비만환자의 비만감소를 위한 침술요법에 대한 국내외 논문 분석 (Analyses of stidies on acupuncture therapies for reducing obesity in obeses patients)

  • 본 연구의 목적은 비만환자의 비만을 감소시키기 위한 침술요법의 현황을 파악하고 침술요법의 내용과 효과를 파악하는 것이다. 문헌검색은 '비만', '침', '이침', '전기침' 등을 검색어로 하여 한국학술정보, 한국교육학술정보원, 국회도서관, Pubmed, CINAHL 등을 통해 2003년 1월부터 2017년 12월까지 발표된 연구논문을 검색하였다. 연구자가 원본을 확인한 후, 선정한 19편의 논문을 최종 분석 하였다. Scottish Intercollegiate Guideline Network의 체크리스트로 논문의 질적 평가를 실시한 결과 10점 만점에 9점인 연구가 1편, 10점 만점에 7-8점인 연구가 15편, 10점 만점에 6점인 연구가 2편, 10점 만점에 3점인 연구가 1편이었다. 침술요법의 내용을 파악한 결과 비만을 감소시키기 위한 경혈점은 shemen, hunger, LI-11(Quchi), LI-4(Hegu), St-25(Tianshu), St-44(Neiting) 등이었다. 침술요법 총 시행 횟수는 평균 16.9회이었고, 침술요법 중재 기간이 20일 혹은 72일인 연구에서의 침술요법 중재 빈도는 1회/1-2일이었고, 침술요법 중재 기간이 4-18주인 연구에서의 침술요법 중재빈도는 1-3회/주이었다. 분석한 연구결과에 의하면 침술요법은 신체계측수치의 감소, 혈당 및 혈중 지질 농도의 감소, 혈중호르몬 변화 등을 일으켜 비만환자의 비만을 감소시키는 효과가 있었다. 향후 비만환자의 비만감소를 위한 침술요법의 효과성을 평가하기 위해 잘 설계된 연구가 시급히 필요하다.

편두통의 침도 치료에 대한 체계적 문헌고찰 (Effectiveness of Acupotomy for Migraine: A Systematic Review)

  • Objectives : This study aims to assess the impact of acupotomy on migraine through an examination of clinical studies conducted since 2015. Methods : We conducted a comprehensive search for randomized controlled trials (RCTs) and non-randomized controlled trials (nRCTs) related to acupotomy treatment for migraine, utilizing five Korean online databases (OASIS, Science ON, DBPIA, KISS, RISS), as well as four foreign online databases (CNKI, PubMed, EMBASE, Cochrane Library). We identified a total of 10 relevant studies for analysis. Participants characteristics, treatment points, combination treatments, treatment cycles or frequencies, evaluation indices, efficacy, and adverse events were analyzed. The risk of bias in the 10 RCTs was assessed using the Revised Cochrane risk-of-bias tool for randomized trials (RoB 2.0). Results : A total of 931 participants were included in 10 studies. In the intervention group, the average duration of migraine morbidity ranged from 15.5±4.5 months to 15.9±4.2 years. Six studies based their diagnoses on the International Classification of Headache Disorders (ICHD), while five studies relied on Chinese diagnostic criteria. All studies specified the treatment area as the region exhibiting tenderness or induration on the head and neck. Treatment cycles ranged from a minimum of 2 days to a maximum of 1 week, with the number of days per treatment course varied from 5 days to 4 weeks. The diameter of acupuncture needles used varied between 0.3 mm and 1 mm. Of the eight studies specifying needle length, the shortest was 20 mm, and the longest was 40 mm. A total of eight evaluation indices were employed, with total efficacy rate (TER) and visual analogue scale (VAS) being the most frequently used. Statistically, all intervention groups showed more significant results compared to the control groups. Adverse events were reported in only two studies within the intervention group. Overall, the risk of bias assessment for the selected RCTs ranged from 'some concerns' to 'high risk of bias.' Conclusions : This study showed that acupotomy treatments for migraine were effective.

