A meta-analysis of intervention studies on the effects of self-management in knee osteoarthritis (무릎 골관절염 대상자에게 적용한 자가관리 중재의 효과 메타분석)

  • The purpose of the this study was to provide objective evidence through a systematic review and meta-analysis regarding the effects of self-management in knee osteoarthritis. Articles published between 1999 and 2013 from periodicals indexed Ovid Medline, CINAHL, PubMed, RISS, KISS and other databases were selected, using the following key words: Osteoarthriti* OR OA, self-(management OR care OR help). A selection of 11 studies(7 randomized controlled trials and 4 quasi-experimental interventions that were used were exercise, education, massage and self-help group. As a result of mera-analysis, self-management significantly effected pain, physical function, activities of daily living, and self-efficacy. In particular, self-management showed great effect on ADL, and moderate effect on pain and physical function. Based upon these result, it is necessary to develop a standardized self-management program for knee osteoarthritis patients.

Korean Medicine Therapy to Ruptured Anterior Cruciate Ligament with Meniscal Tears: Report of 4 Cases (반월상 연골판 손상을 동반한 전방 십자인대 부분파열 환자의 한의학적 치료 효과: 증례보고)

  • The purpose of this study is to report the effectiveness of complex Korean Medicine treatment for anterior cruciate ligament (ACL) injuries with meniscus tear. Four patients were treated with complex Korean Medicine by acupuncture, pharmacopuncture and herbal medication. We evaluated the improvement of knee pain and function by Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index). After treatment, we found that knee pain was reduced and joint function was improved by NRS and WOMAC index in all cases. In the evaluation of health-related quality of life through EQ-5D index, there was no significant difference in patients with degenerative knee osteoarthritis and severe meniscal injury. This results show that complex Korean Medicine may be an effective option for ACL injuries with meniscus tear. Further clinical studies are needed to clarify the effect of Korean Medicine therapy on ACL injuries with meniscus tear.

Influence of Lower Extremity Strengthening Exercise on Sway Area of Knee Osteoarthritis (하지 근력 강화 운동이 퇴행성 슬관절염 환자의 동요 면적에 미치는 영향)

  • Purpose : The main purpose of this study was to investigate the influence of muscle strengthening exercise on balance ability of knee osteoarthritis. The subject were consisted of 30 women patients with knee osteoarthritis. Method : The strengthening exercise group with modality treatment. The strengthening exercise for 40 minutes per day and three times a week during 6 weeks period. Short Form McGill Pain Questionnaire(SFMPQ) was used to measure patient's pain level. Patient Specific Functional Scale(PSFS) was used to measure patient's functional disability level. BPM was used to measure sway area. Global Perceived Effect Scale(GPES) was used to measure recovery or worse of patient's condition. Results : 1. SFMPQ was muscle strengthening exercise showed significantly decreased more than before treatment(p<.05). 2. PSFS was muscle strengthening exercise showed significantly increased more than before treatment (p<.05). 3. Sway area was muscle strengthening exercise showed significantly limited area more than before treatment(p<.05). 4. GPES was muscle strengthening exercise showed significantly increased more than before treatment(p<.05). Conclusion : This study will be used as exercise method of patient with osteoarhtritis.

The Effect of Bee Venom Pharmacopuncture Therapy on the Condition of Different Concentration in Rheumatoid Arthritis Rat Model (흰쥐의 류마티스 관절염 모델에서 봉약침의 농도별 처리 조건에 따른 치료 효과)

  • Objectives : The aim was to study the effect of bee venom pharmacopuncture therapy with different concentration on rheumatoid arthritis rat model. Methods : We enforced a bee venom pharmacopuncture therapy with different concentration on rheumatoid arthritis rat model by the intradermal injection of chicken type II collagen emulsified. 14 days after the onset of the rheumatoid arthritis rat model, a fixed volume of bee venom was daily injected to ST-35 acupoint in the rat's knee joint for 2-3 weeks. The hind paw volume, arthritic index, arthritic flexion pain test, pain threshold, and serum analysis (CRP, $PGE_2$, ALT, AST) were analyzed, and the expression profiles of COX-2, c-fos, and substance-P at the dorsal horn region of the spinal cord and subchondral bone of the knee joint were also analyzed by using the immunohistochemistry. Results : After the treatment of rheumatoid arthritis rats with bee venom pharmacopuncture, the paw volume of edema of arthritic rats were almost restored to the level of normal group, and behavior tests were very effective. Also the evaluation on the blood serum analysis was remarkable. COX-2, c-fos, and substance-P positive cells in the immunohistological section of dorsal horn region of the spinal cord and subchondral bone of the knee joints were significantly decreased. also the bee venom pharmacopuncture was effective to alleviate their rheumatoid arthritic inflammation cytokine inhibition as regards to the behavior tests and joint histological appearance. Conclusions : Based on the results in this study, bee venom pharmacopuncture with concentrated treatment condition was very effective in low fixed quantity and progressive low increased quantity.

