복직근 유리피판 거상 후 합병된 대퇴 신경손상 1례 (Femoral Nerve Injury after Rectus Abdominis Muscle Slap Harvesting: A Case Report)

  • Purpose: The Rectus abdominis muscle free flap is utilized in various reconstruction surgeries due to easiness in harvesting, consistency of vascular pedicle and reduced donor site morbidity. But rarely, femoral nerve injury during rectus abdominis harvesting can be resulted. We report a case of femoral nerve injury after rectus muscle harvesting and discuss the injury mechanism with the follow-up process of this injury. Methods: To reconstruct the defect of middle cranial base after wide excision of cystic adenocarcinoma of the external ear, rectus muscle free flap was havested in usual manner. To achieve a long vessel, inferior epigastric artery was dissected to the dividing portion of femoral artery and cut. Results: One week after the surgery, the patient noted sensory decrease in the lower leg, weakness in muscle strength, and disabilities in extension of the knee joint resulting in immobilization. EMG and NCV results showed no response on stimulation of the femoral nerve of the left leg, due to the defects in femoral nerve superior to the inguinal ligament. With routine neurologic evaluations and physical therapy, on the 75th day after the operation, the patient showed improvement in pain, sensation and muscle strength, and was able to move with walking frame. In 6 months after the operation, recovery of the muscle strength of the knee joint was observed with normal flexion and extension movements. Conclusion: Rarely, during dissection of the inferior epigastric artery, injuries to the femoral nerve can be resulted, probably due to excessive traction or pressure from the blade of the traction device. Therefore, femoral nerve injury can be prevented by avoiding excessive traction during surgery.

봉독이 Papain으로 유도된 골관절염 동물 모델에 미치는 영향 (Effects of Bee Venom on Papain-induced Osteoarthritis in an Animal Model)

  • Osteoarthritis (OA) is an age-related joint disease characterized by degradation of articular cartilage and its association with chronic pain. Purified Bee venom (PBV) has been traditionally used to the treatment for inflammatory diseases. The purpose of the current study is to examine whether PBV regulates the pro-inflammation against a mouse model of knee OA induced by papain. We studied the effect of PBV on papain-induced OA in the knee joints of mice. Mice were split into following groups: normal control, papain induced OA, OA treated with PBV, OA treated with meloxicam as positive control. Proteoglycan deposition was analyzed by safranin O-fast green staining and H&E staining. Papain injection significantly degraded the proteoglycan in OA mice at 42 days. Cartilage proteoglycan density was significantly higher in PBV treated OA group than those of the positive control groups. These results demonstrate that PBV efficiently suppresses pathological processes in an OA model. Thus, PBV could be a potential therapeutic strategy for the treatment of OA.

Resurfacing the donor sites of reverse sural artery flaps using thoracodorsal artery perforator flaps

  • Background The reverse sural artery (RSA) flap is widely used for lower extremity reconstruction. However, patients sometimes suffer from donor site complications such as scar contracture and paresthesia, resulting in dissatisfaction with the aesthetic outcomes. This study investigated the characteristics of donor site morbidity associated with RSA flaps and described our experiences of dealing with complications by performing resurfacing surgery using thoracodorsal artery perforator (TDAP) flaps. Methods From April 2008 to August 2018, a total of 11 patients underwent contracture release and resurfacing surgery using TDAP flaps due to donor morbidity associated with RSA flaps. All affected donor sites were covered with a skin graft, the most common of which was a meshed split-thickness skin graft (six cases). Results Eight of the 11 patients (72.7%) suffered from pain and discomfort due to scar contracture, and seven (63.6%) complained of a depression scar. The donor sites were located 6.3±4.1 cm below the knee joint, and their average size was 140.1 cm2. After resurfacing using TDAP flaps, significant improvements were found in the Lower Extremity Functional Scale (LEFS) scores and the active and passive ranges of motion (AROM and PROM) of the knee joint. The LEFS scores increased from 45.1 to 56.7 postoperatively (P=0.003), AROM increased from 108.2° to 118.6° (P=0.003), and PROM from 121.4° to 126.4° (P=0.021). Conclusions Planning of RSA flaps should take into account donor site morbidity. If complications occur at the donor site, resurfacing surgery using TDAP flaps achieves aesthetic and functional improvements.

다양한 만성 통증 질환에서 5% 리도카인 패치의 유용성 연구 (An Open-Label Trial of the 5% Lidocaine Patches for the Treatment of Chronic Pain)

  • Background: There have been limited reports on the effectiveness of 5% lidocaine patches (L5Ps) for treating a few types of chronic pain. We utilized L5Ps for chronic pain patients with various diagnoses and who had incompletely responded to their current treatment regimen. This study aimed at describing the results of a retrospective review of an open-label L5P trial to assess L5Ps' effectiveness and safety for treating various chronic pain patients. Methods: The chronic pain patients with pain lasting longer than 6-month duration were offered a 2-week L5P treatment trial. The patients were maintained on their other analgesic regimens. The treatment effect was measured according to the change from the baseline visual analog scale (VAS) to the week 2 VAS. After a 2-week trial, the patients were asked if they perceived pain improvement with L5Ps by using a four-item Pain Relief Scale (1 = a lot of relief, 2 = slight relief, 3 = no change, 4 = worse pain). Results: In the combined patient population (n = 177), 2-week treatment with the L5Ps significantly improved the week 2 VAS (P = 0.000). Significant improvement in the VAS was reported by the chronic pain patients with postherpetic neuralgia, intercostal neuralgia, degenerative osteoarthritis at knee joint, and other maladies. A higher proportion of the chronic pain patients reported improving their pain by the L5Ps. Seven patients experienced mild or moderate patch-related adverse events. Conclusions: The L5P provided clinically meaningful pain relief in some refractory chronic pain patients without any severe adverse events.

