Comparison of Anterior Cruciate Ligament Reconstructions Using Hamstring Tendon Autograft and Tibialis Tendon Allograft (자가 슬괵건 및 동종 경골건을 이용한 전방 십자 인대 재건술의 결과 비교)

  • Purpose: To compare the clinical and radiological results of anterior cruciate ligament(ACL) reconstruction using hamstring autograft and tibialis tendon allograft. Materials and Methods: Twenty four ACL reconstructions using hamstring autograft and 30 using tibialis anterior tendon altograft were followed up at least 1 year. We performed femoral tunnel fixation with Ligament Anchor(LA) screw and tibial tunnel fixation with biodegradable interference screw. Evaluations included Lysholm knee(LK) score, Tegner activity scale, Lachman test, Pivot-Shift test, Quardriceps atrophy, incision site numbness, anterior knee pain and instrumented anterior laxity with $Telos^{(R)}$ device. Results: Preoperativ mean LK score was $60.3(18{\sim}82)$ in autograft group and 61.2(25-80) in allograft group. Mean LK score improved to $91.6(68{\sim}100)\;and\;92.6(77{\sim}100)$ respectively. Activity level, using Tegner activity scale, slightly decreased compared with that of Preinjury state in both groups. Lachman test, pivot-shift test, Quadriceps atrophy, anterior knee pain, incision site numbness, and anterior drawer test using $Telos^{(R)}$ device showed no significant difference between two groups (p>0.05). Conclusion: In performing the ACL reconstruction, there was no statistically significant difference between hamstring autograft group and tibilis anterior allograft group in clinical or in radiological results.

The Effects of Knee Joint Taping Yherapy on ${\beta}$-endorphin and Serotonin of Pain Substances of Blood (슬관절 테이핑요법이 혈중통증물질인 ${\beta}$-endorphin과 serotonin에 미치는 영향)

  • The purpose of this investigation is comparing two groups one applied with taping therapy the other with controlled causes variation in pain substances of blood ${\beta}$-endorphin, serotonin with taping therapy. 12 male students of S college divided into two groups each 6 experimental and controlled with no history of flexion and extention of lower extremity focused on knee joint. Experimental group applied with elastic taping before experiment at quadriceps, calf muscles, hamstrings and tibialis anterior, controlled group didn't applied any taping therapy. Bruce protocol of maximal progressive loading exercise implemented 5 minutes after blood samples were extracted. And 5 minutes after the exercise blood samples also were taken and made investigation. Data before and after investigation were operated on SPSS Ver. 12.0 for Window(Kor.). P value 0.05. The affirmative effects of this investigation was proved with increased ${\beta}$-endorphin and decreased serotonin that cause reducing pain.

  • The natural history of the anterior cruciate ligament(ACL)-deficient knee remains controversial. although numerous investigation have tried to ascertain the course that the knee would follow once the ACL has lost functional integrity. An extensive review of the literature performed according to symptoms. physical examinations, associated surrounding tissue injuries. returns to activity level and radiological changes in the knee joint. An active individual with a non-functioning ACL was susceptible to injury to the menisci and deteriorate the articular cartilage, followed radiographic changes. An activity levels in general also changed after injury. The most common symptom was pain. But instability varied in these individuals. Conclusively we believe that all these factors will eventually, if not initially, result in a symptomatic knee. which will result in significant limitations to the individual's desired level. So we recommend an aggressive approach in person who desired to return to a relatively active life style in young person as well as in middle aged individuals who have significant symptomatic ACL deficient knee.

  • The purpose of this study was to investigate the health status and to examine the relationship between health related variables of the clients with arthritis. This descriptive-correlational study was measured physical health status(pain, fatigue, physical disability, joint flexibility) and psycho-social health status(social function disability, self-efficacy, depression). The clients were consisted of 127 arthritis clients who were randomly selected from health center, 2 sub health center and 6 primary health care center in Wonju City. The data was collected from November 2000 to January 2001. The results of the study analyzed using a SPSS win were as follows: 1) The subjects' average age was 64.81 years old(female: 86.6%, male: 13.4%). 2) In physical health status, the average score of fatigue was 63 scores and pain was 69.77. Physical disability score was higher than total disability score. In psychosocial disability, housekeeping management disability score was higher than social function disability score. In psycho-social health status, the average score of self-efficacy was 70.86 and the subjects have higher level of depression, more than 20 scores. 3) On pain sites, almost of subjects complained of a knee joint pain. The waist, wrist, and shoulder joint pain were followed. The numbers of pain site were 8.3 per individuals. 4) The flexibility of knee and shoulder joint were lower than another joints. 5) Correlation with fatigue, pain, physical disability, social disability, self-efficacy and depression was revealed highly significant. We suggest that health promotion program considering our research results should be provided for arthritis clients in community level.

