FES 치료 시 자극 주파수 차이가 퇴행성 슬관절염 환자의 대퇴근 기능에 미치는 영향 (Effects of Different Frequency on Muscle Function of the Thigh in Patients with Degenerative Knee Arthritis during the Functional Electrical Stimulation )

  • Objective: The purpose of this study was to investigate the effects of different frequency on muscle function of the thigh in patients with degenerative knee arthritis during the functional electrical stimulation (FES). Method: For this study, 16 male participants over 65 who patients with degenerative knee arthritis were recruited as research participants. In this research, isokinetic muscular function, EMG, and joint position sensation were performed after FES treatment was applied for three conditions (FES 20, FES 50, and Without FES). For each dependent variable, one-way ANOVA with repeated measures was to determine whether there were significant differences among three different conditions (p<.05). When a significant difference was found, post hoc analyses were performed by using the contrast procedure. Results: When compared to FES 50 and without FES, FES 20 causes significant increase in isometric knee extension strength. No significant differences were found in EMG values across different EMS conditions. Conclusion: The present study examined isokinetic muscular function, EMG, and joint position sensation in order to investigate the effects of different frequency muscle function of knee extensors during the functional electrical stimulation. The results of this study showed that FES with 20 Hz frequency had positive effect on knee extensor. Based on the findings of the present study, FES with lower frequency may help the performer to focus on developing strength in knee extensor muscles.

  • The aim of this study was to analyze the characteristics of knee joint sound in frequency domain and classify the knee joint diseases. The spectral analysis of knee joint sounds was performed using LPC(Linear Predictive Coding) and Wigner-Ville distribution. Ten normal subjects and 5 patients with meniscal tearing were enrolled. Each subject was seated on a chair and underwent active knee flexion and extension for 60 seconds. Sampling frequency was 10kHz and electronic stethoscope and electro-goniometer were applied during the knee motion for data collection. The spectral analysis showed 3 peaks in both groups and the difference energy distribution in time-frequency domain. These results suggest that the diagnosis of knee joint pathology using the auscultation could be easier and more correct.

  • Objective: The purpose of this study was to investigate the effects of different frequency on of knee extensors muscle function during electrical muscle stimulation (EMS). Method: In this research, 40 subjects who have no musculoskeletal disorder, and less than a year workout experience were recruited in order to analyze effects of EMS with different stimulus frequency. Forty subjects were randomly divided into four groups of ten subjects in each group. A EMS training program with different frequencies (without EMS [WE], EMS with frequency 30 Hz [E30], EMS with frequency 60 Hz [E60], EMS with frequency 90 Hz [E90]) was assigned to each group. Throughout eight weeks of training, test subjects were simultaneously carried out knee extension exercises such as squat, leg extension, and leg-press while using EMS with different frequency (20 min, pulse width 250 ㎲, on-off ratio 1:1). Isokinetic knee extension strength, muscle activity of the rectus femoris (RF), the vastus medialis (VM), and the vastus lateralis (VL), and the median frequency of the RF, the VM, and the VL were collected and compared between pre and post training in order to find effects of applying EMS with different frequencies. For each dependent variable, a one-way ANOVA was to determine whether there were significant differences among four different conditions (p<.05). When a significant difference was found, post hoc analyses were performed using the contrast procedure. Results: When compared to WE and E90, E30 causes significant increase in isokinetic knee extension strength. No significant differences were found in EMG values across different EMS conditions. However, the median frequency of the VM in E30 was significantly increased than the corresponding value for WE. Conclusion: The results of this study showed that EMS training with 30 Hz frequency had positive effect on knee extensor. Based of the findings of the present study, EMS training with lower frequency may help the performer to focus on developing strength in knee extensor muscles.

