국내와 대만의 전통약제제 생산.허가.보험등재 관련 제도 (A Comparative Analysis of the Systems Related to the Production, Authorization, and Listing for Insurance of Herbal Medicine Products in South Korea and Taiwan)

  • Objectives: Systems related to the production, authorization, and listing for insurance of herbal medicine products were compared between South Korea and Taiwan to illuminate herbal medicine products system issues in South Korea. Methods: Papers, and laws and policies related to the production, authorization, and listing for insurance of herbal medicine products in South Korea and Taiwan are analyzed to create the primary documents. The documents from South Korea were screened with the advice of a specialist, while those from Taiwan have been verified through local investigation and with the help of a related specialist. The screened documents were then compared and analyzed in the order of the systems related to the production, authorization, and listing for insurance of herbal medicine products. Results: The systems related to the production of herbal medicine products satisfy GMP requirements in both countries, while Taiwan has more specialized systems related to the production of herbal medicine products and a more strict authorization program as compared to South Korea. While South Korea has most of the herbal medicine products classified as non-prescription drugs, Taiwan has them as prescription drugs. And while South Korea does not allow new herbal medicine products to be listed for insurance, Taiwan allows for once-a-year application toward listing for insurance. Conclusions: In order to ensure the safe and effective use of herbal medicine products, systems related to the production, authorization, and listing for insurance of herbal medicine products are to be established, while the categorization of medicine products principally used by Korean medicine doctors should be prepared. Furthermore, prescription by a Korean medicine doctor for new drugs made with natural products and their listing for insurance need to be encouraged.

한약의 방사선 부작용 억제효과에 관한 경향 분석 (Study for Research Trends on Radioprotective Effects of Herbs)

  • Cancer is already a well-recognized main cause of mortality and the incidence of cancer is increasing steadily. Because conventional treatment modalities for cancer accompanies severe side effects, traditional medicine has been considered as alternatives to reduce the adverse effects and its use has continued to rise in cancer therapy. This study aims to summarize and make a reference of radioprotective effects of herbs worldwide. In this process, this review surveyed all papers of radioprotective-focused studies using herbal medicine in PubMed database and finally 44 papers were included. The type of materials, formation of experiments, type of herbal medicine, their action and mechanisms, and type of cancer were analyzed. The number of studies on radioprotective effects of herbal medicine has increased since 2000. The main formation of experiments was clinical study and the portion was 45% and the proportion of the research using prescriptions was 51% and the research using herbal products was 25%. Herbs and prescriptions having the effects of tonifying and nourishment were used the most. Most of herbal medicine in this study can enhance immune function, increase anti-oxidant effect, regulate cell cycle and increase sensitivity to radiotherapy. This study will provide the useful information on development of herbal medicine having radioprotective effects.

Hypothesis Proposal about Predictive Factors and Optimal Age for Response to Herbal Medicine Treatment for Height Gain in Children: a Retrospective Review

  • Introduction: We aimed to investigate the predictive factors and optimal age for response to herbal medicine treatment for height gain in children. Methods: This retrospective chart review included 61 children (age range, 5-16 years) treated for height gain between 2011 and 2015. A predictive model was established by multiple linear regression analysis. Dependent variables were defined by the differences in percentile before and after herbal medicine treatment. The optimal cutoff value of patient age was determined by receiver operating curve analysis. Results : The age of initiation of herbal medicine therapy (p = 0.012) and administration of Forsythiae fructus (p = 0.002) were significant variables for treatment response. The adjusted R2 value was 0.231. The mean ages of the responder and non-responder groups were significantly different (p = 0.023). The optimal cutoff value of age for predicting treatment response was 9.75 years. Treatment response was better among children below 9.75 years of age. Conclusions: Patient age and administration of Forsythiae fructus were identified as determinants of response to herbal medicine treatment. Treatment of rhinitis and initiation of height gain treatment at an early age are critical for better response. These findings will provide fundamental data for further research.