  • Anterior knee pain is a major problem among adolescents and young adults especially those who participates in sports. The most common pathogenesis of anterior knee pain can arise from compression and shear forces in the patellofemoral joint. It is also caused by impingement of infrapatellar fat pad. Fat pad impingement can occur when the fat pad becomes swollen and inflamed due to a direct blow or chronic irritation. As a result, the bottom tip (or inferior pole) of the patella can pinch the fat pad. One of the many causes of swollen fat pad can be secondary to anterior cruciate ligament (ACL) injury. The aim of this study was to compare the infrapatellar fat pad volume in patients with acute ACL injury and a group of age-, gender-, and activity- matched controls with intact ligament. Axial magnetic resonance (MR) images have been performed on 32 patients with torn ACL and 40 control patients. The volume of the fat pad was measured digitally from MR image by using a 3d Reconstruction software, ellipsoidal approximation, and a MATLAB code. The results were compared between patients with torn ACL and control group. Patients with a torn ACL had a significantly larger fat pad than the controls (P=0.01). There was no significant difference between the methods used to measure the infrapatellar fat pad volume (P=0.83-0.87). Thus, lesions of the infrapatellar fat pad is often associated with ACL injury.

Subjective Symptoms of Cumulative Trauma Disorder in Furniture Manufacturing Workers (가구제조업 근로자의 누적외상성장해에 대한 자각증상)

  • This study was carried out from January to March in 1996 to investigate subjective symptoms of cumulative trauma disorder (CTD). This survey was done by questionnaire of the Personal Ergonomics Profiles (PEP), which was developed by Johnson & Johnson. Number of respondents was 365 employees who were working in three furniture manufactories in Kwangjoo at Province Kyungki. The results were as follows : 1. There was no difference between clerical and labor workers in pain frequencies classified by general characteristics. However, labor workers showed statistically significant differences between working process in pain frequencies. Employees undertaking lifting work showed highest scores in frequencies of pain complaints. Comparing the number of pain occurrences, the married and the female was resulted in high score than the single and the male. There was no differences between ages and working experiences in group. 2. According to the mean frequency of body pain, the back was the most part to be complained in 1. 16 and followed by shoulder, neck, knee, wrist/hand, and forearm in order. Although, the study results showed that the workers working in furniture manufacture were not shown that they have had severe CTD, the female, the married and the the lifting workers were indicated as risk groups to CTD. Therefore, the development of backache prevention programe is necessary to manage pain control and to reduce health risks of CTD employees in their work environment.

The Immediately Effect of Narrow Squats on the Knee Joint Biomechanics During a Gait and Distance Between the Knees of Person With Genu-varum (내로우 스쿼트 운동이 내반슬 성인의 무릎 사이 거리와 보행 시 무릎 관절의 생체역학에 미치는 즉각적인 영향)

  • Background: Genu varum is also known as bow leg. It is a deformity wherein there is lateral bowing of the legs at the knee. it does give rise to pain, and persistent bowing can often give rise to discomfort in knees, hips and ankles. Objects: This study investigated the effect of narrow squats on the knee joint during a gait and distance between the knees of person with genu varum. Methods: This study analyzed 23 patient with genu varum that grade III, 12 narrow squat group and 11 genenal squat group in motion analysis laboratory. The subjects of experiment took gait before and after intervention, the range of joint motion, moment of knee joint adduction, power, distance of the knees were measured. And in order to make an analysis between groups, an paiered t-test and independent t-test was carried out. For statistical significance testing, it was decided that significance level ${\alpha}$ be .05. Results: It was shown that the group of narrow squat exercise significantly decreased in distance of knees (p<.05),In moment of adduction of knee joint, it was shown to significantly decrease in two groups (p<.05), was significantly decreased in adduction, abduction, and rotation (p<.05). In relation of peak-knee adduction moment and valgus angle, there was significant decrease in narrow squat group (p<.05). Conclusion: When the above result of study were examined, a narrow squat exercise given to the genu varum patients significantly decreased the distance between the knees, range of knee adduction and abduction, knee adduction moment, knee power. And stability gains through the decrease of excursion of knee medial part be effective for the correction of genu varum deformation.

