Background: Diagnosis procedure combination (DPC) has recently been introduced in Korea as a demonstration project and it has aimed the improvement of accuracy in bundled payment instead of Diagnosis related group (DRG). The purpose of this study is to investigate that the model of end-stage liver disease (MELD) score as the severity classification of liver diseases is adequate for improving reimbursement of DPC. Methods: The subjects of this study were 329 patients of liver disease (Korean DRG ver. 3.2 H603) who had discharged from National Health Insurance Corporation Ilsan Hospital which is target hospital of DPC demonstration project, between January 1, 2007 and July 31, 2010. We tested the cost differences by severity classifications which were DRG severity classification and clinical severity classification-MELD score. We used a multiple regression model to find the impacts of severity on total medical cost controlling for demographic factor and characteristics of medical services. The within group homogeneity of cost were measured by calculating the coefficient of variation and extremal quotient. Results: This study investigates the relationship between medical costs and other variables especially severity classifications of liver disease. Length of stay has strong effect on medical costs and other characteristics of patients or episode also effect on medical costs. MELD score for severity classification explained the variation of costs more than DRG severity classification. Conclusion: The accuracy of DRG based payment might be improved by using various clinical data collected by clinical situations but it should have objectivity with considering availability. Adequate compensation for severity should be considered mainly in DRG based payment. Disease specific severity classification would be an alternative like MELD score for liver diseases.
All Deficiency section (諸虛門) in Euibangyoochui (醫方類聚), fills volumes 143-154 of Euibangyoochui's 266 volumes. In it, diverse and meaningful concepts such as Consumptive Disease (虛勞) and Fatigue Due to Overexertion (勞倦) are introduced to describe deficiency-oriented diseases. Before the 10th century, there are classified into six categories: muscle exhaustion, pulse exhaustion, flush exhaustion, Qi exhaustion, essence exhaustion, bone exhaustion, and prescribing treatments for treating diseases corresponding to each exhaustion. As medical knowledge was integrated through the medical books compiled by the government, awareness of Consumptive Disease was advanced to clarify the concept of pathways. The Confucian doctors have led to changes in recognition of the cause of the Consumptive Disease from damage of human factors to an internal problem. It can be seen that the classification of hurdles has become more diverse just before the outbreak of Euibangyoochui and that they include various diseases.

Metabolomics is the systematic identification and quantification of all metabolites in an organism or biological sample. NMR has been used as a major application tool in plant metabolomics such as quality control, chemotaxonomy, and analysis of genetically modified plants. Herbal medicines are the important therapeutics and are used to manage common diseases such as cold, inflammation, pain, heart diseases, liver cirrhosis, diabetes and central nerve system diseases. Herb plants include various kinds of species such as geranium, mint, and thyme and so on and contain different kinds of metabolites. We performed NMR-based metabolomics study on the seven different species of herb plants using $^1H$ NMR experiments and OPLS-DA to understand the correlation between the classification of herb plants and their metabolite contents. This study showed clear metabolic discrimination among various herb plants. This metabolmics study found several diagnostic NMR signals which are able to be used as bio-markers for identification of the specific herb plants among various species. Clear metabolic discrimination of herb plants suggests three chemotaxonomic groups of herb species.

Purpose: The current Classification of Periodontal and Peri-Implant Diseases and Conditions, published and disseminated in 2018, involves some difficulties and causes diagnostic conflicts due to its criteria, especially for inexperienced clinicians. The aim of this study was to design a decision system based on machine learning algorithms by using clinical measurements and radiographic images in order to determine and facilitate the staging and grading of periodontitis. Methods: In the first part of this study, machine learning models were created using the Python programming language based on clinical data from 144 individuals who presented to the Department of Periodontology, Faculty of Dentistry, Süleyman Demirel University. In the second part, panoramic radiographic images were processed and classification was carried out with deep learning algorithms. Results: Using clinical data, the accuracy of staging with the tree algorithm reached 97.2%, while the random forest and k-nearest neighbor algorithms reached 98.6% accuracy. The best staging accuracy for processing panoramic radiographic images was provided by a hybrid network model algorithm combining the proposed ResNet50 architecture and the support vector machine algorithm. For this, the images were preprocessed, and high success was obtained, with a classification accuracy of 88.2% for staging. However, in general, it was observed that the radiographic images provided a low level of success, in terms of accuracy, for modeling the grading of periodontitis. Conclusions: The machine learning-based decision system presented herein can facilitate periodontal diagnoses despite its current limitations. Further studies are planned to optimize the algorithm and improve the results.
Plant diseases and pests affect the growth of various plants, so it is very important to identify pests at an early stage. Although many machine learning (ML) models have already been used for the inspection and classification of plant pests, advances in deep learning (DL), a subset of machine learning, have led to many advances in this field of research. In this study, disease and pest inspection of abnormal crops and maturity classification were performed for normal crops using YOLOX detector and MobileNet classifier. Through this method, various plant pest features can be effectively extracted. For the experiment, image datasets of various resolutions related to strawberries, peppers, and tomatoes were prepared and used for plant pest classification. According to the experimental results, it was confirmed that the average test accuracy was 84% and the maturity classification accuracy was 83.91% in images with complex background conditions. This model was able to effectively detect 6 diseases of 3 plants and classify the maturity of each plant in natural conditions.

