theory of concept modeling and ontology. and analyses 4 cases of conceptual modeling of medical tenrminologies by ontology, The findings ot this study display these specific characteristics in medscal ontologies: (UHe standardization of terrrinology on MeSH. (2) The conceptualization of tenninology on UMLS. (l) ane. (2) are showed as untormal ontologies. (3) The theory of ontology integration in ON9, (4) The reference model of medical knowledge with formalization in GALEN. (3) and (4) are showed as formal ontologies. The application and construction of ontology should be differentiated according to the level of the proposed system, and then this analysis will provide useful information for the researcher and developer of the system.

A Study on Automatic Recommendation of Keywords for Sub-Classification of National Science and Technology Standard Classification System Using AttentionMesh (AttentionMesh를 활용한 국가과학기술표준분류체계 소분류 키워드 자동추천에 관한 연구)

  • The purpose of this study is to transform the sub-categorization terms of the National Science and Technology Standards Classification System into technical keywords by applying a machine learning algorithm. For this purpose, AttentionMeSH was used as a learning algorithm suitable for topic word recommendation. For source data, four-year research status files from 2017 to 2020, refined by the Korea Institute of Science and Technology Planning and Evaluation, were used. For learning, four attributes that well express the research content were used: task name, research goal, research abstract, and expected effect. As a result, it was confirmed that the result of MiF 0.6377 was derived when the threshold was 0.5. In order to utilize machine learning in actual work in the future and to secure technical keywords, it is expected that it will be necessary to establish a term management system and secure data of various attributes.

A Comparative Analysis of Research Trends in Korean Modern Medicine: Focusing on Two Journals of Medical School (근대의학 논문의 계량학적 방법을 통한 연구 경향 비교 분석 - 의학전문학교 학술지 2종을 중심으로 -)

  • This study aimed to analyze the research trends of journal articles published by medical schools representing Korean modern. A total of 682 were selected from two journals published by Medical College in Keijo and Keijo Imperial University Medical Faculty. In results, the affiliations of authors who participated in Acta Medicinalia in Keijo included various schools and hospitals, and the authors' major was found to be similar in basic medicine and clinical medicine. In The Keijo Journal of Medicine, only school-affiliated authors participated, and 96.33% of the authors were majors in basic medicine. Co-occurrence network analysis was conducted on MeSH terms from the title of the article using MeSH on Demand, and the keyword that derived in both journals was 'erythrocytes', which analyzed the condition of red blood cells according to organs and diseases. In frequency analysis, a common area of research in both journals was the study focusing on blood and blood cells, and the study of anemia and tuberculosis, which were prevalent diseases at the time. As for comparing each journal, Acta Medicinalia in Keijo has focused on inflammatory diseases and clinical pathological studies in humans, and The Keijo Journal of Medicine has focused on anatomical studies on animals and pharmacological studies on medicines. Through this study, it was possible to identify the research topics and major keywords in two medical schools with different founding goals.

Gastric Cancer and Angiogenesis: Is VEGF a Useful Biomarker to Assess Progression and Remission?

  • Gastric cancer (GC) has high mortality owing to its aggressive nature. Tumor angiogenesis plays an essential role in the growth, invasion, and metastatic spread of GC. The aim of this work was to review the angiogenic biomarkers related to the behavior of GC, documented in the literature. A search of the PubMed database was conducted with the MeSH terms: "Stomach neoplasms/blood [MeSH] or stomach neoplasms/blood supply [MeSH] and angiogenic proteins/blood [Major]". A total of 30 articles were initially collected, and 4 were subsequently excluded. Among the 26 articles collected, 16 examined the role of vascular endothelial growth factor (VEGF), 4 studied endostatin, 3 investigated angiopoietin (Ang)-2, 2 studied the Ang-like protein 2 (ANGTPL2), and 1 each examined interleukin (IL)-12, IL-8, and hypoxia inducible factor. Regarding VEGF, 6 articles concluded that the protein was related to lymph node metastasis or distant metastases. Five articles concluded that VEGF levels were elevated in the presence of GC and decreased following tumor regression, suggesting that VEGF levels could be a predictor of recurrence. Four articles concluded that high VEGF levels were correlated with poor prognosis and lower survival rates. Ang-2 and ANGTPL2 were elevated in GC and associated with more aggressive disease. Endostatin was associated with intestinal GC. VEGF is the most extensively studied angiogenic factor. It is associated with the presence of neoplastic disease and lymph node metastasis. It appears to be a good biomarker for disease progression and remission, but not for diagnosis. The data regarding other biomarkers are inconclusive.