섬유근통의 통증 및 우울증상에 대한 침치료의 효과성: 체계적 문헌고찰 및 메타분석 (Effectiveness of Acupuncture for Pain and Depressive Symptoms in Fibromyalgia: Systematic Review and Meta-Analysis)

  • Objectives: To review studies evaluating effects of acupuncture on pain and depressive symptoms in fibromyalgia. Methods: Quantitative evidences (RCTs) were systematically reviewed. Literature were searched for a combination of fibromyalgia and depression (The Cochrane Central Register of Controlled Trials (CENTRAL), EMBASE, medline (via PubMed), Kmbase, KISS, ScienceON, OASIS, CiNii, CNKI). Quantitative research findings were critically appraised by Cochrane risk of bias (RoB) tool and pooled. Meta-analysis was then conducted using Review Manager (RevMan) 5.4. Results: Eighteen studies were selected. American College of Rheumatology (ACR) classification criteria for Fibromyalgia Syndrome was most frequently used as diagnostic criteria for fibromyalgia. As for outcome measurement, Hamilton Rating Scale for Depression (HAMD), Visual Analog Scale (VAS), and Total Effective Rate (TER) were used most commonly. Meta-analysis of ten studies revealed that both Depression and VAS scores of the Acupuncture+Western Medicine group were significantly lower than those of Western Medicine group (Depression: SMD, -0.94, 95% CI, -1.17 to -0.70; VAS: MD, -1.51, 95% CI, -1.83 to -1.19). Also, TERs of both Acupuncture group and Acupuncture+Western Acupuncture+Western Medicine group were significantly higher than those of the Western Medicine group (OR: 2.38, 95% CI: 1.29 to 4.41; and OR: 7.40, 95% CI: 3.41 to 16.07). There was no significant difference in Depression or VAS score between the Acupuncture Group and the Western Medicine Group. Conclusions: Acupuncture might be an effective option for pain and depressive symptoms of fibromyalgia when it is combined with Western Medicine treatment. For more accurate results, more types of Korean medicine treatment should be conducted.

거짓침을 대조군으로 사용한 국내 침 임상시험에 대한 체계적 고찰과 메타분석 (A Systematic Review and Meta-analysis of Acupuncture Trials in Republic of Korea that Used Sham Acupuncture as a Control Group)

  • Objectives : This study aimed to review randomized controlled trials of acupuncture performed in South Korea that used sham acupuncture as a control group. Methods : The following databases were searched through the end of September 2011: Koreanstudies information service system (KISS), Korean medical database (KMbase), national discovery OR science leaders (NDSL), oriental medicine advance searching integrated system (OASIS), and research information service system (RISS). The following search terms were used: acupuncture AND (sham or placebo). The reference lists of searched articles and Korea institute of oriental medicine (KIOM) reports(2005~2009) were identified. The following data were extracted: year/first author, disease, number of participants, blinding, intervention, outcome, and result. Where appropriate, we performed meta-analysis. The methodological quality was assessed according to the Jadad scale and 'risk of bias' by Cochrane Handbook procedure. Results : Twenty-nine studies were included in this review. In eighteen studies, penetrating sham controls were used as the control intervention, whereas the remaining eleven studies adopted non-penetrating sham controls such as the Park Sham Device or blunt auricular acupuncture. Nine studies showed statistically significant difference in outcomes. Twelve studies concerning insomnia after stroke, chronic tension-type headache, idiopathic Parkinson's disease, Hwa-Byung, and smoking cessation were included in meta-analysis. A meta-analysis of insomnia after stroke only found significant difference(MD -4.31, 95% Cl -6.19 to -2.42, $p$<0.00001). In general, all of the studies showed low methodological quality(Jadad score: mean 2.1). Risk of bias by Cochrane Handbook procedure varied. Conclusions : The results of this study could not suggest conclusive evidence that acupuncture is more effective than sham acupuncture in several diseases. In the future, more studies with rigorous acupuncture trials using sham controls should be conducted.

