무릎손상 엘리트 배구선수에 관절가동운동이 무릎통증, 등속성 근력, 근긴장도, 근경직 개선에 미치는 효과 (Effect of joint mobilization on improvement of knee pain, isokinetic strength, muscle tone, muscle stiffness in an elite volleyball player with knee injury)

  • 본 연구는 무릎손상 엘리트 배구선수에 관절가동운동이 무릎통증, 등속성근력, 근긴장도, 근경직에 미치는 효과를 확인한 사례연구이다. 사례자는 왼쪽 무릎관절에 연골결손과 수술경험이 있고 평소 지속적인 통증과 운동 시 통증을 호소하였다. 연구기간은 2015년 8월 5일부터 12월 24일이며, 왼쪽 무릎관절에 관절가동술을 1일 1회 15분씩 총 8회 적용하였다. 연구의 측정은 시각적사상척도를 이용하여 무릎통증, 등속성 근력측정장비를 사용하여 넙다리네갈래근과 넙다리뒤근육 동심성 최대회전력, 근긴장도측정장비를 사용하여 손상쪽에 넙다리곧은근, 안쪽넓은근, 가쪽넓은근의 근긴장도와 근경직을 중재 전과 중재 후 실시하였다. 연구 결과, 단기간 관절가동운동이 손상 쪽 무릎통증 감소, 양쪽 넙다리네갈래근과 넙다리뒤근육 동심성 최대회전력의 증가, 손상 쪽 넙다리네갈래근의 근경직 증가에 긍정적인 효과가 있었다. 특히 손상 쪽 넙다리네갈래근의 경우 관절가동운동 횟수가 증가할수록 동심성 최대회전력이 크게 증가되어 손상 쪽과 정상 쪽 다리의 동심성 최대회전력의 차이가 크게 감소되었다. 하지만 손상 쪽 넙다리뒤근육/넙다리네갈래근의 근력 비율의 개선에는 효과가 없었다. 본 연구를 통해 엘리트 배구선수에 무릎통증, 등속성 근력, 근경직 개선을 위해 단기간에 관절가동운동이 효과적인 중재방법이 될 수 있지만 적절한 근력비율을 위해 트레이닝을 함께 실시할 필요가 있겠다.

복직근 유리피판 거상 후 합병된 대퇴 신경손상 1례 (Femoral Nerve Injury after Rectus Abdominis Muscle Slap Harvesting: A Case Report)

  • Purpose: The Rectus abdominis muscle free flap is utilized in various reconstruction surgeries due to easiness in harvesting, consistency of vascular pedicle and reduced donor site morbidity. But rarely, femoral nerve injury during rectus abdominis harvesting can be resulted. We report a case of femoral nerve injury after rectus muscle harvesting and discuss the injury mechanism with the follow-up process of this injury. Methods: To reconstruct the defect of middle cranial base after wide excision of cystic adenocarcinoma of the external ear, rectus muscle free flap was havested in usual manner. To achieve a long vessel, inferior epigastric artery was dissected to the dividing portion of femoral artery and cut. Results: One week after the surgery, the patient noted sensory decrease in the lower leg, weakness in muscle strength, and disabilities in extension of the knee joint resulting in immobilization. EMG and NCV results showed no response on stimulation of the femoral nerve of the left leg, due to the defects in femoral nerve superior to the inguinal ligament. With routine neurologic evaluations and physical therapy, on the 75th day after the operation, the patient showed improvement in pain, sensation and muscle strength, and was able to move with walking frame. In 6 months after the operation, recovery of the muscle strength of the knee joint was observed with normal flexion and extension movements. Conclusion: Rarely, during dissection of the inferior epigastric artery, injuries to the femoral nerve can be resulted, probably due to excessive traction or pressure from the blade of the traction device. Therefore, femoral nerve injury can be prevented by avoiding excessive traction during surgery.