The purposes of this article are understanding the meaning of yin deficiency interpreted with a perspective of Traditional Korean Medicine and a modern perspective a study and assigning modern diseases to yin deficiency pattern types. Clinical papers were searched in China National Knowledge Infrastructure(CNKI) from 1995 to 2013. Results are as follows. First, yin deficiency written in the "Neijing" has been understood in many ways. It is translated such as deficiency of yin qi, inner qi, essence, cubit pulse, yin meridians qi, viscera yin and kidney. Second, yin deficiency pattern are related with disorders of the endocrine system, immunity, energy metabolism, blood circulation, cytokine, microelements, lipid metabolism and capability of getting rid of oxygen free radicals. Third, from pattern types, diverse diseases classified in types involving the heat from yin deficiency, which reflects pathologic conditions of deficiency heat which is distinct characteristics of yin deficiency pattern. Various diseases classified in types related with liver or kidney are reported, which reflects two viscera are more related with yin deficiency than other viscera. Fourth, levels of pattern types surveyed are more specific than Korean Standard Classification of Diseases(KCD) and specific enough to be applied clinically. This article surveyed the categories of modern diseases yin deficiency pattern types is assigned to but the detailed relation between them will be necessary to be studied in the future.

This article is a study on to which categories of modern diseases qi deficiency pattern types are assigned by reference to modern clinical papers to analyze and understand modern diseases with the perspective of Korean Medicine. Clinical papers were searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI) from 1994 to 2013. Conclusions are as follows. First, qi deficiency pattern types are roughly classified as qi deficiency pattern, qi-yin dual deficiency pattern and qi deficiency pattern related with viscera and bowels. Second, there are many patterns combined with static blood, qi stagnation, phlegm, dampness, heat, toxin, water or fluid deficiency and the level of pattern designation is more specific than pattern types in Korean Standard Classification of Diseases(KCD), which makes the pattern types more useful to clinical application. Third, static blood due to qi deficiency is the most frequent combined pattern and diseases related with blood circulation such as angina, atherosclerosis, hyperlipidemia and chronic obstructive pulmonary disease(COPD) were reported on that pattern. The detailed relation between modern diseases and pattern types can be an another topic.

Donguibogam, Korea's cultural heritage and traditional Oriental medicine book listed in the World Heritage three years ago, is indeed great in its importance and value. Korea's posterity has sought treatment methods through the medical book and their researches on that book have continued. However, it is rare to make the concrete analysis on the medicines for prescriptions recorded in the book. In this study, we reviewed "Ears" in the Section "Bodily Appearance" among the five chapters of Donguibogam, and analyzed the 55 types of prescriptions for ear diseases, the medical characteristics, tastes, prime classification of medical herbs, and acupunctural effects, and the dosage forms depending on prescriptions. For insect-entering ear diseases and 20 kinds of a single-medicine prescription also, the treatments are presented in the book, but we presented the texts only and excluded them in the analysis. We also excluded overlapping prescriptions and adding and reducing prescriptions of the 55 prescriptions, and thus classified the left 42 proscriptions by dosage form. As a result: 1. With respect to medical characteristics, the medicine with warm characteristics reached 9 types(40.9%), the highest rate, followed by the medicine with cold characteristics 8 types (36.4%). 2. Concerning medical tastes, the medicine with bitter taste included 12 types (54.5%), the medicine with sour taste 10 types (45.5%), and the medicine with sweet taste 7 types (31.8%). It was unusual that of the 22 medicines used for multipurpose, no medicine with the taste was found. 3. For the acupunctural effects, the medicine with acupuncture effects for spleen medirian reached 12 types in total (54.5%). On the contrary, the medicine with the effects for pericardium meridian reached only one (4.5%) among the total of 22 medicines. 4. In the dosage form of 42 prescriptions, powder medicine was 15 types (35.7%), and decoction medicine was 14 (33.3%). Powder, dedoction, and pill medicines accounted for 90.4% of the total medicine type. It was found that in particular, for ear diseases powder medicine was used more frequently than decoction medicine.

This article is a study on to which categories of modern diseases blood deficiency pattern types are assigned by reference to modern clinical papers. Clinical papers were searched in China National Knowledge Infrastructure(CNKI) from 1994 to 2013. Results are as follows. First, diverse diseases classified in qi-blood depletion pattern and pattern of blood deficiency and wind-dryness are reported and pattern types designated by the name of viscera are the minority. Second, among pattern types in Korean Standard Classification of Diseases(KCD), diseases classified in heart blood deficiency pattern, liver blood deficiency pattern and heart-liver blood deficiency pattern are a few. Third, the level of designation by the combined patterns such as qi deficiency, fluid deficiency, yin deficiency, kidney deficiency, essence deficiency, wind-cold, cold-dampness, dampness-heat, liver hyperactivity, liver depression and static blood is more specific than KCD, which makes pattern types more useful to clinical application. The detailed relation between modern diseases and pattern types can be an another topic.
This study statistically analyzed the laboratory accidents by investigating 806 laboratory accident survey reports which were officially submitted to government from 2013 to June 2017. After comparing domestic and foreign accident classification criteria, the laboratory accidents were classified using KCD7(Korean Standard Classification of Diseases) and OIICS(Occupational Injury and Illness Classification System) criteria. For the type and part of injury, KCD7 classification criteria was adopted. And, for the cause and occurrence type of accidents, OIICS was adopted to analyze the laboratory accidents. Most of injuries happened to the wrist and hand caused by sharp materials or chemical materials. The analysis of accident cause showed that accidents resulted in medical practice and accidents from handtools and chemical materials such as acid and alkali frequently occurred. The major occurrence types of laboratory accidents was body exposure to the chemical materials such as hydrochloric acid and sulfuric acid. In addition, the accidents resulted in destroy of grasped object or falling object were frequently reported.