본태성 고혈압에 대한 기국지황환(杞菊地黄丸)의 효과에 관한 체계적 고찰 및 메타 분석 (Qu-Ji-Du-Huang-Wan for Essential Hypertension : Systemic Review and Meta-analysis)

  • The purpose of this study is to investigate the blood pressure reducing effect of Qi-Ju-Di-Huang-Wan (QJDHW) in adults with essential hypertension by using methods of systemic review and meta-analysis. Major search engines, such as PubMed, EMBASE, Cochrane library, Web of Science, CNKI, CiNii, J-STAGE, KISS, NDSL, RISS, OASIS, DBpia and so on, were used. The search period we used is from the start date of the search engine to October 30, 2016 and no language limits were placed. Randomized controlled trials using QJDHW in adults with essential hypertension were searched and extracted by two independent researchers. Meta-analysis was performed on outcome variables of the total effective rate (TER), systolic blood pressure (SBP) and diastolic blood pressure (DBP). Risk of bias (RoB) of Cochrane was used to assess methodological quality. Thirteen studies were finally selected. We observed that the combined treatment of QJDHW and antihypertensive drug had 3.6 times the odds ratio of TER for blood pressure lowering than a single use of an antihypertensive drug. Additionally, mean differences of SBP and DBP were -8.88 mmHg (95% Confidential Interval (CI) -12.77 mmHg, -5.00 mmHg, P<0.00001), -7.09 mmHg (95% CI -9.93, -4.25, P <0.00001), respectively. Single use of QJDHW did not reduce blood pressure more than an antihypertensive drug. All items of RoB were unclear and the methodological quality was low. Our analysis suggests that the combination of QJDHW and antihypertensive drugs may be more effective in reducing blood pressure than a single antihypertensive drug. But due to low methodological quality, careful interpretation will be needed and systematic long-term clinical trials will be required.

유헬스케어(U-health Care)에서 양도락의 활용 방안 (The Application of Ryodoraku in the U-health Care System)

  • Objectives : The purpose of this study was to propose the utilization plan of Ryodoraku in the U-health care systems. Methods : Computerized literature searches were performed for Ryodoraku related articles using the following databases: KISS, RISS, DBPIA, NDSL from 1990 to Oct 2010. Search terms were '양도락' or 'Ryodoraku' or 'U-health'. Due to Ryodoraku coming from Japan, additional literature review(articles published by 2008) on Japanese journal of Ryodoraku medicine was done for compensation. Results : 1. Introduction of U-health : As rapid progress of population aging and strong interest in health, the demand for the traditional Korean medical service is increasing. Until now healthcare service has provided post treatment by face-to-face manner. But according to related researches, proactive treatment is resulted to be more effective for preventing diseases. Particularly, the existing healthcare services have limitations in preventing and managing chronic geriatric degenrative diseases such as metabolic syndrome, CVA, coronary heart diseases, parkinson's diseases, degenerative joint disease, spondylosis, etc., because the cause of the above is complex and even related to life habit. As the advent of ubiquitous technology, patients with the chronic geriatric degenrative diseases can improve life habit such as poor eating habits and physical inactivity without the constraints of time and space through u-healthcare service. Therefor, lots of researches for u-healthcare service focus on providing the personalized healthcare service for preventing and managing that. To cope with this situations, The concept of u-healthcare service should be adopted in the traditional korean medicine and diagnostic devices suitable for it should be also devised and developed based on traditonal korean medine. 2. Review of existing Ryodoraku related articles for applying to U-health : articles investigating feasibility applying Ryodoraku to meridian diagnosis and raising problems of it, articles providing recent research trends of Ryodoraku, Ryodoraku related articles considering usefulness for U-health, and articles confirming the repeatability and reproducity of Ryodoraku were included. Based on the review of the above Ryodoraku related articles, several application of Ryodoraku in the U-health care system. Conclusions : To make preparations for the increasing need of traditional Korean medicine due to rising morbidity rate of chronic geriatric degenerative diseases, it is necessary to appropriately apply Ryodoraku to the U-health care system. The application of Ryodoraku is as follows. 1. To use Ryodoraku additionally to the established diagnostic device of metabolic syndrome, CVA, coronary heart diseases, parkinson's diseases, degenerative joint disease, spondylosis. 2. To apply Ryodoraku to the symptoms or diseases having a tendency to be diagnosed by correlation between the affected meridian and the lesion such as headache, nuchal pain, shoulder pain, low back pain, sciatica, HNP, etc. 3. To secure the repeatability and reproducity of Ryodoraku. 4. To devise and develop Ryodoraku appliance in order to overcome the known drawbacks and to improve error of measurement.