스쿼트 동작 시 수동적 발목 가동범위와 무릎 관절 운동역학적 변인 간 상관성 분석 (Analysis of correlation between passive ankle movement range and knee joint kinetic variables during squat movement)

  • 본 연구의 목적은 딥 스쿼트 동작 시 발목 관절 유연성이 무릎 관절의 운동역학적 요인들간의 관련성을 분석하는데 있었다. 본 연구는 최근 1년간 하지 근골격계 병력이 없는 성인 남성 19명과 여성 8명이 연구대상자로 참여하였다. 딥 스쿼드 시 발목 관절 유연성과 하지 관절의 운동역학적 요인들과 상관관계를 검증하기 위해 pearson의 적률상관계수(pearson's correlation coefficient)를 이용하였고(SPSS 24.0, Armonk, NY, USA), 통계적으로 유의미한 상관성을 나타낸 변인들은 단순회기분석(simple regression analysis)을 실시하였으며, 유의 수준은 .05로 설정하였다. 본 연구를 통해 발목 관절 유연성과 무릎 관절의 압력을 결정하는 최대 관절모멘트와 관절반발력 요인들 간의 관련성을 확인할 수 있었다. 그러므로 근력 트레이닝 시 딥 스쿼트와 같은 무릎 관절에 많은 부하를 발생시킬 수 있는 운동을 적용할 때 개인에 신체적 특성 중 발목 관절의 유연성의 정도를 확인하는 것은 신체의 안정성과 무릎 관절의 상해 위험성을 감소시킬 수 있는 운동 강도를 설정하는데 도움이 될 수 있을 것으로 기대한다.

반원상 연골판 손상에서의 관절선 압통의 해석 (The Interpretation of Joint Line Tenderness in Meniscal Injury)

  • 목적: 슬관절 손상의 많은 빈도를 차지하고, 가장 흔하게 수술의 원인이 되는 반월상 연골판 손상을 외래에서 쉽게 일차적으로 검사하는 관절선 압통의 유용성을 알아보고자 하였다. 대상 및 방법: 2005년 5월에서 2006년 6월까지 군병원에서 동일한 시술자에게서 반월상 연골판 손상이 의심되어 수술을 시행한 76명의 환자를 대상으로 관절선 압통(joint line tenderness)에 대한 민감도, 특이도, 정확도, 양성 예측율, 음성 예측율 등을 분석하였다. 결과: 관절선 압통의 민감도, 특이도, 정확도, 양성 예측을, 음성 예측율은 순서대로 78.8%, 32.6%, 52.6%, 47.3%, 66.7%로 나타났으며 외측과 내측 및 불안정성을 동반한 환자를 대상으로 동일하게 분석하여도 상기 결과와 비슷하게 높지 않은 결과를 나타내었다. 결론: 관절선 압통 단일 검사는 외래에서 손쉽게 행해지는 간편한 검사이나, 그 단독 검사로서는 유용성이 떨어지는 것으로 보인다. 그러므로, 여러가지 이학적 검사를 병행 시행하는 것이 좀 더 높은 진단율을 나타낼 것으로 보이고, 필요시에는 자기공명영상(MRI)이나 진단적 관절경 검사도 필요할 것으로 보인다.

드롭 착지 시 스포츠 테이핑이 하지의 충격력과 근육 조율에 미치는 영향 (Effect of Sports Taping on Impact Forces and Muscle Tuning during Drop Landing)

  • The purpose of this study was to evaluate the biomechanical effect of sports taping on the lower limb during drop landing. Twelve male university students who have no musculoskeletal disorder were recruited as the subjects. Principal strain, median frequency, vertical GRF, loading rate, angular velocity and resultant joint moment were determined for each trial. For each dependent variable, paired t-test was performed to test if significant difference existed between taped and untaped conditions(p<.05). The results showed that principal strain of the thigh and the shank in taping group were significantly less than those found in control group. These indicated that sports taping may prevent excessive mechanical strain caused by impact force during the deceleration phase. Flexion(-)-extension(+) and varus(-)-valgus(+) resultant joint moment of the knee joint in taping group were greater than corresponding value for control group. It seems that extensor muscle of the knee joint were not only supported by sports taping during knee flexion but also sports taping is effective for minimizing the possibility of injury.