관절통에 관한 동물모델에서 약침에 의한 기계적 자극에 대한 관절 감각신경 활동의 억제 (Inhibition of Articular Sensory Activities to Mechanical Stimulation by Aqua-acupuncture in an Animal Model of Arthritic Pain)

  • 본 연구는 관절통 모델에서 웅담 우황과 웅담 우황 사향의 약침액의 효과를 검사하기 위해 수행되었다. 할로탄 마취하에서 관절통은 수컷 쥐의 관절강내에 $2\%$ carrageenan을 주입하여 유발시켰다. 운동자극에 대한 감각신경의 반응은 약침을 시술하기 전과 후에 기록하였다. 경혈에 주입한 약침은 유해한 운동 자극에 대한 신경의 반응을 억제시켰다 족삼리에 시술한 약침은 합곡에 비해 유해한 자극에 대한 관절 감각신경의 반응을 더 많이 억제시켰다. 이러한 결과는 웅담 우황과 웅담 우황 사향의약침이 관절통을 완화하는 데 효과적인 치료법을 제공할 수 있다는 것을 시사한다.

동종 반월상 연골 이식술 후 중장기 추시 결과 (Mid to Long - Term Results of Meniscal Allograft Transplantation)

  • 목적: 동종 반월상 연골판 이식술 후 중.장기 추시 결과를 보고하고, 예후 인자를 알아보고자 하였다. 대상 및 방법: 1999년 12월부터 2002년 9월까지 신선 동결 동종 연골판을 이식한 24명 환자, 25례를 대상으로 하였다. 외측 반월상 연골판 19례, 내측 반월상 연골판 6례였다. 평균 연령은 33.6세 (17~50세)였으며, 추시 관찰은 평균 54.8개월 (6~116개월)이었다. 술 전 방사선 사진에 mm 표시된 전.후방 사진을 얻어 반월상 연골판의 크기를 측정하였다. 외측 반월상 연골판은 열쇠 구멍 고정 방법으로(Key hole method) 고정하였으며, 내측 반월상 연골판은 양 골 조각 고정술(double bone plug) 방법으로 고정하였다. 전례에서 KASS (knee assessment scoring system)과 Lysholm knee score를 이용하여 임상적 결과를 평가하였고 Tegner activity scale을 이용하여 스포츠 회복력을 조사하였다. 결과: 모든 환자에서 술 전 증상은 호전되었다. KASS score는 술 전 평균 61.7점(34~80점)에서 술 후 평균 83.8점(61~95점)으로 호전되었으며, Lysholm knee score는 술 전 평균 77.7점(48~79점)에서 술 후 평균 87.7점(63~97점)으로 향상되었다.(우수 3례, 양호 17례, 보통 4례, 불량 1례). 통증을 동반한 부종이 2례, 감각 이상 1례, 비흡수성 봉합사에 의한 육아종이 1례 발생하였다. 총비골신경 마비가 1례 발생하였으나 술 후 6주에 회복이 되었다. 결론: 반월상 연골판 아전 또는 전 절제술을 시행한 환자에서 동종 반월상 연골판 이식술은 동통을 완화시키고, 슬관절의 기능을 향상시킨다. 술 전 정확한 반월상 연골판의 크기 측정과 술 중 견고한 고정은 좋은 임상적 결과를 위한 중요한 인자로 사료된다.

감염성 슬관절염의 관절경적 치료 이후 예후 인자에 대한 분석 (Prognostic Factors after Arthroscopic Treatment of Infectious Knee Arthritis)

