아로마 마사지요법이 퇴행성 슬관절염 노인의 통증, 수면 및 활보장에 미치는 영향 (The Effects of Aromatherapy Massage on Pain, Sleep, and Stride Length in the Elderly with Knee Osteoarthritis)

  • Purpose: The purpose of this study was to investigate the effects of aromatherapy massage in the elderly with knee osteoarthritis. Methods: Participants were assigned randomly to an intervention (n=21) group and a control (n=21) group. In order to increase a lasting effect of aromatherapy massage, an essential oil used in the form of cream. Each participant had aromatherapy massage on lower legs for 20 minutes each time twice a week for four weeks. Numerical rating scale was used for self-report of pain intensity. Sleep disturbance was measured by Korean Sleep Scale consisting of 15 items. Stride length had been obtained by measuring the distance from the heel of one foot to the heel of the other foot. Results: There was a significant difference between the pain scores of the experimental group and that of control group after the aromatherapy massage (p=.001). Whereas, no significant differences between sleep scores of the two groups (p=.592) was found. The experimental group's stride length significantly increased than those of the control group (p=.009). Conclusion: Aromatherapy massage could be recommended as an effective intervention to decease pain and to increase stride length in the elderly with knee osteoarthritis.

슬관절 전치환술 후 하지 근육강화 운동 프로그램 효과 (Effects of a Muscle Strengthening Exercise Program after Total Knee Arthroplasty)

  • Purpose: This research was conducted to compare the effects of a muscle strengthening exercise program on pain, straight leg raising ability, passive range of motion, independent walking time and satisfaction with total knee arthroplasty(TKA). Method: A nonequivalent control group posttest only design was used for this study. A total of 62 patients were randomly divided into the experimental group(31) and the control group (31). The experimental group received a 1-hour exercise intervention, while the control group received the usual care. Data was analyzed by t-test using the SPSS/WIN 11.0. Results: The experimental group showed significant improvement in pain score, straight leg raising ability, passive range of motion of knee joint, independent walking time, and the patient's satisfaction. Conclusion: The muscle strengthening exercise program was effective on decreasing pain and increasing mobility and satisfaction in patients with TKA.

만성 관절 통증 여성 노인환자에 있어 관절염 태극권의 효과에 대한 pilot study (The Effects of Tai Chi for Arthritis on Chronic Arthritic Pain of Senior Female : A Pilot Study)

  • Objectives : Tai Chi for arthritis was developed in 1997 by Paul Lam, M.D. of family medicine in Australia. It is an exercise treatment program and a good approaching method for arthritis. However its application is limited to arthritis of knee joint only. Even there have been no clinical study in korean traditional medical society. The aim of this study is to investigate effect of the Tai Chi for arthritis on chronic pain of body. Methods : We designed this trial as objectives were (1) senior female over 60 years, (2) suffering for osteoarthritis and chronic body pain, (3) have no physical or mental dysfunctions (4) able to walk and exercise alone. Objectives mactched these criteria had been trained the Tai Chi for arthritis 2 hours a day for 21 weeks. Before and after training, pain of 8 joints (neck, shoulder, elbow, wrist, waist, hip, knee, ankle) were estimated with the VAS(Visual Analog Scale). Results : Training the Tai Chi for arthritis relieved multiple joint pain(especially neck, lower back, shoulder, wrist and knee) and these were statistically significant. Conclusions : This study is a pilot study to investigate effect of the Tai Chi for arthritis on chronic pain of whole body. Absence of control group, and other scale to test joint function except VAS were limitations of this study. It is the first clinical approach about Tai Chi in korean medicine field, also the first study of Tai Chi for pain of the whole body.

양측 슬관절 전치환술 후 정맥 내 통증자가조절 시 대퇴신경차단의 병용이 미치는 영향 (The Effect of Bilateral Femoral Nerve Block Combined with Intravenous Patient-controlled Analgesia after a Bilateral Total Knee Replacement)