The Effects of Therapeutic exercise with Electrical Stimulation on Pain, Range of motion, Muscle strength in patients after Anterior Cruciate Ligament Reconstruction (전기자극치료를 동반한 운동치료가 전방십자인대 재건술 환자의 통증, 관절가동범위, 근력에 미치는 영향)

  • This study was to investigate the effect of therapeutic exercise with electrical stimulation on knee joint pain, range of motion and muscle strength in patients who underwent anterior cruciate ligament reconstruction. The subjects of this study were recruited who diagnosed with anterior cruciate ligament rupture and undergoing anterior cruciate ligament reconstruction, total of 20 patients were randomly divided to the 10 control groups and 10 experimental groups. The therapeutic exercise was carried out for 3 weeks with three exercises made by referring to the previous study. The measurement tools used were knee joint pain measurements were made using the visual analogue scale, range of motion was measured using a goniometer, and the muscle strength was measured using a handheld dynamometer. In the statistical analysis, to compare about pre and post test the difference in each same groups was accomplished by using the paired t-test, and compare the difference between the different each groups was accomplished by using the independent t-test. The results of the study showed that the experimental group showed significantly enhanced results than the control group(p<.05). Based on these results, it is concluded that it is effective for the recovery of the patient if the therapeutic exercise with electrical stimulation in parallel with knee joint therapy are performed for the patients with anterior cruciate ligament reconstruction.

The Effect of Applying Various Tools to the Stiffness and Muscle Tone of Hamstring Muscles (다양한 도구의 적용이 뒤넙다리근의 뻣뻣함과 근 긴장도에 미치는 영향)

  • Purpose : The purpose of this study was to examine the effects of tools (i., extracorporeal shock wave therapy, massage gun, and foam roller) on range of motion, muscle tone and pain threshold among patients with hamstring stiffness. Methods : Fourteen participants with hamstrings stiffness were recruited. Interventions were performed 6 times, and each session was for 30 seconds using the three tools. The range of motion, muscle tone, and pain threshold were measured. The order of the use of the three tools was randomly determined. The foam roller was made to move from the bottom of the hip crease to the upper part of the back of the hamstring. Additionally, velocity 5 vibration stimulation was performed on the hamstring using a massage gun. Moreover, vibration stimulation was performed on the hamstring with extracorporeal shock wave therapy 5 minutes, 5 Hz, and 1,500 strokes. The flexibility of the posterior thigh muscle was based on maintaining the knee and hip joints in a 90 ° bend in the supine position. The joint angle of the knee was measured, when the knee was actively extended, at the maximum point where the posterior thigh muscle was stretched. The elasticity of the posterior thigh muscle was measured while the subject was prone and in a relaxed state without any force. Measurements were made at the muscle abdominal area of the semitendinosus muscle of the posterior femur, and the area to be measured was marked with a pen. The measurement of the tenderness threshold of the posterior femur was measured using a tenderness meter(Commander Algometer, J-Tech, USA). The force value at the point at which the pressure sensation change to pain was measured after applying vertical pressure to the posterior femur muscle, which was the halfway point between the ischial tuberosity and the popliteal surface of the subject lying on their stomach. Results : The extracorporeal shock wave therapy increased stiffness and, muscle tone, and caused changes in the pain threshold, whereas the other two tools had no effect on these indices. Conclusion : Extracorporeal shock wave therapy has important effects on range of motion and muscle stiffness and can be used in warmup protocols.

A Study on the Rehabilitation Exercise by Bike Pain and Injury and Fitting

  • The purpose of this study is to improve pain and injury through bicycle fitting and to propose rehabilitation methods. Pain and injuries caused by bicycles can be largely divided into knee and ankle pain, wrist, shoulder, neck, and back pain, and hip pain. The cause of these pains and injuries stems from incorrect bicycle fitting and posture. In order to improve these pain and injuries and prevent recurrence, appropriate bicycle fitting and rehabilitation exercise are needed. Pain and injury are divided into kinetic pain and pain by a fixed posture. In the case of kinetic pain, rehabilitation exercise is required along with inflammatory treatment, and pain by a fixed posture should understand the exact characteristics of the body and learn and practice bicycle posture according to the body's characteristics. Through this method, it can contribute to preventing the recurrence of bicycle pain and injuries and further creating an environment where a safe bicycle culture can be established.