  • Background: This study aimed to determine the effects of complex Korean medicine treatments on a patient suffering from tetraplegia caused by spinal cord injury. Case Summary: A 64-year-old female patient diagnosed with tetraplegia was treated using acupuncture, electroacupuncture, pharmacopuncture, knee joint motion style treatment (MST), and herbal medicine. Clinical symptoms were measured using the numeric rating scale (NRS) and manual muscle test (MMT). After 67 days of treatment, the NRS score for lower back pain and knee pain decreased from 7 to 3. For the shoulder joint the MMT grade improved from 3+/4+(Rt./Lt.) to 4/5-; in the hip joint, it improved from 3-/3+ to 4/4+. No side effects were observed from the treatments used in this case report. Conclusion: The complex Korean Medicine treatments appeared to be effective in recovering muscle strength and reducing pain in patients with tetraplegia caused by spinal cord injury.

슬관절의 통증과 기능장애의 평가도구에 관한 연구 (The Research of Pain and Functional Disability Assessment Scales for Knee Joint Disease)

  • Objectives : The knee pain is the main reason of getting acupuncture treatment. In order to have confidence in the results of a study, it is necessary to establish that questionnaire is both valid and reliable in questionnaire related study. The aim of this study was to review the instruments that are currently in sue for assessing the knee joint. Methods : A literature study was performed to choose appropriate scales for assessment for pain and the function of the knee. Theoretically based scales were selected for review. Therefore, 18 scales for knee and 9 scale for pain were reviewed. the status of scales involved in knee treatment of acupuncture throughout several countries was evaluated. Results & Conclusion : Lysholm Knee Scoring Scale, Cincinnati knee rating system(CKRS) are adequate for ligament injury in knee. International Knee Documentation Committee scoring system(IKDC) may become a publication requirement for journals in view of the international standing of the committee. The available outcome measure for use in osteoarthritis are Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC), Lequesne Functional Severity Index(LFI) and Knee Injury and Osteoarthritis Outcome Score(KOOS), and in rheumatoid arthritis are McMaster-Toronto Arthritis patient function preference questionnaire(MACTAR), Quality of Life-Rheumatoid Arthritis Scale(QoL-RA Scale). Visual analog scale(VAS), verbal rating scale(VRS) are commonly used for the standard pain scale. For long term follow-up study The Medical Outcomes Study Short Form-36(SF-36), Arthritis Impact Measurement Scales(AIMS), Health Assessment Questionnaire(HAQ) should be included. Each measurement has its own composition and characteristics. Their validity, reliability, responsiveness and practical characteristics were already evaluated. We found 20 domestic and 28 overseas papers about knee treatment using acupuncture assessed with knee scales.

대퇴신경 손상 환아의 보행분석 : 사례연구 (Gait Analysis of a Pediatric-Patient with Femoral Nerve Injury : A Case Study)

  • The femoral nerve innervates the quadriceps muscles and its dermatome supplies anteromedial thigh and medial foot. Paralysis of the quadriceps muscles due to the injury of the femoral nerve results in disability of the knee joint extension and loss of sensory of the thigh. A child could walk independently even though he had injured his femoral nerve severely due to the penetrating wound in the medial thigh. We measured and analyzed his gait performance in order to find the mechanisms that enabled him to walk independently. The child was eleven-year-old boy and he could not extend his knee voluntarily at all during a month after the injury. His gait analysis was performed five times (GA1~GA5) for sixteen months. His temporal-spatial parameters were not significantly different after the GA2 or GA3 test, and significant asymmetry was not observed except the single support time in GA1 results. The Lower limb joint angles in affected side had large differences in GA1 compared with the normal normative patterns. There were little knee joint flexion and extension motion during the stance phase in GA1 The maximum ankle plantar/dorsi flexion angles and the maximum knee extension angles were different from the normal values in the sound side. Asymmetries of the joint angles were analyzed by using the peak values. Significant asymmetries were found in GA1with seven parameters (ankle: peak planter flexion angle in stance phase, range of motion; ROM, knee: peak flexion angles during both stance and swing phase, ROM, hip: peak extension angle, ROM) while only two parameters (maximum hip extension angle and ROM of hip joint) had significant differences in GA5. The mid-stance valleys were not observed in both right and left sides of vertical ground reaction force (GRF) in the GA1, GA2. The loading response peak was far larger than the terminal stance peak of vertical ground reaction curve in the affected side of the GA3, GA4, GA5. The measured joint moment curves of the GA1, GA2, GA3 had large deviations and all of kinetic results had differences with the normal patterns. EMG signals described an absence of the rectus femoris muscle activity in the GA1 and GA2 (affected side). The EMG signals were detected in the GA3 and GA4 but their patterns were not normal yet, then their normal patterns were detected in the GA5. Through these following gait analysis of a child who had selective injuries on the knee extensor muscles, we could verify the actual functions of the knee extensor muscles during gait, and we also could observe his recovery and asymmetry with quantitative data during his rehabilitation.