무릎관절 각도가 발목 근육의 근전도 활동에 미치는 영향 (Influence of the Knee Angles on the Electromyographic Activites and Fatigue of the Ankle Muscles in Healthy Subjects)

  • The purpose of this study was to investigate the influence of the various knee angles and ground state on the muscular activities and fatigue of the ankle muscles by integrated electromyograms (iEMG) and median frequency of tibialis anterior (TA), peroneus longus (PL), flexor digitorum longus (FDL) and gastrocnemius (GC). Ten healthy male subjects were participated into stable and balance ball sessions at four angles of knee joint. The surface electromyograms (sEMG) were recorded from the TA, PL, FDL and GC on stable and balance ball with full weight bearing at four knee angles of $0^{\circ}$, $15^{\circ}$, $30^{\circ}$ and $45^{\circ}$. The time serial data of the surface electromyographic signals were transformed into integrated and frequency serial data by fast fourier transformation. On the stable ground, the iEMG signals of the TA, PL, FDL and GC were significantly higher at $45^{\circ}$ and $30^{\circ}$ of knee angles than $0^{\circ}$ and $15^{\circ}$ of knee flexion (p<0.05). On the balance ball, the iEMG of the TA, PL, FDL and GC were significantly higher at $45^{\circ}$ and $30^{\circ}$ of knee angles than $0^{\circ}$ and $15^{\circ}$ of knee flexion (p<0.05). The median frequency of the TA, PL, FDL and GC were significantly lower at $45^{\circ}$ and $30^{\circ}$ of knee angles than $0^{\circ}$ and $15^{\circ}$ of knee on the stable ground (p<0.05). On the balance ball, also the median frequency of the TA, PL, FDL and GC were significantly lower at $45^{\circ}$ and $30^{\circ}$ of knee angles than $0^{\circ}$ and $15^{\circ}$ of knee flexion (p<0.05). The iEMG of the TA, PL, FDL and GC were significantly higher on the balance ball at $0^{\circ}$, $15^{\circ}$, $30^{\circ}$ and $45^{\circ}$ of knee angles compared with stable ground. The median frequency of the TA, PL, FDL and GC were significantly lower on the balance ball at $0^{\circ}$, $15^{\circ}$, $30^{\circ}$ and $45^{\circ}$ of knee angles compared with stable ground. These results indicate that the ground conditions and angles of the knee joint involved to muscular activities and fatigue of ankles muscles, may performed at first on stable ground and then balance ball in order to $0^{\circ}$, $15^{\circ}$, $30^{\circ}$ and $45^{\circ}$ of knee flexion.

정상 성인 여성의 무릎관절 위치감각 (The Knee Joint Position Sense in Healthy Women)

  • Purpose : The purpose of this study is to ascertain whether age, body mass index(BMI) and exercise frequency(EF) are correlated with knee joint position sense in healthy women. Methods : Healthy women of 150 who participated in this study were tested knee joint position sense; each reposition error was analyzed according to the age, BMI and EF. Reposition error was measured with a Myrin goniometer. Results : The age groups, BMI groups and EF groups demonstrated significant differences of the knee joint position sense. The older the healthy women are, the higher BMI is, and the lower EF is, the more decreased knee joint position sense has become. Conclusion : The older the women are, the higher BMI is, and the lower EF is, the more decreased the knee joint position sense in healthy women is. Therefore it needs to be considered to require management of obesity and regular exercise for prevention of knee injuries due to decreased joint position sense.

슬관절 운동 평가를 위한 생체 임피던스 측정용 전도성 섬유 센서의 여기 주파수별 특성 평가 (Excitation Frequency Characteristics of a Conductive Fabric Sensor Using the Bio-impedance for Estimating Knee Joint Movements)

  • This study describes a conductive fabric sensor and determines an optimum excitation frequency of the sensor to evaluate knee joint movements. Subjects were composed of 15 males (age: $30.7{\pm}5.3$) with no known problems with their knee joints. The upper side of subjects' lower limbs was divided into two areas and the lower side of subjects' lower limbs was divided into three areas. The sensors were attached to 1 for 3 spot from a hip joint and to 3 for 4 spot from a knee joint which are the optimum conductive fabric sensor configuration to evaluate knee joint movements. As a result, the optimum excitation frequency for evaluating knee joint movements using conductive fabric sensors was 25 kHz. Average and standard deviation of bio-impedance changes from 15 subjects were $92.1{\pm}137.2{\Omega}$ at 25 kHz. The difference of bio-impedance changes between 25 kHz and 50 kHz was statistically significant (p<0.05) and the difference of bio-impedance changes between 25 kHz and 100 kHz was also statistically significant (p<0.001). These results showed that conductive fabric sensors are more sensitive to measure bio-impedance for evaluating knee joint movements as an excitation frequency decreases.