Analysis on Biomechanical Differences Depending on Changes in Postures during Farm Work

  • Objective: This study looks into biomechanical variables occurring when one moves in a sitting posture, and presents objective references to make improvements in work environments of farm workers. Background: The farmers have more common musculoskeletal disorders compared to other professions, because they are much more exposed to biomechanical risk factors. The sitting posture that is the representative form of the squatted, can cause typical knee joint diseases, such as osteoarthritis or patellofemoral pain syndrome of the knee joint. Therefore, a quantitative study of knee load upon the movement in a squatting posture is required. Method: In order to proceed with its investigation, the study examined movements in a sitting posture with and without a lower body supporter through a threedimensional image analysis and by using Surface EMG. The study compared and analyzed the average muscle activity and the maximum muscle activity as well. Results: Every movement in a sitting posture is related to loads onto the knee joints and, when the farm workers move to sides, the study observed a high level of bowlegged moment. The study also noticed differences in muscle activity of medial gastrocnemius with and without the lower body supporter. Conclusion and Application: The study argues that what has been discussed so far is evidence to prove how the farm working environments should be improved in consideration of these movements observed when the farm workers move in a sitting posture.

Effects of Knee and Foot Position on EMG Activity and Ratio of the Vastus Medialis Oblique and Vastus Lateralis during Squat Exercise (스쿼트 운동시 자세가 안쪽빗넓은근과 가쪽넓은근의 근활성도 및 근활성비에 미치는 효과)

  • Purpose: The purpose of this study was to examine EMG activities and VMO/VL ratio of the vastus medialis oblique, vastus lateralis, and rectus femoris during squat exercise (knee angle: 15, 45, and 60 degrees; tibial rotation: internal rotation, neutral, and external rotation). Methods: Twelve subjects performed squat exercise at each knee angle and tibial rotation while electromyographic (EMG) activity was collected. Statistical analysis consisted of two-way repeated measures analysis of variance with post hoc analysis. Results: There were significant main effects of knee angles and foot positions and interaction effect on EMG activities of vastus medialis oblique and vastus lateralis. VMO/VL ratios were significantly different by tibial rotations and there was an interaction effect. A neutral position produced significantly more VMO/VL activity ratio than that from internally rotated position and externally rotated position at 60 degrees. Conclusion: Considering the interaction effects for EMG activity across quadriceps muscles tested, the 60 degrees knee angle with a neutral foot position may provide the most effective condition for patients with acute patellofemoral syndrome.

The Pain Behavior of Patients with Joint Pain (관절통환자의 통증정도와 통증연관 행위에 관한 연구)

  • The purposes of this study were : 1) to assess the level of pain and to identity the varieties and the degree of pain-related behavior, 2) to measure the level of correlation between the level of pain and the degree of pain- related behavior, 3) to test the correlation between the Korean Pain Rating Scale (KPRS) and Graphic Rating Scale(GRS), and 4) to gather data relevant to the Socio-demographic status of the subjects. The level of pain was measured by KPRS and GRS developed by the researchers. The KPRS consists of three dimensions ; the sensory, the affective and the miscellaneous and the GRS of two separate scales ; the intensity scale and the unpleasantness scale. Of the 2, 025 who had visited orthopedic and neurosurgical out-patients department of 11 university hospitals in various districts of Korea with the episode of Joint pain, 405 subjects were self-selected by responding to the data gathering tools and questionaires mailed. The results are summaried as follows : 1. Maale(217, 53.6%) exceeded female patients(188, 46.4%) in number and the onset of joint pain was more prevalent in the age groups of the 20s and the 30s. 160(39.5%) had been hospitalized for the treatment of, and 87(21.5%) had retired because of the joint pain. 2. Mean pain score measured by KPRS was 128.31 (range; 0-1.344.8) ; mean sensory score was 43.23(range ; 0-645.88%), mean affective score was 46.09(range ; 0- 356.72), and mean miscellaneous score was 39.99(range ; 0-341.68). Mean pain scores measured by GRS were ; sensory intensity score ; 109.1(range ; 0-200) and distress score ; 99.1 (range ; 0-200). 3. The prevalent sites of joint pain revealed to be the right knee joint(203; 50.1%), left knee joint(181(44.7%), left ilium(147, 36.3%), lumbar region (106; 26.2%), hip joint(92; 22.7%) and the ankle(84; 20.7%). 4. The average sleep hour was 6.8hours per day and the average rest hour during the day hours was 3.3hours (range 0-20). 5. The average duration of suffering from joint pain was 49.1 months. 6. Most of the subjects(298; 73.6%) used some sorts of pain relieving practices ; the most prevalent pain relieving practice was the compliance with the physician prescribed treatments(34.4%). 7. The level of discomfort in carrying out the ADL(activities of daily living was 101.16(38.83) and the level of needs for aid in carrying out the ADL was 76.62(31.79). 8. The interrelation between KPRS total score and GRS sensory intensity score(.4438), as well as that of GRS distress score(r=.4446) were not highly correlated, however, sensory and affective dimension within KPRS (.7547) and pain intensity and distress score of GRS(.6975) revealed moderate intercorrelation. 9. Pain-related behaviors such as discomfort in carrying out ADL, the need for aids in carrying out ADL, frequency of pain relieving practices, varieties of pain sites and length of rest hours during the day hours revealed to be highly correlated with the level of pain measured by KPRS, GRS sensory intensity scale and GRS distress scale. The following are recommended ; 1. Test for the correlation of KPRS total score and the summated score of GRS ; sensory intensity and distress scores. 2. Possibilities of utilization of the pain-related behaviors which revealed high correlation as indirect assessment tool for measuring the level of pain.