  • Background: Postoperative pain after bilateral total knee replacement (TKR) is expected to be more severe than unilateral TKR. Intravenous patient-controlled analgesia (IV PCA) is less effective than other methods of pain management especially immediately after an operation even though it is an easily controlled method for managing pain. This study was designed to evaluate the effect of femoral nerve blocks combined with IV PCA after bilateral TKR for postoperative pain control. Methods: The patients in group I (n = 20) were given only IV PCA with morphine and group II (n = 20) were given bilateral femoral nerve blocks with 12 ml of 0.25% bupivacaine and epinephrine 1 : 400,000 before extubation followed by an IV PCA. Main outcome measures included numerical rating pain score, cumulative opioid consumption, hourly dose during each time interval, and side effects. Results: The pain score in group II was significantly lower than that in group I immediately after recovery of awareness and at 3, 6, 12 hours postoperatively. Cumulative opioid consumption was significantly decreased in group II during the first 48 hours postoperatively. The hourly dose in group II was also significantly lower than that in group I until 12 hours postoperatively. There was no difference in side effects between the groups. Conclusions: We concluded that bilateral femoral nerve blocks improve analgesia and decrease morphine use during IV PCA after bilateral TKR.

협응이동훈련과 슬링운동 결합 프로그램이 무릎넙다리통증 환자의 무릎통증 및 기능적 움직임에 미치는 영향: 단일사례 연구 (Effect of CLT and Sling Exercise Combined Program on Knee Pain in Patellofemoral Pain Patient: A Single-Subject Study)

  • Background: Patellofemoral pain is one of the common diseases of the musculoskeletal system. Many previous studies have recommended the application of exercise therapy to patellofemoral pain patients for treatment. The purpose of this study was to investigate effect of coordinative locomotor training (CLT) and sling exercise combined program on knee pain and functional movement in patellofemoral pain patient. Methods: In this study, single-subject design (A-B-A') was conducted for 6 weeks. A repeted-measure analysis was conducted to assess results of the anterior knee pain scale (AKPS), Clarke's test (CT), eccentric step down test (ESDT). During the intervention (B), the CLT and sling exercise combined program was conducted three times a week for 4 weeks. Results: From baseline period A to intervention period B, the AKPS, CT, ESDT were improved from 61 to 48 (27%), from 8.33 to 3 (64%), from 7.67 to 3,58 (53%). From baseline period A to baseline period A', the AKPS, CT, ESDT were improved from 70.67 to 48 (47%), from 0.67 to 3 (92%), from 1.33 to 3,58 (83%). Conclusion: Based on the results of this study, we recommend the application of CLT and sling exercise combined program to improve the pain and functional movement in patellofemoral pain patients.

  • Background: The purpose of this study is to examine the correlation between high heels and body imbalance among female college students in their twenties who mainly wear high heels and prevent associated problems. Methods: The subjects included 89 female college students in their twenties. They were measured in plantar pressure with a gait analyzer. Their legs were measured in length with a tape measure. Their pain intensity and pain frequency were measured in visual analog scale and in pain rating score. Results: There were statistically positive correlations between right leg length and low back pain frequency (p<.05) and negative correlations between the left hindfoot and low back pain frequency (p<.05). There were statistically positive correlations between right leg length and knee pain frequency (p<.05) and positive correlations between the ankle pain intensity and right leg length (p<.05). Conclusion: The stronger the pain was in the ankle, the stronger and more frequent their lumber pain was. When the pressure of the left heel was lower, the frequency of lumbar pain increased.

슬관절 전치환술 환자를 위한 구조화된 운동교육 프로그램 후 한국형 슬관절 점수와 슬관절 운동범위 변화: 후향적 조사연구 (Changes in Korean Knee Score and Range of Motion after the Implementation of Structured Nursing Exercise Programs for Patients underwent Total Knee Arthroplasty: A Retrospective Study)

  • Purpose: The purpose of this study was to examine the factors affecting changes in Korean Knee Score (KKS) and ranges of motion (ROM) of the knee after the structured exercise programs for the patients with total knee arthroplasty. Methods: This was a retrospective study using electronic medical records from January 2015 to February 2017, and the subject of this study was a total of 124 out of 434 patients underwent total knee replacement operation. They took part in a structured step-by-step exercise program conducted by orthopedic nurses, and then were evaluated for KKS and Knee ROM for 12 weeks after operation. Results: Post-intervention scores increased significantly in the KKS subdomains including pain and symptoms (t=-22.31, p<.001), function (t=-20.68, p<.001), evaluation of floor life (t=-14.18, p<.001), socioemotional function (t=-28.94, p<.001) over time. As for the change in the ROM, knee extension (t=9.23, p<.001) and knee flexion (t=4.04, p<.001) showed a statistically significant changes over time. Conclusion: This study illuminated the factors affecting the changes in pain and symptom, physical function, evaluation of floor life, socioemotional function and range of motion after structured exercise training programs for knee arthritis patients.