국내 수면장애 성인을 위한 비약물적 중재 효과에 대한 메타분석 (Effects of non-pharmacological interventions for adults with insomnia in Korea: A meta-analysis)

  • 본 연구는 국내 수면장애를 가진 성인을 대상으로 비약물적 중재를 실시한 논문을 메타분석하여 중재 효과의 크기를 확인하고자 실시되었다. 본 연구의 자료검색은 국가과학기술정보 통합서비스 (NDSL), 한국교육학술정보원 (RISS), 한국학술정보원 (KISS), 학술데이터베이스서비스 (DBpia)에서 이루어졌으며, 검색된 2000년부터 2016년까지의 연구 1,334편 중 16편의 연구를 선정하였다. 분석은 메타분석 R(version 3.3.1)을 활용하였으며, 랜덤효과모형을 이용하여 비약물적 중재의 효과크기를 산출하였다. 메타분석 결과 수면장애가 있는 성인에게 수행된 비약물적 중재의 효과크기 Hedges' g는 2.36 (95% CI: 1.44~3.28)로 큰 효과크기이었으며, 통계적으로 유의한 것으로 나타났다. 조절효과 분석에서 연령 집단, 중재방법, 입원유무 및 결과측정 도구에 따른 효과크기는 통계적으로 유의한 차이가 있었으나 연구설계 및 참여자 유형에 따른 효과크기는 유의한 차이가 없었다. 중재 중 이압요법이 가장 큰 효과크기를 보였고, 발반사 마사지, 아로마요법, 기타 및 웃음치료 순이었다. 연구결과는 수면증진을 위한 비약물적 중재가 수면에 효과적임을 확인하게 되었고, 실무에서 수면장애를 가진 성인을 위한 중재를 선택하고 실행하는데 객관적인 근거를 제공할 수 있을 것이다.

논문 검색을 통한 피부 부작용 사례 고찰 (A Review on Cases of Skin Adverse Reactions through the Search for Articles)

  • Objectives : The aim of this review is to investigate studies on skin adverse reactions and to demonstrate subjects related to the adverse effects in dermatology. Methods : Electric searches were performed with KISS(Korean studies Information Search System) and the key words were combination of 'skin' and 'adverse effect'. 87 literatures investigated in this review were issued from 1900 to 2016. Results : Among the 87 papers, dermatologic adverse reactions were reported in 83 papers in medicine, accounting for 95.4%. Of the adverse effects seen on the skin, 84 discoloration such as erythema, pigmentation and hemotelangiosis were the most common, accounting for 21%. Among the medical adverse reactions not seen on the skin, 21 infection were the most common, accounting for 25%. Among the subjective adverse reactions, of which 32 pruritus were the most common, accounting for 43%. Among the 87 papers, there were 3 cases with underlying diabetes and 3 cases with underlying hypertension, followed by 2 cases with chronic renal failure, HBV, atopic dermatitis and respectively 1 case with alcoholism, depression addiction, multiple myeloma, arthritis and psoriasis. The most frequent period until adverse reactions appeared was within 2 weeks, accounting for 13 papers. And 4 were the most frequent adverse reactions lasting less than 1 month, and 4 were more than 3 months and less than 6 months. There were 48 cases where adverse reactions were caused by nonmedical practioner's treatment. The adverse reactions by the pharmacist were the highest at 11 cases (23%). There were 17 cases of adverse reactions due to medical treatment, among which dermatologists and nondermatologists accounted for the majority of 5 cases, 29%. The most common cause of adverse reactions was the application of external medicine (41 cases), followed by 36 cases of foreign body implantation, eyebrow tattooing, ear piercing, etc. Conclusions : In this report, we demonstrated patterns of adverse reactions in the medical field of dermatology caused by non-medical personnel than medical personnel. We suggest that more effort should be followed by medical personnel to establish clear awareness of skin disease and by patients to be aware of the risks of the illegal medical treatment by non-medical personnel.