간 각질 세포 및 마우스 비장 세포를 이용한 방풍통성산의 항알러지 효능 연구 (In vitro Anti-allergic Effects of Bangpungtongseong-san in Human Keratinocytes and Primary Mouse Splenocytes)

  • Bangpungtongseong-san(BPTSS, Fangfengtongsheng-san in Chineses) is a traditional herbal formula comprising 18 medicinal herbs. In the present study, we performed the simultaneous analysis for four compounds of BPTSS and examined anti-allergic effects in human keratinocytes and mouse splenocytes. The column for separation of four compounds was used Luna C18 column and maintained at 40℃. The mobile phase for gradient elution consisted of two solvent systems. The analysis was carried out at a flow rate of 1.0 mL/min with PDA detection at 254 and 280 nm. To evaluate production and expression of Th2 chemokines, ELISA and RT-PCR were conducted in tumor necrosis factor (TNF)-α and interferon (IFN)-γ-stimulated HaCaT cells with or without BPTSS or silymarin, a positive control for skin inflammation. To measure Th2 cytokines, primary mouse splenocytes were treated with BPTSS and performed ELISA for interleukin (IL)-4, 5, 13. Calibration curves were acquired with r2>0.9999. The contents of geniposide, liquiritin, baicalin, and glycyrrhizin in BPTSS were 5.06 ㎎/g, 7.33 ㎎/g, 27.56 ㎎/g, and 7.81 ㎎/g, respectively. BPTSS reduced TARC and RANTES production and mRNA expression in TNF-α and IFN-γ-treated HaCaT cells. BPTSS inhibited IL-4, 5, and 13 production in mouse splenocytes. Our data will be a helpful information to upgrade quality control and anti-allergic effects of BPTSS.

한약의 류마티스 관절염 동물 모델에 대한 치료효과: 국내 및 국외 논문에 대한 체계적 문헌 고찰 (The Effect of Herbal Medicine on Rheumatoid Arthritis Animal Model: A systematic Review of Animal Experiment Studies Published in Korea and Overseas)

  • Objectives This study was designed to review the effect of herbal medicine on animal model with rheumatoid arthritis. Methods We reviewed studies published both in Korea and overseas, all published after 2008. We investigated 8 databases, 6 Korean databases and 2 of abroad. Searching was conducted through flowchart and 24 studies met our guideline. Datas were extrated and organized as animal model characteristics, experimental period, study design, herbal medicine used as intervention and rheumatoid arthritis indicator. Results 336 articles were primarily selected. After screening, 24 articles were left in the end. All 24 studies showed that the herbal medicine used in the animal experiment was effective on treating rheumatoid arthris, proven by macroscopic, histological, hematological and osteological indicators. Conclusions The result showed that herbal medicine is effective in treating rheumatoid arthritis through various pathways. However, study design and intervention were all different from each other and we could not specify the certain compound, material or substance of efficacy. Further study should be conducted to figure out the effectiveness of herbal medicine on rheumatoid arthritis.

면역학적 이해를 통한 건선의 한약 치료 고찰 (A Study on Herbal Medicine Treatment of Psoriasis through Immunological Understanding)

  • Objectives : The purpose of this review is to investigate the effect of herbal medicine treatment on cytokines and symptom improvement in patients with psoriasis. Methods : We searched four web databases for studies on psoriasis treated with herbal medicines published from 2000 to April 2023. After searching for studies, we analyzed selected studies based on inclusion criteria. Results : Seven randomized controlled trials have been found and analyzed. All of research which selected have reported that herbal medicine treatment for psoriasis patients have an effect on cytokines and was effective in improving symptoms, and all were statistically significant. Conclusions : According to the results, herbal medicine treatment can be an effective treatment for improving symptoms and cytokines in patients with psoriasis. This study will be helpful for future research in herbal medicine.

체외 수정과 한약 치료 중재에 대한 임상연구 동향 - Pubmed를 중심으로 - (Clinical Research of the Effect of Traditional Herbal Medicine with In Vitro Fertilization and Embryo Transfer in Pubmed)

  • Objectives: The purpose of this systematic review is to assess the effects of herbal medicine with in vitro fertilization-embryo transfer (IVF-ET) based on the current evidence. Methods: Eligible randomized controlled trials (RCT) searched from Pubmed which compared a combination of herbal medicine and IVF with IVF alone were included. Results: Sixteen trials, in which 2025 women involved, were included in this review. The review results showed that the effect of traditional herbal medicine can improve the clinical pregnancy rate (herbal medicine intervention: 30.36~63.64%, Control: 9.38~47.5%) and rate of high quality oocytes and embryos. The mechanism may be through regulating follicular fluid to improve microenvironment for oocytes which would lead to a successful embryo implantation. Conclusions: This analysis showed that combination of IVF and traditional herbal medicine used in the included trials improve clinical pregnancy rate and IVF success. During in vitro fertilization, TCM can regulate the microenvironment in the follicular fluid to mature the oocyte, improve the quality of the embryo, and help the development and implantation of the embryo. Further large randomized placebo controlled trials are needed to confirm the effectiveness of traditional herbal medicine with concurrent IVF.