  • 목적: 관절경적 수술을 시행받은 감염성 슬관절염 환자들에서 성별, 나이, 기저질환, 증상 발생 후 수술까지 걸린 기간, 발병 전 침습적 시술의 시행 여부, 관절 천자액의 세균 배양, Gachter 분류를 통한 세균 감염의 정도가 치료 예후에 미치는 영향에 대해 알아보고자 한다. 대상 및 방법: 2014년 6월부터 2016년 12월까지 감염성 슬관절염으로 관절경적 수술을 시행받은 51명의 환자를 대상으로 하였으며, 평균 추시 기간은 14.2±2.1개월(12-20개월)이었다. 남자가 27명(52.9%), 여자가 24명(47.1%)이며, 평균 나이는 55.1±17.6세(13-84세)였다. 수술 전 시행한 관절 천자검사상 백혈구 수가 50,000개 이상, 다형핵 백혈구 수의 비율이 95% 이상인 환자를 감염성 슬관절염으로 진단하고 수술을 시행하였다. 모든 환자에서 관절경적 수술과 수술 후 지속적인 관절 세척을 시행하였다. 결과: C-반응 단백의 초기 평균 수치는 9.55±6.76 mg/dl (1.51-31.06 mg/dl)에서 최종 평균 수치는 0.74±1.26 mg/dl (0.08-6.77 mg/dl)로 감소하였으며, 평균 C-반응 단백의 정상화 기간은 27.6±18.9일(8-93일)이었다. 관절경적 수술과 항생제 사용 후 발열, 동통, 부종 등의 임상증상이 호전되고 C-반응 단백의 수치가 0.5 mg/dl 이하로 감소하여 감염성 슬관절염의 완치 판정을 받은 환자는 51예 중 44예(86.3%)이며, 최종적으로 두 번 이상의 관절경적 수술을 시행한 환자가 5예, prosthesis of antibiotic-loaded acrylic cement 후 인공관절 치환술로 전환한 환자가 2예이다. 결론: 지금까지 문헌들에서 알려진 것처럼 증상 발생 후 수술까지 걸린 기간과 Gächter 분류에 따른 병기는 감염성 슬관절염의 치료 성공 여부에 유의하게 영향을 주었다. 하지만 이외의 인자들은 통계적으로 유의한 상관관계가 없었다. 단 관절 천자액에서 세균이 배양된 환자군은 대조군보다 유의하게 C-반응 단백의 정상화 기간이 짧아 치료 기간을 반영하는 것으로 보인다.

경락 전위를 이용한 경락의 생체물리학적 연구 (Study on the Electric Potentials at Biophysical Meridian System)

  • Objectives : Assuming That the Characteristic of Meridian System Has Been Similar to This of Electric Potentials in Human Body and That Measurements of Electric Potential at Well and Sea Points in Branches of the Twelve Meridians Will Be Representative of Measurements of the Twelve Meridians, to Measure the Electric Potentials in Healthy Volunteers(HG), Patients Diagnosed As a Cerebral Infarction(CG), a Arrhythmia(AG), a Other Intervertebral Disc Disorder(IG), and a Joint Disorder(JG), and Then to Find Out the Characteristic of Biophysical Meridian System, Finally to Compare with the Results of the Electric Potential in Those Groups. Methods : We Selected Who HG Were Diagnosed by a Blood Test, Urine Examination and Differentiation of Syndromes by Five Viscera among Volunteers, CG by CT and Wind-Syndrome Caused by Hyperactivity of the Liver-Yang, AG by EKG, CT, and Deficiency or the Heart Blood, IG by X-ray or CT and Pain in Waist, and JG by X-ray and Knee Pain. Their Electric Potential of Well and Sea Points in the Meridians Were Measured by hysiograph. Results and Conclusions : Measurements Were Analyzed by Factor Analysis, Analysis of Variance, and Logistic Regression Model, We Obtained That Most of the Results, In Conclusion, Hold Good for the Classical Meridian Doctrine.

A Study on Changes in Lower Limb Joint Angles during Stair Walking with High Heel

  • Purpose: The purpose of this study is to compare kinematics on lower limbs between stair walking with high heel and barefoot in healthy adult women. Methods: 18 healthy adult women were recruited in this study. The subjects performed stair ascent and descent with high heels and barefoot. The experiment was conducted in random order and repeated three times for each stair walking with high heels and barefoot. The movements of lower limb joints were measured and analyzed using a three-dimensional analysis system. Results: The ankle, knee, and hip flexion angles on the sagittal plane exhibited statistically significant differences between stair ascent and descent with high heels and barefoot. The pelvic forward tilt angles showed statistically significant differences only during stair ascent. The ankle inversion, hip abduction and pelvic lateral tilt angles on the frontal plane showed statistically significant differences between stair walking with high heels and barefoot. On the transverse plane, the hip rotation angles showed statistically significant differences between the high-heeled and barefoot gait during stair ascent and descent. However, the pelvic rotation angles showed no statistically significant differences. Conclusion: Therefore, wearing high-heeled shoes during stair walking in daily life is considered to influence lower limb kinematics due to the high heel, and thus poses the risks of pain, and low stability and joint damage caused by changes in the movement of lower limb joints.

3차원 보행 영상 기반 퇴행성 관절염 환자 분류 알고리즘 개발 (Developing Degenerative Arthritis Patient Classification Algorithm based on 3D Walking Video)

  • Degenerative arthritis is a common joint disease that affects many elderly people and is typically diagnosed through radiography. However, the need for remote diagnosis is increasing because knee pain and walking disorders caused by degenerative arthritis make face-to-face treatment difficult. This study collects three-dimensional joint coordinates in real time using Azure Kinect DK and calculates 6 gait features through visualization and one-way ANOVA verification. The random forest classifier, trained with these characteristics, classified degenerative arthritis with an accuracy of 97.52%, and the model's basis for classification was identified through classification algorithm by features. Overall, this study not only compensated for the shortcomings of existing diagnostic methods, but also constructed a high-accuracy prediction model using statistically verified gait features and provided detailed prediction results.