측두하악장애 임상연구에 사용된 진단 및 평가기준에 대한 문헌적 고찰 (A Literatural Investigation of Diagnosis Methods and Evaluation Outcomes for the Clinical Trials on Temporomandibular Disorders)

  • Objectives The aim of this review is to provide fundamental data for temporomandibular disorders diagnosis and evaluation criteria which can be used in clinical trial. Methods We investigated the clinical studies on temporomandibular disorders via PubMed. Also, we searched domestic articles through "OASIS", "NDSL", "KISS", "Korean Traditional Knowledge Portal (KTKP)". The articles we focused on were the recent decade from 2007 to 2016. A total of 139 studies were analyzed: 42 domestic articles and 97 overseas articles. This study focuses on the diagnosis and evaluation criteria on temporomandibular disorders. Results 1) In diagnosis criteria, Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) was used 14 times in domestic articles and 63 times in overseas articles. Clinical symptoms were used 13 times in domestic articles, 17 times in overseas articles. 2) In evaluation criteria, Visual Analog Scale (VAS) was used as a pain scale 12 times in domestic articles, 63 times in overseas articles. Pressure Pain Threshold (PPT) was used 16 times in overseas articles. Whereas, no clinical trials used PPT in domestic articles. In psychological scale, Symptom Checklist-90-Revised (SCL-90-R) was used 6 times. However, SCL-90-R was used 2 times in domestic articles. ROM(Maximum Mouth Opening (MMO), Maximum Comfortable Opening (MCO) were used 24 times in overseas. In domestic articles, MMO was counted 12 times, while MCO was counted 5 times. Conclusions This research reviewed the tendency of using diagnosis methods and evaluating outcomes of the clinical on TMD. It is expected that this investigation would develop further treatment for TMD in the Korean Medicine.

노인 낙상 예방을 위한 운동 가이드라인의 적절성과 적용가능성 (Assessment of Applicability of Guidelines for Fall Prevention Exercise of Elderly People)

  • Purpose: The purpose of this study is to analyze the suitability and applicability of the recommendations for the exercise intervention program in fall prevention guidelines for the elderly. Methods: We searched systematically RISS, KISS, National Assembly Library, KoreaMed, and KM base for Korean guidelines and used OVID-MEDLINE, EMBASE, Cochrane library, Trip database, GIN, NGC, WHO, and CDC for international guidelines including fall prevention exercises for the elderly until August 2016. Results: A total of 11 guidelines were selected as the result of the literature search. After evaluating the quality of guidelines, 9 guidelines were used to analyze the recommendations. A total of 19 recommendations were derived from 9 guidelines. Of 19 recommendations, 12 recommendations were considered to be appropriate and applicable. As a result, we recommend for the elderly to do a single type of balance exercise, strength exercise, aerobic exercise or multiple forms of exercise for 20~30 minutes per a day, 2~3 days or more than 3 days per a week, and consistently for more than 10 weeks. Conclusion: To prevent falls in the elderly, it is important for the elderly to perform a regular balance, strength, or aerobic exercise for 20~30 minutes per a day, 2~3 days per a week.