Research Trends in the Journal of the PNF and Movement ('PNF and Movement'의 연구 동향)

  • Purpose: This study investigates research trends in the Proprioceptive Neuromuscular Facilitation (PNF) and Movement journal. Methods: This study analyzes the frequency of keywords and their coincidences with medical subject headings (MeSH) over 15 years in 315 papers from volume 1, issue 1 to volume 15, issue 3 of a journal published by the Korean Proprioceptive Neuromuscular Facilitation Association. The research types and levels are also analyzed, and the journals are classified by subject, diagnosis, application of PNF, and technique used when PNF is applied. All of the variables are classified in five-year units and their trends are examined. Results: A total of 315 papers were published in 40 issues, and 1190 keywords were used over 15 years. The most frequently used keyword was "PNF." For the keywords that coincided with the MeSH, there were 235 (19.74%) complete coincidence words, 167 (14.03%) incomplete coincidence words, and 788 (66.21%) complete incoincidence words. Thus, the number of complete incoincidence words was the largest. For research types, there were 196 (61.90%) experimental studies, which was the most studied research type. For research levels, there were 155 (49.21%) Level 3 studies (non-randomized trial), which was the research level with the largest number of papers. Normal people were the most common subjects (121 cases, 38.41%), and the number of papers that did not use PNF was 187 (59.37%), which was larger than those that used PNF. The most frequently used combination technique was isotonics when PNF was used. Conclusion: Basic data on PNF-related research was obtained by analyzing papers published over the past 15 years. This information can be used to suggest future directions for PNF research.

A Study on the Organizing Web Materials in the Field of Medicine (의학 분야 웹 자료의 분류에 대한 개선 방안 연구)

  • There are lots of Web materials in the field of medicine and many search engines classify the medical materials on the Web through directories. But the organization of these directories are wanting in consistency and systematization. In order for manager of search engines to organize medical materials on the Web systematically, this paper suggests several guidelines. NLMC, a special classification system for medicine, need to be applied to develop directories of medicine in search engines. Also, items of the directories should be arranged based on the relevance of subjects among subfields of medical science. For classifying an item to several directories repeatedly, clear criteria should be established. In addition to, controlled vocabularies or glossaries for medicine such as MeSH and the English-Korean, Korean-English Medical Terminology Collection should be used for selection of the name of items in medical directories.

Effect of MeOH Extract of Cibotium barometz for Repair and Regeneration of Nogo A-injuried Neuroblastoma Cells (구척(狗脊) 메탄올추출액이 신경세포의 재생 및 회복효과에 미치는 영향)

  • The effect of MeOH extract of Cibotium barometz (or Cibaro) on nogo-A expression was studied by neurite cone collapse and neurite outgrowth assay. The degrees of mRNA expression of BDNF, GDNF, and Caspase-3 in nogo-A were also examined with SK-N-SH cell lines using RT-PCR and confocal microscopy methods. We have shown that Cibaro treatment inhibits nogo-A activation in SK-N-SH cell lines. It has been shown that Cibaro increases the expression rates of neurofilament and enhances neurite outgrowth in neuroblastoma cells as increasing the amount of Cibaro. It has been also shown that Cibaro increases the expression rates of BDNF, GDNF mRNA in neuroblastoma cells as increasing the amount of Cibaro. These results suggest that Cibaro induces neutrite outgrowth by nogo-A inactivation and is, therefore, crucial for the treatments against anaplastic disc and spinal neuronal anesthesia.

A study on the Structure of the Subject Headings (주제명표목의 구조에 관한 연구)

  • The study confirmed that the methods for expanding headings and combined structures among the headings, subdivisions, qualifiers and inverted terms were diverse, and that the headings used for similar purposes do not have identical structural forms. It is also found that the standard symbols representing generic relationship in theasurus, which are introduced in the list of subject headings, do not show precisely the hiearchical relationship of phrase headings. MeSH in pre-coordinated system will be available for retrieval in the computer environment because it can make a link between headings and subdivisions in searching stage. The changes in retrieval circumstances and the seeking behaviors of the users demand that a new structure of subject headings be developed. It was proposed that the modification of the structure in the Hangul subject headings be essential.

Identification of differentially expressed Genes by methyl mercury in neuroblastoma cell line using SSH

  • Methylmercury (MeHg), one of the heavy metal compound, can cause severe damage to the central nervous system in humans. Many reports have contributed MeHg poisoning to contaminated foods and release into the environment. Despite many studies on the pathogenesis of MeHg-induced central neuropathy, no useful mechanism of toxicity has been established. To find genes differentially expressed by MeHg in neuronal cell, we peformed forward and reverse suppression subtractive hybridization (SSH) method on mRNA derived from neuroblastoma cell line, SH-SY5Y treated with solvent (DMSO) and 6.25 uM (IC$\sub$50/) MeHg. Differentially expressed CDNA clones were sequenced and the mRNAs were re-examined on Northern blots. These sequences were identified by BLAST homology search to known genes or expressed sequence tags (ESTs). Analysis of these sequences has provided an insight into the biological effects of MeHg in the pathogenesis of neurodegenerative disease and a possibility to develop more efficient and exact monitoring system of heavy metals as common environmental pollutants.