항염증 효능을 가진 한약물에 관한 고찰 - 시험관내 실험을 중심으로 - (Research of Traditional Herbal Medicines for Anti-inflammatory Effects - Focusing on in vitro experiments -)

  • This study aims to summarize and makes a reference of anti-inflammatory activities of herbal medicines. In this process, this review collated papers of anti-inflammation-focused studies using herbal medicines in Oriental medical journals since 2003. Finally 221 papers were included and the type of materials, the type and effective classification of herbal medicines, the type of cells used in the experiments and the action and mechanisms of herbal medicine were analysed. The herbal medicines having the effects of decreasing fire and tonifying and nourishment were used the most. Most herbal medicines in this study can decrease proinflammatory cytokines, NO and prostaglandin 2 (PGE 2) production, inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) expression by regulating of nuclear factor kappa B (NF-${\kappa}B$) and/or mitogen activated protein kinases (MAPKs).

EBM 기반구축을 위한 사물탕 연구 문헌 분석 (Analysis of Studies on Samul-tang for Fundamental Establishment of Evidence Based Medicine)

  • To establish the fundament for EBM of Traditional Korean Medicine, the papers on Samul-tang which was frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international literatures. The papers were classified by the registration of domestic or international journals, by the year of publishment, by experimental methods, by laboratory animals used in biological experiment and by the kinds of studies on biological efficacy. Of total 67 papers on Samul-tang, 58 volumes were registered in domestic journals and 9 volumes were in international journals of which 8 volumes were in SCI journals. Since 1978, publishments of papers have continuously increased. The papers on instrumental analyses were 6, biological studies were 58 volumes, clinical studies were 3. Instrumental analyses were preceeded with standard compounds(gallic acid, albiflorin, paeoniflorin, benzoic acid, ferulic acid, 5-HMF). And biological studies showed improvement of cardiovascular function and circulation, antianemia, brain protection, immunoregulation, antistress, radioprotection, antifatigue, antiinflammation and antiallergy, antioxidative effect. Through clinical studies, antifatigue, improvement of insomnia and osteoporosis were reported. Samul-tang could be used to tonify and activate blood. And further study on clinical field need to be conducted in accordance with biological study.

한약 추출물이 실험적 관절염 모델에 미치는 영향에 대한 연구 분석 (Review of the Studies on the Influence of Experimental Arthritis Model of Herbal Medicine Extract)

  • Objectives The aim of this study was to analyze the herbal prescription and composition of the leading experimental studies of herbal medicine extract in arthritis model and to analyze the way of setting model and the way of validating effect of the leading experimental studies of herbal medicine extract in arthritis model. Methods The Subjects were selected by select criteria and exclusion criteria from January 1, 2000 to September 30, 2017. The herbal prescription and composition of the selected subjects were analyzed. The effects and statistical significance of selected studies were verified by comparing the control and treatment groups of frequently used cytokines (tumor necrosis factor $[TNF]-{\alpha}$, interleukin $[IL]-1{\beta}$, IL-6). Results 52 kinds of herbal prescriptions and 118 kinds of individual herbs were used in 64 studies. Among the 52 kinds of herbal prescriptions, Daeganghwal-tang (4 times) was used most frequently. Among the 118 kinds of individual herbs, Glycyrrhizae Radix (49 times), Angelicae Giantis Radix (34 times), Osterici Radix (24 times) were used most frequently in order. The prescription that has the highest $TNF-{\alpha}$ decline is Changchooldoin-tang. The prescription that has the highest $IL-1{\beta}$ decline is Banggibokryeong-tang. The prescription that has the highest IL-6 decline is Gwanjeol8ho-bang. Conclusions Daeganghwal-tang (herbal prescription) and Glycyrrhizae Radix, Angelicae Giantis Radix, Osterici Radix (individual herbs) were used most frequently. The prescriptions that have the highest cytokines decline were Changchooldoin-tang, Banggibokryeong-tang and Gwanjeol8ho-bang.