무릎관절에서의 단계별 도수견인에 따른 견인력과 연성변이와의 관계 (Relationship between Traction Force and Elongated Displacement Under Graded Manual Traction of Knee Joint)

  • Although manual traction, one of pain therapies, was applied in clinic to relief pain, the study was rare on the manual force and displacement of ligaments at knee joint during manual traction. The aim of this study is to quantify not only manual force at knee joint but also elongated displacement of joint ligament by C-arm scanning and motion analysis. Twenty-one healthy subjects were tested with manual traction from grade I to grade III under neutral position by a physical therapist. We calculated traction force using joint farces of both hands and elongated displacement of joint ligament were measured. The results showed that traction forces by C-arm scanning analysis were averagely 1.67-fold greater than those by motion analysis, but elongated displacements were instead averagely 2.36-fold smaller than motion analysis. Finally, we could estimate relationship between traction force and elongated displacement at knee joint by two methods.

미세전류치료와 초음파치료가 슬관절전치환술 후 통증경감과 기능회복에 미치는 영향 (The Effects of Microcurrent Treatment and Ultrasound Treatment on the Pain Relief and Functional Recovery after Total Knee Replacement)

  • Purpose: The purpose of this study was to investigate pain relief and functional recovery after total knee replacement. Methods: The treatment was performed by dividing individuals into a control group ($n_1=5$), ultrasound treatment group ($n_2=5$), and micro-current treatment group ($n_3=5$). The control group applied the hot pack for 15 minutes, Transcutaneous Electrical Nerve Stimulation (TENS) for 15 minutes, and Continuous Passive Movement (CPM) for 40 minutes. The ultrasound therapy group applied the frequency of 1 MHz, intensity of 1.0 $W/cm^2$ for five minutes following the same treatment as the control group. The micro-current therapy group applied the intensity of 25 ${\mu}A$, and pulsation frequency 5 pps for 15 minutes following the same treatment as the control group. After treatment, Visual Analogue Scale (VAS), Korean Western Ontario and McMaster Universities Arthritis Index (K-WOMAC), Berg Balance Scale (BBS), Range of Movement (ROM) and wound length was measured. Results: VAS showed significant effect in the control group and micro-current therapy group during the treatment period. According to the treatment of K-WOMACK, BBS, ROM, and Healing wounds showed main effects between groups. Conclusion: According to the results of this study, data showed improvement of pain relief, wound healing effects, and range of motion recovery. Thus, these selected treatments were effective after total knee replacement. In other words, electrical treatment continues to influence pain relief and functional recovery after total knee replacement.

Neuromuscular electrical stimulation improves strength, pain and weight distribution on patients with knee instability post surgery

  • Objective: The purpose of this study was to investigate the effects of an exercise with and without neuromuscular electrical stimulation (NMES) of the quadriceps femoris muscle, on strength, pain, and weight distribution in patients with knee instability post surgery. Design: Randomized controlled trial. Methods: Twenty patients in the early stage of rehabilitation after knee surgery were recruited as subjects and were randomly divided into either experimental group (exercise combined with NMES) (n=10) or control group (n=10). Both groups received strength training of the lower limb for 20 min/day, 5 days/week for 4 weeks. The experimental group used NMES for unilateral quadriceps femoris training with incremental increases in the intensity of isometric contraction over 4 weeks. Outcome measurements were assessed using the digital manual muscle testing, 30-chair stand test (30CST), numeric pain rating scale (NPRS) and weight distribution using the foot analyzer before and after 4 weeks of training. Results: After the 4-week intervention, knee extensor strength increased significantly in the experimental group post intervention (p<0.05), and there was a significant improvement in the experimental group compared with the control group (p<0.05). The 30CST and NPRS scores improved significantly in the experimental group compared to the control group (p<0.05), and there was a significant difference between the two groups (p<0.05). Weight distribution was significantly improved in the experimental group compared with the control group, (p<0.05), but there was no significant difference in improvement between the two groups. Conclusions: This study showed that NMES combined with strengthening exercises of the lower limbs is effective in improving lower limb pain and strength in patients with instability after knee surgery.