  • This paper describes the possibility of evaluating and classifying arthritic pathology using the acoustical analysis of knee joint sound. Six normal subjects and 11 patients with knee problems were enrolled. Patients were divided into the 1st patient group which required an orthopeadic surgery and the 2nd patient group of osteoarthritis. During sitting and standing periods, subjects' active knee flexion and extension were monitored. Fundamental frequency, mean amplitude of pitch, jitter and shimmer were analyzed according to the position. The results demonstrate that the values of fundamental frequency, jitter and shimmer of the 2nd patient group were larger than others and changed unstably. The values of the standing position were larger than the sitting position.

어깨뼈 익상에 대한 푸쉬업플러스 시 부가적 진동의 주파수와 진폭이 어깨안정근 근활성도에 미치는 영향 (The Effects of Vibration Frequency and Amplitude on Serratus Anterior Muscle Activation During Knee Push-up Plus Exercise in Individuals with Scapular Winging)

  • PURPOSE: This study was conducted to investigate the effects of vibration frequency and amplitude on scapular winging during the knee push-up plus exercise. METHODS: A total of 26 female subjects with scapular winging were evaluated while performing the knee push-up plus exercise with no vibration, low-frequency/low-amplitude (5 Hz/3 mm) vibration, low-frequency/high-amplitude (5 Hz/9 mm) vibration, high-frequency/low-amplitude (15 Hz/3 mm) vibration, and high-frequency/high-amplitude (15 Hz/9 mm) vibration. The surface EMG of the serratus anterior (SA) muscle was compared between the vibration frequency and amplitude. The EMG amplitude was normalized using the maximal voluntary isometric contraction (MVIC). The statistical significance of the results was evaluated using one-way ANOVA. RESULTS: The SA muscle EMG values increased at low-frequency/low-amplitude vibration and at low-frequency/high-amplitude vibration compared to no vibration. Furthermore, the same values increased at high-frequency/low-amplitude vibration and high-frequency/high-amplitude vibration compared to no vibration. In general, a higher vibration frequency and amplitude was associated with higher EMG values of the SA muscle, with particularly greater increases observed during high-frequency/high-amplitude vibration. There was also a significant difference between each condition with a high-frequency/high-amplitude vibration (p<.05). CONCLUSION: This study suggests that there were remarkable clinical effect of the knee push-up plus exercise with vibration, which enhanced the SA muscle activation in persons with scapular winging. Furthermore, applying a higher vibration frequency and amplitude more effectively increased for increasing SA muscle activation.

슬관절음의 각도별 피치 변동에 대한 분석 (The Pitch Perturbation of Knee Joint Sounds according to Angle movement)

  • In this study, we have evaluated and classified arthritic pathology using the auscultation of knee joint sound. Six normal persons and 11 patients with knee problem were enrolled. Six patients of Group 1 needed an orthopeadic surgery because of the ruptured wounds of meniscus or ACL(Anterior Cruciate Ligament) and 5 patients of Group 2 diagnosed as osteoarthritis. Subjects were taken knee flexion and extension being seated in a chair for 20 seconds which repeated 3 times. Also subjects stood up and sit down repeatedly in the same way. After the movement of knee was divided into 18 degrees, the pitch perturbation according to partial degrees was analyzed and the DTW(Dynamic Time Warping) method was applied for normalizing a time-axis and unpaired t-test was used for statistic results among groups. As a result, the amplitude and frequency perturbations of group 2 was higher than group 1(p<0.05) and showed a characteristic 'w-shape' in angle-amplitude graph. These results suggest that the analysis of knee joint sound might assist in early diagnosis of knee joint disease.