The Effect of Foot Landing Type on Lower-extremity Kinematics, Kinetics, and Energy Absorption during Single-leg Landing

  • Objective: The aim of this study was to examine the effect of foot landing type (forefoot vs. rearfoot landing) on kinematics, kinetics, and energy absorption of hip, knee, and ankle joints. Method: Twenty-five healthy men performed single-leg landings with two different foot landing types: forefoot and rearfoot landing. A motion-capture system equipped with eight infrared cameras and a synchronized force plate embedded in the floor was used. Three-dimensional kinematic and kinetic parameters were compared using paired two-tailed Student's t-tests at a significance level of .05. Results: On initial contact, a greater knee flexion angle was shown during rearfoot landing (p < .001), but the lower knee flexion angle was found at peak vertical ground reaction force (GRF) (p < .001). On initial contact, ankles showed plantarflexion, inversion, and external rotation during forefoot landing, while dorsiflexion, eversion, and internal rotation were shown during rearfoot landing (p < .001, all). At peak vertical GRF, the knee extension moment and ankle plantarflexion moment were lower in rearfoot landing than in forefoot landing (p = .003 and p < .001, respectively). From initial contact to peak vertical GRF, the negative work of the hip, knee, and ankle joint was significantly reduced during rearfoot landing (p < .001, all). The contribution to the total work of the ankle joint was the greatest during forefoot landing, whereas the contribution to the total work of the hip joint was the greatest during rearfoot landing. Conclusion: These results suggest that the energy absorption strategy was changed during rearfoot landing compared with forefoot landing according to lower-extremity joint kinematics and kinetics.

글라스톤 기법을 이용한 연부조직가동술과 자가근막이완술이 넙다리뒤근 유연성에 미치는 영향 (Effects of the Graston Technique and Self-myofascial Release on the Range of Motion of a Knee Joint)

  • PURPOSE: The purpose of this study was to compare the effects of Graston and self-myofascial release (SMR) techniques on knee joint flexibility, hamstring, and quadriceps strength. METHODS: Twenty subjects with hamstring shortness participated in this study. The subjects were assigned randomly to one of two groups: The Graston technique (GT) group received intervention using a Graston instrument for one minute, and the SMR group performed self-exercises using a foam roll for one minute. The range of motion (ROM) of the knee joint was measured by active knee extension test, and a handheld dynamometer was utilized to collect the hamstring and quadriceps muscle strength. This experiment was performed by two physical therapists. The significant level was set at ${\alpha}$=0.05. RESULTS: The results were as follows: 1) The ROM of the knee joint and quadriceps muscle strength were significantly increased in both groups. 2) Hamstring muscle strength was significantly reduced in both groups. 3) There were no significant differences between the GT group and SMR group for any variable. CONCLUSION: The results of this study suggest that SMR is an effective and easy technique for restoring proper muscle length and strength in subjects with hamstring shortness. We recommend that SMR technique be used for treat hamstring shortness in clinical setting and home-program.