E-mail Survey for Developing Clinical Trial Protocol on Acupuncture Treatment for Knee Pain (슬통의 침구임상 진료지침 프로토콜 개발을 위한 전자우편 설문조사)

  • Objectives : This survey was done in order to find out how Korean medical doctors derive pattern identification for acupuncture prescriptions in treating knee pain in real clinical practice. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. The questionnaires were distributed via e-mail to 75 members of Korean Acupuncture & moxibustion society from March 26th to April 14th in 2009. 57 members completed answers, and the computerized data were analyzed by SPSS 17.0 statistical program. Results : 1.54 Korean medical doctors selected meridian pattern identification based on the course of the meridians(52.5%), visceral pattern identification(27.1%), pattern identification based on cause of disease(8.5%) as the most commonly used pattern identification methods for acupuncture prescription when treating knee pain patients in real clinical practice. 2. In meridian pattern identification based on the course of the meridians, liver meridian of the medial knee region(13.2%), bladder meridian of the posterior knee region(12.0%), spleen meridian of the lateral knee region(11.7%), stomach meridian of the anterior knee region(9.8%) and kidney meridian of the medial knee region(8.6%) were selected. 3. In visceral pattern identification, blood stasis of sinews due to liver and kidney deficiency(5.3%), damp joint with yang deficiency of liver and kidney(4.9%), kidney qi deficiency with congealing cold(4.5%), yin deficiency of liver and kidney(4.1%) were selected. Conclusions : In our e-mail survey, Korean medical doctors answered that Meridian Pattern Identification based on the course of the meridians is the most often used diagnosis method. Visceral pattern identification, pattern identification based on cause of disease, pattern identification based on symptom and pattern identification based on qi-blood-yin-yang theory in order of frequency used, were selected for knee pain diagnosis in real clinical practice.

  • Degenerative arthritis is characterized by progressive degenerative change, particularly change occurring to the joint due to the burden of body weight. Arthritis means inflammation occurring to the joint: it causes pain and limits exercise involving movement of the joint. Edema is not a disease but a condition which is accompanied by progression of a certain disease. Degenerative arthritis patients also have various diseases and mostly brings edema. Even though the cause is known to be abrasion of the joints, there are many cases in which walking difficulties are caused by pressure and pain due to edema. We found patients with walking difficulties caused by pain and increase of pressure in the joints walk better after treatment of both the pain and edema.

The Immediately Effect of Narrow Squats on the Knee Joint Biomechanics During a Gait and Distance Between the Knees of Person With Genu-varum (내로우 스쿼트 운동이 내반슬 성인의 무릎 사이 거리와 보행 시 무릎 관절의 생체역학에 미치는 즉각적인 영향)

  • Background: Genu varum is also known as bow leg. It is a deformity wherein there is lateral bowing of the legs at the knee. it does give rise to pain, and persistent bowing can often give rise to discomfort in knees, hips and ankles. Objects: This study investigated the effect of narrow squats on the knee joint during a gait and distance between the knees of person with genu varum. Methods: This study analyzed 23 patient with genu varum that grade III, 12 narrow squat group and 11 genenal squat group in motion analysis laboratory. The subjects of experiment took gait before and after intervention, the range of joint motion, moment of knee joint adduction, power, distance of the knees were measured. And in order to make an analysis between groups, an paiered t-test and independent t-test was carried out. For statistical significance testing, it was decided that significance level ${\alpha}$ be .05. Results: It was shown that the group of narrow squat exercise significantly decreased in distance of knees (p<.05),In moment of adduction of knee joint, it was shown to significantly decrease in two groups (p<.05), was significantly decreased in adduction, abduction, and rotation (p<.05). In relation of peak-knee adduction moment and valgus angle, there was significant decrease in narrow squat group (p<.05). Conclusion: When the above result of study were examined, a narrow squat exercise given to the genu varum patients significantly decreased the distance between the knees, range of knee adduction and abduction, knee adduction moment, knee power. And stability gains through the decrease of excursion of knee medial part be effective for the correction of genu varum deformation.
